Delayed regional metastasis from small face/scalp cutaneous squamous carcinoma: outcomes and predictors.

OBJECTIVE: CSCC (cutaneous squamous cell carcinoma) of the face/scalp generally has a favorable outcome. However, patients with delayed regional metastasis from scalp/facial CSCC do very poorly. Historically, prophylactic treatment of the N0 neck for the T1-T2 (as staged in the AJCC 7th edition) face/scalp cancers has not been recommended. This study aims to determine predictors of delayed regional metastases in < 4 cm CSCC of the face and scalp. METHODS: We retrospectively analyzed medical records of patients diagnosed regional metastasis of CSCC of the scalp/face at least 6 months after the

The Proximity Prediction Hypothesis: How predictive coding of CT-touch explains Autonomous Sensory Meridian Response and its therapeutic applications.

Autonomous Sensory Meridian Response (ASMR) is a pleasant tingling sensation felt across the scalp and neck, widely reported to reduce anxiety and improve sleep. The Proximity Prediction Hypothesis (PPH) is the first comprehensive predictive coding model explaining ASMR's underlying neural mechanism. PPH posits that near-field acoustic cues from common ASMR triggers (e.g., brushing sounds, whispered speech) engage the audio-tactile Peripersonal Space Network, generating a top-down prediction of gentle C-tactile (CT) touch on CT fibre-rich skin of the scalp and neck. This prediction suppresses

What is the clinical utility of histological diagnosis between pilomatricoma and trichilemmal cyst in children?

Pilomatricoma and trichilemmal cyst are two benign tumors of hair follicles that can affect children. We performed a retrospective study to histologically differentiate the two tumors to determine if this has clinical utility. For 5 years, we conducted a retrospective study on the patients who underwent surgery and evaluated some parameters (site, recurrence and scar outcome) categorizing them based on their histological type. We treated 71 patients, with an average age of 7.6 years. Most of the lesions were localized on the scalp, head and neck. Histological examination diagnosed 55 pilomatri

Squamous cell carcinoma originating in folliculitis decalvans.

Folliculitis decalvans is a chronic and progressive scarring alopecia at the vertex and occipital scalp with a predilection for middle-aged men. Squamous cell carcinoma is an exceedingly rare complication of folliculitis decalvans, reported in 5 cases to date. Herein, we present a case of squamous cell carcinoma of the scalp in a patient diagnosed with recalcitrant folliculitis decalvans and review the clinicopathologic characteristics of all reported cases in the literature.

Acral lentiginous melanoma with subsequent spontaneous vitiligo vulgaris: a case report and literature review.

Acral lentiginous melanoma (ALM) is an infrequent and often missed subtype of melanoma. Its association with spontaneous vitiligo vulgaris is an exceedingly rare condition. The current study presents a rare case of ALM with spontaneous vitiligo vulgaris after complete resection of the ALM. An 84-year-old male patient presented with multiple white skin lesions on the scalp, neck, trunk, and upper and lower limbs for a three-month duration. One year prior, a diagnosis of ALM was confirmed on histopathological examination with a pathological stage of T4bNxMx. The patient did not receive any treat

The Use of Novosorb Biodegradable Temporizing Matrix for Reconstruction in Head and Neck Cancer: A Simple Answer to a Complex Problem.

BACKGROUND: Skin cancer defects in the head and neck can pose a reconstructive challenge. We aimed to evaluate our experience of Novosorb biodegradable temporizing matrix (BTM) in reconstructing complex defects in the head and neck region. METHODS: Assessment of all patients where BTM was used for head and neck skin cancer reconstruction was undertaken at our facility. This included tumor type; defect size; second-stage reconstruction (SSR) method; and complications, including failure. RESULTS: Fifty-four patients were identified with 63 lesions reconstructed with BTM. The mean age was 79. H

Special Considerations for Head and Neck Melanoma.

Approximately 25% of all cutaneous melanomas arise in the head and neck region. Common locations include the face (40%-60%), scalp (14%-49%), neck (20%-29%), and ear (8%-11%) with the most common locations being the occipital scalp and cheek. The complexity of the head and neck anatomic subsites makes definitive management with wide local excision and reconstruction a challenge owing to both functional and cosmetic sequela. Although complex, the key to treatment remains early diagnosis, treatment, and close multidisciplinary management.

Metastatic Melanoma to the Parotid Gland: A Case Report and Review of Current Diagnostic and Treatment Approaches.

Metastatic melanoma to the parotid gland is rare and represents a significant diagnostic challenge due to its atypical presentation, often resembling benign conditions, resulting in delays in diagnosis. Early and accurate detection is crucial for optimizing patient outcomes. We report the case of a 27-year-old woman who presented with a slowly growing, painless mass in her right parotid gland, which had been enlarging over the past three months. Her past medical history included the excision of an atypical blue nevus located on the scalp six years earlier. Imaging studies, including ultrasound

Angiomatoid melanoma.

