Birth by cesarean section remodels neonatal brain barriers and associated immune and metabolic pathways in mice.

Delivery mode shapes early-life physiology through multiple pathways, including stress and hormonal signalling, neonatal oxygenation, and exposure to maternal microbes. Cesarean section (CS) bypasses several of these processes and has been associated with altered immune maturation and neurodevelopmental outcomes in humans and animal models. Given that CS is known to disrupt gut barrier integrity and that gut and brain barriers operate as a coordinated network, we investigated whether CS affects early postnatal brain barriers. Using a mouse model, we compared CS-delivered pups to vaginally born

Automated Regional Detection of Intervortex Venous Anastomosis on Ultra-Widefield Indocyanine Green Angiography Using a Two-Stage Deep Learning Framework.

Objectives: This study aimed to develop and validate a deep learning-based system for automated detection of intervortex venous anastomosis (IVA) on ultra-widefield indocyanine green angiography (UWF-ICGA). Methods: A total of 183 UWF-ICGA images with an original resolution of 3900 × 3072 pixels were included. A two-stage deep learning framework was developed. In the first stage, an object detection model automatically localized four predefined quadrants within each UWF-ICGA image. In the second stage, each extracted quadrant was classified according to the presence or absence of IVA. Quadrant

OCTA Vessel Segmentation via UNet Transformers and Deformable Receptive Fields: A Motion-Artefact-Resilient Deep Learning Framework.

Optical coherence tomography angiography (OCTA) is a non-invasive medical imaging technique that helps doctors see the blood vessels of the retina and choroid in fine detail. However, OCTA images often suffer from motion artefacts, which lower the accuracy of vessel segmentation in the retina. In this paper, we propose V2Vnet, a robust volume-to-volume deep learning framework that combines a UNet transformer with deformable receptive fields (DRF) to overcome this problem. The transformer encoder captures the overall structure of the input volume and gathers multi-scale information about retina

Oral eplerenone in first-episode acute central serous chorioretinopathy: results of a double-masked randomized trial.

BACKGROUND: Central serous chorioretinopathy (CSCR) is a pachychoroid disorder characterized by serous retinal detachment driven by choroidal vascular dysfunction and mineralocorticoid receptor (MR) overactivation. Eplerenone, a selective MR antagonist, has been proposed as a therapeutic option, although clinical evidence remains inconsistent. METHODS: In a randomized, double-blind, placebo-controlled trial, 44 patients with first-episode acute CSCR received either oral eplerenone (25 mg twice daily) or placebo for three months. Outcomes included best-corrected visual acuity (BCVA), central m

Choroid plexus remodeling associated with inflammatory CSF exposure in preterm acquired hydrocephalus.

Acquired hydrocephalus in preterm infants follows intraventricular hemorrhage (IVH) or central nervous system infection and is associated with inflammatory cerebrospinal fluid (CSF) profiles. We integrated clinical magnetic resonance imaging (MRI), CSF cytokine profiling, primary human choroid plexus (ChP) epithelial and endothelial cell assays, and independent transcriptomic and proteomic datasets to investigate ChP remodeling. Infants requiring CSF diversion exhibited greater ChP thickness independent of gestational age, ventricular size, and IVH severity, supporting its potential as a bioma

In Vivo Dynamic D2O-MRI for Quantifying Apparent CSF Water Renewal Across Ventricular and Extraventricular Compartments.

PURPOSE: The choroid plexus has long been regarded as the principal source of cerebrospinal fluid (CSF), yet this view remains unresolved. We developed a dynamic indirect D2O MRI approach to quantify regional apparent CSF water renewal in vivo in rats at 9.4 T. METHODS: Rats were scanned at 9.4 T using spin-echo echo-planar imaging during intravenous D2O infusion (10.69-s temporal resolution, 150 × 150 μm2 in-plane resolution). A deliberately long echo time (TE = 150 ms) was used to preferentially suppress signals from brain parenchyma, blood, and interstitial fluid, thereby enhancing sensiti

Optical coherence tomography angiography of the macular retina and choroid in glaucoma: a narrative review.

Glaucoma is a progressive optic neuropathy characterized by retinal ganglion cell degeneration, structural remodeling of the retina and choroid, and irreversible visual field loss. Increasing evidence indicates that microvascular dysfunction plays a crucial role in its onset and progression. Optical coherence tomography angiography (OCTA), as a noninvasive, high-resolution imaging technique, enables quantitative visualization of retinal and choroidal microcirculation, providing new insights into the structural-vascular-functional relationships in glaucoma. This narrative review summarizes rece

Monocyte-derived pro-angiogenic ocular macrophages identified by spatial transcriptomics are essential for choroidal neovascularization.

The human retina and underlying choroid have multiple well-organized vascular layers. However, retinal and choroidal neovascularization can develop in diseases such as diabetic retinopathy or neovascular age-related macular degeneration, which can result in destructive vision loss. Ocular macrophages are implicated in the pathogenesis of such neovascular diseases. To identify a potential pro-angiogenic macrophage subtype, we integrated 13 single-cell RNA sequencing datasets from human and mouse eyes comprising 173 unique samples. From 99,184 mononuclear phagocytes, we identified a single macro

Multifocal Posterior Pigment Epitheliopathy in a Pachychoroid Eye Following Ahmed Glaucoma Valve Implantation: A Case Report.

We report a case of multifocal posterior pigment epitheliopathy (MPPE), a bullous variant within the pachychoroid disease spectrum, following Ahmed Glaucoma Valve (AGV) implantation in a patient with exfoliation glaucoma (XFG). An 81-year-old man with XFG underwent AGV implantation for uncontrolled intraocular pressure (IOP) of 48 mmHg despite maximal medical therapy. Preoperative best-corrected visual acuity (BCVA) was 20/20. Preoperative optical coherence tomography (OCT) demonstrated dilated pachyvessels in Haller's layer with attenuation of the inner choroid, consistent with an underlying

Development of the eye in grass carp (Ctenopharyngodon idella): Ocular histomorphology, corneal surface ultrastructure, and retinal calretinin and tyrosine hydroxylase immunoreactivity.

Eye posthatching development was examined in grass carp (Ctenopharyngodon idella) from 1 day posthatching (1 dph) to 365 dph. Eye histology and morphometry, retinal expression of calretinin (CR) and tyrosine hydroxylase (TH), and corneal ultrastructure were studied using light and scanning electron microscopy. Basic eye structure was detectable at 1 dph, though important features, including retina and iris pigmentation as well as photoreceptor differentiation, were delayed till 4 dph. Differentiated photoreceptors seemed to emerge at 4 dph with evident increases in their count and shape divers

A spatiotemporal atlas of radiation-induced brain injury reveals dysfunction of the blood-CSF barrier.

Radiotherapy for brain tumors frequently induces irreversible cognitive decline, yet the underlying mechanisms remain poorly understood. Here, we present an integrated single-cell and spatial transcriptomic atlas of radiation-induced brain injury (RIBI) in mice, spanning the acute to chronic phases. We reveal that the early-delayed phase following irradiation represents a critical window during which the blood-cerebrospinal fluid barrier (BCSFB) integrity is compromised. This disruption results from the selective depletion of a radiosensitive Tmsb4x+ epithelial subpopulation in the choroid ple
