Biochemical Biomarker Combinations in Human Traumatic Spinal Cord Injury: An Exploratory Study.

Background/Objectives: Early diagnosis and prognosis after traumatic spinal cord injury (TSCI) remain challenging, and single biochemical biomarkers have shown limited stand-alone utility. Combining different biomarkers may improve prediction. We explored combinations of biomarkers in blood and cerebrospinal fluid (CSF) and their association with injury severity and neurological outcome after TSCI. Methods: In this post hoc analysis of a previously reported prospective cohort, blood and CSF were collected from 15 patients with TSCI at a median of 13 h after injury. Biomarkers comprised 13 infl

Overcoming Biological Barriers with Sononanomedicine: New Horizons in Oncology and Neurology.

Biological barriers, including the tumor microenvironment and the blood-brain barrier, represent the primary bottleneck for the clinical translation of nanomedicines in oncology and neurology. Ultrasound, characterized by noninvasiveness, deep tissue penetration, and high spatiotemporal precision, provides a transformative platform to address this challenge. This review systematically presents sononanomedicine as an integrated paradigm that couples acoustic physics with nanoplatform engineering to overcome these physiological barriers. We delineate the core physical mechanisms─cavitation, sono

Molecular mechanism of total flavonoids of hawthorn leaves in spinal cord injury treatment: A combined network pharmacology and experimental study.

BACKGROUND AND OBJECTIVE: Spinal cord injury (SCI) is a devastating neurological disorder characterized by irreversible motor and sensory dysfunction, largely driven by secondary injury mechanisms including oxidative stress, inflammation, and apoptosis. Total flavonoids of hawthorn leaves (TFHL), a traditional Chinese medicine, have demonstrated antioxidant and anti-inflammatory properties; however, their therapeutic effects and mechanisms in SCI remain unclear. METHODS: Network pharmacology was employed to identify the active compounds, potential targets, and key signaling pathways of TFHL i

Artificial Intelligence in Vascular and Endovascular Surgery: From Prediction to Surgeon-Led Clinical Implementation.

Artificial intelligence (AI) has shown promising performance in vascular imaging, risk prediction, procedural support, surveillance, and education, but its translation into routine surgical care remains limited. This narrative review addresses a focused question: what evidence supports the transition of vascular AI from technical prediction to surgeon-led clinical implementation? A targeted PubMed search through July 24, 2026 identified international and Japan-based studies across the perioperative pathway. Deep-learning systems can segment aortic anatomy, characterize aneurysm or dissection,

Electrosurgical septotomy across the full spectrum of aortic dissection.

OBJECTIVES: Transcatheter electrosurgical septotomy divides the dissection membrane to reconstitute a single aortic lumen. Reported experience is largely confined to chronic post-dissection aneurysms. We assessed the technique across the full spectrum of descending aortic dissection, including stand-alone treatment of malperfusion. METHODS: Retrospective intention-to-treat analysis of 21 consecutive patients scheduled for septotomy between June 2022 and May 2026. The membrane was crossed by needle re-entry and divided with an electrified through-and-through guidewire at 40-60 W under fluorosc

Pruritus-dominant herpes zoster with concurrent contralateral upper limb pain in a patient with complete spinal cord injury: A case report.

CONTEXT: Sensory impairment after spinal cord injury (SCI) may complicate herpes zoster (HZ) recognition. FINDINGS: A 76-year-old man with complete T4* paraplegia developed pruritus and vesicles in the right T4 dermatome without eruption-site pain. The asterisk denotes adjustment for stroke-related left upper-limb weakness. Empiric intravenous acyclovir preceded dermatology assessment. Contralateral shoulder and hand pain developed the day after eruption and was absent on the final treatment day. CONCLUSION/CLINICAL RELEVANCE: Focal pruritus warrants skin examination. When HZ is strongly sus

Primary isolated thoracic vertebral hydatid disease with multicompartment extension: A case report.

Vertebral hydatid disease is a rare manifestation of cystic echinococcosis, accounting for less than 1% of all cases, while primary isolated vertebral involvement is exceptionally uncommon. We report the case of a 33-year-old woman presenting with progressive spastic paraplegia. Magnetic resonance imaging revealed a multilobulated left thoracic costovertebral lesion centered at the T6-T7 level, composed of multicystic and enhancing solid components. The lesion involved the T6 and T7 vertebral bodies and posterior elements, with extensive intraspinal, pleural, and paravertebral soft-tissue exte

Clinically unresponsive patients with acute brain injury activate a brain-computer interface.

