Tau isoform mapping across tissues and biofluids reveals a signature distinguishing Alzheimer's disease from amyotrophic lateral sclerosis.

Blood phosphorylated tau 217 (p-tau217) accurately detects Alzheimer's disease (AD) pathology but is also elevated in amyotrophic lateral sclerosis (ALS), likely reflecting peripheral (big) tau released from muscle. Assays selectively measuring brain-derived (BD-) tau and BD-p-tau217 are therefore needed. We analysed serum BD-tau and p-tau217 in ALS (n = 139), AD (n = 101) and controls (n = 75) using direct immunoassays and after BD-Tau immunoprecipitation (BD-Tau-IP), and tau species in brain (n = 16), spinal cord (n = 11) and muscle (n = 10) by targeted mass spectrometry. With the direct imm

Chronic spinal cord injury in a patient with Klippel-Trenaunay syndrome: Long-term rehabilitation challenges.

CONTEXT: Spinal involvement in Klippel-Trenaunay syndrome (KTS) is uncommon, and long-term outcomes after extensive spinal surgery remain poorly characterized. FINDINGS: A 64-year-old man with KTS developed progressive myelopathy and chronic incomplete thoracic spinal cord injury from an intraspinal vascular lesion causing cord compression. He underwent embolization, decompression, lesion resection, corrective osteotomies, and long-segment fusion. Over subsequent years, neurological and functional decline progressed with bilateral rod fractures, pseudoarthrosis, and sagittal imbalance. Rehabi

[Research progress on the autonomic-immune mechanism of acupuncture in regulating ovarian function].

Ovarian dysfunction is an important cause of ovulatory disorders, infertility and endocrine disorders in women, which is commonly observed in conditions including polycystic ovary syndrome, primary ovarian insufficiency, diminished ovarian reserve and ovarian aging-related states. Recent studies have demonstrated that besides abnormalities in the classic hypothalamic-pituitary-ovarian axis, autonomic nervous imbalance and immune-inflammatory disturbance also serve as critical mechanisms underlying impaired ovarian function. Central inflammatory responses, chronic stress and metabolic abnormali

Multichannel, biomimetic scaffolds improve functional connectivity following traumatic long-gap peripheral nerve injury.

The gold standard for repair of peripheral nerve injuries is a nerve autograft. Donor nerve harvest creates a second surgical site, invariable numbness and donor site chronic neuropathic pain in 20% of cases. We developed a poly-caprolactone (PCL) multichannel, biomimetic scaffold with surface features promoting cell and vascular attachment and rapid, linear axonal regeneration. The cross-sectional area available for axonal regeneration exceeds that of autografts. When compared to nerve autografts and FDA-approved nerve conduits in 1cm-long rat and 3cm-long rabbit sciatic gap injury, biomimeti

Effects of virtual reality and gamified digital rehabilitation on functional recovery in individuals with Spinal Cord Injury: a systematic review and Bayesian multilevel meta-analysis.

BACKGROUND: Virtual reality (VR) and gamified digital rehabilitation can increase task-specific practice and feedback for individuals with Spinal Cord Injury (SCI); however, their incremental effects on functional recovery remain uncertain. METHODS: We searched PubMed, Web of Science, Embase, and the Cochrane Library from database inception through July 1, 2026, for randomized controlled trials (RCTs) evaluating VR or gamified digital rehabilitation in adults with SCI. Comparator conditions were classified as active rehabilitation, sham or attention control, or minimal/no intervention. The pr

[Arachnoid web presenting as tetraparesis].

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.

From simulation to stimulation: a predictive synergy-based control strategy for functional electrical stimulation cycling in a person with spinal cord injury.

BACKGROUND: Functional electrical stimulation (FES) cycling stimulation strategies are often derived from heuristics, or able-bodied kinematics and electromyography, which may be unsafe and non-representative for people with complete spinal cord injury. Optimally controlled predictive musculoskeletal simulations with synergy implementation offer a way to generate individualized, physiologically plausible muscle trajectories without prior experimental data. Building on a previous proof of concept, this study developed and experimentally validated a simulation-derived stimulation strategy for FE

Connexins are essential for the contribution of latent progenitors to healing after spinal cord injury.

Gap junctions are important regulators of the biology of neural stem cells. Both in vertebrates with regenerative abilities and neonatal rodents, ependymal cells communicate via connexin (Cx) 43 and Cx26. Gap junction coupling and Cx26 are down-regulated as the ependyma becomes quiescent in adulthood, but injury overrules this developmental down-regulation suggesting a role for Cx signaling in the reactivation of ependymal cells. However, direct evidence of the involvement of these Cxs in the reactivation of the ependymal stem cell niche is lacking. Here, we aim to explore the role of Cx26 and

Comparison of non-invasive neuromodulation for motor function rehabilitation in incomplete spinal cord injury: a systematic review and network meta-analysis.

BACKGROUND: Spinal cord injury (SCI) disrupts structural and functional connectivity between the brain and spinal cord, resulting in severe motor dysfunction. Various neuromodulation techniques have been applied in SCI rehabilitation and have promoted motor recovery to varying degrees. A Bayesian network meta-analysis was therefore undertaken to compare and rank the relative effects of non-invasive neuromodulation techniques on motor-related outcomes in adults with incomplete SCI. METHODS: PubMed, Web of Science, Embase, and The Cochrane Library were searched for randomized controlled trials

tACS suppresses M1 polarization of microglia after spinal cord injury through the cGMP-PKG signaling pathway.

