Klumpke's Palsy Following Minor Trauma: A Rare Presentation in a Young Adult.

The brachial plexus is a complex network that transmits motor and sensory signals between the spinal cord and the upper limbs. Klumpke's palsy, a rare injury affecting the lower brachial plexus (C8-T1 roots), commonly occurs in newborns or adults after severe trauma and is characterized by a claw-hand deformity, sensory loss along the medial forearm and hand and weakness in the small muscles of the hand. Adult-onset Klumpke's palsy following minor trauma is exceptionally rare. We report a unique case of isolated right-sided Klumpke's palsy in a 31-year-old male after a trivial fall from bed. N

Tasting pain-relief through tongue stimulation: results from a feasibility study looking into the use of cranial nerve non-invasive neuromodulation in healthy individuals and chronic poststroke patients.

PURPOSE: Chronic pain is a common consequence of stroke, often impairing survivors' quality of life. By targeting cranial nerves and brainstem regions involved in pain modulation, cranial nerve non-invasive neuromodulation (CN-NINM) offers a promising approach to pain relief. The aims of this study were to investigate the feasibility of using CN-NINM in people with chronic stroke and in healthy individuals, and to test the immediate effect of this modality on experimental pain. PARTICIPANTS AND METHODS: Thirteen adults with chronic stroke and 13 age-matched healthy adults were recruited for a

Identification of low threshold off-target activation pathways during stimulation of carotid baroreceptor afferents in swine.

OBJECTIVE: Electrical stimulation of the baroreceptors pathways at the carotid sinus bulb - known as baroreflex activation therapy (BAT) - is intended to change autonomic tone and ultimately reduce blood pressure and heart rate. BAT is pre-market approved by the United States Food and Drug Administration (FDA) for the treatment of heart failure and received an FDA humanitarian device exemption for drug resistant hypertension. However, responder rates are limited by side-effects including numbness in the head and neck, altered speech, respiratory constriction, dry cough, vomiting, and altered s

Usefulness of a 3 Dimensional-printed Head Model with Two-layered Dura and Cerebellar Tentorium for Transpetrosal Approach Training.

The combined transpetrosal approach provides effective exposure to the petroclival region and ventral brainstem, although its technical demands necessitate advanced neurosurgical expertise. Proficiency in skull base drilling and meticulous separation of the dual dural layers is crucial. Recently, three-dimensional printing has gained attention as a tool for surgical training. To evaluate its potential in transpetrosal approach education, we developed a three-dimensional head model that reproduces the soft brain tissue, cranial nerves, vasculature, and tumors. A petrotentorial meningioma case w

Functional Improvement in Cranial Neuropathies From Perineural Cutaneous Squamous Cell Carcinoma After Immunotherapy: A Multicenter Case Series and Review.

PURPOSE: Cutaneous squamous cell carcinoma has a propensity for perineural invasion. Treating perineural spread has involved surgery and adjuvant chemoradiation, and more recently, immune checkpoint inhibitors. This study aims to evaluate motor and sensory functional outcomes in patients with cranial neuropathies from perineural spread of squamous cell carcinoma, particularly those undergoing immunotherapy. METHODS: This was a multicenter case series and literature review. RESULTS: Eighteen patients with cranial neuropathies from perineural spread had a mean age of 72.5 ± 9.7 years. Cranial

Clinical characteristics and risk factors of delayed ophthalmoplegia following cavernous sinus exploration in endoscopic pituitary adenoma surgery : Delayed transient ophthalmoplegia.

PURPOSE: Postoperative cranial nerve dysfunction remains a significant concern in endoscopic endonasal surgery for pituitary neuroendocrine tumors (PitNETs, also known as pituitary adenomas) that have invaded the cavernous sinus, but the incidence, timing, and recovery patterns vary widely across reports. This study aimed to characterize the clinical features, risk factors, and recovery patterns of patients with postoperative ophthalmoplegia following cavernous sinus exploration. METHODS: We retrospectively analyzed 127 consecutive patients who underwent endoscopic skull base surgery with cav

[An autopsy case of glioblastoma with diffuse leptomeningeal spread that was difficult to diagnose before death].

The patient was a male in his 50s. He had been aware of headache since the end of April, X year, and was admitted to the Neurosurgery Department of X Hospital in early May; however, his headache and posterior neck pain worsened. He was discharged from the previous hospital and admitted to this hospital in late May. His consciousness was clear, and there were no abnormalities in his cranial nerves or limb motor systems. Tendon reflexes were normal and Babinski's sign was negative. He complained of posterior neck pain and remained in bed all day with his neck flexed backward. Head MRI showed a h

Combined lateral and ventral approach for 360° decompression of an intraosseous suprasellar meningioma encasing the optic nerve.

Suprasellar meningiomas present operative challenges because of their involvement with cranial nerves and neurovascular structures. The optimal approach for suprasellar meningiomas encasing the optic nerve remains debated. This video presents a patient with profound vision loss limited to hand motion, arising from a calcified suprasellar meningioma with 360° optic nerve encasement, which was resected via an anterolateral approach with extradural anterior clinoidectomy followed by an endoscopic endonasal approach. Postoperatively, vision improved to 20/30 at 2 months with no recurrence at 2 yea

High-resolution T2-weighted MRI versus tractography to predict cranial nerve anatomy in skull base tumors: a retrospective 132-case series.

