Fix the phalanx: A meta-analysis comparing intramedullary screws, pinning, plates, and lag screws for closed extra-articular proximal phalanx fractures.

BACKGROUND: Proximal phalanx fractures are common hand injuries with multiple fixation options. This meta-analysis compared outcomes of closed extra-articular fractures treated with intramedullary screws (IMS), percutaneous pinning (PCP), plating, or lag screws. METHODS: A systematic review of MEDLINE, Embase, CINAHL, and Cochrane databases (January 2010-May 2025) was conducted according to PRISMA guidelines. Primary outcomes were complication and revision rates; secondary outcomes included immobilization duration, total active motion [TAM] and grip strength. Data were pooled using random- or

Surgical management of Kienböck's disease with non-negative ulnar variance: A systematic review.

BACKGROUND: Kienböck's disease is a complex disorder characterized by avascular necrosis of the lunate, influenced by variations in vascular anatomy and abnormal carpal loading. Management remains challenging, particularly in patients with neutral or positive ulnar variance, where standard radial shortening is biomechanically inappropriate. The present systematic review evaluates reported surgical strategies for Kienböck's disease in this population, comparing isolated capitate shortening osteotomy with combined intracarpal procedures and exploring the adjunctive role of vascularized bone graf

Use of a magnetic intramedullary nail for axial compression in endoprosthetic reconstruction of the humerus: a description of technique.

Pathological fractures, both completed and impending, of the humeral diaphysis from primary or metastatic disease demand a reconstruction that restores stability while preserving shoulder function and alleviating pain. Conventional reconstructive approaches include the use of long-stemmed endoprosthetic devices, plate-and-screw fixation or intramedullary nailing, all of which can be limited in their success when residual bone is short or biologically weak. This case study details the management of a patient with metastatic leiomyosarcoma who presented with refracture through a previously cemen

Five-Year Radiographic and Clinical Outcomes of Pyrocarbon Hemiarthroplasty for Glenohumeral Arthritis and Osteonecrosis.

BACKGROUND: This study evaluated the progression of humeral head medialization in patients who underwent pyrocarbon hemiarthroplasty (PyC-HA). The authors hypothesized that glenoid erosion would not dramatically progress between the short-term and final imaging evaluations, and that there would be excellent clinical outcomes at ≥5-year follow-up. METHODS: Patients who underwent PyC-HA with ≥60 months of follow-up were included in this prospective study. Relevant data included preoperative demographic characteristics, Walch glenoid classification, changes in clinical outcomes, and revision-fre

Spontaneous Reduction of Radial Head Dislocation by Gradual Ulnar Lengthening in Patients With Ulnar Growth Disturbance.

PURPOSE: Radial head dislocation associated with a relatively short ulna is the most severe deformity of the forearm. Surgical reduction of the radial head is challenging and often yields unpredictable results. This study investigated whether, in pediatric patients, a dislocated radial head could be spontaneously reduced through gradual ulnar lengthening. METHODS: In this retrospective study, we reviewed patients who underwent ulnar lengthening by distraction osteogenesis from June 2008 to March 2024. Patients with short ulnas accompanied by dislocated radial heads were included. The radial h

A Radiographic Classification of Capitate Morphology and Its Association With Kienböck Disease.

PURPOSE: Kienböck disease (KD), osteonecrosis of the lunate, has an unclear etiology. While there is a strong and clear association between KD and negative ulnar variance (UV), it is likely not the only mechanical factor. The capitate, which has a large joint surface with the lunate, may play a role in KD pathogenesis. This study investigated the association between capitate morphology and KD. We classified the capitate into three types based on the angle and length ratio of the capitotrapezoid (CT) and scaphocapitate joint lines: type I (short, vertical CT facet), type II (pentagonal with a l

Placement of humeral transcondylar screws in dogs with humeral intracondylar fissures using preoperative computed tomography planning and a novel aiming guide.

OBJECTIVES: To evaluate the clinical use of preoperative computed tomography (CT) planning and a novel aiming guide to place humeral transcondylar screws in dogs with humeral intracondylar fissures. MATERIALS AND METHODS: Measurements from preoperative CT scans were used to identify entry and exit points of the screw. These points were identified at surgery using anatomical landmarks and measurements from articular surfaces. An aiming device was used to place transcondylar positional locking screws from medial to lateral. Assessment of position within the humeral condyle was made from postope

Comparing clinical and functional outcomes following anatomic total shoulder arthroplasty based on humeral component length (Equinoxe; Exactech, Inc.).

