Regional Proteomic Characterization of Uterine Leiomyomas: Implications for Molecular Pathways and Tumor Biology.

Uterine leiomyomas are the most common benign smooth muscle tumors in women of reproductive age. Recent technological advances have enhanced the potential of proteomic studies to identify proteins and related signaling pathways involved in leiomyoma pathogenesis. This study performed comprehensive proteomic analyses of three leiomyoma subtypes, classified by their localizations, to provide new insights and a valuable resource for further studies. Tissue samples from both leiomyoma and normal myometrial tissues were collected from individuals undergoing hysterectomy for symptomatic leiomyomas.

Hysteroscopic surgery for treating intramural fibroids at the cesarean scar area: a case report and literature review.

BACKGROUND: Uterine fibroids located at cesarean section scar sites are rare and differ clinically from conventional fibroids, frequently causing abnormal uterine bleeding and infertility. Traditional surgical approaches (laparotomy, laparoscopy, or vaginal surgery) necessitate incision of the scar site, resulting in significant tissue damage. We present a successful case of hysteroscopic resection for such fibroids, demonstrating the minimally invasive advantages of this approach. CASE SUMMARY: A 49-year-old multiparous woman with a history of cesarean delivery presented with a 2-year histor

Case Report: Uterine mesothelial cyst in a nulliparous woman-diagnostic and clinical challenges of a rare benign lesion.

Uterine mesothelial cysts (UMCs) are rare, benign lesions that arise from the mesothelial lining of the uterus and are only sporadically documented in the literature. Because of their extremely low incidence, the clinical presentation and imaging features of UMCs are often nonspecific, which makes preoperative diagnosis particularly challenging. We report the case of a 46-year-old nulliparous woman who presented with an intramural cystic mass of the uterus. Preoperative imaging suggested a benign lesion, but the differential diagnosis included cystic degeneration of leiomyoma, adenomyotic cyst

Pregnancy Complicated by Ruptured Degenerating Leiomyoma and Hemoperitoneum: Case Report.

Uterine fibroids are the most common benign gynecologic tumors; in pregnancy, degeneration may occur but is uncommon, whereas spontaneous rupture is an exceedingly rare complication. We present the case of a 43-year-old woman, gravida 2 para 2, at 17 weeks and 6 days of gestation, who was admitted with progressive abdominal pain and distension. Imaging revealed a large degenerating intramural fibroid with hemoperitoneum and a focal contour defect, raising concern for rupture. Despite stable vital signs, the patient experienced spontaneous abortion during hospitalization. Persistent abdominal p

Early ultrasound diagnosis and management of intramural pregnancy: a case series report utilizing the FIGO leiomyoma classification system and 3D sonographic mapping.

BACKGROUND: Intramural pregnancy, a rare ectopic pregnancy with the gestational sac implanted within the myometrium, poses significant diagnostic and therapeutic challenges. Current diagnostic criteria lack anatomical precision, leading to delayed management and high morbidity. This study proposes a novel diagnostic framework integrating the International Federation of Gynecology and Obstetrics (FIGO) leiomyoma classification system and advanced 3D transvaginal ultrasound, enabling anatomical risk stratification and individualized management. METHODS: We retrospectively analyzed four intramur

Outcomes of cesarean myomectomy using novel and safe uterine isthmic circumferential technique.

BACKGROUND: To evaluate the feasibility and safety of performing myomectomy during cesarean delivery using a novel bleeding-reducing suture technique. METHODS: This retrospective cohort study included pregnant women with intramural uterine fibroids. A total of 120 patients were divided into two groups: Group A consisted of women who underwent myomectomy during cesarean section between January 2020 and June 2024, and Group B included women who underwent cesarean section alone without myomectomy. The following data were obtained from hospital records and compared between the groups: maternal ag

Spontaneous uterine rupture after myomectomy in patients during pregnancy: Clinical cases in a single university center.

Uterine rupture is an infrequent yet sometimes fatal complication of a subsequent vaginal birth attempt following a previous uterine surgery. We have chosen to write about spontaneous uterine ruptures following myomectomy due to the scarcity of data on this subject, stemming from the limited number of reported cases. Furthermore, with the increasing trend of advanced maternal age, there is a growing cohort of pregnant women with a history of myomectomy, thereby presenting a timely opportunity to examine this phenomenon in greater depth. A total of 28 studies reporting pregnancies after prior m

Myomectomy for intramural fibroids prior to in vitro fertilization/intracytoplasmic sperm injection cycles, a conundrum: Systematic review and meta-analysis.

