Treatment Patterns and Decision Drivers in Chinese Patients with Locally Advanced Cervical Cancer: Results from a National Physician Survey.

INTRODUCTION: Cervical cancer (CC) is the fourth-most common and lethal cancer in women worldwide, with ~ 23% of cases in Chinese patients. We sought to characterize real-world treatment patterns and decision drivers for locally advanced CC (LACC) in China. METHODS: We conducted a cross-sectional, 3-phase survey (quantitative development, cognitive, and quantitative interviews) of 360 physicians across 21 cities in China. Primary outcomes included treatment patterns and decision drivers for Chinese patients with LACC. Cancer stage was reported per International Federation of Gynecology and Ob

Cervical strain elastography improves mid-trimester prediction of spontaneous preterm birth beyond cervical length: a single-center cohort study.

OBJECTIVE: Spontaneous preterm birth (sPTB) remains a leading cause of neonatal morbidity and mortality, and cervical length (CL) alone has limited predictive performance in asymptomatic women. Cervical strain elastography may capture biomechanical changes that precede cervical shortening. We aimed to evaluate the predictive value of mid-trimester cervical strain elastography parameters, alone and in combination with CL, for sPTB. METHODS: In this single-center cohort study with prospectively collected data, 275 singleton pregnancies underwent transvaginal assessment at 18 + 0 to 23 + 6 weeks

Immunohistochemical characterization of nerve endings in the vaginal fornix and cervix of small ruminants.

This study assessed nerve endings in the vaginal fornix and the cervix of five Dorper ewes and five Saanen does, quantifying their distribution within epithelial and connective tissues, providing a preliminary assessment of the potential painful conditions they represent. Nerve endings were detected in all animals, with variations in their quantity among the organs and tissues analyzed. Overall, the presence and distribution of nerve endings suggest a sensory role, implicating a potential sensitivity to nociceptive stimuli in these organs.

Mid-pregnancy cervical length and pregnancy outcomes in nulliparous and low-risk multiparous singleton pregnancies: A prospective cohort study (the Quadruple P Screening Study).

OBJECTIVES: This study evaluated the association between midtrimester cervical length (CL) and pregnancy outcomes in low-risk nulliparous and multiparous singletons and assessed whether combining CL with obstetric and medical history improved the prediction of spontaneous preterm birth (sPTB). STUDY DESIGN: This multicenter prospective cohort study included singletons without prior sPTB < 34 weeks who had CL measured at 18-22 weeks GA. Outcomes were retrieved through linkage with the national perinatal registry. Primary outcome was sPTB < 37 weeks. Secondary outcomes included PTB rates, NICU

Neuroendocrine Carcinoma of Cervix: Comprehensive Review of Epidemiology, Pathology, and Advanced Imaging Modalities.

Neuroendocrine carcinoma of the cervix (NECC) is a rare subtype of cervical cancer, making up only about 1%-2% of cases, yet it contributes disproportionately to cancer-related deaths. These tumors are classified into well-differentiated tumors and poorly differentiated small- and large-cell neuroendocrine carcinomas. High-grade NECC is associated with high-risk HPV18 and HPV16 and usually has alterations in TP53, PIK3CA, KRAS, and the Rb and DNA damage response pathways. Immunohistochemical expression of markers such as synaptophysin, chromogranin, CD56, and INSM1 supports the diagnosis. Thes

Cervical cerclage outcomes by indication and cervical length across gestational-age thresholds: a historical cohort study.

BACKGROUND: Cervical cerclage reduces the risk of preterm birth, but factors associated with its success may not be uniform. Among singleton pregnancies with ultrasound- or physical examination-indicated cerclage (no history-based indication), we examined whether factors associated with delivery differ by gestational-age threshold. METHODS: This single-center historical cohort included 114 women undergoing cerclage between May 2020 and December 2025: ultrasound-indicated with cervical length 10 to < 20 mm (Group 1, n = 41) or < 10 mm (Group 2, n = 32), and physical examination-indicated cervi

Can Acupuncture Be Effective in Treating Insufficiently Dilated Cervix Following Intrapartum Uterine Torsion in Cows?

BACKGROUND: This prospective clinical field study evaluated the effect of acupuncture on cervical dilatation following manual correction of intrapartum uterine torsion in cows. METHODS: A total of 93 cows of different breeds and ages diagnosed with intrapartum uterine torsion were randomly assigned to two groups: Group I (n = 45), which received standardised acupuncture prior to retorsion, and Group II (n = 48), which served as a control without acupuncture treatment. Acupuncture was applied approximately 15 min before the initiation of the retorsion procedure. During this interval, all anima

Preoperative considerations for cervical cerclage (part 1).

