From cranium to brain: Rethinking tES neuromodulation through the peripheral pathways.

Transcranial electrical stimulation (tES) is traditionally framed as a purely cortical intervention, and weak direct or alternating currents delivered through scalp electrodes are assumed to modulate excitability or entrain oscillations directly under electrodes. However, a growing body of evidence suggests that this transcranial view is incomplete. We synthesized converging human and animal evidence showing that tES also co-stimulates cranial and cervical nerves, especially branches of the trigeminal and greater occipital nerves, which engage deep brainstem neuromodulatory nuclei, such as the

CHARGE Syndrome: A Narrative Review and Update on Diagnosis, Assessment and Management.

BACKGROUND: CHARGE syndrome (CS) is a rare multisystemic genetic condition caused by a pathogenic variant in the DNA-binding protein-7 CHD7 gene. The condition affects the development of neural crest cells, which give rise to craniofacial structures, cranial nerves, ears, eyes and the heart, resulting in diverse and complex clinical features. CS is the most common genetic cause of congenital deafblindness. PURPOSE: This narrative review provides an updated and systematic approach to diagnosis, assessment and management of individuals with CS. METHODS: A broad literature search was performed

Pre-operative embolization of skull base meningiomas.

Skull base meningiomas are challenging tumors to resect and confer a high risk of surgical morbidity due to their location in deep-seated regions of the cranium. These tumors can have robust and variable blood supply from branches of the internal and external carotid artery, as well as from posterior circulation vasculature, and may have anastomotic connections that supply cranial nerves. If tumoral blood supply cannot be readily accessed early during surgery, pre-operative embolization may be considered in select cases to facilitate subsequent surgical resection. However, the indications and

Imaging of the Cavernous Sinus.

Pathology of the cavernous sinus encompasses a broad and heterogeneous group of conditions that reflect the complex anatomy and critical neurovascular contents of this region. Accurate diagnosis relies on careful integration of clinical presentation with high-resolution imaging that delineates lesion location, extent, and behavior relative to the cavernous sinus walls, cranial nerves, and internal carotid artery. While many disease entities share overlapping imaging and clinical features, recognition of key distinguishing characteristics allows meaningful narrowing of the differential diagnosi

Recurrent ophthalmic arteries supplying posterior meningiomas: embryologic background and 3D anatomical visualization.

PURPOSE: Recurrent branches of the ophthalmic artery rarely function as feeding arteries to posteriorly located meningiomas. This study describes two such cases and examines the embryologic basis and collateral pathways that enable this atypical supply. The utility of three-dimensional (3D) simulation for delineating skull base vascular relationships is also assessed. METHODS: Two patients with posteriorly located meningiomas supplied by recurrent ophthalmic artery branches underwent detailed angiographic evaluation. The embryogenesis of the superficial recurrent ophthalmic artery (SROA) and

Toward a deeper and more comprehensive understanding of neurological symptoms related to Allgrove syndrome: a case report.

INTRODUCTION AND IMPORTANCE: Allgrove syndrome (AS) is a rare autosomal recessive disorder caused by triple A syndrome gene mutations, characterized by alacrima, achalasia, and adrenocorticotropic hormone (ACTH)-resistant adrenal insufficiency, with potential neurological complications. Early signs include absent tears, feeding difficulties, vomiting, and growth issues, while adrenal crises and neurological symptoms may appear later. Diagnosis is often delayed when only one feature is present. CASE PRESENTATION: We report a 13-year-old boy with AS on oral hydrocortisone who presented with rec

Cephalic Tetanus Following Auricular Metallic Trauma in an Adult: A Rare Clinical Case Presentation.

INTRODUCTION: Cephalic tetanus of otitic origin is a well-documented clinical entity. It is characterized by trismus associated with damage to one or more cranial nerves. Although rare, this form poses a real diagnostic challenge because of its often atypical clinical picture, which justifies rapid and appropriate treatment to limit morbidity and mortality. CASE PRESENTATION: We report the case of a 31-year-old patient, working as a welder and whose tetanus vaccination status was not up to date with recommended booster doses, who developed purulent otitis following auricular trauma caused by

Perineural Tumor Spread along the Cervical Plexus in Head and Neck Cancer: Imaging Features and Clinical Characteristics from a Single-Center Case Series.

