CD1a-Mediated Presentation of Canonical Microbial Peptides to T Cells.

Langerhans cells express the nonpolymorphic antigen-presenting molecule CD1a, positioning them as contributors to host immunity against Mycobacterium leprae in human leprosy. CD1a was originally shown to present non-canonical lipopeptide antigens such as dideoxymycobactin and chemically diverse hydrophobic ligands. Here, we generated CD4⁺ T cell lines from leprosy lesions that recognized M. leprae in a CD1a-restricted manner. Unexpectedly, antigen recognition was protease-sensitive, prompting biochemical purification that identified two microbial protein antigens: LppX, a 25-kDa lipoglycoprote

Lepromatous leprosy presenting with chronic painless ulcers and inflammatory arthropathy: a case report from a non-endemic setting.

BACKGROUND: Hansen's disease is a chronic infectious disease caused commonly by Mycobacterium leprae. Although it is rare in Europe, sporadic cases occur due to migration from endemic areas. In Portugal there are 4 cases reported in 2023 and 4 cases reported in 2024. Lepromatous leprosy represents the anergic pole of the disease spectrum, characterized by diffuse bacillary proliferation and multisystem involvement, often mimicking autoimmune conditions. CASE PRESENTATION: A 36-year-old Brazilian male from Rio de Janeiro living in Portugal presented with recurrent painless leg ulcers and viola

Lepromatous Leprosy Mimicking Sarcoidosis With Elevated Serum ACE Levels: A Deceptive Granulomatous Presentation.

Granulomatous diseases often present with overlapping clinical and histopathological features, posing a diagnostic challenge, particularly in regions endemic for tuberculosis. We report the case of a 68-year-old diabetic male with a significant smoking history who presented with progressive shortness of breath and cough for almost three weeks' duration and multiple cutaneous nodules, present for six years, that increased in size and number over two months before presentation. Initial evaluation revealed normal chest imaging, negative microbiological tests for tuberculosis, a non-reactive Manto

Recurrent uveitis and nodular skin lesions revealing lepromatous leprosy.

A 51-year old woman with type 2 diabetes mellitus, presenting with recurrent episodes of bilateral anterior uveitis for five years, had also developed recurrent tender erythematous nodules and plaques over the extremities, abdomen and back. Evaluation ultimately confirmed lepromatous leprosy. After multidrug therapy and systemic corticosteroids, complete resolution of skin lesions and uveitis was maintained at 1-year follow-up, thus rescuing her eyesight.

Dovramilast for Erythema Nodosum Leprosum in Patients With Leprosy in Nepal: Protocol for a Phase 2 Open-Label Pilot Study.

BACKGROUND: Erythema nodosum leprosum (ENL) is an immunological complication affecting up to 10% of borderline lepromatous and 50% of lepromatous leprosy cases with a high bacterial index (3-6+). Current treatments often require prolonged therapy over years and cause significant long-term side effects, highlighting the need for new therapies. Therefore, it is imperative to identify new therapies for ENL. Phosphodiesterase 4 inhibitors are a class of compounds that mediate immune homeostasis and have the potential to treat ENL with fewer side effects. Dovramilast (formerly CC-11050) is an anti-

Leprosy masquerading as systemic lupus erythematosus: a case report and systematic review of the literature.

BACKGROUND: Leprosy can mimic systemic lupus erythematosus (SLE) due to overlapping clinical, laboratory, and immunological features, frequently resulting in misdiagnosis and inappropriate immunosuppressive treatment. We report a case of leprosy initially misdiagnosed as SLE and systematically review the literature on leprosy cases mimicking SLE, emphasizing clinical, laboratory, therapeutic, and outcome characteristics, evaluated in light of the 2019 ACR/EULAR classification criteria for SLE. METHODS: A systematic review was conducted according to PRISMA guidelines. PubMed, SciELO, and Googl

Lepromatous Leprosy Presenting With Preserved Sensation: A Case Report.

Leprosy, which is also called Hansen's disease, is a chronic infectious disease that predominantly affects the skin and the nerves. It is caused by Mycobacterium leprae. The skin, peripheral nerves, upper respiratory tract mucous surface, and the eyes are mostly affected during the course of the disease. Leprosy can occur at any age, from early infancy to very old age. This case report describes a 43-year-old Nigerian man who works as a timber contractor. He presented with a month's history of the eruption of non-itchy papules first noticed on the face and progressively worsened till the patie

Retiform Purpura Due to Microvascular Occlusion: An Archetypical Manifestation of Lucio Phenomenon in Lepromatous Leprosy.

Retiform purpura is a morphological pattern that is a clinical reflection of microvascular occlusion. Although several infectious etiologies are described, leprosy remains an underrecognized cause. Lucio phenomenon is a rare vasculonecrotic reaction of untreated lepromatous leprosy, characterized by endothelial invasion by Mycobacterium leprae (M. leprae), leading to thrombosis and necrotizing vasculitic skin lesions. The present report involves a case of a 70-year-old woman who presented with purpuric and necrotic lesions over the extremities, digital discoloration, and neuritic pain, with a

Relapsed Type II Lepromatous Reaction With Drug-Induced Hemolysis: A Case Report Highlighting Therapeutic Challenges.

