Nonsurgical Treatment Versus Intramedullary Fixation of Displaced Metacarpal Shaft Fractures.

PURPOSE: Retrograde intramedullary headless compression screw (IMHS) fixation offers potential advantages over traditional surgical treatment of metacarpal shaft fractures. The purpose of this study was to compare the rates of recovery of short-term functional outcomes for displaced metacarpal shaft fractures treated nonsurgically and those treated using a minimally invasive intramedullary screw technique. We hypothesized that IMHS fixation would lead to earlier return of function with equivalent outcomes at final follow-up. METHODS: We identified patients between 2017 and 2022 with a displac

Duplication in the SHOX Gene as a Rare Genetic Cause of Short Stature and/or Skeletal Abnormalities: A Clinical Report and Review of the Literature.

The SHOX (short stature homeobox containing gene) haploinsufficiency can result in phenotypes ranging from idiopathic short stature to Leri-Weill dyschondrosteosis (LWD). It has been reported to have been detected in 5-17% of children diagnosed with idiopathic short stature, and in 60-90% of children with LWD. SHOX duplications, although typically associated with tall stature, can result in short stature and/or extremity anomalies in rare cases when partial or complete duplications involving the SHOX region occur. In this case report, two patients with extremity anomalies who were found to hav

Matched Pair Analysis of Rotational Stability With 1- Versus 2-Screw Antegrade Fixation in Distal Metaphyseal Ulnar Shortening Osteotomy Constructs.

BACKGROUND: The purpose of this study is to evaluate the rotational stability of 1-screw versus 2-screw antegrade fixation in distal metaphyseal ulnar shortening osteotomies (DMUSOs) using matched cadaveric forearm specimens. In addition, we aim to assess the mode of failure for each construct, specifically determining whether failure occurs due to hardware failure or peri-implant fractures. METHODS: Sixteen fresh-frozen cadaveric forearm specimens, organized into 8 matched pairs, were used. Ulnas were harvested, transected at the midshaft, and all soft tissue attachments were removed. A 5-mm

Introducing V-Soft Pro: A Modular Platform for a Transhumeral Prosthesis with Controllable Stiffness.

Current upper limb prostheses aim to enhance user independence in daily activities by incorporating basic motor functions. However, they fall short of replicating the natural movement and interaction capabilities of the human arm. In contrast, human limbs leverage intrinsic compliance and actively modulate joint stiffness, enabling adaptive responses to varying tasks, impact absorption, and efficient energy transfer during dynamic actions. Inspired by this adaptability, we developed a transhumeral prosthesis with Variable Stiffness Actuators (VSAs) to replicate the controllable compliance foun

Bilateral symmetrical brachymetacarpia of the ring fingers: a case report.

Brachymetacarpia is the term for the shortening of the digit as a result of the metacarpal shortening. Isolated idiopathic brachymetacarpia is rarely reported in African countries, and its incidence is still unknown in the literature compared to brachymetatarsia, which is extensively reported. We report an uncommon case of a 26-year-old male patient who presented with short, symmetrical ring fingers without an associated medical condition. Clinical examination revealed signs in keeping with brachymetacarpia, while plain radiographs showed a metacarpal sign. Non-operative treatment was given fo

AUTS2 disruption underlies radioulnar synostosis and skeletal dysmorphogenesis: evidence from four unrelated cases.

BACKGROUND: While AUTS2 is recognised as a pivotal neurodevelopmental gene, its role in skeletal morphogenesis has remained unexplored. We investigated the contribution of AUTS2 to radioulnar synostosis (RUS) and associated skeletal dysplasias through integrated molecular and phenotypic analyses of unrelated probands. METHODS: Comprehensive genetic profiling was performed on patients with RUS, including G-banding karyotyping, translocation breakpoint mapping via low-coverage whole-genome sequencing with PCR/Sanger validation, CNV detection using SNP array (Infinium OmniZhongHua-8) and qPCR, a

A novel concealed posterior axillary approach for scapula glenoid fractures - a multicenter clinical study.

Glenoid fractures of the scapula can be treated via anterior deltopectoral, Judet, modified Judet, or posterior minimally invasive approaches; however, these may result in soft tissue injury, hematoma formation, nerve damage, or inadequate internal fixation stability. While arthroscopy minimizes soft tissue dissection, it may pose challenges in visualizing inferior glenoid fragments. To address these potential complications, we designed a new posterior axillary approach, the Tian Yun-Wu Dankai approach (T-W approach), in the treatment of certain glenoid fractures of scapula. We retrospectively

Intra-patient comparison of trapeziectomy with LRTI and dual mobility prosthesis for trapeziometacarpal osteoarthritis: a multicenter observational study.

PURPOSE: Thumb osteoarthritis is a debilitating condition that affects a large portion of the elderly population. Conservative treatments for this condition often fail, and a surgical solution is required. Many different surgical techniques have been described, but the current literature has not yet demonstrated the superiority of one over the others. In this study, we analyzed the clinical and radiological findings of a population of 26 patients who were operated on both hands but with different techniques. METHODS: One hand underwent trapeziectomy with suspension arthroplasty, while the oth

Recommendations Regarding Suspected Scaphoid Fractures Are Associated With Patient and Surgeon Comfort With Uncertainty.

