Case Report: Beyond chronicity: sustained multidimensional response to spinal cord stimulation three decades after traumatic spinal cord injury.

INTRODUCTION: Spinal cord stimulation (SCS) is an established treatment for refractory neuropathic pain conditions, yet evidence for its effectiveness in spinal cord injury (SCI)-related neuropathic pain, particularly after decades of persistent symptoms, remains limited. It remains unclear whether unusually prolonged pain chronicity and established neurological deficits affect the potential for a meaningful response to neuromodulation. CASE PRESENTATION: We report a 60-year-old man with chronic incomplete SCI, bilateral spastic paraparesis markedly more pronounced on the left, and severe chr

Mitochondrial electron transport chain activity is not essential for the acute hypoxic response of spinal oxygen sensors.

Spinal sympathetic preganglionic neurons (SSPNs) are oxygen sensitive. These spinal oxygen sensors (SOS) increase sympathetic output when subjected to acute hypoxia, orchestrating cardiorespiratory responses to protect against the deleterious effects of oxygen deprivation. Most other oxygen-sensing systems rely on mitochondrial mechanisms to detect and signal decreases in oxygen. Here, we apply pharmacological blockers of the mitochondrial respiratory chain and a mitochondrial-targeted antioxidant to a rat perfused in situ spinal cord preparation to test the role of mitochondria in the SOS. We

Cost-effectiveness of cladribine tablets for the treatment of active relapsing-remitting multiple sclerosis in the United Kingdom.

OBJECTIVE: The approved indication of cladribine tablets has recently been expanded in the United Kingdom for the treatment of all adult patients with active relapsing multiple sclerosis. This analysis assessed the cost-effectiveness of cladribine tablets from the perspective of the National Health Service in patients with active relapsing-remitting multiple sclerosis (RRMS). METHODS: A cohort-based Markov model with 11 health states (10 disability states plus death) was developed. In addition to best supportive care without disease-modifying therapy (DMT), 12 recommended DMTs were included a

Early catastrophic posterior migration of a cervical disc replacement polyethylene insert resulting in complete spinal cord injury: illustrative case.

BACKGROUND: Posterior migration of a mobile-bearing cervical disc replacement (CDR) can be catastrophic. The authors report a single-patient case of catastrophic failure after recent trauma and implant loosening. OBSERVATIONS: A 48-year-old woman had received C5-6 and C6-7 Mobi-C CDRs at an outside institution 3 weeks before presentation and had sustained a ground-level fall 8 days before presentation. Outside imaging from after the fall showed implant loosening. After subsequently lifting a laundry basket, the patient developed acute quadriparesis with complete motor and sensory loss below C

Gαi2 promotes microtubule-driven incorporation of neurofilaments to stabilise nascent axons in vivo.

Axon initiation is a critical step in the establishment of neuronal morphology. However, the mechanisms regulating stable axon initiation during embryonic development remain poorly understood. Here, using fixed-tissue super-resolution and high-resolution live-tissue imaging in the developing chick spinal cord, we reveal that stable axon initiation is characterised by microtubule-directed neurofilament accumulation in the emerging axon, and identify the multifunctional G-protein Gαi2 as a regulator of this process. Before axon initiation, microtubule network orientation shifts towards the site

Ratiometric Fluorescence Sensing of Human/Bovine Serum Albumin Using In Situ Formed Fe3+ Complex.

Oligo[poly(ethylene glycol) fumarate] (OPF) is an efficient gel-forming macromer for challenging tissue engineering applications, including repair of the spinal cord (SC) after complete lesion. Existing OPF-derived SC-bridging hydrogels are being applied with cellular products and/or electrical stimulation to promote regeneration. A novel biologics-free composite OPF hydrogel was designed as a ready-to-use neuroactive scaffold with defined (bio-)chemical, porous structure, and multi-channel system to support cellular/axonal growth. It was constructed as a highly permeable macroporous cryogel a

Chronic post-traumatic stress disorder-related behavioral traits develop in female rats exposed to repetitive low-level blast injury and are reversible with (2R,6R)-hydroxynorketamine.

INTRODUCTION: Many military Veterans who experienced blast-related injuries during military service suffer from chronic cognitive and mental health problems. Women constitute more than 15% of active-duty U. S. military personnel and are the fastest-growing population of U. S. Veterans. For more than a decade, we have studied a rat model of repetitive low-level blast exposure designed to mimic the type of blast-related exposure that is common in military settings. Male rats develop chronic cognitive and post-traumatic stress disorder-related traits that appear in a delayed fashion and persist f

From trauma capacity to neurotrauma capacity: A scoping review of existing assessment frameworks.

