The clinical diagnosis of human leptospirosis is challenging owing to the non-specific nature of its symptoms, making laboratory confirmation essential. The microscopic agglutination test (MAT), recommended by the World Health Organisation as the reference serological method for diagnosing leptospirosis, requires ongoing efforts towards optimisation. The present study evaluated the impact of thermal inactivation of serum samples on MAT performance for the diagnosis of human leptospirosis. Thermal inactivation did not compromise antibody detection capacity. Discrepancies observed between inacti

BACKGROUND: The golden Syrian hamster is the main experimental model for leptospirosis. However, there are no validated reference genes for reverse transcription quantitative PCR (RT-qPCR) analysis of blood samples.
METHODS AND RESULTS: The expression stability of 11 candidate reference genes was evaluated in hamster blood under distinct immunological conditions, including Leptospira interrogans infection and different immunization strategies. Stability was assessed using BestKeeper, geNorm, NormFinder, and the comparative ΔCt method, with the consensus ranking calculated using the RefFinder

Leptospirosis is an economically important zoonotic bacterial infection of livestock that causes abortions, stillbirths, infertility, and loss of milk production. Bovine leptospirosis occurs worldwide and is caused by infection with pathogenic bacteria in the genus Leptospira that can colonize the kidney or genital tract of both males and females to maintain disease transmission via shedding in urine, the reproductive tract, or via vertical transmission to a fetus. Cattle are a reservoir host for L. borgpetersenii serovar Hardjo but can also shed other pathogenic species and serovars associate

Leptospirosis can masquerade as acute cholangitis with cholestatic jaundice, fever, and leukocytosis. In febrile patients with cholestatic liver profile without biliary obstruction, especially farmers with calf pain and conjunctival suffusion, consider leptospirosis early to initiate timely antibiotics and prevent poor outcomes.

The nonspecific presentation of leptospirosis and delays in serologic confirmation often make its diagnosis challenging. The presence of comorbidities or potential drug toxicities, which can mimic or obscure clinical findings, further adds to this diagnostic difficulty. Here, we present the case of a 46-year-old male with well-controlled HIV infection on antiretroviral therapy, who presented with a four-day history of fever, malaise, and jaundice. The patient is a pilot with recent travel to the United Kingdom and prior travel throughout North America, Central America, and the Caribbean. On ad

BACKGROUND: Leptospirosis is a zoonotic infection caused by pathogenic Leptospira species and remains a significant cause of acute febrile illness in tropical regions. Its clinical spectrum ranges from mild, self-limiting illness to severe multi-organ dysfunction. The disease is broadly classified into anicteric and icteric (Weil's disease) forms, the latter associated with jaundice and increased morbidity. In India, diagnosis is often challenging due to overlapping features with other endemic infections such as dengue and scrub typhus. Coinfections and changing epidemiological patterns, inclu

Leptospirosis is a zoonosis prevalent on Réunion Island. Severe forms with acute respiratory failure can lead to refractory hypoxemia, requiring the use of extracorporeal membrane oxygenation (ECMO). The aim for the present study was to describe all cases of leptospirosis complicated by refractory acute respiratory distress syndrome (ARDS) requiring veno-venous ECMO on Réunion Island and to conduct a literature review. This retrospective observational study was conducted in a multidisciplinary intensive care unit of the University Hospital of Réunion Island. The aim was to describe the clinica

Leptospirosis is a zoonotic disease caused by pathogenic bacteria of the genus Leptospira and represents a major public health concern worldwide. Although its systemic manifestations are well documented, the consequences of leptospirosis for reproduction and pregnancy, particularly in humans, remain poorly understood. This narrative review summarizes the principal reproductive outcomes associated with leptospirosis and examines the mechanisms that may contribute to pregnancy complications. In livestock, leptospirosis is a well-established cause of reproductive failure, including abortion and i

Leptospirosis is a zoonotic disease that poses public health risks associated with stray and shelter dogs. We examined Leptospira infection in shelter dogs from Okayama and Yamaguchi Prefectures, Japan, using serological and molecular approaches. Anti-Leptospira antibodies were detected by microscopic agglutination test in 48% (10/21) of dogs from Okayama and 49% (24/49) of dogs from Yamaguchi, with serogroup Hebdomadis predominating. Pathogenic Leptospira DNA was detected in urine samples from two of 12 dogs; both were asymptomatic and infected with Leptospira interrogans ST36 belonging to se

INTRODUCTION: Leptospirosis is a neglected zoonotic disease caused by Leptospira spirochetes and remains a significant public health concern in tropical and subtropical regions of India. The humid subtropical climate of Eastern Uttar Pradesh favors its transmission.
METHODS: To assess the occurrence, demographic characteristics, seasonal trends, and epidemiological distribution of leptospirosis in Eastern Uttar Pradesh. A cross-sectional study was conducted on 2430 clinically suspected cases. Serum samples were tested for Leptospira-specific IgM antibodies using enzyme-linked immunosorbent as

BACKGROUND: Leptospirosis is a re-emerging public health threat in French Guiana with a broad clinical spectrum. Neurological involvement (neuroleptospirosis) lacks a consensus definition and may be underrecognized. This study aimed to characterize its epidemiology, clinical features, outcomes, and associated factors.
METHODS: A multicenter retrospective study was conducted across healthcare facilities in French Guiana from 2015 to 2021. Consecutive hospital-managed patients with laboratory-confirmed or probable leptospirosis were included. Neuroleptospirosis was defined by neurological sympt

BACKGROUND: Leptospirosis is a globally prevalent zoonotic disease that remains challenging to diagnose due to its nonspecific symptoms and the limitations of current diagnostic methods and targets. To address these challenges, we developed and validated a naked-eye loop-mediated isothermal amplification (LAMP) assay targeting the newly identified orpH region for the rapid detection of pathogenic Leptospira species.
METHODS: Comparative genomic analysis identified orpH as a conserved sequence among pathogenic Leptospira. A colorimetric LAMP assay (LAMP-orpH) was developed and evaluated using

Objective: To investigate the risk factors for pulmonary hemorrhage and severe infection in adult leptospirosis patients and to construct a prediction model. Method: A retrospective analysis was conducted on patients with leptospirosis admitted to the First Affiliated Hospital of Wenzhou Medical University from August 2017 to September 2025. Patients were divided into a non-pulmonary hemorrhage group (n=95) and a pulmonary hemorrhage group (n=63) based on the presence or absence of pulmonary hemorrhage. Multivariate logistic regression model was used to analyze the risk factors for pulmonary h

The Amapa State, Brazil, in the Amazon region and bordering French Guiana, is a strategic sentinel location for infectious disease surveillance. Ecological pressures such as deforestation, artisanal gold mining, and high human mobility contribute to complex transmission dynamics of arboviruses, bacterial infections, and other pathogens. This study aims to characterize the etiologies of acute febrile illness (AFI) reported in Amapa State in 2018-2020, focusing on arbovirus circulation, bacterial contributions, and emerging pathogens. A retrospective analysis was carried out with 727 AFI cases.

Brazilian spotted fever, caused by Rickettsia rickettsii, is a life-threatening tick-borne disease endemic to southeastern Brazil and is associated with high mortality when treatment is delayed. Central nervous system involvement poses a particular diagnostic challenge because of nonspecific early symptoms and frequent misdiagnosis as dengue or leptospirosis. We report a case illustrating characteristic neuroimaging findings and therapeutic considerations in rickettsial encephalitis. We describe the clinical course, cerebrospinal fluid (CSF) findings, magnetic resonance imaging (MRI) findings,