BACKGROUND: Strategies for repairing spinal cord injury (SCI) emphasize blocking the shift of microglia to the pro-inflammatory M1 phenotype, lowering neuroinflammation, and fostering neuronal survival. The cGMP-PKG pathway modulates immune cell function by decreasing the secretion of pro-inflammatory elements such as IL-1β and TNF and boosting the levels of the anti-inflammatory element IL-10, thereby alleviating inflammatory responses. Additionally, transcranial alternating current stimulation (tACS) at specific frequencies may indirectly regulate immune cell activity and inflammatory cytoki

Rare multi - regional involvement in posterior reversible encephalopathy syndrome with spinal cord involvement of a young hypertensive male: a case report.

BACKGROUND: Posterior reversible encephalopathy syndrome (PRES) is an acute or subacute cerebral syndrome which primarily manifests as headache, encephalopathy, seizures, and visual disturbances in various combinations. PRES with spinal cord involvement (PRES-SCI) is extremely rare and considered to be a variant of PRES. CASE PRESENTATION: We present a case of PRES-SCI where the patient primarily manifested with bilateral blurred vision. The brain and spinal cord MRI showed that the lesions simultaneously affected the supratentorial region, infratentorial region, and spinal cord, and malignan

Deep brain stimulation and spinal cord stimulation for chronic disorders of consciousness: a decade of clinical evidence, mechanistic insights, and critical appraisal.

Deep brain stimulation (DBS) and spinal cord stimulation (SCS) have been investigated for over four decades as neuromodulatory treatments for chronic disorders of consciousness (DOC), including the vegetative state/unresponsive wakefulness syndrome (VS/UWS) and the minimally conscious state (MCS), for which no disease-modifying pharmacological therapy exists. This narrative review synthesizes literature published between 2016 and 2026 on the neurobiological rationale, clinical evidence, neurophysiological biomarkers, and safety of DBS and SCS for chronic DOC. The 'mesocircuit hypothesis', supp

Single-cell analysis reveals temporal heterogeneity of neutrophil-lineage states after spinal cord injury.

Spinal cord injury (SCI) elicits a complex immune response that evolves from acute inflammation to chronic tissue remodeling, but the temporal organization of neutrophil-lineage cells remains incompletely defined. Here, we reanalyzed three public SCI single-cell RNA sequencing datasets, including immune-cell-enriched and whole-lesion datasets. We combined neutrophil-state annotation, maturation assessment, differential expression, functional module scoring and ligand-receptor inference with immunofluorescence validation in a C57BL/6 mouse contusion model. Neutrophil-lineage cells were detected

Principles, concepts, and perspectives for the improvement of decompression procedures: a Neo-Haldanian approach bridging bubble physics and decompression physiology.

INTRODUCTION: This paper is a narrative review and theoretical perspective presenting principles, concepts, and perspectives for improving decompression procedures. It distinguishes three levels of evidence: established observations, hypotheses with experimental support, and mechanistic scenarios that remain hypothetical. METHODS: We organize decompression around two linked dimensions: bubbles as the primary physical event and microparticles (MPs) as a physiological consequence. We hypothesize that a substantial proportion of decompression-related microbubbles originate from pre-existing gas

[Factors associated with body image in people with spinal cord injury].

OBJECTIVE: Spinal cord injury (SCI) represents a significant public health concern due to its substantial impact on functioning, independence, and quality of life. It can also significantly affect body image (BI), leading to complex emotional responses and alterations in body perception. The aim of this paper was to establish the factors associated to BI in people with SCI in Boyacá (Colombia). METHODS: The type of design was cross-sectional in a sample of eighty-seven subjects; for body image, the BI for Physically Disabled Persons (PICDF) instrument was used. The statistical analysis was pe

Postoperative functional rehabilitation for cervical spondylotic myelopathy: An integrative review guided by Orem's self-care deficit theory.

BACKGROUND: Cervical spondylotic myelopathy is the predominant cause of spinal cord dysfunction in adults, for which surgical decompression remains the mainstay of treatment. Surgical success alone does not guarantee functional restoration, and postoperative rehabilitation is a critical determinant of long-term outcome. A comprehensive, theory-grounded nursing framework to guide rehabilitation practice for this population has not been available. OBJECTIVES: To synthesise the current evidence on postoperative rehabilitation nursing for patients with cervical spondylotic myelopathy, to develop

Case Report: Beyond chronicity: sustained multidimensional response to spinal cord stimulation three decades after traumatic spinal cord injury.

INTRODUCTION: Spinal cord stimulation (SCS) is an established treatment for refractory neuropathic pain conditions, yet evidence for its effectiveness in spinal cord injury (SCI)-related neuropathic pain, particularly after decades of persistent symptoms, remains limited. It remains unclear whether unusually prolonged pain chronicity and established neurological deficits affect the potential for a meaningful response to neuromodulation. CASE PRESENTATION: We report a 60-year-old man with chronic incomplete SCI, bilateral spastic paraparesis markedly more pronounced on the left, and severe chr

Mitochondrial electron transport chain activity is not essential for the acute hypoxic response of spinal oxygen sensors.

Spinal sympathetic preganglionic neurons (SSPNs) are oxygen sensitive. These spinal oxygen sensors (SOS) increase sympathetic output when subjected to acute hypoxia, orchestrating cardiorespiratory responses to protect against the deleterious effects of oxygen deprivation. Most other oxygen-sensing systems rely on mitochondrial mechanisms to detect and signal decreases in oxygen. Here, we apply pharmacological blockers of the mitochondrial respiratory chain and a mitochondrial-targeted antioxidant to a rat perfused in situ spinal cord preparation to test the role of mitochondria in the SOS. We
