Intramyometrial ectopic pregnancy masquerading as intramural leiomyoma: a diagnostic challenge.

One of the extremely rare locations for an ectopic pregnancy is myometrium. Intramyometrial ectopic pregnancy is characterised by an unusual form of implantation wherein the gestational sac is situated deep in the myometrium, separated from the endometrial cavity and fallopian tubes. It accounts for <1% of all ectopic pregnancies. The preoperative diagnosis based on ultrasound findings may mimic a degenerated leiomyoma, thereby posing further diagnostic challenges. We report a case of a woman in her late 20s presenting with abnormal uterine bleeding diagnosed ultrasonographically as posterior

Genome-wide meta-analysis identifies novel risk loci for uterine fibroids within and across multiple ancestry groups.

Uterine leiomyomata or fibroids are highly heritable, common, and benign tumors of the uterus with poorly understood etiology. Previous GWAS have reported 72 associated genes but included limited numbers of non-European individuals. Here, we identify 11 novel genes associated with fibroids across multi-ancestry and ancestry-stratified GWAS analyses. We replicate a known fibroid GWAS gene in African ancestry individuals and estimate the SNP-based heritability of fibroids in African ancestry populations as 15.9%. Using genetically predicted gene expression and colocalization analyses, we identif

Focal Area of Low T2 Signal on MRI Scans in a Heterogeneous Uterine Leiomyoma Does Not Exclude the Possibility of Malignancy: A Report of Two Cases.

Background: Uterine sarcomas are uncommon malignant tumors with a grim prognosis, accounting for less than 1% of all gynecologic malignancies. Radiological series often include a limited number of patients, and diagnostic approaches can vary. While the presence of low T2 signal intensity in leiomyomas on MRI has been proposed as a criterion to exclude sarcoma, exceptions to this rule exist. We present two cases that challenge this notion. Case reports: The first patient was a 48-year-old woman presenting with metrorrhagia. MRI revealed a large intramural leiomyoma characterized by extensive hy

Vaginal myomectomy of multiple cervical myomas.

Cervical myomas are rare and pose a treatment challenge, particularly in childbearing years. We describe a case of a woman in her 30s with symptomatic multiple cervical myomas, two of them intramural, who desired a future pregnancy. She was successfully submitted to vaginal myomectomy, an unusual approach in intramural cervical myomas, which allowed uterine preservation and a normal appearing cervix on speculum and ultrasound examinations on follow-up.

Impact of definitive uterine artery occlusion on ovarian reserve markers in laparoscopic myomectomy: a randomized controlled trial with 2-year follow-up.

STUDY QUESTION: Does definitive occlusion of uterine arteries have a short- or long-term impact on ovarian reserve markers in reproductive-age women undergoing laparoscopic myomectomy? SUMMARY ANSWER: Preventive definitive uterine artery occlusion (UAO) during laparoscopic myomectomy reduces intraoperative blood loss but does not impact serum AMH levels after short- and long-term follow-up in reproductive-age women. WHAT IS KNOWN ALREADY: Uterine leiomyomas are the most common benign tumours in women of reproductive age. For symptomatic women willing to retain their uterus, especially for a

Bevacizumab and Erlotinib in Hereditary and Sporadic Papillary Kidney Cancer.

BACKGROUND: Hereditary leiomyomatosis and renal-cell cancer (HLRCC) is an inherited disorder characterized by germline pathogenic variants in the gene encoding fumarate hydratase and an increased risk of papillary renal-cell carcinoma. No effective therapy is known for patients with advanced HLRCC-associated papillary renal-cell carcinoma, and most patients die from progressive disease. METHODS: In this open-label, phase 2 study, we evaluated the efficacy of bevacizumab (10 mg per kilogram of body weight every 2 weeks) and erlotinib (150 mg once daily) in patients with advanced HLRCC-associat

Primary squamous cell carcinoma of the uterine wall associated with adenomyosis: A rare case report and literature review.

Primary squamous cell carcinoma (SCC) of the myometrium associated with adenomyosis is exceedingly rare. We present a case involving a 49-year-old female patient who experienced irregular menstruation for 3 months and was initially misdiagnosed with an anterior intramural uterine leiomyoma. The patient underwent a total abdominal hysterectomy with bilateral salpingectomy. Histopathological analysis revealed infiltrative moderately differentiated squamous cell carcinoma. Adenomyosis was also present, with no lesions detected in the cervix, endocervical canal, or uterine cavity. The lesion was l

Childhood social and economic disadvantage and the risk of uterine fibroids among Black women.

