INTRODUCTION: Cat Scratch Disease, caused by the bacterium Bartonella henselae, typically manifests with fever, headache, anorexia, weight loss, tender lymphadenopathy, and other systemic symptoms. Transmission commonly occurs through a cat scratch or bite. The clinical course varies depending on the patient's immune status, notably in individuals with conditions such as renal failure. While generally localized, Cat Scratch Disease can occasionally present as a systemic illness with diverse manifestations. This report aims to elucidate the etiology of fever of unknown origin in hemodialysis pa

Fever of unknown origin (FUO) presents a significant diagnostic challenge in renal transplant recipients due to their immunosuppressed state, which predisposes them to a broad spectrum of potential non-infectious and infectious causes, including atypical pathogens. Among these, Bartonella henselae, the agent of bartonellosis or cat scratch disease (CSD), is a rare but significant pathogen in this population, capable of causing several systemic manifestations, including hepatosplenic involvement. We describe the case of a 60-year-old male renal transplant recipient who presented with FUO, diarr

This case report aims to demonstrate the use of amniotic membrane transplantation (AMT) as a treatment for a refractory full-thickness macular hole (FTMH) that developed secondary to neuroretinitis. A 14-year-old girl presented to our hospital with a complaint of blurred vision persisting for one week. The patient was diagnosed with neuroretinitis due to cat scratch disease with a FTMH that developed secondarily. A pars plana vitrectomy, internal limiting membrane (ILM) peel, and ILM transplantation were performed. The FTMH did not close completely. The third surgery was performed using the AM

Many human infections are transmitted through contact with household pets. The purpose of this cross-sectional study was to determine the occurrence of ticks and fleas on pets (dogs and cats) in local veterinary clinics in Statesboro, Georgia. We screened ectoparasites for Rickettsia and Bartonella and assessed owner knowledge and practices related to tick- and flea-borne diseases. Ectoparasites were collected and identified using standard taxonomic keys, and their genomic DNA was extracted. Three TaqMan assays were used to test ectoparasites for flea-borne Rickettsia DNA. Nested PCR targeting

Bartonella henselae, a gram-negative rod, is the etiologic agent of cat scratch disease, which may manifest as a self-limiting local infection or as an atypical, invasive disease. Establishing B. henselae as the causative organism can be challenging as it is a fastidious organism that does not grow on traditional media. Diagnosis is generally performed with serology, which has variable turnaround times, or microbial cell-free DNA (mcfDNA) sequencing, which has a high cost. Therefore, a high index of suspicion is necessary for timely diagnosis and management of these invasive infections. We dep

Cat scratch disease, first described in France in the 1930s, is a benign infectious disease with often atypical presentation. Its clinical symptoms and radiologic features can overlap with those of a sarcoma. Here we present the case of a 12-year-old female with cat-scratch disease that was initially misdiagnosed as a soft tissue sarcoma of the elbow. The patient was then successfully treated with an antibiotic course. Our case emphasizes the importance of early consultation by a specialized multidisciplinary team given the lack of specificity and overlap of presentation among benign and malig

In the last decade, cases of bartonellosis, Q fever and Brazilian spotted fever have been recorded in Brazil. Despite this, their occurrence is still underestimated. This study aimed to investigate Bartonella spp., Coxiella burnetii and Rickettsia rickettsii DNA in organs of urban bats collected from the São Paulo state, southeastern Brazil. Spleen and liver samples were collected from 102 bats from urban areas and evaluated by real-time PCR. Positive samples were submitted to conventional PCR and subsequent sequencing for species identification. In total, 3.9% of the bats were positive for Ba

CASE: A 58-year-old veterinarian presented with a dorsal-radial hand and wrist infection after a catch scratch. Despite multiple courses of broad-spectrum antibiotics, her symptoms progressed culminating in rupture of the first dorsal compartment tendons. An unspecified Mycoplasma species was identified through broad-range bacterial PCR testing. The patient was then started on oral doxycycline which, combined with serial debridement and eventual tenolysis, resulted in successful eradication of her infection and functional improvement.
CONCLUSION: Our case highlights the power of clinical util