Perineural tumor spread of head and neck cancer has been associated with poor prognosis and is a diagnostic challenge for neuroradiologists who may be unfamiliar with neural anatomy. Although much of the literature has focused on understanding anatomy and perineural tumor spread along the cranial nerves, there has been relatively little focus on the cervical plexus, due to a lack of familiarity with its complex anatomy or recognition that perineural tumor spread can occur along its vast neural network. Consequently, perineural tumor spread in the cervical plexus is underrecognized and underdia

Risk stratification of advanced extranodal extension in retropharyngeal lymph nodes for nasopharyngeal carcinoma: a multi-center study.

BACKGROUND: This study aimed to refine risk stratification of advanced extranodal extension (ENE) in retropharyngeal lymph nodes (RLNs) and evaluate its prognostic significance. METHODS: A total of 1,373 non-metastatic NPC patients at three centers between 2011 and 2021 were retrospectively enrolled. For advanced RLN ENE, we documented all involved structures, maximum axial diameter (MAD), presence of lymph node necrosis (LNN), and bilaterality. High-risk structures were identified using adjusted hazard ratios and LASSO-Cox models. Multivariable Cox models and Kaplan-Meier analyses were used

DjFNDC3A is associated with DjSmad4 to regulate cranial nerve regeneration in planarian (Dugesia japonica).

FNDC3A protein, widely expressed in various tissues, is involved in a variety of biological processes, such as, tail bud development and caudal fin regeneration in zebrafish. However, functional studies of FNDC3A in planarians have not been reported. In this study, we examined the spatiotemporal expression pattern of DjFNDC3A and investigated its functions in planarians regeneration. Knockdown of DjFNDC3A resulted in severe defects, including poorly defined brain lobes, complete loss of brain lateral branches and reduction of multiple neuronal populations. Silencing DjFNDC3A did not affect the

Role of Radiotherapy in an Inflammatory Pseudotumor of the Maxillary Sinus and Imaging Findings: A Case Report.

Inflammatory pseudotumor (IPT) is a benign lesion of inflammatory origin that can mimic a malignant neoplasm because of its aggressive clinical and imaging behavior, particularly in uncommon locations such as the paranasal sinuses. In this article, we present the case of a patient diagnosed with IPT of the left maxillary sinus, which demonstrated extension into adjacent soft tissues and secondary involvement of cranial nerves due to extensive local invasion. Findings on computed tomography (CT) and magnetic resonance imaging (MRI) revealed bone destruction and extension into neighboring anatom

The Addition of a 3D Balanced Steady-State Free Precession Pulse Sequence Improves Magnetic Resonance Imaging Identification of Certain Canine Cranial Nerves.

In people, magnetic resonance imaging (MRI) with high-resolution, high T2-weighted (T2w) contrast-balanced steady-state free precession (b-SSFP) pulse sequences and 3D fast spoiled gradient-echo (FSPGR) pulse sequences can improve visualization of cranial nerves (CNs) and associated osseous foramina. This prospective, observational study aimed to determine whether the addition of b-SSFP and FSPGR sequences improves confidence in the identification of canine CNs compared to the standard imaging protocol at 1.5 T. The head of 10 canine cadavers was imaged, including transverse T1-weighted (T1w)

Follicular Thyroid Carcinoma with Cavernous Sinus Metastasis and Perineural Spread: A Rare Manifestation of Radioiodine-refractory Disease.

Cavernous sinus metastasis from follicular thyroid carcinoma (FTC) is exceedingly rare and typically indicates advanced, radioiodine-refractory disease. We report a 37-year-old man with minimally invasive FTC who was treated with total thyroidectomy, neck dissection, and cumulative radioiodine therapy totaling 720 mCi for persistent iodine-avid disease. Four years later, he developed progressive left-sided palsies of cranial nerves III, IV, and VI. Imaging revealed a cavernous sinus mass with orbital apex extension and perineural spread. Serum thyroglobulin was markedly elevated (>500 ng/mL),

How I do it: pediatric lateral-type posterior fossa ependymoma resection.