AIMS: In a scenario-based survey experiment, we evaluated the contributions of patient and surgeon tolerance for uncertainty to variation in management strategies for suspected scaphoid fractures. We asked the following: (1) Are patient and surgeon factors, including comfort with uncertainty, associated with the choice of management strategy for a suspected scaphoid fracture? and (2) What patient and surgeon factors are associated with comfort letting the patient decide how to manage their suspected scaphoid fracture? METHODS: Surgeon members of the Science of Variation Group completed the sh

Individualizing glenoid lateralization to optimize range of motion after reverse shoulder arthroplasty with the Arthrex Univers reverse: a virtual assessment of the influence of scapular neck length using a statistical shape model.

BACKGROUND: Preoperative scapular neck length (SNL) varies widely due to anatomical factors and pathologic glenoid wear. A short SNL can lead to early impingement, decreased range of motion (ROM), and increased rates of scapular notching after reverse shoulder arthroplasty (rTSA). Glenoid-sided implant lateralization can avoid these issues, but it is unclear how much is necessary for varying SNLs. The objective was to use a statistical shape model, varying SNL, to evaluate impingement-free rotational ROM across different glenoid positions and lateralizations. METHODS: A total of 100 scans wer

A fetal case of Stüve-Wiedemann syndrome due to a novel homozygous truncating variant in IL6ST.

Stüve-Wiedemann syndrome is a rare skeletal dysplasia characterized by severe shortening and bowing of the long bones and by immunological and autonomous dysfunction, usually resulting in early death. Bi-allelic loss-of-function variants in either the leukemia inhibitory factor receptor encoding LIFR or the interleukin-6 cytokine family signal transducer encoding IL6ST are causative. So far, five individuals from three unrelated families were described with IL6ST associated Stüve-Wiedemann syndrome. We here report on a sixth case that came to attention at 21 weeks of gestation with short and b

Poly-l/d-Lactide Implant (RegJoint) Versus Tendon Interposition for Trapeziometacarpal Inflammatory Arthritis: A 15-Year Follow-up of a Randomized Controlled Clinical Trial.

BACKGROUND: Traditional surgical management of trapeziometacarpal (TMC) joint destruction in rheumatoid arthritis involves trapezial resection with tendon interposition. The poly-l-d-lactide RegJoint (Scaffdex, Tampere, Finland) implant offers an alternative with promising short-term results in rheumatoid patients. METHODS: This study reports long-term outcomes from a randomized controlled trial comparing partial trapeziectomy with the RegJoint versus tendon interposition TMC arthroplasties in patients with inflammatory arthritis, including seropositive and juvenile rheumatoid arthritis, as w

Ulnar head replacement or head resection in patients with distal radioulnar arthritis: a prospective cohort study of clinical and patient-reported outcomes up to 2 years after surgery.

BACKGROUND AND PURPOSE: Traditional surgery for arthritis of the distal radioulnar joint (DRUJ), which typically involves resecting the ulnar head, is being increasingly challenged by newer techniques, such as prosthetic ulnar head replacement. The aim of our prospective cohort study was to investigate the clinical and patient-reported functional results, up to 2 years postoperatively, among patients with DRUJ arthritis treated with ulnar head replacement or resection. METHODS: 40 patients were included and underwent either ulnar head replacement (n = 22) or ulnar head resections (n = 18), du

Costo-Osteochondral Autograft for the Non-salvageable Proximal Scaphoid: A Systematic Review.

Background: Scaphoid non-union is a challenging condition that can lead to chronic wrist pain, decreased range of motion (ROM) and functional impairment. One promising approach involves the use of costo-osteochondral rib autografts, offering a non-vascularised and structurally supportive environment for bone regeneration. This systematic review aims to evaluate the current evidence regarding the management of non-salvageable scaphoid proximal poles and non-union using rib costo-osteochondral autografts. Methods: A comprehensive search of electronic databases was conducted. Data extraction was

Short-term Complication Rate in Single- Versus Dual-Mobility Thumb Carpometacarpal Joint Arthroplasty.

BACKGROUND: Single-mobility carpometacarpal (CMC) joint arthroplasty shows good long-term outcomes but has a dislocation risk of 5% to 8%. Newer dual-mobility designs aim to reduce this risk. This study compares the early outcomes and complications of single-mobility Arpe (Zimmer Biomet) versus dual-mobility Touch (Kerimedical) CMC joint prostheses. METHODS: We retrospectively reviewed patients who underwent CMC joint arthroplasty at our hospital from August 2018 to December 2022 by a single surgeon. We were able to obtain follow-up information on all included patients. The Arpe prosthesis wa

Is Scaphoid Reconstruction Still an Option for Scaphoid Nonunion With Established Arthritis? A Retrospective Study of a Group of Patients.

PURPOSE: Although many studies have supported salvage procedures as the best option for scaphoid nonunions with established arthritis, there remains uncertainty regarding when procedures to gain union are favored over salvage procedures. The purpose of this study was to evaluate whether the short-term results from scaphoid nonunion with established arthritis treated by scaphoid reconstruction are acceptable. METHODS: We retrospectively studied the clinical outcomes of scaphoid nonunions with established arthritis treated by scaphoid reconstruction between January 2019 and December 2021. Eight

Isolated Spiral Fracture of the Right Fourth Metacarpal: "Power Drill Kickback Fracture"-A Distinct Fracture Pattern Caused by a Specific Mechanism of Injury.

BACKGROUND: Isolated spiral fractures of the metacarpals are relatively common injuries. However, we observed a distinct and consistent fracture pattern associated with a specific occupational mechanism of injury, which, to our knowledge, has not been previously reported in the literature. METHODS: We retrospectively reviewed the charts of 43 construction workers who presented with metacarpal fractures following hand injuries sustained due to sudden drill kickback. This mechanism typically involved the drill bit becoming abruptly lodged in concrete, causing a forceful rotational load to be tr
