Epidemiology of mid-life fracture: Self-Reported and Hospital Episode Data from the UK Biobank.

Fracture risk in mid-life (ages 35-65) is under-recognised despite its implications for long-term skeletal health. Using UK Biobank data, we aimed to characterize fracture epidemiology in this critical age group through two complementary approaches. We conducted two studies: a cross-sectional analysis of self-reported fractures (2006-2010) to estimate annualized incidence risk per 10,000 people, and a longitudinal cohort analysis using linked Hospital Episode Statistics (2001-2022) to calculate incidence rates per 10,000 person years, both stratified by sex, skeletal site, and 10-year age band

Suprapatellar versus infrapatellar tibial nailing: the ongoing debate.

PURPOSE: This study aims to critically appraise both suprapatellar (SPN) and infrapatellar (IPN) tibial nailing techniques in terms of fracture reduction, fluoroscopy use, anterior knee pain, and overall clinical outcomes, exploring whether one technique offers clear superiority over the other for all tibia fractures or in a select subset of fractures. METHODS: A narrative review of current literature, including randomised controlled trials, meta-analyses, and high-volume cohort studies, was conducted. Outcomes assessed included malalignment rates, operative time, fluoroscopy exposure, postope

Biomechanical comparison of different fixation methods for tibiofibular syndesmosis injuries: a finite element analysis study.

BACKGROUND: Syndesmosis injuries require effective fixation to restore ankle stability and function. Therefore, the aim of this study was to use the finite element method to evaluate and compare the biomechanical efficiency of three different fixation techniques used to treat syndesmosis injuries: ANK nails, cortical screws, and suture buttons. METHODS: A three-dimensional model of the ankle complex of a healthy individual was created from computed tomography (CT) images for finite element analysis (FEA). In the model, the tibia, fibula, talus, calcaneus, and navicular bones and ligament struc

Intradural spinal bizarre parosteal osteochondromatous proliferation (BPOP): case report and review of the literature.

Bizarre parosteal osteochondromatous proliferation (BPOP), or Nora's lesion, is a benign but locally aggressive osteochondroid proliferation that almost exclusively involves the small bones of the hands and feet. Spinal involvement is exceptionally rare, with only two cases previously reported. We describe a 13-year-old boy who presented with progressive lower-limb weakness, gait instability, urinary urgency, and significant weight loss. MRI demonstrated a well-circumscribed intradural extramedullary mass at L1 with mixed signal characteristics and peripheral enhancement, initially raising con

Flexor Hallucis Longus Preservation vs. Harvest in Fibula Free Flap Surgery: Functional Recovery and Gait Outcomes at 12 Months.

BACKGROUND: The flexor hallucis longus (FHL) is often manipulated during free fibula flap (FFF) harvest. While its resection facilitates exposure, the impact on long-term donor-limb function remains unclear. METHODS: This prospective cohort study compared 12-month functional outcomes between patients undergoing FFF harvest with intentional FHL preservation (n = 46) and those requiring FHL resection (n = 33). A standardized rehabilitation protocol was implemented. Outcomes were assessed via patient-reported measures (AOFAS-AH, FFI), objective clinical tests (manual muscle testing, single-leg h

Clinical information regarding the fibula flap gained from digital subtraction angiography useful for flap elevation.

The fibula free flap (FFF) is commonly used in mandible reconstruction due to its good bone length and volume, and availability of a long vascular pedicle - the fibular artery (FA). In this study, digital subtraction angiography (DSA) was utilized to evaluate the lower leg arteries in 130 patients (48 female, 82 male). The mean distance from the fibula head to the fibular artery (FA) origin was 72.8 mm, with significant differences between sexes (P=0.019) and sides in males (P=0.033). FA diameters were significantly larger in males (P=0.042); in females, right-side FA diameters were larger tha

Lipoma of the Infrapatellar Fat Pad: A Report of Three Cases.

Although lipoma is the most common soft tissue tumor, lipoma in the infrapatellar fat pad (IFP) is rare. Herein, we report three cases of lipoma in the IFP that penetrated the joint capsule and extended subcutaneously. All patients presented with unusual MRI findings. Patients 1, 2, and 3 were 63, 74, and 64 years old, respectively, and all were female. Their chief complaint was a knee mass; however, they did not experience pain or limitations in range of motion. The interval from initial awareness of the mass to first consultation was long (3, 8, and 13 years, respectively). MRI revealed that

A giant solitary osteochondroma of the fibular neck with nerve compression in a 35-year-old male: a case report from Afghanistan.