BACKGROUND: Lateral-type ependymomas, or cerebellopontine ependymomas, of the posterior fossa are rare tumors extending into the cerebellopontine angle, encasing important neurovascular structures, presenting as a particular surgical challenge. METHOD: We performed a radical resection of lateral-type ependymoma in a 2-year-old patient with anamnesis of torticollis and vomiting. Resection of the tumor was preceded by insertion of external ventricular drainage. CONCLUSION: Optimal treatment improving prognosis requires achieving gross total resection of the tumor.

Operative Technique for Embolization and Resection of a Carotid Body Tumor.

Carotid body tumors are rare, highly vascular paragangliomas that arise from neural crest-derived chemoreceptor cells of the carotid body, a specialized structure located at the carotid bifurcation adjacent to the carotid sinus and involved in oxygen and carbon dioxide sensing. For symptomatic or enlarging lesions, surgical resection remains the definitive treatment. Preoperative embolization can reduce vascularity and improve intraoperative safety as supported by the literature demonstrating its safety and efficacy.1-3 This video demonstrates the microsurgical technique for resection of a car

Bilateral Blindness in a Patient With Vestibular Schwannoma and Empty Sella Syndrome: A Case Report.

We report a rare clinical presentation of a T4b vestibular schwannoma causing severe compression of the brainstem and fourth ventricle, associated with obstructive hydrocephalus and empty sella syndrome, in a patient presenting with profound neurological and ophthalmological deficits, most notably complete bilateral blindness. Radical microsurgical resection was achieved via a lateral suboccipital (retromastoid) approach, resulting in gross-total tumor removal with preservation of most cranial nerves. Postoperatively, the patient developed House-Brackmann grade III facial palsy. Hydrocephalus

Comprehensive management of carotid body paragangliomas: A case report on diagnosis, surgical resection, and postoperative surveillance.

Carotid body paragangliomas (CBPs) are rare, highly vascular neuroendocrine tumors arising at the carotid bifurcation and represent a diagnostic and therapeutic challenge because of their close anatomical relationship with major cervical vessels and cranial nerves. We report the case of a 45-year-old woman who presented with a slowly progressive, painless left lateral cervical mass evolving over a 10-month period. Multimodal imaging evaluation, including high-resolution cervical ultrasound, contrast-enhanced computed tomography, and digital subtraction angiography, demonstrated a well-defined

Successful Decompression and Reduction of Displaced Sphenoid Wing Fracture into the Superior Orbital Fissure Causing Complete Ophthalmoplegia Secondary to Compression of Cranial Nerves III, IV, and VI.

Superior orbital fissure syndrome (SOFS) is a rare, vision-threatening condition caused by injury or compression of cranial nerves III, IV, and VI. We report a 16-year-old male who sustained extensive craniofacial trauma, including a displaced left sphenoid wing fracture, after a motorized scooter accident. Initially, he had limited extraocular motility, but by hospital day 5, he developed complete ophthalmoplegia due to progressive nerve compression. Urgent surgical decompression of the superior orbital fissure and reduction of the fracture were performed via a pterional craniotomy on day 7.

Using a hand-held pupillometer to quantify pupil constriction as part of the accommodation reflex.

BACKGROUND: Quantitative pupillometry (QP) provides a reliable measure of the pupillary light reflex. The pupil accommodation reflex (P-AR) occurs when there is a change in visual focus from near to far or from far to near. The ability to measure the P-AR using handheld QP has not been described. We aimed to explore whether QP can objectively detect changes in pupil diameter associated with differences in gaze fixation distance. METHODS: This prospective study enrolled 100 healthy adult volunteers. Bilateral QP measurements were obtained with each observation lasting 16 s (s) for both near an