Angiomatoid melanoma is a rare variant of malignant melanoma not currently recognized by the World Health Organization classification system. In this study, we present a case of primary angiomatoid melanoma in a 57-year-old man who presented to clinic with a complaint of a growing lesion on the frontal scalp for four months. Microscopic examination of the lesion revealed cavernous spaces filled with extravasated red blood cells. Immunohistochemical markers were positive for melanoma and negative for vascular endothelium. We also review the literature on angiomatoid melanoma, finding distinct m

Management of exceptional tumor: MITF::CREM-rearranged tumor on the scalp.

A 57-year old woman presented with a 18-month history of a vertex lesion. The first excisional biopsy showed an histopathology dermal proliferation of epithelioid splinded cells, arranged in nests or trabeculae, suggesting a tumoral pathology such as sarcoma or melanoma, without a definitive diagnosis. Further surgical treatment was realised. The definitive diagnosis is a clear cell tumor with melanocytic differentiation and MITF::CREM translocation. It is a recently described rare tumor, with less than 5 cases reported. This article addresses the management of a rare tumor in a plastic surger

Surgical Reconstruction Toxicity Following Hypofractionated Adjuvant Radiation Therapy for Primary Cutaneous Melanoma.

PURPOSE: Adjuvant radiation therapy (RT) to a cutaneous target has been associated with elevated risk of surgical complications such as graft, flap, or skin substitute reconstruction failure. We sought to better quantify the risk of surgical site complications after hypofractionated adjuvant RT delivered in the modern era to patients undergoing surgical reconstruction for their primary site cutaneous melanoma. METHODS AND MATERIALS: We reviewed clinical data on all patients treated for cutaneous melanoma at our center between 2008 and 2021 with primary tumor resection and reconstruction (graf

Impact of Anatomical Subsite on Achieved Surgical Margins in Head and Neck Basal Cell Carcinoma.

<b>Introduction:</b> Basal cell carcinoma (BCC) most commonly affects the sun-exposed head and neck skin, where oncologic clearance must be balanced against functional and cosmetic outcomes. Whether anatomical subsite independently influences achieved histopathological margins is clinically relevant for ENT practice.<b>Aim:</b> To assess the association between tumor location and achieved surgical margins (minimum lateral and deep) in head and neck BCC, and to identify subsites most prone to narrow/positive margins and adverse histopathological features.<b>Materia

Head and neck dermatitis: a variant of atopic dermatitis.

Atopic dermatitis involves the head and neck area across all age groups. This manifestation is frequently referred to as "the head and neck dermatitis." Aside from a considerable deterioration of the quality of life, it poses a significant diagnostic and therapeutic challenge. The head and neck dermatitis may be mimicked by other inflammatory conditions such as seborrheic dermatitis or contact dermatitis. Furthermore, it can be associated with a wide range of infectious, ocular, psychiatric and hair disorders, which should raise clinical alertness and encourage a multidisciplinary management o

Metastatic Gastrointestinal Stromal Tumor to the Scalp: A Case Report Highlighting Diagnostic Challenges and Molecular Correlation.

Gastrointestinal stromal tumors (GISTs) are rare gastrointestinal mesenchymal neoplasms. While the liver and peritoneum are the most common metastatic sites, skin involvement is rare. Nevertheless, recognizing skin metastases is crucial as it guides targeted treatment and indicates the possibility of widespread disease. Skin metastases pose diagnostic challenges due to histopathologic and clinical variability, often resembling other tumors. We present an 81-year-old female with metastatic GIST involving the scalp with corroborative molecular data. This case underscores the fundamental nature o

An Unusual Culprit: Staphylococcus capitis Native Mitral Valve Endocarditis Following Scalp Surgery.

Staphylococcus capitis is a coagulase-negative Staphylococcus (CoNS) that colonizes the skin, particularly the scalp, and is often dismissed as a blood culture contaminant in the absence of prosthetic material. Reports of invasive adult infections, particularly native valve endocarditis (NVE), are rare. We describe the case of a man in his 50s, previously healthy and with no known cardiac pathology, who presented with a three-week history of night sweats, myalgia, chest discomfort, and severe neck pain. Clinical examination revealed a pansystolic murmur, and five sets of blood cultures were po

Two-month-old with diffuse erythema: A case report.

We present a case of a 2-month-old female with an acute onset and rapidly progressing diffuse erythroderma involving the scalp, face, neck folds, torso, and groin. She had a history of seborrheic and diaper dermatitis and developed a worsening rash that initially appeared as a "blister-like" lesion on the cheek. Examination revealed greasy yellow scaling and intertriginous erythema. Workup ruled out herpes simplex virus/varicella-zoster virus, and blood cultures grew presumed contaminant Staphylococcus epidermidis. She was treated empirically with antibiotics, antivirals, anti-fungals, and top
