Preoperative risk stratification of discharge disposition using the RAPT score in brain tumor surgery.

PURPOSE: Discharge planning following brain tumor resection is often variable, contributing to delays in care coordination after discharge. While structured discharge prediction tools are widely used in orthopedic and spine surgery, no standardized preoperative framework exists in neurosurgical oncology. We conducted an exploratory analysis of the utility of the Risk Assessment and Prediction Tool (RAPT) for preoperative discharge stratification in supratentorial brain tumor resection patients. METHODS: We performed a retrospective cohort study of adults undergoing supratentorial craniotomy f

Reconnecting the disconnected forebrain: multimodal resting-state bold dynamics underlying zolpidem-induced arousal in incomplete Locked-in syndrome.

PRIMARY OBJECTIVE: Zolpidem paradoxically restores behavioral responsiveness and communication in select patients with severe disorders of consciousness and profound motor deafferentation. While the anterior forebrain mesocircuit hypothesis attributes this awakening to central thalamic disinhibition, macroscopic network dynamics and lower brainstem gating remain under-characterized in locked-in syndrome (LIS). METHODS AND PROCEDURES: A 45-year-old male with ventral pontine necrosis initially misdiagnosed as vegetative state/unresponsive wakefulness syndrome (VS/UWS) was formally identified wi

Early plasma cell-free DNA diagnosis of fatal Acanthamoeba granulomatous amebic encephalitis in a liver transplant recipient.

BACKGROUND: Acanthamoeba granulomatous amebic encephalitis (GAE) is a rare, highly lethal opportunistic infection in immunocompromised hosts. Diagnosis is difficult because clinical and radiographic findings are nonspecific and conventional cerebrospinal fluid (CSF) studies may be nondiagnostic. CASE: We report a 68-year-old woman who developed fever, seizures, and progressive encephalopathy 13 days after orthotopic liver transplantation. Brain MRI demonstrated multifocal supratentorial and infratentorial enhancing lesions with vasogenic edema and peripheral diffusion restriction. Initial CSF

Intracranial Mesenchymal Tumor with FET-CREB Gene Fusion: A Case Report of Late Recurrence.

Intracranial mesenchymal tumors with FET‑CREB fusion represent a rare tumor entity newly defined in the 5th edition of the WHO classification of central nervous system tumors. Due to overlapping morphological features, these neoplasms were previously diagnosed under different names, including intracranial angiomatoid fibrous histiocytoma and intracranial myxoid mesenchymal tumor. Clinically, they need to be differentiated from meningioma, high‑grade glioma, solitary fibrous tumor, and other intracranial neoplasms. Most supratentorial lesions achieve good outcomes after gross total resection, a

Functional Mapping-Guided Resection of Eloquent Diffuse Low-Grade Gliomas: A Single-Institution Experience from a Low-Middle Income Country.

AIM: To report our experience in implementing functional mapping-guided resection for eloquent diffuse low-grade gliomas (DLGGs) in a low- and middle-income country (LMIC) setting. There are logistical barriers to achieving onco-functional balance through glioma surgery in LMICs. MATERIAL and METHODS: The study is a retrospective chart review of adult patients who underwent surgical resection of their DLGGs that were located within supratentorial eloquent brain areas (within 15 mm). Functional mapping-guided surgery was performed using electrophysiological brain mapping techniques, namely intr

Langerhans cell sarcoma in the central nervous system mimicking a meningioma: case report and literature review.

Langerhans cell sarcoma (LCS) and Langerhans cell histiocytosis (LCH) are proliferative conditions involving cells that exhibit Langerhans cell features. While LCH can have a variable behavior ranging from benign hyperplasia to a fatal multisystemic disease, LCS represents a rare, aggressive, high-grade malignancy. Central Nervous System (CNS) involvement is rare, and extra-axial lesions can frequently mimic meningiomas or carcinomatous infiltrates, posing a significant diagnostic challenge. We report the case of a 31-year-old male who presented with severe holocranial headache, projectile vom

SurgiCorr: A Physics-Based Computational Framework for Patient-Specific Surgical Corridor Planning in Brain Tumor Surgery.