Leprosy is a chronic infectious disease caused by Mycobacterium leprae (ML), and it is also known as Hansen's disease. It is transmitted mainly through prolonged close contact with an untreated infected person. The disease primarily affects the skin and peripheral nerves. Leprosy can, however, present with a broad spectrum of clinical manifestations depending on the host's immune response, ranging from localized tuberculoid disease to disseminated lepromatous disease. Diagnosis of leprosy relies primarily on clinical findings and is supported by histopathological examination of a skin biopsy.

Strongyloides hyperinfection presenting with ulcerative duodenitis and septic shock in a steroid-treated patient.

Strongyloides stercoralis can cause chronic infections that may escalate into life-threatening hyperinfection, especially in immunocompromised patients. Severe intestinal strongyloidiasis has been associated with bacteraemia and sepsis, classically due to translocation of enteric organisms across a disrupted intestinal mucosal barrier. We report a patient with lepromatous leprosy on steroids who developed Strongyloides hyperinfection presenting with ulcerative duodenitis and septic shock.

Longitudinal performance of the ENLIST ENL severity scale in individuals with severe erythema nodosum leprosum: Responsiveness, trajectories and clinical features - a secondary analysis of the Methotrexate and Prednisolone study - MaPs in ENL.

BACKGROUND: Erythema nodosum leprosum (ENL) is a severe inflammatory complication of lepromatous leprosy characterised by recurrent inflammatory episodes often requiring prolonged immunosuppression. The severity of ENL can be quantified using the validated and reliable ENLIST ENL Severity Scale (EESS). The longitudinal course of ENL and how it is captured using standardised severity measures has not been well described. We prospectively evaluated the changes in ENL severity over time using the EESS in a randomised clinical trial. METHODS: Ethics statement. The trial was performed according to

Dapsone-Induced Photosensitivity: A Rare and Overlooked Clinical Dilemma.

INTRODUCTION: Dapsone (4,4'- diamino diphenyl sulfone, DDS) is an antimicrobial sulfonamide used for non-dermatological and dermatological disorders. Dapsone has been associated with a wide range of cutaneous adverse effects, with photosensitivity being an unusual side effect. CASE PRESENTATION: A case of dapsone-induced photosensitivity was reported in a patient with lepromatous leprosy. A 46-year-old male patient complained of redness and scaling, with itching over his face, nape, the flexural aspect of his neck, and bilateral upper and lower limbs, during the second month of multidrug mult

When a mimicker is mimicked: A case report on acid-fast atypical lepromatous leprosy mimicking tuberculosis.

Leprosy is caused by Mycobacterium leprae, may mimic tuberculosis. We describe a 22 year old woman with prolonged fever and cervical and inguinal lymphadenopathy. Initial lymph node aspirate from an outside centre showed acid fast bacilli (AFB) on ZN staining using 20% sulfuric acid, leading to a diagnosis of tuberculosis and initiation of anti-tubercular therapy. Additionally, she developed ulcerative lesions over the feet after six weeks. At our centre, lesion pus revealed abundant AFB with differential decolorization. CBNAAT for tuberculosis was negative. Slit-skin smears, Fite Faraco stain

Alopecia areata universalis in Leonardo da Vinci's Mona Lisa: A systematic retrospective iconodiagnosis.

The face of Leonardo da Vinci's Mona Lisa (ca 1503-1519) has attracted several medical presentations, most recently the hypothesis of frontal fibrosing alopecia masquerading as Renaissance fashion. I present a systematic retrospective iconodiagnosis grounded in an explicit methodological framework, in the technical literature on the painting, including 13-band multispectral imaging, and in the documented biography of the sitter, Lisa Gherardini (ca 1479-1542). Four reproducible findings are legible on the panel: complete bilateral absence of the eyebrows; complete bilateral absence of the eyel

Acute Necrotic Violaceous Plaques and Hemorrhagic Bullae in a 53-Year-Old Man.

A 53-year-old man from northeast Mexico presented with acute painful violaceous plaques, hemorrhagic bullae and stellate necrotic eschars on the lower limbs, dorsal hands and external ears. Examination also revealed diffuse non-nodular cutaneous infiltration, madarosis and milphosis, and generalized hypoesthesia-subtle signs of underlying diffuse lepromatous leprosy. IgG anticardiolipin antibodies were positive, raising the possibility of antiphospholipid syndrome. Immunotherapeutic effect of MIP vaccine along with multidrug therapy (MDT) in leprosy: A systematic review and meta-analysis.

Background and objectives Leprosy is a neglected tropical disease caused by Mycobacterium leprae often resulting in nerve damage, numbness, disfiguring lesions, and disability. India accounts for ∼52% of the world's new leprosy patients. Immunotherapy has been reported to achieve faster bacterial clearance in multi-bacillary (MB) leprosy patients. We conducted this systematic review to synthesise the global evidence available for the efficacy and safety of the MIP vaccine (Mycobacterium indicus pranii), previously known as Mw, as an immunotherapeutic agent along with standard chemotherapy, on

Eyebrow Madarosis: An Updated Review of the Etiology and Management, Part II- Scarring Disorders.

BACKGROUND: Eyebrows play an essential role in facial symmetry, communication, and aesthetic identity. Scarring eyebrow alopecia results from inflammatory, autoimmune, infectious, genetic, traumatic, and iatrogenic processes that irreversibly damage follicular stem cells and limit regrowth potential. Despite its clinical and psychosocial impact, evidence-based guidance for diagnosis and management remains limited. METHODS: A comprehensive PubMed search (1960–2025) was performed using terms related to "eyebrow alopecia," "eyebrow madarosis," and etiology-specific conditions. English-lang