The association of childhood social and economic disadvantage ("disadvantage") and uterine fibroid risk is understudied. We examined the association between disadvantage and fibroid incidence using standardized ultrasound exams at repeated visits, among 1230 participants 23 to 35 years of age in the Study of Environment, Lifestyle and Fibroids. Six disadvantage variables collected at baseline (ie, food insecurity, neighborhood safety, childhood income, mother's educational attainment, household composition, and quiet bedroom for sleep) were evaluated separately, and using a latent class dichot

Submucosal Tunneling and Endoscopic Resection of an Esophageal Leiomyoma in a 13-Year-Old Male.

INTRODUCTION: Esophageal leiomyoma, while rare in children, is the most common benign tumor of the esophagus.1 Many children with leiomyomas present with diffuse disease.2-4 Isolated tumors have been managed by primary resection or enucleation with thoracotomy or thoracoscopy.5,6 More recently, submucosal tunneling and endoscopic resection (STER) has been used for resection of intramural esophageal masses.7-14 This technique for endoscopic surgical resection takes advantage of the submucosal plane, as employed in the per-oral endoscopic myotomy procedure. We describe the first use of STER to t

Combined transcervical radiofrequency ablation and hysteroscopic myomectomy: expanding treatment to diverse fibroid types.

INTRODUCTION: Uterine fibroids are highly prevalent and often symptomatic, leading to abnormal uterine bleeding (AUB) and impaired quality of life. While hysteroscopic myomectomy (HSC) is the gold-standard treatment for submucosal fibroids, it is limited in addressing deeper lesions. Transcervical radiofrequency ablation (TFA) offers a minimally invasive alternative for intramural and transmural fibroids. This study evaluated the safety and effectiveness of combining TFA with HSC in a single session compared to HSC alone. STUDY DESIGN: We conducted a retrospective multicenter analysis of 127

Feasibility of Laparoscopic Removal of the Largest Documented Uterine Fibroid Without Morcellation.

Background and Clinical Significance: Uterine fibroids affect up to 25% of women of reproductive age and can lead to significant symptoms or impact fertility, often requiring surgical management. While hysteroscopic myomectomy is suitable for intracavitary fibroids, intramural and subserosal fibroids typically necessitate open or minimally invasive surgery (MIS). Laparoscopic approaches offer notable advantages, including reduced postoperative pain and faster recovery. However, MIS is frequently avoided in cases of very large fibroids due to technical difficulty and concerns about safe tissue

Leiomyomatosis peritonealis disseminata presenting as acute abdomen: A rare case report.

INTRODUCTION AND IMPORTANCE: Leiomyomatosis peritonealis disseminata (LPD) is an unusual condition characterized by the proliferation of uterine smooth muscle neoplasms onto the peritoneum and other organs in the abdomen. The cause is unknown, but it may be hormonal or uterine trauma. Most symptoms remain vague and are most common among women of reproductive age. Although they are not as common, acute presentations can include hemorrhagic features, which can lead to considerable suffering. CASE PRESENTATION: A 39-year-old para 1 with a history of one first-trimester spontaneous abortion prese

Navigating the Landscape of Uterine Fibroid Treatment Using a Novel Scarless Technology (GYIDE): A Case Report.

Uterine fibroids (UFs) affect 70%-80% of women by age 50. Traditional treatments include medical management and invasive surgery. Recent advancements in fibroid treatment introduce less invasive methods like radiofrequency ablation (RFA), microwave ablation, uterine artery embolization, and MR-guided focused ultrasound. Accurate fibroid mapping and electrode placement are major challenges in the case of fibroid ablation. GYIDE, a scarless technology, demonstrates the feasibility for ultrasound-guided transcervical RFA. A 40-year-old woman with multiple intramural fibroids underwent RFA using G

Recurrent cotyledonoid dissecting leiomyoma of the uterus: A case report.

RATIONALE: Cotyledonoid dissecting leiomyoma (CDL) is an exceptionally rare and morphologically unusual benign uterine leiomyoma. Its malignant-mimicking radiographic and intraoperative features pose a significant diagnostic challenge, often leading to misinterpretation and potentially overtreatment. This case is reported for its rarity and to highlight the critical importance of pathological recognition. PATIENT CONCERNS: A 23-year-old female presented with a recurrent pelvic mass detected during a routine follow-up examination 2 years after the initial surgical resection of a uterine mass.

Vitamin D deficiency and uterine leiomyoma in unexplained infertility.

Uterine leiomyomas are the most common benign tumors of the female genital tract, and alongside hormonal and genetic factors, emerging evidence implicates vitamin D deficiency in their pathogenesis. We investigated the association between serum 25-hydroxyvitamin D [25(OH)D] and the presence of uterine leiomyomas in women with unexplained infertility. In this retrospective case-control study, 148 women aged 18-45 years presenting to the Infertility Clinic of Ankara Bilkent City Hospital between July 2019 and February 2024 were included: 74 had imaging-confirmed leiomyomas (non-submucosal; FIGO