INTRODUCTION: Neurotrauma, comprising traumatic brain injury (TBI) and traumatic spinal cord injury (tSCI), remains a major global health issue, with low- and middle-income countries (LMICs) bearing a disproportionate burden. Effective neurotrauma care requires timely and coordinated access to prehospital care, imaging, surgery, neurocritical care, and rehabilitation, yet current assessment frameworks often fail to capture these interdependent requirements. RESEARCH QUESTION: What instruments are currently available to evaluate trauma and neurosurgical capacity across diverse global settings,

Basilar Artery Occlusion Associated With Paradoxical Embolism After Traumatic Spinal Cord Injury: A Case Report.

Basilar artery occlusion is a neurological emergency associated with high mortality and severe disability. In young patients without conventional vascular risk factors, identifying an uncommon embolic mechanism is essential for secondary stroke prevention. We report the case of a previously healthy 21-year-old man who sustained a 6-meter fall resulting in T9-T10 compression fractures with spinal canal compromise and complete traumatic spinal cord injury. He underwent posterior thoracic spinal instrumentation. During hospitalization, he developed sudden neurological deterioration characterized

Clinical phenotypes and outcomes of spinal cord stimulation in painful peripheral polyneuropathy: a cohort including diabetic, chemotherapy-induced, and idiopathic etiologies.

BACKGROUND AND OBJECTIVE: Chronic painful peripheral polyneuropathy (PPN) is a leading global cause of chronic pain and may arise from a wide range of etiologies. When pharmacological treatments fail to provide adequate pain relief, spinal cord stimulation (SCS) has demonstrated efficacy in selected subtypes. This study investigated the effectiveness of SCS in alleviating pain, neurological symptoms and signs, and other health-related outcomes in patients with diverse forms of PPN, including diabetic (DPN) and chemotherapy-induced (CIPN) peripheral neuropathies, and chronic idiopathic axonal p

Neurosurgical manifestations and management of mucopolysaccharidoses: A systematic review.

Mucopolysaccharidoses (MPS) represent a group of rare inherited lysosomal storage disorders, of which neurosurgical issues arise during childhood and adolescence. The literature however remains fragmented in terms of these specific outcomes. Correspondingly, we sought to synthesize a primer for neurosurgeons in regard to MPS, summarizing the neurosurgical manifestations and management reported in the contemporary literature. A systematic interrogation of the literature was performed according to PRISMA guidelines. Electronic databases were searched from inception through June 2026. Studies rep

Aging-enhanced accumulation of fibroblasts excludes oligodendrocytes in demyelinated lesions.

Fibroblast dysregulation contributes to pathological fibrosis and aberrant repair. Emerging evidence suggest that fibroblasts accumulate in lesions following central nervous system injury, but whether and how they influence oligodendrocyte repair responses, including in aging, is uncertain. Here we report that fibroblasts accumulate in the parenchyma of spinal cord white matter lesions of 6 - 10 week old young mice after lysolecithin-induced demyelination. This was first observed through immunofluorescence microscopy that employed several markers attributed to fibroblasts, including platelet-d

The role of vascular pathology in SMA: implications for pathogenesis and treatment.

Spinal muscular atrophy (SMA) has become a paradigm for precision medicine following the development of transformative therapies targeting the disease-causing deficiency of survival motor neuron (SMN) protein. As more than 98% of SMA cases arise from mutations in the SMN1 gene, restoration of SMN expression through nusinersen (Spinraza; Biogen, Cambridge, MA), risdiplam (Evrysdi; Roche, Basel, Switzerland) and onasemnogene abeparvovec (Zolgensma and Itvisma; AveXis, Novartis, Chicago, IL) has produced unprecedented clinical benefits and established SMA as the first treatable motor neuron disea

Zinc Hexacyanoferrate Nanoenzymes Attenuate Neuroinflammation After Spinal Cord Injury by Modulating Mitochondrial Function and Microglial Polarization.

BACKGROUND: Secondary pathology after acute spinal cord injury (SCI) is driven by pathological iron accumulation, reactive oxygen species (ROS) overproduction, and mitochondrial failure, which together sustain chronic neuroinflammation. Methylprednisolone (MP), the most widely used ROS-scavenging agent for acute SCI, provides limited clinical benefit and carries considerable side effects. To address this, we engineered zinc hexacyanoferrate nanoenzymes (ZnHPB) that chelate excess ferrous ions while displaying intrinsic SOD- and CAT-like activities, attenuating oxidative injury at its source ra

WHEEL-LEARN: Evaluating the usability, feasibility, and proof of concept of an internet-based healthy lifestyle behavior change intervention for people with spinal cord injury.

BACKGROUND: Individuals with spinal cord injury (SCI) are less physically active compared to the general population and face a higher risk of cardiometabolic diseases, some of which are modifiable through physical activity (PA). Personal and environmental barriers prevent people with SCI from meeting PA recommendations. WHEEL-LEARN, a 4-week virtual behavior change program, was developed to reduce PA barriers. OBJECTIVE: To evaluate the usability, acceptability, feasibility, and proof of concept of WHEEL-LEARN in people with SCI. METHODS: A pre-/postintervention study was conducted using mix
