The Utility of Perioperative Products for the Prevention of Surgical Site Infections in Total Knee Arthroplasty and Lower Extremity Arthroplasty: A Systematic Review.

Surgical site infections (SSIs) are among the most prevalent and devastating complications following lower extremity total joint arthroplasty (TJA). Strategies to reduce the rates can be divided into preoperative, perioperatives, and postoperative measures. A multicenter trial is underway to evaluate the efficacy of implementing a bundled care program for SSI prevention in lower extremity TJA including: (1) nasal decolonization; (2) surgical skin antisepsis; (3) antimicrobial incise draping; (4) temperature management; and (5) negative-pressure wound therapy for selected high-risk patients. Significance of Asymmetry of Groin and Thigh Skin Creases in Developmental Dysplasia of the Hip Revisited: Results of a Comparative Study.

BACKGROUND: No high level of scientific evidence exists about the use of asymmetry of skin creases (ASC) on the groin and thigh regions in diagnosing developmental dysplasia of the hip. The aim of this comparative study was to revisit whether or not ASC was a significant clinical finding in developmental dysplasia of the hip. METHODS: This was retrospective analysis of a prospectively collected data. Control group was composed of 1000 consecutive healthy infants (mean age, 46.7 d) whose both hips were considered Graf type I (normal). Study group (SG) was composed of 246 consecutive patients (

The impact of discharge contracture on return to work after burn injury: A Burn Model System investigation.

INTRODUCTION: Despite many advances in burn care, the development of extremity contracture remains a common and vexing problem. Extremity contractures have been documented in up to one third of severely burned patients at discharge. However, little is known about the long-term impact of these contractures. The purpose of this study was to examine the association of extremity contractures with employment after burn injury. METHODS: We obtained data from the Burn Model System database from 1994 to 2003. We included in the study cohort all adult patients who were working prior to injury and iden

Reconstruction of the lower limb bones from digitised anatomical landmarks using statistical shape modelling.

BACKGROUND: Bone shapes strongly influence force and moment predictions of kinematic and musculoskeletal models used in motion analysis. The precise determination of joint reference frames is essential for accurate predictions. Since clinical motion analysis typically does not include medical imaging, from which bone shapes may be obtained, scaling methods using reference subjects to create subject-specific bone geometries are widely used. RESEARCH QUESTION: This study investigated if lower limb bone shape predictions from skin-based measurements, utilising an underlying statistical shape mod

Cutibacterium colonization of the anterior and lateral thigh.

AIMS: Of growing concern in arthroplasty is the emergence of atypical infections, particularly Cutibacterium (formerly Propionibacterium) sp. infections. Currently, the dermal colonization rate of Cutibacterium about the hip is unknown. Therefore, the aim of this study was to investigate colonization rates of Cutibacterium sp. at locations approximating anterior and posterolateral approaches to the hip joint. METHODS: For this non-randomized non-blinded study, 101 adult patients scheduled for hip or knee surgery were recruited. For each, four 3 mm dermal punch biopsies were collected after ad

Analysis of Difficulties and Complications Following Implantation of Modular Prostheses in Patients with Lower Limb Bone Tumours.

BACKGROUND: The implantation of a modular prosthesis is the most popular method of reconstruction of bone defects in oncological patients. Along with clear-cut benefits associated with this procedure, there is also an increased risk of complications. Common complications include deep infections, aseptic loosening and mechanical implant damage. This study assessed the risk of complications following the implantation of a resection prosthesis in patients with lower limb bone tumours and to evaluate difficulties encountered during the treatment. MATERIAL AND METHODS: A total of 149 patients with

Clinical significance of asymmetric skin folds in the medial thigh for the infantile screening of developmental dysplasia of the hip.

BACKGROUND: The presence of asymmetric skin folds in the medial thigh (ASM) is often used as one of the screening items for developmental dysplasia of the hip (DDH). However, few studies have evaluated the clinical relevance of ASM in DDH. METHODS: We reviewed the data of 584 infants aged 4-12 months who were referred to our institute because of suspicious findings in periodic infantile DDH screenings. The reasons for referral were classified into ASM, gluteal asymmetry, limb-length discrepancy, limitation of hip abduction, and click/clunk of the hip. A simple radiograph and hip examination i

A safe percutaneous technique for the reduction of irreducible femoral neck fractures using ultrasound localization of the femoral vascular and nervous structures at the hip.

We present a safe percutaneous technique for the placement of Kirschner wires into the femoral head to assist in the reduction of irreducible femoral neck fractures using ultrasound to identify the vascular and nervous structures about the hip. From January 2011 to June 2014, a total of 36 patients (25 males and 11 females) were enrolled in this study. Patients were placed on a fracture reduction table for limb traction. After 3 unsuccessful reductions with limb traction, ultrasound-guided localization of the patient's femoral artery, vein, and nerve at the hip was performed. These structures w

Asymmetrical thigh creases or isolated thigh crease may be a false positive sign with low predictive value in the diagnosis of developmental dysplasia of the hip in infants: a prospective cohort study of 117 patients.

BACKGROUND: The clinical importance of asymmetrical thigh creases in the diagnosis of developmental dysplasia of the hip (DDH) in infants remains controversial. The aim of this study is to investigate the association of asymmetrical skinfolds of the thigh as the only clinical finding with the DDH in infants. METHODS: One hundred and thirty-four infants between January 2010 and December 2015 were referred to our clinics for DDH with the only clinical sign being asymmetrical or isolated thigh creases and met the inclusion criteria of the study. Patients with another clinical sign for DDH in our
