Endometriosis-related peritoneal adhesion formation: molecular mechanisms and clinical significance.

BACKGROUND: Peritoneal adhesions are a major and often under-recognized cause of morbidity following abdominopelvic inflammation and surgery, contributing to chronic pelvic pain, infertility, bowel obstruction, and surgical complications. Endometriosis, affecting 10-11% of reproductive-age women, is strongly associated with adhesions; however, whether adhesion formation in endometriosis reflects general peritoneal injury or disease-specific mechanisms remains unclear. OBJECTIVE AND RATIONALE: This narrative review examines (i) physiological peritoneal healing and mechanisms of adhesion format

Untangling the Labyrinth: An Integrated View of the Mesothelial and Epithelial Linings of the Female Adnexa Based on Newly Characterized Anatomic and Histologic Junctions.

Epithelial junctions such as the cervical transformation zone and the gastroesophageal and anorectal junctions are hotspots for carcinogenesis. Three junctional regions in the female adnexa have been recently described and characterized: the fallopian tube-peritoneal junction at the distal end of the fimbriae, the ovarian surface epithelial-peritoneal junction at the ovarian hilus, and the fimbria ovarica connecting the distal fallopian tube to the ovarian surface. As most extrauterine high-grade serous carcinomas have been determined to arise in the distal fallopian tube, this new paradigm re

Pack-Size-Dependent Effects of Bone Cement Use in Unicompartmental Knee Arthroplasty: A Prospective Randomized Trial.

UNLABELLED: The current therapeutic agents for the treatments of visceral leishmaniasis are ineffective, and cytotoxic. Therefore, there is the urgent need for new chemotypes for the treatment of the disease with novel mechanisms of action. In our previous investigation, we identified the triazolopyridazine, STOCK6S-84928 as a potential inhibitor of Leishmania donovani sterol methyltransferase (LdSMT) with IC50 value of 118.3 µM. To improve the biological activity of the initial hit compound, we hereby describe the results of structural-activity relationship studies on STOCK6S-84928 via chemic

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Mullerian adenosarcoma of the uterus, ovary and peritoneum: Whole transcriptomic digital spatial profiling (DSP) in a cohort of 42 tumors.

Mullerian adenosarcoma (MA) is a rare biphasic tumor with a malignant stromal component and benign glandular epithelium with periglandular stromal condensation occurring mainly in the uterus. The diagnosis is essentially morphologic and may be difficult particularly in distinguishing MA from other biphasic tumors at the lower end of the spectrum with histologic features similar to, but falling short-off adenosarcoma; i.e., uterine atypical polyp/low-grade or well-differentiated adenosarcoma with low mitotic count and lacking significant nuclear atypia. We investigated the transcriptomic profil

Aligned vascular endothelial growth factor-functionalized gelatin/polycaprolactone/heparin nanofibrous scaffolds with cyclic stretching for mesothelial tissue regeneration.

Mesothelial tissue regeneration requires scaffolds that integrate structural, biochemical, and mechanical cues to mimic the native microenvironment. In this work, random and aligned gelatin/polycaprolactone/heparin nanofibrous scaffolds (rGPH and aGPH) were fabricated by electrospinning. The aligned scaffold (aGPH) provided topographical guidance for mesothelial cell orientation and cytoskeletal organization. To enhance bioactivity, vascular endothelial growth factor (VEGF) was immobilized via heparin binding. This enables the combination of topographical cues with tailored biochemical signali

Molecular and Functional Characterization of the Peritoneal Mesothelium, a Barrier for Solute Transport.

Peritoneal dialysis (PD) is an increasingly needed, life-maintaining kidney replacement therapy; efficient solute transport is critical for patient outcome. While the role of peritoneal perfusion on solute transport in PD has been described, the role of cellular barriers is uncertain, the mesothelium has been considered irrelevant. We calculated peritoneal blood microvascular endothelial surface area (BESA) to mesothelial surface area (MSA) ratio in human peritonea in health, chronic kidney disease, and on PD, and performed molecular transport related gene profiling and single molecule localiz

Eosinophil and B-cell dynamics in the milky spots from Schistosoma mansoni-infected mice: comparison with spleen and bone marrow, and extramedullary eosinopoiesis.

The milky spots (MS) are structures found in the omentum of humans and other vertebrates, representing a fraction of the lymphomyeloid tissue associated with the celom. They majorly consist of B lymphocytes, T lymphocytes, and macrophages. Also found in smaller quantities are mesothelial, stromal, dendritic, and rare mast cells. In an experimental model of Schistosoma mansoni infection, there is significant activation of the omentum and MS, which exhibit numerous eosinophils. Despite being described for many years, the complete profile of cells found in MS and their functions remains largely u

Mesothelial cell responses to acute appendicitis or small bowel obstruction reactive ascites: Insights into immunoregulation of abdominal adhesion.