BACKGROUND AND OBJECTIVES: Transcortical corridor selection is guided by anatomic intuition and neuronavigation, with no quantitative tool to estimate retraction injury to brain tissue. Such injury depends on corridor-specific tissue properties, opening width, and hold duration, variables existing systems do not model. We present SurgiCorr, a preoperative framework that generates patient-specific corridor candidates, simulates each biomechanically, and ranks them by a composite risk score (CRS) integrating strain, viscoelastic dose, imaging burden, and ventricular proximity. METHODS: MRI volu

Role of Multimodal Use of Surgery Adjuncts for Overall Survival of Patients with Supratentorial WHO Grade 4 Gliomas.

BACKGROUND: In recent years, numerous advanced techniques supporting surgery (TSS) have been utilized to maximize glioma resection. However, the impact of the multimodal use of TSS, such as awake surgery, intraoperative neurophysiological monitoring, neuronavigation, fluorescence-guided surgery, functional magnetic resonance imaging m(fMRI), and diffusion tensor tractography (DTT), on improving overall survival (OS) remains uncertain. METHODS: In retrospective analysis of 81 patients with supratentorial WHO grade 4 gliomas, we assessed the effect of age, sex, tumor location, number of TSS use

Minimal invasive surgery for primary brainstem intracerebral hemorrhage: a critical mini review.

CONTEXT: Intracerebral hemorrhage (ICH) accounts for half of stroke attributable deaths and two thirds of DALYs loss. Past well designed trials (2005 STICH, 2013 STICH 2, 2019 MISTIE 3, 2024 ENRICH, 2024 SWITCH) did not demonstrate benefits of surgery, aside from ENRICH which suggested improved functional outcome thanks to minimal invasive surgery for lobar ICH, but not for deep seated thalamic ICH. Since 2020, retrospective studies suggest great functional benefit of minimal invasive surgery for brainstem ICH. MATERIALS AND METHODS: Review of weel designed trials and guidelines regarding sur

Lupus anticoagulant testing before and after direct oral anticoagulant removal in patients suspected of antiphospholipid syndrome: a real-life experience.

INTRODUCTION: Edema is a significant cause of neuromorbidity in children and adults with brain tumors. Agents used to control this effect, such as corticosteroids, have their own associated morbidities. Sulfonylurea receptor 1 (SUR1) is a transmembrane protein that regulates the activity of ion channels in neurons, glia, and endothelial cells. SUR1 expression is upregulated in neuroinflammatory conditions. Inhibition of SUR1 with glyburide decreases edema and neuroinflammation by countering cytotoxic edema and apoptosis in rodent models of subarachnoid hemorrhage, stroke, trauma, and cerebral

Developing an advanced risk stratification model for pediatric intracranial ependymoma based on the prospective trial E-HIT2000 and subsequent registries.

ABSTRACT: BackgroundCurrent treatment strategies for pediatric intracranial ependymoma do not consider molecular heterogeneity. Here, we evaluated molecular group-specific determinants of outcome and developed an improved risk stratification model. METHODS: Patients aged 0-21 years with localized intracranial ependymoma were enrolled into the prospective clinical trial E-HIT2000. Treatment included maximum safe surgery, local radiotherapy, and chemotherapy, stratified according to age, histology and, following a major amendment, residual tumor. Clinical data were analyzed in a pooled molecula

Integrated Single-Nucleus and Spatial Transcriptomics Elucidate Heterogeneity and Hypoxia-Driven Organization of Supratentorial Ependymoma.

UNLABELLED: Supratentorial ependymomas (ST-EPN) are often life-threatening brain tumors that are characterized by significant molecular heterogeneity. Hypoxia contributes to this heterogeneity, but the extent and nature of this relationship remain unclear. This poses challenges for effective treatment strategies, demanding a more holistic and detailed understanding of the underlying biology. In this study, we employed single-nucleus (n = 63) and spatial (n = 30) transcriptomics to delineate the cellular and spatial landscape of ST-EPN. Characterizing the transcriptional consensus programs of t

Cerebral Proliferative Angiopathy: Imaging Spectrum and Diagnostic Insights.

Cerebral proliferative angiopathy (CPA) is a rare cerebrovascular malformation that differs fundamentally from classic arteriovenous malformations (AVMs). Unlike AVMs, CPA is characterized by a diffuse vascular network intermingled with normal brain parenchyma, absence of a compact nidus or dominant feeders, and transdural supply. We conducted a comprehensive review of 75 published studies comprising 153 patients to summarize the full imaging spectrum. CPA most commonly presents in adolescence or young adulthood, with a slight female predominance, and generally involves the supratentorial brai
