The femoral hypoplasia-unusual facies syndrome comprises short femurs and a facies with short nose, elongated philtrum, thin upper lip and micrognathia. It is felt to be sporadic in nature. An affected father and daughter are described, implying dominant inheritance.

A girl with a "new" variant of mesomelic dysplasia is reported. The disorder is characterized by mesomelic brachymely, especially of the arms, a short ulna, relatively long fibula, brachymetacarpy, minor symmetrical changes at the hands, developmental arrest of the spine, contractures, and micrognathia. The clinical and radiological differential diagnostic features are discussed.

In 5 patients with congenital deficiency of the fibula, the degree of fibular deficiency ranged from unilaterally short to bilaterally absent. The patients each had at least one missing metatarsal and, in addition, dysplasia of a midline metatarsal. In view of these findings, some missing metatarsals associated with congenital fibular deficiency could be midline metatarsals. The abnormality could represent a deficiency acquired after the development of the embryonic limb bud cartilage models.

A review of the literature concerning the classification, etiology, histology and treatment of congenital pseudarthrosis of the tibia is presented. The authors describe two patients in whom resection of the lesion and reconstruction of the extermity with free vascularized fibular grafts were performed. In a retrospective trial 235 patients with fractures of the tibia and/or fibula shafts were followed up. In a consecutive series of 91 patients with 92 injured knees in which the anterior cruciate ligament was irrevocably destroyed, 79 were males; 12 were females. Eleven cases of osteogenesis imperfecta were treated by corrective osteotomies and fixation with expanding intramedullary rods. The authors have used the Bailey-Dubow nail. Despite a short follow-up (average one year) they think that the device will avoid future repeated changes of rods during the growing period. Several technical pitfalls are described. It is thought that the same technique could be applied to congenital pseudarthrosis of the tibia.

This paper reports fifteen cases of ball and socket articulation at the ankle followed up for an average of twelve years. All patients showed inequality of leg length. Ten patients showed coalitions of the bones of the hindfoot and nine patients had a reduction in the number of bony elements of the forefoot. Other associated anomalies are described. The abnormality seems to be part of a congenital short-limb malformation, perhaps modified by adaptive change associated with rigidity of the hindfoot. Surgical intervention was not required in any patient in the series.

Results of surgery and chemotherapy in osteosarcoma are based on a retrospective investigation of 122 patients from 1930 till 1978 treated at the Orthopaedic Department of Vienna University and documented at the Viennese Bone Tumour Registry. This paper reviews the extraction of solutes through capillaries in bone, by means of outflow dilution techniques in the canine tibia. Solutes leave capillaries in bone by passive diffusion dependent on molecular size. The fluid space in bone is also examined by means of analyzing washout curves of radioactive substances. It is proposed that there are 4 definable compartments in bone; a vascular space, a perivascular fluid space a bone fluid space and a bone space. The relevance of these spaces is in bone mineral short and long term ion exchange activity.

Experiments were carried out to determine the optimum conformity between the femoral and tibial condyles in condylar replacement knee prostheses. Wear tests and observations from removed prostheses indicated that both high and low conformity produced characteristic abrasion and fatigue. Partly conforming condyles provided stability under load-bearing but allowed laxity to occur. Fixation to resist the various forces on the tibial components was enhanced by a short central intramedullary peg. Partial conformity is proposed as the optimum configuration between femoral and tibial components.