Brain activation to verbal commands can be detected by EEG in clinically unresponsive patients. These patients with cognitive motor dissociation (CMD) are unable to communicate with the environment despite preserved consciousness. Advanced brain-computer interfaces (BCI) are promising to support motor function in patients with motor neuron disease or spinal cord injury. Our primary objective was to determine if clinically unresponsive patients can control a self-paced, one-way BCI early after acute brain injury. Secondarily, we assessed the association of BCI control with short- and long-term

Microvasculopathy in spinal muscular atrophy: The endothelium as a target of SMN deficiency and an amplifier of disease.

Spinal muscular atrophy (SMA) is caused by deficiency of the ubiquitous survival motor neuron (SMN) protein. Degeneration of anterior horn motor neurons defines the disease, but many organs are affected, including heart, liver, kidney, and bone. The vascular compartment is among them: digital necrosis in infants with the fewest SMN2 copies, capillary rarefaction in SMA mice, and decreased von Willebrand factor with fibrinogen extravasation in human post-mortem spinal cord. Whether these reflect endothelial injury by SMN deficiency itself or capillary adaptation to atrophic, denervated tissue h

Dynamic force sensing by bone delays aging and extends lifespan.

Physical activity promotes health primarily by decelerating the aging process. Bone is one of the tissues capable of sensing mechanical force and can adapt to different physical demands. However, its role in whole-body aging is unknown. We found that reduced mechano-sensing in osteocytes, caused by deletion of the mechanosensory channel, Piezo1, accelerated whole-body aging in mice. Dynamic tibial axial loading (DTAL) in mice at 20-24 months of age extended both healthspan and lifespan (9.4%/79.5 days). Furthermore, DTAL reversed bone marrow immunosenescence and induced anti-aging and protecti

Dynamic Conformal Neural Electroactive Biointerfacing Sustains Bioelectronic Coupling for Spinal Cord Repair.

Stable bioelectronic coupling in mechanically active tissues remains challenging because conventional interfaces rely on static architectures that cannot dissipate deformation-induced interfacial stress, resulting in progressive contact failure and signal instability. Here, we report an injectable magnetoelectric hydrogel that forms a dynamic conformal neural electroactive tissue-biointerface (DyCONNECT) with deformable neural tissues. By integrating polydopamine-modified magnetoelectric nanoparticles within a dynamically adaptive conductive network, DyCONNECT combines tissue adhesion, electri

Development of Enterovirus D68 3C Protease Inhibitors with In Vivo Efficacy in a Neurological Infection Model.

Enterovirus D68 (EV-D68) is a non-polio enterovirus that primarily causes respiratory illness but can also lead to severe disease, including acute flaccid myelitis. No approved vaccine or antiviral is available. Here, we report the rational design and structure-property optimization of dipeptidyl EV-D68 3C protease (3Cpro) inhibitors. Structure-guided optimization and prodrug modification identified Jun13133 as a potent antiviral active against multiple contemporary EV-D68 isolates and EV-A71, with high selectivity. Computational modeling supported a stable substrate-mimetic binding mode consi

Precision Nanomedicine for Traumatic Neural Injury: Functional Paradigms and Translational Frameworks.

Traumatic neural injuries, including traumatic brain injury (TBI), spinal cord injury (SCI), and peripheral nerve injury (PNI), impose a substantial global burden. These injuries converge on oxidative stress, neuroinflammation, regulated cell death, demyelination, and extracellular matrix remodeling, while diverging in timing, cellular effectors, and regenerative capacity. Current management still fails to arrest secondary injury or restore functional neural circuitry. Nanomedicine integrates targeted delivery, microenvironmental regulation, and regenerative support, yet existing strategies an

Therapeutic role of cell signalling pathways and immunomodulatory approaches for recovery of spinal cord injury in dogs and cats.

Spinal cord injury (SCI) can lead to paralysis and other disorders in domestic animals, compromising welfare due to limited treatment techniques and their side effects. Modulating cell signalling pathways, such as the Wnt, BDNF (brain-derived neurotrophic factor)-TrkB and JAK/STAT pathways, plays a potential role in more rapid recovery of spinal cord trauma with fewer side effects. The Wnt signalling pathway plays a critical role in neuronal regeneration and reduction of neuronal apoptosis. The BDNF-TrkB pathway stimulates recovery of spinal cord trauma by modulating synaptic plasticity and re