Central nervous manifestations of bartonella infection are uncommon and can vary from encephalitis, meningitis, nerve palsy as well as ocular involvement. Most of these manifestations are described in immunocompromised individuals. We report a case of a previously healthy 42-year-old male who presented to the hospital with 4-weeks of fever and headache and found with meningeal signs on physical exam. He reported been scratched by a stray cat at work some days before his symptoms started. He remained febrile in the hospital. Initial work up was unrevealing, but CSF analysis showed pleocytosis,

We report a case of Cat Scratch Disease (CSD) in a 39-year-old male who presented to the emergency department of an infectious diseases center with high fever, chills, myalgia, arthralgia, sweating, dry cough, nausea, weakness, right axillary lymphadenopathy, and fatigue. Initial laboratory findings showed elevated inflammatory markers consistent with bacterial infection. Imaging revealed right axillary lymphadenopathy with multiple hypovascular lesions in the liver and spleen. Patient history revealed multiple cat bites over the past 5 years. Initial treatment with ceftriaxone and azithromyci

Neuroretinitis is an inflammatory type of optic nerve damage evidenced by the appearance of papillary edema. It also involves inflammation of the retinal layers, as evidenced by the thickening of these layers and the presence of intra- and subretinal fluid. Chorioretinitis is a condition in which inflammation of the posterior component of the uvea, the choroid, leads to further damage to the retina, causing it to become inflamed. The most common causes of neuroretinitis and chorioretinitis in the pediatric population are represented by infectious etiologies. Most cases of neuroretinitis in chi

With an incidence of 0.3 per 100,000, spondylodiscitis is a rare condition in children. It is typically bacterial in origin and most commonly caused by Staphylococcus aureus. Bone involvement in cat-scratch disease (CSD) due to Bartonella henselae is exceedingly rare, occurring in only 0.17-0.27% of cases. We present the case of a 3-year-old boy with a two-week history of intermittent back pain and a recent onset fever. Initial laboratory findings were unremarkable, and MRI revealed spondylodiscitis at L3/4 without abscess formation. Empirical antibiotic treatment with ampicillin/sulbactam sho

Bartonella henselae is an important zoonotic pathogen with a wide range of clinical manifestations in humans. Despite advances in understanding its pathogenesis, there is no broadly applicable laboratory animal model for bartonellosis. This study aimed to assess the potential utility of an experimental model of chronic B. henselae infection using ferrets, a species previously utilized in various human pathogen studies. Six ferrets (n = 6) were divided into three groups: a control group (n = 2), a low-dose infection group (n = 2), and a high-dose infection group (n = 2). The two infection group

BACKGROUND: Cat scratch disease (Bartonella henselae) typically presents with fever and granulomatous lymphadenopathy. Immunocompromised individuals, particularly organ transplant recipients, are at increased risk of severe infections with potential multisystem involvement. In renal transplant recipients, the optimal duration of treatment and antibiotic selection remain uncertain due to drug interactions and nephrotoxicity concerns.
METHODS: We present a 13-year-old male renal transplant recipient who was diagnosed with hepatosplenic Bartonella infection after presenting to the pediatric infe

Cat-scratch disease (CSD) is a systemic infection caused by the facultative, rod-shaped, nonmotile, Gram-negative, intracellular zoonotic bacillus Bartonella henselae. The bacteria responsible for CSD were not discovered until decades after the condition was first characterized in 1931. The prevalence of CSD is more common in warm, humid areas and is typically seasonal, peaking in the fall and winter. The pathogenesis of CSD starts when a tiny wound from an infected cat's bite or scratch allows the bacteria B. henselae to enter the human body. The innate immune system, which includes neutrophi
