OBJECTIVE: The study assessed the incidence, diagnosis, management, and causes of ureteral injuries during major laparoscopic operations at our department in a 6-year period.
STUDY DESIGN: The study was a chart review of 790 consecutive major laparoscopic procedures in 711 patients (mean age 37.4 +/- 11.6 years, mean weight 64.3 +/- 29.1 kg).

In minilaparotomy-assisted vaginal hysterectomy, the operation begins vaginally by opening the peritoneal folds and ligating the uterine vessels and uterosacral ligaments. Minilaparotomy is then performed for myomectomy, cutting the fallopian tubes and the utero-ovarian ligaments and detaching any adhesions on the anterior peritoneal angle. Twin ectopic gestations are rare; the majority involve one or both fallopian tubes. The case presented is the first known report of a bilateral twin ectopic pregnancy of this type. The patient experienced a concurrent right broad ligament ectopic pregnancy along with a left interstitial pregnancy. Her history was significant for a left cornual resection eight years previously. This case is additionally noteworthy in that intraligamentous gestations are rare, while interstitial pregnancies following cornual resection are even more uncommon.

Malignant female adnexal tumor of probable wolffian origin is distinctly rare. To my knowledge, only four other cases have been reported in the English literature. This article reports a malignant female adnexal tumor of probable wolffian origin of the right broad ligament with extensive metastases to the omentum in an 81-year-old woman. The patient died 3 months after the diagnosis. The site and characteristic histologic features of the tumor and the absence of carcinoma of the ovaries, fallopian tubes, and uterus indicate that this tumor is of wolffian origin.

Laparoscopic surgery is currently the preferred diagnostic and treatment modality for ectopic pregnancy (EP). Cornual pregnancy, a rare and dangerous form of EP, is usually treated by cornual resection or hysterectomy. We report the first case to be treated by laparoscopic surgery. A 28-year-old married, Korean woman was diagnosed with a right unicornuate uterus, a left hypoplastic tube, and a right ovarian necrotic corpus luteal cyst. We performed a right oophorectomy and an excision of the left hypoplastic tube and then transferred the left ovary to the right broad ligament with intact vascular pedicles. Thus the remaining left ovary and right fallopian tube were placed in proximity. The patient conceived 2 months after the operation and delivered a living male infant.
