A 20-year-old primigravida presented at 39 weeks' gestation with the fetus lying in the transverse position. Ultrasound examination indicated an anterior placenta praevia grade II-III. During a vertical lower uterine segment caesarean section, a giant intramural leiomyoma of approximately 25 X 25 cm was found. The uterine incision was extended into the upper segment and a healthy male fetus of 2 568 g delivered. The placenta was situated anteriorly but did not extend into the lower uterine segment. Benign nonepithelial tumor of the urinary bladder is very rare. We experienced a case of a 33-year-old woman who complained of total urinary retention. Vaginal examination revealed a hen's egg sized retrovesical tumor. IVP revealed a filling defect on the cystogram. Cystoscopy revealed protrusion of left side of the bladder neck and intact mucosa. Computed tomography (CT) was performed in 9 consecutive cases of primary gastric neoplasm. Intramural leiomyoma is the most common benign esophageal tumor. The treatment of choice is enucleation without mucosal encroachment. Esophageal resection with reconstruction is rarely necessary, obviating the need for wide exposure. This report demonstrates the technical simplicity of the cervical approach to leiomyomata of the proximal thoracic esophagus. The operative technique avoids the morbidity of the commonly employed thoracotomy incision and deserves consideration in the treatment of accessible lesions.

So far there have been no clinical methods claiming reasonable estimation of the patterns of submucosal tumors. Esophageal location of bronchogenic cysts is rare especially if two cysts are present. We report such a case in a girl 17 years old who had several intramural esophageal smooth defects which looked like multiple leiomyomatosis of the esophagus. At surgery there were two bronchogenic cysts arising in the wall of the esophagus and lying on each side of the azygos arch.

Seven cases of gastric leiomyoblastoma are reported, all of them benign. The subject of gastric leiomyoblastoma is reviewed from both clinical and pathologic aspects based on a review of 307 cases in the literature. Hemorrhage is a frequent sign (51.8%) but in 21.2% of cases leiomyoblastoma is asymptomatic. While such tumors have a malignant potential, a benign course was pursued in most of the cases (79.5%). Benign esophageal tumors are rare. Intraluminal and intramural tumors may be distinguished according to localization,--intramural leiomyomas occur most often. Smooth muscle tumors are the second most common benign tumors of the stomach after adenomatous polyps. Most often they present as solitary submucosal lesion. Occassionally they may be multiple. They usually present as a smooth mass, although they may be pedunculated and rarely present as gastroduodenal intusuception, when they are located in the distal antrum and prolapse into the duodenum. Leiomyomas may be submucosal, intramural or subserosal. Because of this peculiarity they may grow predominantly intraluminal as well as extraluminally.

Only about 250 cases of benign gastric leiomyoblastoma of the stomach have been reported. Most occur in the smooth muscle of the antrum, and may be intramural, intraluminal, or exogastric in position. A pedunculated gastric leiomyoblastoma occurs less frequently, and those containing radiographically identifiable calcifications are extremely rare.

Only occasionally is there a good indication for either abdominal myomectomy or vaginal myomectomy. Four cases of intramural exogastric lesions studied by ultrasound are presented. In all four patients, a large mass was identified in the left mid- and upper abdomen with a strip of solid tissue extending toward the left hemidiaphragm anteriorly, adjacent to the left lobe of the liver. This finding, best demonstrated on longitudinal scanning, suggested that the mass originated in the stomach rather than in the spleen or splenic flexure of the colon, which are posterior in location. Two of the lesions were leiomyoblastomas and two leiomyosarcomas.

Endoscopic examination carried out in 20,000 patients allowed 7 benign tumors of the oesophagus to be detected. Four of them were intramural tumors corresponding to leiomyomas. Guided biopsies are not sufficient to assure an accurate diagnosis, but a mucosal lesion, particularly a carcinoma, should be ruled out. Endoscopic resection of the 3 intraluminal tumours was performed without incidents, allowing an accurate histologic diagnosis and a reasonable therapeutic approach. Endoscopic supervision is necessary in case of epithelial tumors.

Multiple leiomyomas of the esophagus are rare. These benign intramural, extramucosal tumors present a sufficiently characteristic appearance during a barium examination to suggest a specific roentgenologic diagnosis. A gastric leiomyoblastoma can occur in any age group, but the prognosis and sex distribution are different in the child-adolescent group than in adults. In the adult, the lesion is generally considered to be benign, though in the child-adolescent group it may metastasize to regional lymph nodes. The possibility of a leiomyoblastoma is suggested by multicentric intramural gastric tumors discovered in a female child or adolescent who has had gastrointestinal hemorrhage.
