The objective of this study was to compare the degree of osteoarthritis caused by different periods of repeated immobilization and mobilization. Existing knowledge of the pathogenic factors causing osteoarthritis is deficient, but it is well known that long-term immobilization produces degenerative joint changes. In this experimental study it is shown that short repetitive periods of immobilization also induce osteoarthritis. A description is given of a prosthesis for replacement of tumour-destroyed diaphyseal bone in the lower extremity. It can be used in a salvage procedure and as an alternative to intramedullary nailing or plating procedures. The method is not complicated provided that mechanically sound devices are used and technical errors are avoided. The operation time is short and the patients can be mobilized early.

The metaphysis of rat tibia was examined by scanning electron microscopy (SEM) using the HCl-collagenase method (EVAN et al., 1976) and the three-dimensional structure of osteocytes and the relationship between osteocytes and osteoblasts were demonstrated. After the HCl-collagenase treatment, the bone matrix near the exposed surface was completely removed but the cellular components retained their structural integrity. Twenty-four cases of Werner's syndrome were studied to define the clinical manifestations in comparison with those found in a clinical review of 153 cases in the Japanese medical literature. A retrospective study of patients who sustained a fracture of the amputation stump was carried out to determine how these injuries might be prevented and to develop an optimum method of treatment. Review of the records of the Mayo Clinic from 1956 through 1978 revealed that 14 patients had a fracture in the amputation stump and 2 patients had two separate episodes of fracture, for a total of 16 fractures. In the space flights by the USA and USSR to date adequate experience has been obtained on the adaptation of man to living conditions in space--with the technical assistance of an artificial protective atmosphere in the space ship or space suit--and to the singular factor of "weightlessness" (microgravity, low gravity) and how he re-adjusts again after returning to a gravitational field. There is no ultimate or absolute physiological barrier for long term space flight--with one exception: decalcification of the skeleton. The distribution and localization of Tc-99m methylene diphosphonate (Tc-MDP) in the epiphyseal growth plates of the rat were elucidated by contact and microautoradiography. The fasciocutaneous flap is very useful in the repair of soft tissue defects on the lower leg. It is easy to design and construct large flaps that are safe because of the good circulation. The operating time is short for an experienced surgeon and no extra equipment is needed. It is perfectly possible to carry out surgery on the underlying bones at the same time. The postoperative treatment is simple for both the patient and the staff. The surgical technique is described and the results of 23 flaps are critically reviewed.

Twenty patients with Köhler's disease of the tarsal navicular were treated at the Alfred I. duPont Institute between 1948 and 1974. Three applications of free vascularised bone grafts are described. Free fibular grafts have been performed in 19 cases of avascular necrosis of the femoral head. The grafts are placed along the anterior femoral neck and into the femoral head with a microvascular anastomosis between the nutrient vessels of the fibula and the anterior circumflex pedicle of the hip. Sixty adult male rats were used to study the effect of the anti-inflammatory drug Ibuprofen on fracture callus and perpendicular skeleton.