Transvaginal cervical cerclage remains a central strategy for preventing spontaneous preterm birth in appropriately selected pregnancies. Although decades of research have clarified which patients benefit most from cerclage, substantial heterogeneity persists in pre-, intra-, and postoperative management. This article, the first in a series on cerclage technique, synthesizes the evidence guiding preoperative decision-making. Cerclage is classified by indication as history-indicated cerclage, ultrasound-indicated cerclage and physical examination-indicated cerclage. History-indicated cerclage i

Cervical length assessment using magnetic resonance imaging during pregnancy: correlation and agreement with transvaginal ultrasound.

OBJECTIVE: To assess the correlation between cervical length (CL) measurements obtained via fetal magnetic resonance imaging (MRI) and transvaginal ultrasound (TVS), and the accuracy of CL measurements obtained via MRI in diagnosing a second-trimester short cervix, using TVS as the gold standard. METHODS: This was a retrospective cross-sectional study of pregnancies referred to our center between January 2022 and December 2024 that underwent fetal MRI and TVS examination on the same day. We estimated the correlation between MRI- and TVS-derived CL measurements using Spearman's rank correlatio

Progesterone and cerclage together for short cervix: evaluating potential complementary effects for improved preterm birth prevention.

Management of patients with a short cervical length (CL) on transvaginal ultrasound (TVU) often involves therapy with vaginal progesterone and/or cervical cerclage. Although these interventions are frequently viewed as alternative strategies, they may also represent complementary approaches targeting different mechanisms involved in spontaneous preterm birth. As in other areas of medicine, therapies with different mechanisms are often used together rather than as mutually exclusive options. In line with current level 1 evidence from randomized controlled trials (RCTs) and meta-analyses of RCTs

Biomechanical function of the murine cervix with elastic fiber deficiency.

Preterm birth (PTB) is the leading cause of infant death globally. Improper softening and dilation of the cervix due to cervical insufficiency is a major contributor to PTB. However, the processes describing why or how the cervix prematurely remodels are not well understood. Elastic fibers and smooth muscle cells contribute to contractile and mechanical stability in soft tissues, but their relationship in the cervix is not fully characterized. Towards this end, this study uses the fibulin-5 global knockout mouse model to investigate the impact of elastic fiber deficiency on the contractile and

The Role of Papaverine in the Molecular and Biomechanical Mechanisms of Cervical Ripening: A Narrative Review.

Papaverine (PPV) has long been used in obstetric practice to facilitate cervical dilatation, yet its mechanism of action remains poorly understood. This creates an apparent paradox, as experimental studies indicate that PPV suppresses several inflammatory signaling pathways traditionally considered essential for physiological cervical ripening, despite its reported clinical benefits during labor. In this narrative review, we critically integrate current knowledge of cervical ripening biology with the pharmacological actions of PPV to reconcile this discrepancy. We propose that the reported cli

Quantitative stress-strain cervical elastography characterizes cervical remodeling in pregnancy.

BACKGROUND: Elastography techniques to date have been limited in their ability to provide repeatable measurements across examiners, preventing the ability to compare within the same patient over time and across patients to define population norms. Innovation to define normal and abnormal biomechanical changes in the cervix during remodeling using methodology that is comparable across large populations of patients would facilitate significant clinical and scientific progress. OBJECTIVE: The aim of this study was to test the ability of a novel quantitative, synchronized stress-strain cervical e

Laparoscopic Assisted Neo-Cervico-Vaginal Anastomosis by Rail-Road Procedure In Cervico-Vaginal Atresia.

Cervico-vaginal atresia is a rare entity, difficult to diagnose and treat, with a higher rate of complications. It is classified as type 1b mullerian anomaly per ASRM and subclass C4 per ESHRE classification. The increased rate of re-stenosis and sepsis makes the surgery challenging. Also, the young adolescent age group makes compliance difficult, necessitating an empathetic yet efficient approach. We aim to compare and observe the two repair techniques: laparoscopic v/s open. Techniques for preparation prior to embryo transfer.

RATIONALE: Embryo transfer (ET) is the final and most vulnerable step in in vitro fertilisation (IVF) treatment. Pregnancy rates after ET may be influenced by several factors, including straightening the utero-cervical angle, cervical and endometrial preparation, and techniques for traversing the cervix. Recent research has focused on improving the ET technique in the hope of increasing IVF success rates. This review, first published in 2009, examined preparation techniques, as it remains unclear whether these simple interventions make ET an easier procedure with higher success rates and fewer
