PURPOSE: To evaluate the efficacy of transcatheter embolization of arteriovenous malformations (AVMs) of the shoulder and upper extremity.
PATIENTS AND METHODS: Four men with large AVMs of the shoulder and upper extremity were treated with transcatheter arterial embolization. Symptoms included high cardiac output, pain, paresthesias, and disfigurement. Each patient underwent multiple sessions of embolotherapy.

Cutaneous depressions, commonly known as dimples, are uncommon in locations other than the face. An infant had deep dimple-like depressions near the acromion bilaterally. This most likely represents benign autosomal acromial dimples, a rarely reported autosomal dominant condition. Other causes of dimples include a variety of congenital malformation syndromes, and infectious, metabolic, and traumatic etiologies. Awareness of the etiology and significance of skin dimples is helpful to the clinician when confronted with this unusual finding.

Daily administration of epidermal growth factor (EGF) to neonatal rodents elicits a classic morphogenetic syndrome. Twenty-two patients with pilomatricoma seen from January 1986 to October 1990 were analyzed retrospectively. The incidence of pilomatricoma in our dermatopathologic material is 1 in every 50 cases of skin tumors. Male to female ratio is 1:2. Half of the tumors appeared before the age of 30. Cat scratch disease (CSD) in the setting of HIV infection is associated with lesions of epithelioid angiomatosis but not with granulomatous lesions seen in the normal host. We report a case of CSD in a patient with AIDS and Kaposi's sarcoma with epithelioid angioma of skin, thrombocytopenia, and abnormalities of liver, spleen, lymph node, and pleura that responded to antimicrobial therapy. The latissimus dorsi was transferred on its neurovascular pedicle to reconstruct the shoulder or brachium in nineteen patients. We report 65 cases of a hitherto undescribed neoplasm that occurs chiefly in children and young adults, and has morphologic features reminiscent of both a fibrous histiocytoma and fibromatosis. The median age of the 65 patients was 14.5 years; two-thirds (67.7%) of the patients were younger than 20 years. We report the case of a 56-year old male patient with pulmonary, abdominal and pelvic metastases from a dermatofibrosarcoma (DFS) which had developed 23 years previously in the right scapulo-humeral region and had recurred twice after the first excision, in 1983. The bilobed deltopectoral advancement flap is a versatile reconstructive option for selected cervical defects. This modified version of Bakamjian's classic deltopectoral flap utilizes thin anterolateral neck skin to cover complex anterior neck defects. A technique for temporary ventricular drainage using a subcutaneous pouch is described for use in neonatal hydrocephalus after intraventricular hemorrhage. The advantages include continuous, valve-regulated ventricular decompression, no exposed tubing, avoidance of electrolyte imbalance and of the loss of CSF, obviation of the need for repeated tapping, and the ability to convert to a peritoneal shunt on an elective basis.

One-hundred and forty-six postburn elbow contractures in children were classified into four categories--simple band, complex band (crosses shoulder and/or wrist joint), diffuse scar, and limited scar--to assess the results of surgical treatment. The natural history of malignant fibrous histiocytomas in children has not been well delineated. During a 6-year period, we have treated six patients less than 18 years of age.
