Deciphering endometrial dysfunction in patients with uterine myoma using endometrial organoids: a pilot study.

RESEARCH QUESTION: What influence does an intramural myoma have on the endometrium, and how is this mediated? DESIGN: Endometrium was collected from 13 patients with non-cavity-distorting intramural myomas (diameter ≤4 cm; International Federation of Gynecology and Obstetrics type 4) and 13 patients without myomas undergoing hysterectomy for benign cervical diseases with a similar clinical baseline. Endometrial organoids were established in vitro and induced to reach the secretory phase by oestrogen and progesterone. Transcriptome sequencing was conducted on endometrial organoids in both untr

Benign Metastasizing Leiomyoma in a Patient With No Known History of Uterine Leiomyomas.

Benign metastasizing leiomyoma (BML) is a rare medical condition characterized by metastasis of fibroid tissue from uterine leiomyomas to other areas of the body, most commonly the lungs. While BML is mostly prevalent in women with a prior history of uterine leiomyomas who underwent surgical intervention, this case report explores the case of a 50-year-old female who was diagnosed with pulmonary benign metastasizing leiomyoma (PBML) with no prior history of confirmed leiomyomas. After initially presenting with worsening cough and congestion, chest radiograph and computed tomography revealed mu

Case Report: An Extremely Rare Case of Epitheloid Type Leiomyoma.

Introduction: Uterine leiomyomas are a frequent finding in women of reproductive age. However, rare, unusual growth patterns exist, such as atypical, cellular, mitotically active, myxoid, and epithelioid leiomyomas, and present a major concern as they mimic highly malignant uterine tumors such as uterine leiomyosarcomas. An example of such cases is the epithelial type leiomyoma which is the subject of our report in a 35-year-old female. Case: A 35-year-old, nulliparous lady presented with abnormal uterine bleeding to the emergency department. Workup revealed a suspicious uterine mass that was

Uterine leiomyoma in a 16-year-old young adolescent from Northen Tanzania: A rare case report and review of current literature.

INTRODUCTION AND IMPORTANCE: Uterine leiomyomas, or fibroids, are benign mesenchymal tumors, which represent the most common tumors of the female genital tract. Their occurrence in pediatric and adolescent populations is uncommon, and relatively few cases have been documented in the literature, particularly in lower-resourced settings. This case highlights the importance of recognizing that although uncommon, fibroids can occur in much younger populations. Also highlights the need for healthcare practitioners in resource-limited environments to be vigilant in considering leiomyoma in different

Entirely Intramural Growth Pattern: A Rare Presentation of Esophageal Squamous Cell Carcinoma and Review of the Literature.

Esophageal squamous cell carcinoma (ESCC) is a malignant tumor originating from the squamous epithelium. In contrast, esophageal submucosal tumors are common benign lesions arising from mesenchymal tissues. To date, an entirely intramural growth of ESCC is very rare. This study described a case of an esophageal submucosal tumor resected by endoscopic submucosal dissection (ESD) that was finally diagnosed as ESCC.A 51-year-old woman presented with progressive dysphagia and was provisionally diagnosed with esophageal leiomyoma by further diagnostic modalities. The patient did not have any obviou

Mechanism of Endometrial Receptivity Affected by Fibroids.

Fibroids are the most common benign tumors of the female reproductive system. Most patients with fibroids are asymptomatic, but the presence of fibroids can still cause some abnormal clinical symptoms, such as increased menstrual volume, abnormal uterine bleeding, pelvic pain, urinary tract and gastrointestinal tract compression symptoms, etc. The impact of fibroids on pregnancy is worth discussing. At present, it is believed that submucosal myoma and intramural myoma affecting uterine cavity shape affect the pregnancy outcome of patients, while the impact of type III intramural myoma on pregn

Possible new application of high-intensity focused ultrasound: degradation of uterine myoma classification.

Hysteroscopic myomectomy is widely used to treat submucosal myomas. However, performing hysteroscopic surgery on certain type II (and even type III) or large myomas remains challenging. High-intensity focused ultrasound (HIFU) has recently emerged as a promising alternative for treating various gynecologic conditions. Studies have shown that following HIFU ablation, not only is the myoma volume reduced, but the intramural portion of the myoma may also protrude into the uterine cavity. This reclassification or 'degradation' of the myoma could facilitate the resection of difficult type II and II

Case report: Esophageal bulge with white patch: endoscopic removal of leiomyoma and high-grade intraepithelial neoplasia.

Esophageal leiomyoma is the most common benign intramural tumor of the esophagus. Despite being the most common benign tumor in its category, esophageal leiomyomas constitute only 1.2% of all esophageal tumors. While esophageal leiomyoma itself is uncommon, the occurrence of multiple esophageal leiomyomas is even rarer, and the coexistence of high-grade squamous intraepithelial neoplasia (HGIN), involving both epithelial and mesenchymal tissues, is exceedingly rare. This case report describes a patient with multiple esophageal leiomyomas and localized HGIN, diagnosed using endoscopic ultrasono

Mesothelial Cyst in the Uterine Myometrium: A Rare Localization.

