Can bone SPECT/CT and 18 F-NaF PET/CT diagnose the cause of persistent pain due to motion sparing devices: a case series.

PURPOSE: Can positron emission tomography/CT (18 F-NaF PET/CT) and Bone single-photon emission CT (SPECT/CT) be used to diagnose the cause of persistent pain in motion-sparing devices? Interspinous devices (ISD) and cervical disc arthroplasty (CDA) are used increasingly. While a desired outcome is achieved in a portion of these patients, others develop persistent postoperative symptoms. Etiology of these symptoms is diagnosed with imaging such as x-ray, computed tomography (CT), or magnetic resonance imaging (MRI). Despite these modalities, there is a subset of patients with persistent pain wh

Automated quantification of cervical spine degeneration with development of a segmentation framework based on probabilistic anatomical cognition.

Objective.Ossification of the posterior longitudinal ligament (OPLL) is a prevalent cervical spine degeneration disease leading to significant spinal cord dysfunctions. Due to morphological diversity and data scarcity, traditional OPLL assessment relies on manual measurements, which suffer from low consistency and high cost. To implement automated quantification of the OPLL, a cognition-inspired segmentation framework, named the probabilistic anatomical cognition (PAC) framework, is proposed to encode physicians' anatomical knowledge of the OPLL and mimic their hierarchical logic of inferring

Polydatin for treating spinal cord injury: Multiple mechanisms and challenges.

Spinal cord injury (SCI) is a serious neurological system disease. After SCI, a series of cascade reactions can cause irreversible damage, with a high disability rate and mortality rate. The complexity of the pathological mechanism of SCI limits the efficacy of various traditional therapies, and it is urgent to find new therapeutic means. In recent years, the method of extracting effective components from natural Chinese herbs for treating diseases has attracted widespread attention. Polydatin (PD) is an active ingredient extracted from Polygonum cuspidatum and its structure is similar to the

Retrospective analysis of stage IV pressure injuries managed without operative closure in veterans admitted to a spinal cord injuries/disorders unit.

OBJECTIVE: Patients with spinal cord injuries/disorders (SCI/D) have one of the highest risks of developing pressure injuries, but surgical closure of these wounds has significant pre-operative and post-operative requirements with a high rate of complications. Limited data exists on the outcomes of stage IV pressure injuries that are not managed with surgical closure. Our objective was to review outcomes of patients with stage IV pressure injuries that close by secondary intention. DESIGN: Retrospective case series review Setting: Veterans Affairs hospital. METHODS: Records of veterans hospi

Comparison Between Surgical and Percutaneous Paddles in Spinal Cord Stimulation for Chronic Neuropathic Pain.

Background: Spinal cord stimulation (SCS) is a well-established treatment for chronic neuropathic pain, offering a safe procedure with low complication rates. Both surgical and percutaneous leads can be effective, with similar complication rates. Methods: We analyzed all patients implanted at a reference center since 1996 to compare pain control and complications and determine whether one system was more effective than the other in patients who had experienced both systems. A retrospective observational study was designed. Results: A total of 188 SCS systems were implanted, with a follow-up pe

Targeting kynurenine pathway and A1 /A2 astrocytes polarization in experimentally induced fibromyalgia: Modulatory role of apigenin on kynurenine/ aryl hydrocarbon receptor signaling.

Astrogliosis is thought to be a potential factor in the neuroinflammatory response associated with fibromyalgia (FM); however, the role of A1/A2 astrocyte polarization remains underexplored. Several metabolic processes have been reported to influence astrocyte activation and function. The current study was designed to assess the potential regulatory role of apigenin (API), a natural flavonoid, on astrocytes polarization via modulation of the kynurenine pathway (KP) in rats with FM-like symptoms. Reserpine (Res) (1 mg/kg/day, s.c.) was injected into the rats for three consecutive days to induce

Guillain-Barré Syndrome With Asymmetrical Weakness in a Patient With Charcot-Marie-Tooth Disease Type 1A.

A man in his 60s presented with acute, progressive right lower limb weakness, low back pain, and paraesthesia. Neuroimaging ruled out spinal cord and central pathology. Cerebrospinal fluid analysis revealed albumin-cytological dissociation, and nerve conduction studies confirmed Guillain-Barré syndrome (GBS) superimposed on Charcot-Marie-Tooth disease type 1A (CMT1A). There was no preceding infection or ganglioside antibody positivity. His weakness progressed rapidly, resulting in flaccid paralysis of the right leg. He received intravenous immunoglobulin with marked clinical improvement and re

[Analysis of Plastic Changes in Spinal Neurons in Fibromyalgia and the Exploration of Therapeutic Agents].