BACKGROUND: The role of myomectomy for intramural (IM) fibroids prior to artificial reproductive technique (ART) cycles is still debatable. This study assesses evidence regarding the effect of myomectomy for IM fibroids on in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) cycle outcomes. METHOD: Online search of databases from June 1949 to February 2024 were performed. A literature search was performed using Ovid MEDLINE, EMBASE, and Cochrane Library databases. Medical subject headings and keywords were used to generate a subset of citations for myomectomy, fibroid, leiomyo

Deciphering a diagnostic enigma from ovarian malignancy to disseminated peritoneal leiomyomatosis.

Disseminated peritoneal leiomyomatosis (DPL) is a rare benign disorder of uncertain aetiology that often mimics advanced ovarian or peritoneal malignancy, both clinically and radiologically. The majority of reported cases have been associated with prior laparoscopic uterine myomectomy, particularly when uncontained morcellation was used. Here, we present a case of a nulliparous woman in her 20s who presented with a large abdominopelvic mass and radiological findings suggestive of ovarian carcinoma. However, further history-taking revealed a laparoscopic myomectomy with morcellation performed 2

Uterine leiomyosarcoma diagnostic challenges in a resource-limited setting: a case report and review of the literature.

INTRODUCTION: Uterine leiomyosarcoma, constituting only 1-2% of all uterine malignancies, is a rare and aggressive cancer arising from the smooth muscles of the uterine wall. Owing to its rarity, identifying risk factors and clinical symptoms can be challenging, often leading to misdiagnosis as benign leiomyomas, particularly in settings with limited access to advanced imaging modalities. This case report presents a 40-year-old Ethiopian woman diagnosed with uterine leiomyosarcoma after presenting with nonspecific symptoms, underscoring the diagnostic difficulties associated with this malignan

Two Cases of Giant Uterine Leiomyomas With Myomatous Erythrocytosis Syndrome Treated by Laparoscopic Hysterectomy.

Myomatous erythrocytosis syndrome (MES) is a rare condition characterized by erythrocytosis, the presence of uterine leiomyomas, and the rapid normalization of hematologic parameters following tumor removal. Ectopic erythropoietin (EPO) production by the tumor is considered the main cause. We encountered two postmenopausal women with giant uterine leiomyomas complicated by MES. Case 1 involved a 21-cm subserosal leiomyoma, and Case 2 involved a 14-cm intramural leiomyoma. We performed total laparoscopic hysterectomy with in-bag morcellation in both cases, enabling safe specimen retrieval. Post

Common signs, uncommon condition: A case report of a 41-year-old woman with a large left-sided esophageal hiatus hernia and a benign esophageal leiomyoma.

Benign esophagus tumors are lesions rarely found in the general population. Leiomyomas, the most common of them, arise from the submucosal muscularis propria of the esophagus. They remain asymptomatic for a long time. The lack of typical symptoms, the non-specific abdominal complaints, and the very rare occurrence of these tumors in the population lead to diagnostic failures. This article describes a case of 41-year-old female patient diagnosed with a round lesion found in the chest X-ray. The CT scan revealed a large esophageal hiatal hernia extending to the left ventricular wall. She was qua

Presentation patterns and management outcomes of uterine fibroids: A retrospective study.

Uterine fibroids are common benign pelvic tumors in reproductive-age women, often presenting with heavy menstrual bleeding and pelvic pain. Therefore, it is of interest to analyze 60 patients undergoing myomectomy (66.7%) or hysterectomy (33.3%) between 2022 and 2024. Intramural fibroids (60.0%) with a mean size of 8.2 cm were the most frequent finding. Symptom resolution at 6 months was higher after hysterectomy (95.0%) compared to myomectomy (82.5%, p=0.048), while complication rates were similar. Surgical management provides effective relief, with hysterectomy offering a definitive cure and

Deep learning assisted detection and segmentation of uterine fibroids using multi-orientation magnetic resonance imaging.

