National trends in orthopedic surgical procedures among adults with spina bifida: A MarketScan database study.

BACKGROUND: Prenatal screening and folic acid supplementation have decreased spina bifida (SB) incidence, while advances in multidisciplinary care have increased life expectancy for those born with the condition. Historically, orthopedic outcomes research has focused largely on childhood, as earlier assumptions regarding limited survival into adulthood contributed to the exclusion of adults with SB from longitudinal studies. As a result, knowledge of orthopedic procedures performed on adults with SB may help to guide treatment in childhood and transition to adult care. OBJECTIVE: To describe

Limb Mucormycosis in the Early Postoperative Period Following Kidney Transplantation: A Case Report.

Post-transplant mucormycosis is a rare but life-threatening complication with high mortality. We report a severe case of upper limb mucormycosis in a 58-year-old diabetic kidney transplant recipient. Despite standard immunosuppression, progressive skin lesions developed on the left forearm. Initial debridement failed to arrest progression, revealing extensive muscle necrosis and vascular thrombosis. Rhizopus oryzae was confirmed by histopathology and culture. Surgical management required mid-humeral amputation followed by shoulder disarticulation due to persistent fungal invasion. Antifungal t

Clinical Presentation of Shoulder-Hand Syndrome: A Systematic Review.

BACKGROUND: Shoulder-hand syndrome (SHS) is a neurological disorder characterised by pain, loss of function, and trophic changes in the shoulder and hand of the affected limb. SHS shares a number of features with Complex Regional Pain Syndrome (CRPS). Historically, several terms have been used interchangeably with SHS, obfuscating clinical presentation. This review aimed to characterise the presentation of SHS to provide a full clinical picture for clinicians and researchers. Furthermore, we aimed to examine whether symptoms differ between triggered and idiopathic SHS, as indicated in previous

Chronic graft versus host disease in upper limb: Calling upper limb rehabilitation specialists for improving outcomes in patients.

BACKGROUND: Chronic graft versus host disease (cGVHD) is a complex post-transplant complication resulting in varying degrees of musculoskeletal (MSK) manifestations, including sclerosis-type skin conditions, fasciitis, and osteopenia, avascular necrosis affecting the shoulder, elbow and hands apart from other joints of the body. A large proportion of these can often miss out on the expertise of upper limb therapists. STUDY OBJECTIVES: This paper aims to outline upper limb cGVHD manifestations to raise awareness among upper limb physiotherapists and rehabilitation specialists, so that patients

Effects of manual lymphatic drainage combined with Kinesio taping on post-stroke shoulder-hand syndrome: a study protocol for a randomized controlled trial in the Southwest China region.

BACKGROUND: Shoulder-hand syndrome (SHS) is a common upper limb complication following stroke. It is associated with poor functional recovery of the upper limb and reduced mobility. Both kinesiology tape (KT) and manual lymphatic drainage (MLD) are considered effective interventions for treating pain and swelling associated with SHS. However, there is currently no research examining the limitations and safety profile of combining K-tape with MLD for SHS management. This study aims to investigate the efficacy of KT combined with MLD in patients with post-stroke shoulder-hand syndrome. METHODS:

Timing of Tourniquet Release: A Prospective Randomized Trial.

BACKGROUND: Although the benefits and complications of tourniquet use are well-described, there has been minimal upper extremity research evaluating the timing of tourniquet release and how it affects patient outcomes. The objective of this study was to determine if tourniquet release prior to or after incision closure would affect clinical outcomes in a variety of upper extremity surgeries. METHODS: Two hundred fourteen consecutive patients were randomized into 2 groups: skin incision closure without releasing the tourniquet versus skin closure following tourniquet release. Demographic, intr

Reconstructive Goals in Arm and Elbow Defects Treated with the Pedicled Latissimus Dorsi Flap.

Background/Objectives: Reconstruction of complex soft-tissue defects of the arm and elbow remains challenging because of exposed neurovascular structures, wide joint mobility, and the need to preserve function. The pedicled latissimus dorsi (LD) flap remains a valuable option, particularly when recipient vessels are compromised or functional restoration is required. Given the heterogeneity of these injuries, treatment must be individualized according to each patient's defect characteristics, functional demands, and rehabilitation goals, reflecting personalized medicine principles. This study e

Upper-Limb Complications in Diabetes: A Narrative Review.

Upper-limb complications (ULCs) in diabetes, affecting joints, tendons, muscles, connective tissue, nerves, and skin, are underrecognized but prevalent conditions in type 1 and type 2 diabetes. Advances in diabetes care have extended life expectancy, leading to an aging population with diabetes with increased susceptibility to long-term complications beyond traditional vascular issues. Despite some data on ULCs epidemiology, understanding of their pathogenesis, prevention, and impact on quality of life remains limited, and treatments are often based on clinical experience rather than robust ev

Gracilis Free Flap Technique for Elbow Flexion Reconstruction.

BACKGROUND: Brachial plexus injuries (BPI) can be devastating for patients, often resulting in notable physical, psychological, and socioeconomic distress1. Violent accidents that torque the head away from the shoulder frequently damage the upper brachial plexus roots, with varying severity of damage to the lower roots1. Patients having pan-plexus injuries typically present with a flail extremity, loss of sensory function, and generalized atrophy. To improve activities of daily living, the treatment of pan-plexus injuries focuses on restoring antigravity motion of the upper extremity, with elb