OBJECTIVE: Skull base tumors are difficult to approach because of their deep location and their entrapment by cranial nerves (CNs), arteries, and veins. CN trajectories can be challenging to describe by conventional MRI when they are deformed by skull base tumors. High-resolution (HR) T2-weighted MRI (T2) has been able to depict normal CN cisternal paths; however, tractography has demonstrated value in CN reconstruction when the nerves are displaced by skull base tumors. In the present study, the authors aimed to compare HR T2 to tractography in the detection of CNs displaced by skull base tum

Microsurgical Clipping of Ruptured Posterior Fossa Aneurysms in the Semi-Sitting Position.

OBJECTIVE: Posterior fossa aneurysms, especially ruptured ones, are challenging to treat due to their deep location, complex anatomy, and proximity to the brainstem, and lower cranial nerves. The semi-sitting position, though historically controversial due to concerns over venous air embolism, may offer distinct advantages. The aim of this study was to evaluate the safety and efficacy of the semi-sitting position for the clipping of ruptured posterior fossa aneurysms. METHODS: We retrospectively reviewed 39 consecutive patients with ruptured posterior circulation aneurysms treated with micros

Narrative pedagogies in cranial nerve anatomy: Comparing mythological and sports-based approaches with traditional teaching.

This study used a mixed methods randomized controlled experimental design to evaluate the effectiveness of narrative-based versus traditional pedagogy in teaching cranial nerve anatomy to medicine students. Eighty-eight students who had completed a previous course on the nervous system were randomized into three groups: a control group receiving standard theoretical and practical anatomy instruction, a group using a mythological narrative ("Supper of Gods"), and a group using an animated sports-themed narrative ("Head Coach Hypoglossus"). Knowledge acquisition was assessed using pre- and post-

Clival metastasis of hepatocellular carcinoma: a case report of a rare skull-base malignancy.

INTRODUCTION: Hepatocellular carcinoma is the most common neoplasm of the liver. It metastasizes mainly to local lymph nodes, lungs, adrenal glands, vertebrae, pelvis, ribs and long bones. Metastasis to the clivus is extremely rare. Only a few cases have been described in the literature. Therefore, there is currently no clear consensus on the optimal treatment: biopsy only or maximum safe removal. CASE REPORT: We present a 68-year-old lady who presented with dysfunction of the 6th and 12th cranial nerves. Imaging showed tumor formation in the clivus, most likely a metastasis. Further diagnost

Pretemporal Transcavernous Approach to Cavernous Sinus Meningioma with Suprasellar Extension Synchronous with Sphenoid Wing Meningioma: Operative Technique: 2-Dimensional Operative Video.

Cavernous sinus meningiomas with suprasellar extension are challenging because of their proximity to the optic nerves, pituitary stalk, cranial nerves, and the cavernous/supraclinoid internal carotid artery (ICA). This complex anatomy necessitates precise microsurgical corridors for safe resection. The patient consented for the procedure and the publication of their images. Queen's University ethics board approved this publication. A 67-year-old woman who experienced a tonic-clonic seizure with left-sided weakness was found to have a right sphenoid wing meningioma and a synchronous meningioma

Peripheral Facial Nerve Palsy due to Spontaneous Internal Carotid Artery Dissection.

A man in his 50s experienced novel, continuous, and progressive headache and neck pain prior to the onset of left-sided peripheral facial nerve palsy. Sequential palsies of left lower Cranial Nerves IX and XII followed. Imaging showed spontaneous cervical artery dissection (sCeAD) of the ipsilateral internal carotid artery. Lower cranial nerve palsies in sCeAD are a frequent result of a local mass effect exerted by the formation of a mural hematoma. The only close topographical relationship between the facial nerve and the internal carotid artery is within the petrous part of the temporal bone

Design of FPGA-Based Rehabilitation Effect Assessment Headband.

Stroke is an emergency cerebrovascular event, resulting in damage to cranial nerves, and the subsequent rehabilitation is necessary to restore neurological function and improve patient outcomes. In clinical settings, the rehabilitation effectiveness is assessed subjectively by experienced physicians or through the use of performance assessment scales. Although several techniques, such as electroencephalography (EEG), cardiopulmonary exercise testing (CPET), and functional magnetic resonance imaging (fMRI), may also be utilized in the evaluation of rehabilitation effectiveness, they require hig

Cavernous Hemangioma of the Cerebellopontine Angle With Remarkable Enlargement of the Internal Auditory Canal in a Child.

Cavernous hemangioma (CH) is a benign vascular malformation. Those arising in the central nervous system commonly occur in the supratentorial region or brainstem, but those originating from cranial nerves are rare. The case was an 11-year-old girl. Presenting with left sensorineural hearing loss and unsteadiness, she was found to have a tumorous lesion in the left cerebellopontine angle, accompanied by marked enlargement of the internal auditory canal. The left vestibular schwannoma was suspected, and tumor resection was performed. However, the intraoperative findings differed from this expect

Transection of the glossopharyngeal nerve reduces energy and sugar intake but does not affect fat intake or meal patterns in rats offered a palatable cafeteria diet.

Taste information is transmitted to the brain via branches of cranial nerves VII (chorda tympani, CT, and greater superficial petrosal, GSP), IX (glossopharyngeal, GL), and X (superior laryngeal branch of the vagus) with each nerve innervating relatively segregated fields of taste buds on the tongue, palate, and laryngeal epithelium. Approximately 70% of taste buds reside on the tongue and are innervated by the CT or GL. Previously we found that rats offered a 5-choice cafeteria diet after combined transection of the CT and GL (2NX) ate more fat, less sugar, and larger, but fewer meals each da
