OBJECTIVE: Our purpose was to study the frequency of cleft lip mongolian spot in children with cleft lip of various severity.
METHODS: Sixty-six babies with unilateral cleft lip were divided into three groups: namely, those with microform cleft lip (10 subjects), incomplete cleft lip (30 subjects), and complete cleft lip (26 subjects). The incidence of cleft lip mongolian spot in the three groups was studied.

Marginal defects of the eyelid were repaired using skin-muscle-mucosa composite grafts from the lower lip without cartilage support in 2 patients. The method is a simple one-stage procedure and takes little time to perform. Because of the wide contact between the grafts and the recipient sites, grafting was successful. Texture and color match is not too problematic in elderly persons.

Clinical and histopathologic data were obtained on 353 cases of oral melanotic macules. The mean age at the time of diagnosis for all sites was 43.1 years. A significant predilection for females (p < 0.001) was noted and the most common location was the lower lip (33.0%). The mean size of the lesions was 6.8 mm. Brown was the most common color (64.9%), and most (66.0%) of the lesions were described as flat. Melanin was evident in both the basal cell layer and lamina propria in 93.5% of the cases.

The dismal prognosis for patients with oral malignant melanoma is partly due to patients' delayed recognition of signs of early disease and delayed diagnosis by physicians. Crescentic subcutaneous pedicle flaps prepared on the area lateral to the alar base were designed to cover the skin defects in and around the philtrum. In 1 patient a combination of the lateral lip advancement flap and the crescentic subcutaneous pedicle flap was applied to the skin defect on the lateral lip and philtrum. In another patient, a combination of two crescentic subcutaneous pedicle flaps was applied to the skin defect within the philtrum. Both methods have been successful.

Of the present series comprising 66 newborns with cleft lip, 13 (20%) had pigmented macules in the labial skin on the lateral side of the cleft. Histological examination using serial sections were performed on 47 cases and showed evidence of dermal melanocytosis in 40 cases (85%) consisting of 33 (70%) without clinically detectable macules and 7 (15%) with obvious pigmented macules. The fusiform cells in the dermis were positive for dopa reaction and anti-melanocyte antibody. The high incidence of dermal melanocytosis is comparable to that of common Mongolian spots in Asian babies.

The inverse triangular skin defect between lower eyelid and up lip can be repaired by cheek rotation flap. In order to preventing lower eyelid eversion, the author designed a triangular flap in the lateral canthal area which can change force bearing direction in this area. The flap can repair the defect as wide as 8 X 5 cm when the combination of bilobate flap of the posterior auricular flap and cheek flap was used. A bilateral tissue expanding flap is described for the reconstruction of the mid-vermilion border. With this technique, a defect of about one-half of the vermilion border can be reconstructed with little discomfort to the patient. Because of the high degree of tissue extensibility, this procedure appears to be a versatile one especially in elderly people.

A case of lentigo maligna melanoma on the vermilion of the right lower lip which invaded deeply along the mental nerve is described. The patient also had sensory loss and numbness in the area innervated by the mental nerve, which was the only sign of this rare mode of invasion in this case. Early excision of the lentigo maligna lesion is stressed, and if the patient complains of sensory disturbances in the area of the lesion, this might be a sign of deep invasion of the tumour along the nerve bundle.

A comparison of the ultrastructural features of blue nevus cells with those of schwannoma cells indicates that, among other similarities, both exhibit a surrounding external lamina (basement membrane).
