An innovative predictive scoring system for assessing stone-free rate of lower caliceal stones after flexible ureteroscopic lithotripsy: a retrospective cohort study.

BACKGROUND: Lower caliceal stones (LCS) account for 25%-36% of all renal stones. Flexible ureteroscopic lithotripsy (FURL) is first-line treatment for LCS, but its stone-free rate (SFR) is significantly affected by renal anatomy and stone characteristics. Currently, there is a lack of specific, convenient scoring systems for predicting SFR of LCS after FURL. OBJECTIVES: To assess the factors influencing the SFR of LCS after FURL, and establish a practical, convenient, and accurate scoring system. DESIGN: A single-center retrospective cohort study. METHODS: A total of 149 patients who underw

Laparoscopic Pyeloplasty via Transmesenteric Approach for Pediatric Ureteropelvic Junction Obstruction.

With ongoing advances in minimally invasive surgical techniques, laparoscopic pyeloplasty (LP) has become the standard of care for the treatment of pediatric ureteropelvic junction obstruction (UPJO). Numerous studies have demonstrated that, compared with traditional open surgery, laparoscopic surgery offers significant advantages, including reduced postoperative pain, diminished surgical trauma, accelerated recovery, and superior cosmetic outcomes, while achieving success rates comparable to those of open surgery. Among various surgical approaches, the transmesenteric approach represents an i

The Influence of Age on Cancer-Specific and Other-Cause Mortality in Upper Tract Urothelial Carcinoma.

Background: Upper tract urothelial carcinoma (UTUC) is a rare malignancy with prognosis predicated on clinical and pathological factors. Radical nephroureterectomy is the standard treatment for higher risk localized UTUC, although competing mortality may influence optimal treatment selection in older patients. Methods: A retrospective population-based cohort study was conducted using the Surveillance, Epidemiology, and End Results program. Individuals diagnosed with UTUC of the renal pelvis or ureter between 2000 and 2017 were included. Patients with unknown tumor grade or size were excluded.

Long-Term Empagliflozin Treatment (50 Days) Attenuates Protease Activity, Migration, and Stemness-Associated Phenotypes in BFTC-909 Renal Pelvis Transitional Cell Carcinoma Cells.

Empagliflozin is a sodium-glucose cotransporter 2 inhibitor widely prescribed for the treatment of type 2 diabetes mellitus. It promotes urinary glucose excretion by inhibiting renal glucose reabsorption. Although empagliflozin has been widely used in clinical practice, its effects and underlying mechanisms related to metastasis, cytokine secretion, and stemness-associated phenotypes in renal pelvis transitional cell carcinoma (TCC) remain poorly understood. In this study, we investigated the long-term (50 days) effects of empagliflozin (5 μM) on BFTC-909 human renal pelvis TCC cells. Prolonge

FDG PET/CT in a Case of α-Fetoprotein-Producing Urothelial Carcinoma With Hepatoid Differentiation of the Renal Pelvis.

An α-fetoprotein-producing tumor in the urinary tract is rare. We describe FDG PET/CT findings in a case of α-fetoprotein-producing urothelial carcinoma with hepatoid differentiation of the left renal pelvis with a significantly elevated serum α-fetoprotein level (1035 ng/mL). FDG PET/CT showed intense activity of the left renal pelvic tumor with invasion of the renal parenchyma. This case indicates that α-fetoprotein-producing urothelial carcinoma with hepatoid differentiation should be considered in the differential diagnosis of FDG-avid tumors of the urinary tract in patients with elevated

Predictive value of renal pelvis urine density and perinephric fat stranding for infectious complications after ureteroscopy in patients with obstructive ureteral stones.

Ureteroscopy (URS) is the most commonly used minimally invasive method for treating ureteral stones. Postoperative infection and sepsis are among the most frequent complications following URS. This study investigated the predictive power of renal pelvis urine density (RPUD) and perinephric fat stranding (PFS) on preoperative non-contrast computed tomography for predicting postoperative infectious complications after URS for obstructive ureteral stones. Data from patients who underwent URS for obstructive ureteral stones between January 2020 and December 2025 were examined. Patient groups with

Infantile Homocystinuric Urolithiasis With Staghorn Calculus: A Case Report of Nephron-Sparing Surgical Salvage in an Intraparenchymal Renal Pelvis.

Pediatric urolithiasis, though uncommon, carries significant morbidity when driven by underlying metabolic disorders. Classical homocystinuria, caused by cystathionine beta-synthase (CBS) deficiency, is a rare autosomal recessive inborn error of sulfur amino acid metabolism. While ectopia lentis, skeletal anomalies, and thromboembolic events define its classical triad, urolithiasis presenting within the first year of life remains exceptionally rare in surgical literature. A one-year-old male, born to consanguineous parents with known homocystinuria, presented with gravel-like urinary deposits

Upper tract urothelial carcinoma mimicking emphysematous pyelonephritis: a diagnostic challenge case report.