INTRODUCTION: Osteochondromas are the most common developmental bone tumors, usually presenting in childhood or adolescence as slow-growing, painless swellings. They typically arise from the metaphyseal regions of long bones and are composed of mature bone capped by hyaline cartilage. The most frequent sites are around the knee, particularly the distal femur and proximal tibia. Growth after skeletal maturity is rare and raises concern for malignant transformation. CASE PRESENTATION: We report a 35-year-old male who presented with a progressively enlarging swelling in the right proximal leg, a

Influence of Amphibian Metamorphosis on the Biokinetics and Biodistribution of Radionuclides Cesium and Strontium.

Amphibians currently face a global decline due to numerous stressors, including contaminated ecosystems. Radionuclides, while less prevalent than other anthropogenic contaminants, can pose a threat to aquatic environments due to their long physical half-lives and tendency to accumulate in organisms. This study investigated the accumulation, depuration, and biodistribution of 134Cs and 85Sr in amphibians through metamorphosis. Limnodynastes peronii tadpoles were exposed to dissolved 134Cs and 85Sr for 6 days and then depurated in clean water for up to 12 days. Whole-body Cs and Sr burdens were

Mapping the growing skeleton: Macroscopic analysis of the extracortical topography of long bones in early human development.

External surfaces of developing long bones exhibit complex morphological patterns that reflect coordinated processes of vascular invasion, periosteal apposition, and structural remodeling. Despite their interpretative importance, systematic baseline descriptions of extracortical topography during early ontogeny remain scarce, hampering differentiation between normal developmental variation, pathological alterations, and taphonomic changes. This study provides the first comprehensive macro- and microscopic characterization of surface features in fetal, infant, and child long bones using an iden

Editorial Commentary: Too Many Questions Remain to Consider Slope-Correcting Osteotomy for Most Primary Anterior Cruciate Ligament Reconstructions.

Anterior cruciate ligament (ACL) reconstructions and meniscus repairs can have unacceptably high retear rates among certain cohorts of young athletes. Studies have showed that increased posterior tibial slopes are associated with increased risk of ACL and meniscus retears. Increasing evidence suggests that slope-correcting osteotomy can decrease retear rates following ACL reconstructions and meniscus tears in certain populations. However, preventing ACL graft failures and meniscus retears at all costs should not be the only consideration: risk of major complications and reoperations, ability t

Mid-Term Tibial Tunnel Enlargement and Graft Maturation After ACL Reconstruction: A 5-Year Follow-Up Linking Morphological Dynamics to Knee Function.

OBJECTIVE: Anterior cruciate ligament reconstruction (ACLR) with autologous hamstring tendon is the standard treatment for ACL rupture. However, tibial tunnel enlargement and delayed graft maturation may affect long-term outcomes. Evidence on their mid- to long-term associations with clinical recovery remains limited. The main objectives of this study include: (i) analyze longitudinal changes in tibial tunnel morphology after single-bundle ACLR; (ii) quantitatively evaluate graft maturation at different tunnel regions using the signal intensity ratio (SIR) from MRI; and (iii) examine the corre

Biomechanics of porous lattice-structured tibial implant design for TAR: Role of pore morphology, porosity and implant design.

BACKGROUND: Implant-bone micromotion and stress shielding play a crucial role in the long-term survival of tibial implant for Total Ankle Replacement (TAR). Both these biomechanical considerations are majorly relying on the design of implant and fixation features. The biomechanics of different porous structure features and porosity on biomechanical performance of tibial implant design remain unexplored. The objective is to investigate the role of pore morphology, porosity, and tibial design on the biomechanics of porous lattice-structured tibial implant design for TAR. METHODS: This is achiev

Should We Worry About the Inter-Implant Gap in the Tibia? A Finite Element Analysis of Revision TKA and Distal Plating.

Background and Objectives: The management of periprosthetic tibial fractures distal to revision Total Knee Arthroplasty (TKA) presents a biomechanical challenge, often requiring extramedullary locking plates when long stems preclude nailing. While in femoral fractures the gap between the stem and plate is a well-documented stress riser, requiring implant overlap to prevent an inter-implant fracture, this specific biomechanical scenario has not been studied in the tibia, and it remains unclear if the femoral dogma of mandatory overlap applies to the straight, centrically loaded tibial anatomy.