BACKGROUND: There has been a trend toward the use of shorter humeral components with the goal of preserving bone, decreasing operative time, and a theoretical decrease in stress shielding. The purpose of this study was to compare the clinical and functional outcomes of anatomic total shoulder arthroplasty among standard-length stem, short-stem, and stemless humeral components, with a minimum of 2-year follow-up. METHODS: A retrospective review of a multicenter international database was performed for anatomic total shoulder arthroplasty with a minimum of 2-year follow-up between 2018 and 2021

Heterozygous loss of OSR2 can cause radioulnar synostosis with ancillary skeletal manifestations.

PURPOSE: Although radioulnar synostosis (RUS) and other skeletal anomalies are features of the 8q22.2q22.3 microdeletion syndrome, the precise genetic etiology of RUS remains undefined. Here, we aimed to define the genetic basis of joint fusion in this syndrome. METHODS: We performed combined chromosomal microarray, high-throughput ligation-dependent probe amplification, and exome sequencing on RUS probands and families. Variant effects were assessed through structural modeling, Western blot, and immunofluorescence. Murine Osr2 knockout phenotypes were evaluated via literature review. RESULT

Radiological and clinical evaluation of stemless anatomical shoulder arthoplasty Affinis Short™ (Mathys) with a minimum follow-up of 5 years.

INTRODUCTION: Stemless shoulder implants appeared on the market in 2003. The interest of this type of anatomic shoulder prosthesis is a better adaptation of the implant to the humeral anatomic characteristics and humeral bone preservation. These implants enabled an easier restoration of the rotation centre of the humerus, eliminating the problem of diaphyseal humeral offset, but few publications at long term are related. Therefore we did a retrospective investigation aiming to analyse signs of humeral and glenoid loosening and to assess the clinical results obtained using anatomic stemless sho

Expanding the differential diagnosis of bone-forming tumors: a new entity characterized by a NIPBL::BEND2 fusion.

We report two bone-forming tumors driven by a newly recognized fusion. The first is a lesion in the distal radius of a 59-year-old female subject, which had been present for 10 years. Imaging showed an intramedullary tumor with an exophytic dorsal component and marked sclerosis, histologically best classified as low-grade osteosarcoma. The second was a destructive lesion in the left ulna of a 13-year-old female with a short history of elbow pain. Imaging showed a lytic lesion with subtle tumor ossification, histologically best classified as an atypical osteoblastoma-like tumor. A NIPBL::BEND2

Humeral Head Reconstruction in Anatomic Shoulder Arthroplasty: How to Assess It, How to Avoid Overstuffing, and Whether It Matters.

➢ Humeral head anatomy affects the tension and mechanics of the glenohumeral joint. Thus, aiming for anatomic reconstruction can help to avoid negative consequences of component malpositioning (such as "overstuffing") on soft-tissue tension and impingement-free range of motion.➢ The most common method to assess humeral head reconstruction is comparing the prosthetic humeral articular surface with the "perfect circle" incorporating the lateral cortex of the greater tuberosity, the medial greater tuberosity, and the medial calcar at the anatomic neck. Although this method is quick and helpful in

Short-term outcomes of anatomic total shoulder arthroplasty with nonaugmented glenoid component for Walch B2 and B3 glenoid morphology.

BACKGROUND: The optimal management of posterior glenoid bone loss in glenohumeral osteoarthritis with anatomic total shoulder arthroplasty (aTSA) remains unknown, as the degree of bone loss and retroversion can vary across a wide spectrum in glenoids with Walch B2 and B3 morphology. The objectives of the current study were to evaluate the clinical and radiographic outcomes of utilizing standard, all-polyethylene, nonaugmented anchor-peg glenoid (APG) components in aTSA for patients with mild to moderate B2 or B3 morphology. METHODS: Between January 2010 and September 2019, we identified 79 sh

A multi-centre, participant-blinded, randomized, 3-year study to compare the efficacy of Virtual Surgical Planning (VSP) to Freehand Surgery (FHS) on bony union and quality of life outcomes for mandibular reconstruction with fibular and scapular free flaps: study protocol for a randomized phase II/III trial.

BACKGROUND: Advanced head and neck malignancies with underlying bony involvement often require aggressive oncological resection of large segments of the oral cavity including the mandible. These patients require vascularized donor osseous free tissue transfer to reconstruct significant defects. Traditionally, the donor bone is harvested on its vascular supply and shaped to the defect in a free hand fashion (FHS). However, virtual surgical planning (VSP) has emerged as a method to optimize reconstructive outcomes and decrease operative time. The goals of this study are to assess superiority of