PURPOSE: To develop deep learning models for automated detection and segmentation of uterine fibroids using multi-orientation MRI. METHODS: Pre-treatment sagittal and axial T2-weighted MRI scans acquired from patients diagnosed with uterine fibroids were collected. The proposed segmentation models were constructed based on the three-dimensional nnU-Net framework. Fibroid detection efficacy was assessed, with subgroup analyses by size and location. The segmentation performance was evaluated using Dice similarity coefficients (DSCs), 95% Hausdorff distance (HD95), and average surface distance (

Hymen-Sparing Myomectomy: Innovative Laparoscopic Approach for Prolapsed Pedunculated Cervicovaginal Leiomyoma in a Virgin Case.

Leiomyoma is the most common benign tumor of the female genital tract. It may develop subserous, intramural, or submucous. The submucous subtype accounts for 5% of all cases, and it may become pedunculated or prolapse outside the uterine cavity, resulting in vaginal bleeding and pelvic pain, significantly impacting the quality of life, especially for larger leiomyomas. The management of such cases may require vaginal access, which may disrupt hymen integrity and is not accepted in conservative communities. Here, we present an innovative hymen-conserving Laparoscopic-And-Suprapubic Hysteroscopi

An Unusual Cause of Dysphagia in a 16-Year-Old Girl.

This report presents the case of a 16-year-old girl with progressive dysphagia and weight loss over 6 months. Initial examinations, laboratory findings, and upper endoscopy were unremarkable. High-resolution esophageal manometry was suspicious of achalasia, but subsequent imaging revealed the presence of a non-homogeneous mass in the esophageal wall, identified as esophageal leiomyoma through histopathological and immunohistochemical analyses. Despite the rarity of this tumor in adolescents, our findings emphasize the importance of considering esophageal leiomyoma in the differential diagnoses

Removal of port-site and pelvic parasitic fibroids after laparoscopic myomectomy with morcellation: a case report and literature review.

We present the case of a 39-year-old Chinese woman who presented with a palpable mass in her abdominal wall and pelvic cavity 9 years after undergoing laparoscopic myomectomy. Six abdominal wall fibroids, 18 intramural fibroids, 1 posterior adenomyoma, and 6 uterine rectal fibroids were excised. Abdominal wall fibroids were located near the right trocar site of the previous laparoscopic myomectomy, with a total diameter of 15 cm. Histopathologic examination confirmed leiomyoma. Port-site PL is a rare complication caused by power morcellation following laparoscopic surgery. Even rarer are cases

Association of Fibroids, Endometriosis, and Gynecologic Surgeries with Breast Cancer Incidence and Hormone Receptor Subtypes.

BACKGROUND: Fibroids and endometriosis are sex hormone-mediated and exhibit cancer-like behavior. Breast cancer may be more common in women who have had these conditions, but the literature is conflicting and does not always address factors like hysterectomy/oophorectomy status, race/ethnicity, menopause, and hormone receptor subtypes. METHODS: Data are from the Sister Study, a cohort of 50,884 U.S. women enrolled in 2003 to 2009 and followed through 2020. Cox proportional hazards models with time-varying exposures and covariates assessed the relationship of fibroids or endometriosis with bre

Body Mass Index and Uterine Fibroid Development: A Prospective Study.

OBJECTIVE: Fibroids are hormonally dependent uterine tumors. The literature on adiposity and fibroid prevalence is inconsistent. Previous work usually combined all those with a body mass index (BMI) ≥30 kg/m2 into a single category and relied on clinically diagnosed fibroids, which misclassifies the many women with undiagnosed fibroids. We used a prospective cohort design with periodic ultrasound screening to investigate associations between repeated measures of BMI and fibroid incidence and growth assessed at each follow-up ultrasound. METHODS: The Study of Environment, Lifestyle & Fibroids

Rewiring of RNA methylation by the oncometabolite fumarate in renal cell carcinoma.

Metabolic reprogramming is a hallmark of cancer that facilitates changes in many adaptive biological processes. Mutations in the tricarboxylic acid cycle enzyme fumarate hydratase (FH) lead to fumarate accumulation and cause hereditary leiomyomatosis and renal cell cancer (HLRCC). HLRCC is a rare, inherited disease characterized by the development of non-cancerous smooth muscle tumors of the uterus and skin, and an increased risk of an aggressive form of kidney cancer. Fumarate has been shown to inhibit 2-oxoglutarate-dependent dioxygenases (2OGDDs) involved in the hydroxylation of HIF1α, as w
