Simple clinical decision model for predicting leptospirosis: a secondary analysis of a randomized controlled trial.

BACKGROUND: Leptospirosis is a major cause of febrile illness in tropical regions, but early diagnosis is challenging due to overlapping clinical features and delayed serological confirmation. We developed a simple bedside decision model using readily available clinical variables. METHODS: This secondary analysis used archived data from a randomized controlled trial of adults with undifferentiated febrile illness (October 2020-February 2021) of 5-15 days' duration. Independent predictors of IgM-ELISA positive leptospirosis were identified using multivariable logistic regression. A simplified

Zoonotic transmission risk in flood-prone river basins: community knowledge and behavioral determinants of leptospirosis in eastern Ethiopia.

BACKGROUND: Leptospirosis is a neglected zoonotic disease with a rising incidence in flood-prone tropical regions. In the Shabelle River Basin of eastern Ethiopia, recurrent flooding combined with close human-animal contact creates conditions conducive to transmission. However, community knowledge, attitudes, and behaviors related to leptospirosis remain poorly characterized, limiting effective prevention efforts. METHODS: A community-based cross-sectional study was conducted in flood-affected districts of Mustahil and Fer Fer in eastern Ethiopia. Adult household heads or primary caregivers w

Environmental Distribution of Pathogenic Leptospira spp. in Subtropical Rivers of Japan and Implications for Human Infection.

OBJECTIVES: Leptospirosis is a zoonotic disease caused by Leptospira spp. often linked to freshwater exposure in subtropical and tropical regions. Okinawa Prefecture reports the highest number of leptospirosis cases in Japan based on national surveillance data. Although recreational river activities in Okinawa are a major opportunity for exposure, direct evidence connecting environmental contamination with human infection remains limited. We investigated the spatiotemporal distribution of pathogenic Leptospira spp. in rivers on Iriomote Island, Okinawa, a subtropical region with a high leptosp

Case Report: Diagnosis of leptospirosis presenting as aseptic meningitis using metagenomics CAPture sequencing.

BACKGROUND: Leptospirosis is a globally prevalent zoonotic disease caused by pathogenic Leptospira species. The manifestation of leptospirosis can range widely, from being asymptomatic to causing severe multi-organ failure with a high mortality rate. It is uncommon for leptospirosis to present primarily with neurological complications. In this context, we discuss a notable case of Leptospira borgpetersenii infection manifesting as aseptic meningitis in China. CASE PRESENTATION: In this study, we describe a primary case of neuroleptospirosis leading to symptomatic aseptic meningitis following

Large extrachromosomal replicons are widespread across bacterial lineages and show coordinated replication termination and spatial coupling with the chromosome.

Bacterial genomes frequently harbor extrachromosomal replicons (ERs) that promote genome plasticity and adaptation, ranging from small plasmids to chromosome-scale replicons. In a few model organisms, including Vibrio cholerae and Agrobacterium tumefaciens, large ERs are coordinated with chromosome replication and cell-cycle organization by specific molecular mechanisms. Whether this applies broadly across bacteria remains unknown. Here, we analyzed more than 40,000 complete bacterial genomes to update the distribution of ERs across bacterial taxa. Their GC content converged toward that of the

From Classical Bacterins to Recombinant Vaccines: Critical Aspects of the Immune Response in Ruminants.

Leptospirosis is a neglected zoonosis causing significant economic losses in livestock, primarily through Bovine Genital Leptospirosis (BGL). While current vaccines prevent clinical disease, they typically fail to provide sterilizing immunity against adapted strains. This allows Leptospira to persist in the genitourinary tract, maintaining environmental shedding and zoonotic risk. Achieving sterilizing immunity remains a challenge, and this gap may be closely related to the immune response pattern of ruminants, where effective protection against chronic colonization requires, besides the humor

Piloting Noninvasive Pathogen Assessment and Perceptions at the Snow Leopard (Panthera uncia)-Domestic Animal-Human Interface in Nepal.

Little is known about pathogens circulating in free-living snow leopards (Panthera uncia) and their implications for both species conservation and local communities in Nepal. The close proximity between snow leopards and domestic animals increases the risk of pathogen spillover and represents a critical knowledge gap that has yet to be explored in Nepal. We conducted a pilot noninvasive health assessment of snow leopards in Nepal by screening for the presence of selected pathogens in snow leopards and livestock inhabiting the same area and having local community members complete a knowledge, a

Extracorporeal Membrane Oxygenation Haemoperfusion for Leptospirosis Pulmonary Hemorrhagic Disease: Report of 1 Case.

Severe pulmonary leptospirosis (SPFL), characterized by diffuse alveolar haemorrhage (DAH) and acute respiratory distress syndrome (ARDS), carries a high mortality, often due to diagnostic delays and complex management. We report a case of a 42-year-old male with outdoor occupational exposure who presented with fever and hemoptysis, rapidly progressing to severe respiratory failure and thrombocytopenia. Chest imaging confirmed DAH/ARDS. For refractory hypoxemia, veno-venous extracorporeal membrane oxygenation (VV-ECMO) was initiated without systemic anticoagulation due to active pulmonary haem

Climate change and leptospirosis: a growing environmental and zoonotic threat.

Climate change is increasingly recognized as a key driver in the emergence and re-emergence of infectious diseases, including leptospirosis, a globally distributed bacterial zoonosis. The bacterial agents of leptospirosis infect humans through contact with soil or water contaminated by the urine of animal reservoirs. As a primarily waterborne disease, leptospirosis is strongly influenced by climatic conditions, including rainfall, flooding, and extreme events such as hurricanes and cyclones. Here, we present current knowledge on the links between climate change and leptospirosis, as well as pe

Linking AKIN stages with tropical infection induced acute kidney injury among Indian patients.

Tropical acute febrile illness often causes AKI in India and outcome linkage with AKIN staging is still limited. Hence, we studied 150 adults with tropical infection related AKI staged by AKIN at 48 hours and followed till discharge or death. Leptospirosis was the main cause and higher AKIN stage showed more oliguria organ dysfunction longer stay higher dialysis need with mortality rising to 40.9% in stage III. This study adds a clear stage based risk gradient plus simple bedside red flags and basic labs for early triage and it supports timely aggressive support and dialysis referral to reduce

Leptospiral dissemination is restrained by liver macrophages through Clec4d-driven capture via C/EBPβ activation.

Leptospirosis, caused by pathogenic Leptospira species, is a globally significant zoonotic disease with high morbidity and mortality. However, the organs or cells mainly involved in capturing circulating leptospires and the related mechanisms remain poorly understood. In this study, we firstly proved that the liver was the primary organ that captured leptospires during the very early intravascular phase of infection in mice. Then, we used unbiased flow sorting of leptospires-positive cells and intravital microscopy of mice infected with leptospires, and found that liver macrophages were the ma

Changing landscapes, shifting reservoirs: land-use change andLeptospira ecology in Madagascar.

Land-use change is widely recognized as a driver of zoonotic disease emergence, yet its effects are often context-dependent. In an article published in Applied and Environmental Microbiology, J. A. Rajaonarivelo, K. M. Kauffman, T. M. Randriamoria, J. P. Herrera, et al. (92:e02061-25, 2026, https://doi.org/10.1128/aem.02061-25) show that shifts in small mammal communities across a disturbance gradient in Madagascar are associated with changes in Leptospira prevalence and lineage distribution. Introduced rodents contributed disproportionately to infections, whereas bats exhibited distinct patte