Mesothelial cysts in the uterus are exceedingly rare. A 41-year-old patient presented with complaints of abdominal pain, and transvaginal ultrasonography revealed an enlarged uterus with a hypoechoic intramural cystic mass measuring 7.2 × 3.3 × 3.6 cm in the posterior uterine corpus. Suspecting a degenerated leiomyoma, the patient underwent a total laparoscopic hysterectomy and bilateral salpingectomy. Intraoperatively, a cystic lesion approximately 7 cm in size was observed within the myometrium of the posterior uterine wall. Pathological examination, supported by immunohistochemical findings

Management of subepithelial esophageal tumors.

Subepithelial esophageal tumors (SET) are normally benign intramural esophageal lesions of mesenchymal origin. Although rare, the incidence of SET has increased in recent decades due to the more widespread use of endoscopy and diagnostic imaging. The current review aims to provide an overview of the histopathologic spectrum and the most frequent entities including leiomyoma and gastrointestinal stromal tumor (GIST), diagnostic workup, and multidisciplinary treatment options. Staging for SET should include endoscopy, endoscopic ultrasonography (EUS), and tissue sampling. Current consensus guide

Step-by-step hysteroscopic treatment of International Federation of Gynaecology and Obstetrics type 3 myoma with the cold loop technique.

OBJECTIVE: To demonstrate the "cold loop technique" for the hysteroscopic treatment of International Federation of Gynaecology and Obstetrics (FIGO) type 3 myomas. DESIGN: Step-by-step demonstration of the technique using educative video. EXPOSURE: FIGO type 3 myomas exhibit complete myometrial development while encroaching on the endometrium. This hybrid nature, combining features of both submucous and intramural myomas, may have a detrimental "double hit" effect for patients seeking pregnancy. Currently, there is a dearth of robust evidence regarding the ideal surgical approach for FIGO ty

Role of inflammation and immune response in the pathogenesis of uterine fibroids: Including their negative impact on reproductive outcomes.

Uterine fibroids (UFs), the most common tumors in women of reproductive age, are characterized by sex steroid-dependent growth and excessive deposition of extracellular matrix (ECM) surrounding UF smooth muscle cells. Women with symptomatic UFs experience heavy menstrual bleeding and consequent iron-deficiency anemia. They also have a risk of recurrent pregnancy loss, preterm birth, and cesarean delivery, indicating that UFs can negatively affect reproductive outcomes. Various types of immune cells, including innate and adaptive cells, are observed in UFs; however, the impact of these cells on

Dual treatment approach for leiomyomas of urinary bladder: Experience from tertiary hospital.

INTRODUCTION AND IMPORTANCE: Leiomyomas are rare, benign mesenchymal tumors. They represent 1 to 5 % of all urinary bladder tumors. 20 % are asymptomatic, but most presentations are voiding and storage, followed by hematuria. Surgery has been reported to be a standard treatment, depending on the size of the tumor and its location in the bladder. The following case report, we discuss the case of a bladder leiomyoma presenting with storage and voiding symptoms and managed with dual approach of Transurethral resection of bladder tumor (TURBT) and open urethrotomy. CASE PRESENTATION: A 39-year-ol

Clinical Performance Evaluation of an Artificial Intelligence-Powered Amyloid Brain PET Quantification Method.

BACKGROUND: Fibroid or myoma is the commonest reported tumor of uterus, and is one of the important reasons for hysterectomy in our setting. Different drugs are available for medical management of fibroid uterus including mifepristone, a progesterone antagonist. Varying dosage regimen for mifepristone was studied as medical management of fibroid uterus. The aim of the project was to study the effect of mifepristone on the symptoms and size of fibroids, especially using a low-dose regimen for 6 months. In addition, any symptomatic improvement of menorrhagia and dysmenorrhea was also studied. M

Soy-Based Infant Formula Feeding and Uterine Fibroid Development in a Prospective Ultrasound Study of Black/African-American Women.

BACKGROUND: Uterine fibroids are highly prevalent, benign tumors. They are the leading indication for hysterectomy, and Black women are disproportionally burdened. Soy-based infant formula contains phytoestrogens, and exposure during sensitive developmental windows may adversely affect the developing uterus; early phytoestrogen treatment in rodent studies led to detrimental uterine effects, including increased fibroid risk in Eker rats. Limited epidemiological studies also have suggested increased fibroid development with soy formula infant feeding. OBJECTIVE: The goal of this study was to ex

The effect of ≤6 cm sized noncavity-distorting intramural fibroids on in vitro fertilization outcomes: a systematic review and meta-analysis.

IMPORTANCE: The potential detrimental effects of fibroids on natural fecundity and in vitro fertilization (IVF) outcomes may be influenced by their size, location, and number. The impact of small noncavity-distorting intramural fibroids on reproductive outcomes in IVF is still controversial, with conflicting results. OBJECTIVE(S): To determine whether women with noncavity-distorting intramural fibroids of ≤6 cm size have lower live birth rates (LBRs) in IVF than female age-matched controls with no fibroids.