Previous abdominal surgery (PAS) increases risk of small bowel obstruction (SBO) due to adhesions, and appendectomy (appy) is an independent risk factor for abdominal adhesion-related complications. Peritoneal inflammation, e.g., acute appendicitis (AA), causes formation of reactive ascitic fluid (rA) that activates peritoneum surface mesothelial cells (MCs) to form adhesions. Pathologic adhesions may arise if restoration of MC-regulated fibrinolysis and secretion of glycocalyx (GCX) are disrupted. Proteins affecting these processes may originate from peritoneal rA. This is a prospective obser

Metabolic reprogramming of peritoneal mesothelial cells in peritoneal dialysis-associated fibrosis: therapeutic targets and strategies.

Peritoneal dialysis (PD) is considered a life-saving treatment for end-stage renal disease. However, prolonged PD use can lead to the development of peritoneal fibrosis (PF), diminishing its efficacy. Peritoneal mesothelial cells (PMCs) are key initiators of PF when they become damaged. Exposure to high glucose‑based peritoneal dialysis fluids (PDFs) contributes to PF development by directly affecting highly metabolically active PMCs. Recent research indicates that PMCs undergo metabolic reprogramming when exposed to high-glucose PDFs, including enhanced glycolysis, impaired oxidative phosphor

Inhibition of Caspase-1 Suppresses GSDMD-mediated Peritoneal Mesothelial Cell Pyroptosis and Inflammation in Peritoneal Fibrosis.

Pyroptosis, belonging to programmed cell death, is shown to be mediated by gasdermin D (GSDMD) and gains more and more attention in innate immunity and multiple diseases. However, the role of GSDMD-mediated pyroptosis in peritoneal fibrosis (PF) remains unclear. This study observed NLRP3 inflammasome activation and pyroptosis in the peritoneum of long-term peritoneal dialysis (PD) patients with PF. Moreover, it is found that high glucose (HG) can induce the activation of NLRP3 inflammasome by regulating TLR4/NF-κB and JNK/p38 MAPK signaling in human peritoneal mesothelial cells (HPMCs), leadin

Plasma-activated solutions prevent peritoneal adhesion formation by regulating eNOS expression in mesothelial cells.

INTRODUCTION: Peritoneal adhesions cause significant morbidity due to limited therapeutic options. Current strategies are limited by inconsistent efficacy and potential side effects. Plasma-activated solutions (PAS) exhibit anti-inflammatory and healing promoting properties with good safety, their efficacy in preventing peritoneal adhesions remains further investigation. OBJECTIVES: This study aimed to investigate the therapeutic potential of PAS in preventing peritoneal adhesion formation and to elucidate its mechanisms. METHODS: Two murine peritoneal adhesion models ("ischemic button" and

Platelet-rich fibrin promotes mesothelial cell proliferation and peritoneal repair by up-regulating calretinin to prevent postoperative intestinal adhesion.

Introduction: As the most common postoperative complication, intestinal adhesions can cause intestinal obstruction, female infertility, and even endanger life. The currently developed materials for preventing intestinal adhesions mainly focus on physical barriers and reducing inflammatory reactions, while neglecting the importance of effectively promoting rapid repair of the peritoneum. We previously found that platelet-rich fibrin (PRF) can prevent postoperative intestinal adhesions. The proliferation of mesothelial cells may play a significant role in reducing intestinal adhesions, but the m

Tracing LYVE1+ peritoneal fluid macrophages unveils two paths to resident macrophage repopulation with differing reliance on monocytes.

Mouse resident peritoneal macrophages, called large cavity macrophages (LCM), arise from embryonic progenitors that proliferate as mature, CD73+Gata6+ tissue-specialized macrophages. After injury from irradiation or inflammation, monocytes are thought to replenish CD73+Gata6+ LCMs through a CD73-LYVE1+ LCM intermediate. Here, we show that CD73-LYVE1+ LCMs indeed yield Gata6+CD73+ LCMs through integrin-mediated interactions with mesothelial surfaces. CD73-LYVE1+ LCM repopulation of the peritoneum was reliant upon and quantitatively proportional to recruited monocytes. Unexpectedly, fate mapping

Benign Cystic Mesothelioma of the Peritoneum Arising at the Greater Omentum in a 14-Year-Old Boy.

INTRODUCTION: Mesothelioma arises from mesothelial cells. This tumor is very rare among pediatric abdominal neoplasms. We herein report an extremely rare case of acute abdomen with cystic mesothelioma of the peritoneum in a child. CASE PRESENTATION: A 14-year-old boy was referred to our hospital for emergency surgery. Surgery revealed a primary tumor arising from the greater omentum. The tumor was macroscopically diagnosed as a benign omental lymphangioma and was resected en bloc with the greater omentum. A histopathological examination of the tumor revealed a simple columnar epithelium-like

Uncloaking the Fimbria Ovarica: Histologic Recognition of an Elusive Anatomic Structure.

A portion of the fimbriated end of the fallopian tube known as the fimbria ovarica extends along the lateral edge of the mesosalpinx to the ovary to which it is attached at its lateral pole. Seventy-four examples of fimbrial plicae that were attached to the ovary or broad ligament and lacked features of adhesions were studied. The fimbrio-ovarian attachments were characterized by one or more of the following: continuity between the tubal epithelium and either the ovarian surface epithelium, peritoneum, or both, in 51 cases; direct continuity of the ovarian stroma into the stroma of the fimbria
