BACKGROUND: Cranial nerve involvement is a well-recognized feature in Guillain-Barré syndrome (GBS) but remains less well understood in chronic forms of autoimmune neuropathies. Earlier studies of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) were conducted before updated diagnostic criteria and the recognition of autoimmune nodopathy (AN), which may limit the interpretation of their findings.
METHODS: We retrospectively analyzed 582 patients with chronic autoimmune neuropathies-CIDP (n = 431), multifocal motor neuropathy (MMN) (n = 64), anti-myelin-associated glycoprotein

Ewing sarcoma (ES) is an aggressive tumor of the bone in young adults, commonly originating from the pelvis and long bones. It is characterized by chromosomal translocation t(11;22) (q24;q12), resulting in the EWSR1-FLI1 fusion gene. It presents with metastasis commonly to the lungs and other soft tissues. While the metastasis to lungs is common, orbital involvement with cranial involvement is exceptionally rare. The case highlights the need for the early detection and aggressive treatment in order to reduce mortality in patients ES. A 23-year-old South-Asian male presented with a 2 month hist

Cerebral hemosiderosis is a condition resulting from recurrent or extensive intracranial hemorrhages, leading to hemosiderin deposition in central nervous system structures and affecting cranial nerves. The classic clinical triad includes sensorineural hearing loss, cerebellar ataxia, and pyramidal signs, with less frequent involvement of other cranial nerves. In our case however, we report a unique presentation of multiple lower cranial nerve involvement following a right-myelencephalic subarachnoid hemorrhage secondary to hemorrhagic ventricular effraction in a 58-year-old male patient. We a

BACKGROUND: Posterior fossa penetrating traumatic brain injury (pTBI) in military combat is rare but carries high neurovascular risk.
OBSERVATIONS: A 51-year-old male sustained a mine explosion injury in Ukraine, in which a 4 × 1.5-cm metal fragment penetrated through the skull base into the posterior cranial fossa. The fragment was adjacent to the vertebral artery, sigmoid sinus, lower cranial nerves, and occipital condyle. Digital subtraction angiography was completed on arrival, which showed a patent right vertebral artery, occluded transverse-sigmoid sinus, and partial internal jugular ve

Secondary neuronal degeneration refers to progressive structural changes occurring in neuronal pathways following a primary insult. This article reviews characteristic imaging patterns of anterograde, retrograde, and trans-synaptic degeneration in the central nervous system. Radiologic features are described in the context of common clinical scenarios, including stroke, trauma, tumor, demyelination, and neurodegenerative disease. Imaging findings are presented alongside relevant pathophysiologic mechanisms, focusing on tract involvement such as the corticospinal, pontocerebellar, cerebellooliv

BACKGROUND AND PURPOSE: Visualization of cranial nerves III-VI (CN III-VI) within the orbit has traditionally been challenging on conventional MRI, even with advanced imaging techniques. Detailed nerve anatomy has largely been inferred from anatomical studies, while most previous MR imaging focused on proximal segments such as the cisternal or cavernous portions of CN III-VI. This study evaluated whether fat-suppressed three-dimensional (3D) FLAIR imaging obtained as part of brain MRI can depict these nerves.
MATERIALS AND METHODS: This retrospective, IRB-approved study included 46 subjects (

Skull base chordomas and chondrosarcomas are rare bone tumors predominantly found in the petroclival areas and involve the cranial nerves, internal carotid arteries, and brain stem. Their clinical aggressiveness and high risk of recurrence necessitate multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Surgery is commonly the first step in their treatment, and ABCD rules should be respected; anatomical preservation of the innocent nasal anatomy for potential repeated surgeries in case of recurrence, block-by-block resection, coverage of the intradural resi

Preoperative planning for skull base meningioma surgery requires a detailed understanding of complex anatomical relationships and careful strategy development. Although three-dimensional (3D) simulation has been increasingly used for surgical planning, conventional simulations mainly visualize anatomical structures and rarely reproduce the sequential process of tumor removal. We developed a method to perform a near-complete digital 3D simulation of skull base meningioma surgery using a surgical simulation application. Objective.Electrical stimulation of the baroreceptors pathways at the carotid sinus bulb-known as baroreflex activation therapy (BAT)-is intended to change autonomic tone and ultimately reduce blood pressure (BP) and heart rate. BAT is pre-market approved by the United States Food and Drug Administration (FDA) for the treatment of heart failure and received an FDA humanitarian device exemption for drug resistant hypertension. However, responder rates are limited by side-effects including numbness in the head and neck, altered speech, respiratory constriction, dry cough, vomiting, and altered s

BACKGROUND: The improved management of craniosynostosis has let emerge concomitant anatomical, functional and clinical features that are frequently faced later in life (especially by syndromic patients) and might variably encompass the cranial nerves due to stenosis of bone foramina, abnormal intracranial pressure, or anomalous gene-driven development. Clinical consequences vary according to the affected nerves, the severity and the pathogenesis of nerve involvement but also might depend on early appropriate treatment. Vision, smell and hearing, but also feeding, swallowing and facial mimic or

