Kindlins regulate integrin- and growth factor-dependent ureteric bud formation.

The kidney collecting system develops from the ureteric bud (UB), which undergoes multiple rounds of iterative branching. This process is controlled by growth factors and requires the interaction between the extracellular matrix and β1-containing integrin receptors. Integrin affinity for its ligands is regulated by integrin-binding proteins including kindlins, which bind well-defined motifs within the β subunit cytoplasmic tail. We show that mice expressing β1 integrins with mutations that abrogate kindlin binding in the developing UB have mild medullary hypoplasia and a moderate branching def

Generating Training Data for Ureter Segmentation Using Dual-Energy CT Two-Material Decomposition.

This study aimed to evaluate the utility of dual-energy CT (DECT)-based two-material decomposition in facilitating the generation of training data for ureter segmentation. This retrospective two-center study included 180 patients who underwent DECT urography between April and July 2020, including 150 from Institution 1 and 30 from Institution 2. Virtual unenhanced (VUE) images were generated from the late excretory phase (LEP) images using a two-material decomposition technique. Ground truth segmentation masks were created by segmenting contrast-filled ureteral regions on LEP images and were t

Mirabegron administration for the prevention of ureteral injuries during ureteral access sheath insertion.

PURPOSE: Urolithiasis is a common condition in urological practice. Retrograde intrarenal surgery (RIRS) is widely accepted as a safe and effective treatment modality. However, acute ureteral injuries during ureteral access sheath (UAS) placement remain a significant concern. Although beta-adrenergic receptors have been identified in the ureteral wall, studies investigating beta-agonists for ureteral protection are lacking. This study aimed to assess whether short-term preoperative administration of mirabegron, a beta-3 adrenoreceptor agonist, can reduce the incidence of UAS-related ureteral i

Long-term outcomes of ureteral replacement and epithelial regeneration with tapered demucosalized ileum.

BACKGROUND: Ileal substitution is the mainstream treatment for long-segment ureteral defects (LSUDs) but is complicated by the ileum's wide lumen and mucus secretion. This study investigated the long-term efficacy of a modified technique using tapered demucosalized ileum (TDI) in a canine model, with a specific focus on the type and origin of the regenerated epithelium. METHODS: Twenty-five beagle dogs with modeled LSUDs underwent TDI substitution. In the TDI replacement group (TDIR, n = 15), long-term outcomes were assessed at 3, 6, and 12 months using intravenous urography, intraluminal pre

vNOTES High Uterosacral Ligament Suspensions and Bilateral Adnexectomy Under Real-Time Visualization of Ureters Using Indocyanine Green.

STUDY OBJECTIVE: Transvaginal natural orifice transluminal endoscopic surgery (vNOTES) combines the benefits of traditional vaginal surgery-such as the absence of abdominal incisions, reduced postoperative pain, and improved cosmetic outcomes-with enhanced endoscopic visualization and working space compared to conventional vaginal instrumentation [1-3]. This is particularly relevant in urogynecological surgery, where lower urinary tract injuries, especially bladder and ureteral damage, are among the most frequent complications [4,5]. We present the feasibility and safety of performing high ute

Navigating Through Technical Difficulties in Operating Massive Cervical Fibroid-A Case Report and Narrative Review.

INTRODUCTION: Total abdominal hysterectomy (TAH), commonly referred to as extrafascial hysterectomy, is widely used in standard gynecological practice. However, hysterectomy performed in cervical leiomyoma presents greater challenges and pitfalls that require a modified approach. This case report wishes to highlight the importance of anatomical identification and the application of the intrafascial approach of hysterectomy. CASE REPORT: A 57-year-old multiparous woman was admitted with a significant abdominal mass. During the physical examination, a large globular firm mass, corresponding to

Innervation of the Uterus: An Anatomical Study With Application to Better Understanding Pelvic Pain in Women.

Pelvic pain has a significant impact on quality of life, especially when associated with benign pelvic diseases, such as endometriosis or chronic conditions leading to persistent discomfort. A substantial proportion of women are affected in their reproductive years, making it a public health issue. Even though previous research has provided a map of intrauterine innervation, the origins and pathways of macro-uterine innervation are still poorly understood. A better understanding of the complex network of pelvic nerves is crucial for improving surgical techniques and patient outcomes. We perfor

Chylous Mesenteric Cyst: A Rare Surgical Entity-A Case Report From Nepal.

Chylous Mesenteric Cyst is an uncommon benign abdominal cyst that can arise anywhere along the mesentery of the gastrointestinal tract, from the duodenum to the rectum. It is often asymptomatic but can rarely present with abdominal discomfort or a palpable abdominal mass. Final diagnosis typically relies on histopathology examination following laparoscopic or open excision. We present a 54 years old male who initially presented with right lower abdominal discomfort and lump. Computed Tomography (CT) demonstrated a large thin walled cystic lesion abutting and displacing the right ureter. Pre-op

Determining appropriate indications for the self-expanding, metallic stents in the treatment of ureteral strictures of different etiology - efficiency and safety analysis of 65 stents implantations: a single-center, retrospective cohort study.

BACKGROUND: Ureteral strictures constitute an important clinical problem due to the increasing number of cases. Endoscopy has an established but limited role in the treatment of ureteral strictures. Endoscopic treatment of other than single, short strictures results in a high recurrence rate and treatment failure. Promising tools are metallic stents available now on the urological market; however, there are no standardized indications for the treatment of ureteral strictures with the use of metallic stents. Study aim was to determine the features influencing the results of the self-expanding u

Endoscopically visible Randall's plaques are an independent predictor of future stone events.

Randall’s plaques have been long thought to be precursors for kidney stone formation, but how their presence impacts future stone recurrence is not known. The aim of this study was to determine whether patients with endoscopically visible plaques were more likely to have subsequent stone events compared to those without. The presence of Randall’s plaque in adult patients was prospectively assessed during endoscopic cases as part of the Registry for Stones of the Kidney and Ureter (ReSKU). In each case, the visible Randall’s plaque burden was scored by the attending surgeon as “none,” “minimal,

Open vs. robot-assisted laparoscopic ureteral reimplantation in a contemporary pediatric cohort: a retrospective single-institution analysis.

Ureteral reimplantation (UR) in children is traditionally performed to correct vesicoureteral reflux (VUR). The traditional open approach can be performed using various techniques. More recently, the robotic approach has gained popularity but with concerns regarding cost, complications, and overall efficacy. This study aims to understand clinical outcomes in patients undergoing UR via both approaches. We hypothesize robot-assisted ureteral reimplantation (RALUR) has comparable outcomes to open ureteral reimplant (OUR) in a contemporary cohort. An IRB approved registry was queried for all patie