BACKGROUND: Upper tract urothelial carcinoma is a rare malignancy arising from the renal pelvis or ureter, accounting for 5-10% of urothelial carcinomas. Common clinical manifestations include hematuria and flank pain; however, many patients present with invasive disease at the time of diagnosis due to the absence of characteristic early symptoms. Fever may occur in association with infection or neoplastic fever, which can complicate the diagnostic process. CASE PRESENTATION: A 47-year-old male with a history of recurrent nephrolithiasis and multiple prior right-sided urological interventions

Maternal obstructive uropathy during labor is a risk factor for cesarean delivery.

OBJECTIVES: Obstructive uropathy is the impairment of urinary flow resulting from mechanical compression along the urinary tract between the ureter's origin in the renal pelvis and the urethral outlet. While uncommonly encountered during labor, maternal obstructive uropathy (thought to result from ureteral compression by the fetal head) can be associated with acute kidney injury, and its relationship with clinically meaningful endpoints including risk of cesarean delivery, arrest of labor (previously known as cephalopelvic disproportion (CPD)), and high birthweight remain unknown. METHODS: We

Sequential HER2-targeted antibody-drug conjugate therapy for acquired resistance in a 55-year-old male kidney transplant recipient with metastatic urothelial carcinoma: a case report.

BACKGROUND: Metastatic urothelial carcinoma (mUC) with human epidermal growth factor receptor 2 (HER2) amplification is a highly lethal malignancy, particularly in complex patients. While antibody-drug conjugates (ADCs) are promising, their safety and efficacy in renal transplant recipients maintained on chronic immunosuppression remain largely unexplored. Furthermore, there are a paucity of data regarding the sequential use of different ADCs in this high-risk population. This case highlights its unique clinical importance by demonstrating the feasibility and safety profile of biomarker-driven

Protective Effects and Mechanisms of Extracellular Vesicles in Remote Ischemic Preconditioning on Renal Injury Induced by Inflammation.

It is known that lipopolysaccharides (LPS) could lead to kidney injury and may play a role in intracavitary lithotripsy surgeries such as percutaneous nephrolithotomy (PCNL). Remote ischemic preconditioning (RIPC) has been demonstrated to have protective effects against kidney injury caused by various factors; however, its role in LPS-induced kidney injury remains unclear and its mechanisms require further exploration. Previous studies have shown that extracellular vesicles (EVs) induced by hypoxia mediated the renal protective effects of RIPC. In this study, we established a kidney injury mod

Superselective renal artery embolization for complex duplex kidney with multiple prior surgeries: a case report.

Duplex kidney is a common congenital malformation appearing as duplication of the renal pelvis and ureter. However, complex cases with a history of multiple abdominal surgeries, extensive perirenal adhesions, aberrant vascular anatomy, and long-term anticoagulation remain challenging to manage, and no standardized protocol currently exists. Herein, we report a 54-year-old female patient with a complex duplex kidney who had a history of multiple previous abdominal and renal surgeries, long-term oral warfarin anticoagulation, and concurrent recurrent urinary tract infections, renal abscess, and

Laparoscopic-assisted percutaneous nephrolithotomy for nephrolithiasis in a pancake kidney.

Pancake kidney is an extremely rare renal fusion anomaly characterised by a single discoid parenchymal mass situated within the pelvic cavity. Due to its anomalous rotation and location, it is highly susceptible to urinary stasis, resulting in nephrolithiasis and infections. This report describes the clinical course and management of a man in his 20s who presented with chronic episodic lower abdominal pain and intermittent gross haematuria. Diagnostic imaging revealed a 2.5×2.2 cm obstructive calculus in the renal pelvis of a pancake kidney, causing significant hydronephrosis. Due to the unusu

Evaluating the Resolution of Hydronephrosis Following Pyeloplasty for Ureteropelvic Junction Obstruction in the Adult Population.

OBJECTIVE: Patients with ureteropelvic junction obstruction (UPJO) present with varying degrees of hydronephrosis preoperatively and many heal with persistent hydronephrosis postoperatively despite a successful pyeloplasty, the significance of which is poorly understood. The study aims to illustrate the natural history of pre- and post-pyeloplasty hydronephrosis and determine whether degree of hydronephrosis is predictive of pyeloplasty failure in the adult population. METHODS: We retrospectively reviewed patients 18+ years old who underwent robotic pyeloplasty for UPJO with available imaging