Fibromyalgia (FM) is a chronic disorder characterized by widespread pain and various accompanying symptoms, with complex mechanisms involving both peripheral and central nervous systems that remain unclear. Herein, we introduce a study employing a reserpine-induced rat model of FM, investigating the spinal dorsal horn in particular. In reserpine-induced FM model rats, we observed pronounced mechanical hypersensitivity of cutaneous and muscular C-fiber nociceptors. Moreover, increased expression of the ASIC3 (one of acid-sensing ion channel) in dorsal root ganglia implicated its role in periphe

Case Report: Scalpel Sign and Dorsal Arachnoid Cyst-The Importance of an Accurate Diagnosis.

Background and Clinical Significance: Thoracic dorsal arachnoid web (DAW) is a rare intradural extramedullary condition characterized by a thin band of arachnoid tissue compressing the dorsal spinal cord. A hallmark imaging feature is the "scalpel sign", which refers to anterior displacement of the thoracic spinal cord with dorsal cerebrospinal fluid (CSF) accumulation, producing a sagittal profile resembling a surgical scalpel. Although highly specific for DAW, this sign may also appear in other intradural conditions such as idiopathic ventral spinal cord herniation and arachnoid cysts. The c

The Programme for Manufacture of hPSC‑Based Products for AMD and PD.

Pluripotent stem cells (hPSCs) possess the unique ability to self-renew and differentiate into various functional cell types, positioning them as key "seed cells" in regenerative medicine. The development of PSC-based therapies offers new hope for treating complex diseases, and degenerative diseases, particularly those caused by the loss of specific cellular functions, such as macular degeneration, Parkinson's disease, spinal cord injury, diabetes, and cartilage damage. Cell replacement therapies using hPSC-committed cells can help halt disease progression or potentially cure these conditions.

Motion-Style Scalp Acupuncture Ameliorates Post-Stroke Muscle Spasticity Through Cerebral Blood Flow Augmentation and 5-HT2AR-Mediated Spinal KCC2 Reactivation.

BACKGROUND: Muscle spasticity, a prevalent and debilitating motor impairment after stroke, severely compromises patients' functional recovery and quality of life. Motion-style scalp acupuncture (MSSA), a novel integrative therapy combining scalp acupuncture (SA) with targeted exercise training, has emerged as a promising strategy for post-stroke spasticity (PSS). This study investigated the mechanisms underlying MSSA's therapeutic effects, focusing on 5-HT2AR-mediated KCC2 reactivation in spinal motoneurons. METHODS: Using a rat model of permanent middle cerebral artery (MCA) occlusion, we ev

The effects of mechanical stimulation on the morphology and function of spinal ligament cells.

BACKGROUND: It has been shown that mechanical signaling in the cellular microenvironment is crucial for control at the cellular and tissue levels, affecting human growth and the prevalence of disease. Ligament cells from individuals with posterior longitudinal ligament ossification (OPLL) have been found to respond to external mechanical stimulation. However, the response of mechanical stimulation to OPLL ligament cells is unclear. This study aims to comprehensively assess the effects of mechanical stimulation on the proliferation, viability, shape and function of OPLL ligament cells. METHODS

Nanoparticle and epothilone D combinatorial intervention improves motor performance and regeneration in chronic cervical spinal cord injury.

Spinal cord injury (SCI) causes the loss of motor function below the site of injury due to neuron loss and the severing of spinal tracts. The injury leads to the recruitment of circulating myeloid cells that create an inflammatory microenvironment and exacerbate cell death, with subsequent migration of fibroblasts and astrocytes that contribute to scar tissue that inhibits regeneration. Herein, we investigated a combinatorial treatment in a chronic cervical hemisection model involving cargo-less nanoparticles (NPs) administered acutely, and a multichannel bridge and microtubule stabilizer deli

Functional Recovery by Transplantation of Human iPSC-Derived A2B5 Positive Neural Progenitor Cell After Spinal Cord Injury in Mice.

Human induced pluripotent stem cells (hiPSCs) hold great potential for patient-specific therapies. Transplantation of hiPSC-derived neural progenitor cells (NPCs) is a promising reparative strategy for spinal cord injury (SCI), but clinical translation requires efficient differentiation into desired neural lineages and purification before transplantation. Here, differentiated hiPSCs-reprogrammed from human skin fibroblasts using Sendai virus-mediated expression of OCT4, SOX2, KLF4, and C-MYC-into neural rosettes expressing SOX1 and PAX6, followed by neuronal precursors (β-tubulin III+/NESTIN+)

Basic fibroblast growth factor sustained-release system promotes neurogenesis and tissue repair after spinal cord injury.

Spinal cord injury is accompanied by a substantial loss of neurons. Cell replacement therapy improves motor and sensory dysfunction by replacing dead neurons, and endogenous neurogenesis is an important cell replacement source. Stimulating endogenous neurogenesis is therefore a viable approach for treating spinal cord injury. Given that basic fibroblast growth factor is a potent inducer of neurogenesis, we developed a sustained-release system of basic fibroblast growth factor-chitosan to enhance tissue repair in spinal cord injury. In the present study, we isolated neural stem cells from the s