The jugular foramen (JF) is a complex anatomical region transmitting cranial nerves IX-XI and the internal jugular vein, where variations in its dural and bony septations can influence tumor spread, venous drainage, and surgical access. This study examined the morphology and morphometry of the JF using 200 dry skulls and 39 formalin-fixed cadavers. Extracranial features, including bony septation (BS), jugular dome, and JF-stylomastoid foramen distance, were evaluated in skulls, while intracranial dural septation (DS), sigmoid sinus (SS) diameter, and JF-internal acoustic meatus distance was me

BACKGROUND AND OBJECTIVES: Corticobulbar motor evoked potentials (coMEPs) were introduced in 2005 as an alternative to direct stimulation and free running electromyography for continuous monitoring of the facial nerve during skull base surgery. The technique has been expanded for monitoring of multiple cranial nerves (CNs) and adapted across diverse pathologies. Nonetheless, broad adoption has lagged. This study aimed to understand the basis of this disparity by systematically mapping current practices and characterizing the variations in technical implementation and reporting of coMEPs.

Orofacial behaviors such as mastication, swallowing, and phonation are not organized as simple reflex chains but are generated and coordinated by intrinsic brainstem networks known as central pattern generators (CPGs). Although the existence and organization of these circuits are well established in animal models, direct evidence for their microcircuit organization in humans remains limited, and current human interpretations rely largely on lesion-based, neurophysiological, and imaging-based inference. In this review, we synthesize neuroanatomical, neurophysiological, and clinical evidence to

BACKGROUND: Endolymphatic sac tumours (ELSTs) are rare, locally aggressive neoplasms of the temporal bone that often present at an advanced stage due to their indolent clinical course. Complete surgical excision may be technically challenging when tumours involve the jugular foramen, cerebellopontine angle (CPA) or adjacent cranial nerves and may therefore be associated with significant neurological morbidity.
OBJECTIVE: To evaluate facial nerve and radiological outcomes following planned subtotal resection with selective use of adjuvant radiotherapy in patients presenting with advanced spora

INTRODUCTION: The role of stereotactic radiosurgery (SRS) of vestibular schwannomas (VS) in patients with neurofibromatosis type 2 (NF2) is still not established well. The objective of this retrospective study was evaluation of effectiveness of proactive microanatomy-based low-dose SRS for serviceable hearing preservation in such cases.
METHODS: Study cohort included 16 NF2-associated VS in 13 patients, who underwent SRS between January 2002 and March 2022. Median tumor volume was 0.56 cc (range, 0.12-2.75 cc). Hearing on the side of treated tumor corresponded to Gardner-Robertson classes I (

Collet-Sicard syndrome is a rare neurological condition resulting from lesions at the jugular foramen and hypoglossal canal, leading to combined palsy of cranial nerves IX, X, XI, and XII. Neoplastic causes are most commonly involved, while benign tumors such as paragangliomas are infrequent etiologies. We report a 58-year-old woman who presented with progressive dysphagia and unilateral lower cranial nerve dysfunction. Clinical examination demonstrated right-sided palsy of cranial nerves IX, X, XI, and XII. Magnetic resonance imaging revealed a lobulated, avidly enhancing lesion centered in t

Leptomeningeal carcinomatosis (LMC) is extremely rare in gastric cancer, as is recurrence >10 years after gastrectomy. Herein, we report an autopsy case of LMC that developed 29 years after gastrectomy for gastric signet ring cell carcinoma. A 74-year-old man presented with multiple progressive cranial neuropathies involving cranial nerves III, V, VI, VII, IX, X, and XII. CSF analysis revealed pleocytosis and markedly elevated protein levels; however, cytological examination of five consecutive samples was negative for malignant cells. Multiplex PCR analysis excluded infectious etiologies. MRI

General approaches of head and neck surgery involve varied procedures with developing perioperative care and a renewed effort on regional anaesthesia (RA) for intraoperative and postoperative analgesia. Due to the rich innervation and focus on enhanced recovery after surgery (ERAS), RA techniques, particularly ultrasound-guided ones, are becoming popular as part of an opioid-sparing multimodal analgesic regimen. However, the evidence base is heterogeneous and synthesised clinical guidance is needed. This narrative review, performed according to the Scale for the Assessment of Narrative Review

BACKGROUND: The recommendations to leave the lights on or turn the lights off before testing the pupillary light reflex (PLR) are primarily based on historical precedent. There is a lack of evidence to determine the best practice regarding adjusting ambient light when obtaining pupil measurements using a quantitative pupillometer (QP).
OBJECTIVES: To examine the difference in QP measurements taken from the same subject in bright light versus in a darkened room.
METHOD: Before the human subject study, light readings were obtained with a lux meter to identify the range and central tendency for
