Revisiting the anatomy of the retroperitoneum and renal fascia. Aspects applied to radiology, laparoscopic and robotic surgery - a narrative review.

This review aims to provide a comprehensive analysis of the anatomy of the retroperitoneal and abdominal fascias. We analyzed papers published in the past 70 years in the databases of Pubmed, Embase and Scielo. We excluded case reports, editorials and opinions of specialists. Studies were excluded if they lacked clear anatomical descriptions of the fascias, if full-text access was unavailable, or if they were published in languages other than Portuguese, English, or Spanish. This narrative review addresses the anatomical integration, surgical techniques, and functional importance of the fascia

Approach to the pregnant patient with prolactinoma.

Prolactinomas account for approximately half of the pituitary adenomas and predominantly affect women of reproductive age. Hyperprolactinemia commonly causes infertility by suppressing hypothalamic gonadotropin-releasing hormone (GnRH) secretion and impairing gonadal function. Because dopamine agonist therapy can rapidly restore fertility, clinicians caring for women with prolactinomas should be qualified to manage problems that may arise during pregnancy. Pregnancy is characterized by physiological enlargement of the anterior pituitary gland and progressive hyperprolactinemia, primarily drive

Medullary thyroid carcinoma with pheochromocytoma and cystic parathyroid degeneration in a ret-negative patient: a case report.

INTRODUCTION: Multiple endocrine neoplasia type 2A (MEN2A) is a rare autosomal dominant hereditary cancer syndrome, predominantly caused by mutations of the RET proto-oncogene on chromosome 10q11.2. It typically presents with medullary thyroid carcinoma (MTC), pheochromocytoma, and primary hyperparathyroidism. Cases with a MEN2A phenotype but negative RET testing are uncommon, posing challenges for diagnosis and patient management. CASE PRESENTATION: We report the case of a 63-year-old female who was diagnosed with MTC and pheochromocytoma. and cystic parathyroid gland. Genetic workup was neg

The use of intraoperative parathyroid hormone assay in predicting the success and reduce morbidity of targeted minimal invasive parathyroidectomy for primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) is caused by overproduction of parathyroid hormone (PTH) by a hyperfunctioning parathyroid gland (most commonly a parathyroid adenoma, PTA) or glands. Historically, the most commonly described surgical approach has been bilateral neck exploration (BNE). This study focuses on the outcomes of targeted minimally invasive parathyroidectomy [unilateral neck exploration (UNE), focused exploration (FE)-including endoscope and/or exoscope-assisted parathyroidectomy]. Intraoperative measurement of parathyroid hormone levels (ioPTH) is generally considered

Subcutaneous translocation of residual thyroid tissue: a novel functional-preserving technique for partial thyroidectomy with 1-year follow-up retrospective case series.

BACKGROUND: Benign thyroid nodules frequently require surgical intervention. Balancing thyroid‑function preservation and re‑operation‑related neuro‑parathyroidal risk represents an unmet clinical challenge. This study aimed to investigate the feasibility of translocating residual normal thyroid tissue into the subcutaneous space after resection of benign thyroid tumors. Conventional thyroid resection commonly compromises native thyroid function and leads to postoperative hypothyroidism. Residual thyroid tissue retained in the neck complicates surveillance and increases risks to the recurrent l

Preoperative Preparation With Selective versus Nonselective Alpha-Adrenoceptor Blockade for Pheochromocytomas and Paragangliomas: A Systematic Review and Updated Meta-Analysis.

OBJECTIVE: Preoperative preparation with nonselective alpha-blockers (NABs) or selective alpha-blockers (SABs) is crucial for the maintenance of intraoperative hemodynamic stability during surgical resection of pheochromocytomas and paragangliomas (PPGLs). This study aims to compare the efficacy and safety of NAB versus SAB for preoperative preparation in patients with PPGL. METHODS: We systematically searched PubMed, Embase, Web of Science, and Cochrane CENTRAL databases (up to August 2, 2025) for randomized and observational studies directly comparing NAB with SAB. Meta-analyses were perfor

Plasma adrenocorticotropin concentrations vary with assay used in horses with and without pituitary pars intermedia dysfunction.

Plasma adrenocorticotropin (ACTH) concentration is measured to support a diagnosis of pituitary pars intermedia dysfunction (PPID). However, baseline ACTH and ACTH concentrations after IV administration of thyrotropin releasing hormone (TRH) vary with season and assay used. To compare two ACTH assays (Scantibodies immunoradiometric assay [IRMA] and Immulite 2000Xpi chemiluminescent immunometric assay [CIA]) for diagnosis of PPID, ACTH was measured before and 10 minutes after administration of TRH in three groups of horses: 1) four young horses (6 ± 3 years, no hypertrichosis and a pituitary gl

Catecholamine-Secreting Retroperitoneal Paraganglioma: A Case Report and Literature Review.

INTRODUCTION: Paragangliomas are rare neuroendocrine tumors arising from autonomic paraganglia. These tumors have metastatic potential, and histopathological findings alone cannot reliably predict their biological behavior. Clinical manifestations range from the classic triad of headache, diaphoresis, and palpitations to incidental findings in asymptomatic patients. CASE PRESENTATION: A 75-year-old man was incidentally found to have a 5 cm retroperitoneal mass adjacent to the right renal hilum. Biochemical testing confirmed marked catecholamine hypersecretion, and 18F-FDOPA PET/CT demonstrate

Partial or full preservation of the pituitary stalk in craniopharyngioma surgery: improving endocrinological outcomes without an increased risk of recurrence.

OBJECTIVE: The risk-benefit balance of pituitary stalk (PS) preservation during craniopharyngioma (CP) surgery remains insufficiently investigated. Although preservation requires meticulous microsurgical dissection, frequent invasion of the PS by the CP also raises the question of the feasibility of preservation. Herein, the authors assessed the impact of preserving the PS in a large consecutive series of adult patients who underwent endoscopic endonasal surgery for CP. METHODS: The authors performed a retrospective single-center study of adult, surgery-naive patients with supradiaphragmatic

The Effect of Prophylactic Protection of Inferior Parathyroid Gland Vein Blood Supply on Reducing Postoperative Hypoparathyroidism.

BACKGROUND: The objective of this study was to verify that a strategy involving prophylactic exposure of the inferior parathyroid glands and preservation of their venous blood flow-achieved through early identification and protection of their venous drainage-can enhance the success rate of maintaining glandular perfusion and effectively reduce the incidence of postoperative hypoparathyroidism. METHODS: This study included 600 patients who underwent total thyroidectomy with concomitant bilateral central neck dissection between November 2019 and December 2025. Patients were divided into two gro

Real-world optimization of cutoff values for enzyme immunoassay-based plasma-free fractionated metanephrines for the diagnosis of pheochromocytoma and paraganglioma: a multicenter retrospective study.

Pheochromocytoma and paraganglioma (PPGL) require sensitive biochemical screening to prevent delayed or missed diagnoses. In Japan, plasma-free metanephrines (pMN/pNMN) measured using enzyme immunoassay have been covered by national insurance since 2019; however, the real-world diagnostic performance of the currently applied cutoff values has not been evaluated. We retrospectively identified consecutive adults who underwent pMN/pNMN testing in routine practice at five endocrinology departments between 2019 and 2025, of whom 817 were included in the final analysis. Patients were classified post

Near-complete Response of Bilateral Adrenal Metastases to 177Lu-PSMA Therapy in Castration-resistant Prostate Cancer on Posttherapeutic SPECT-CT Follow Up.

A 72-year-old man with castration-resistant prostate adenocarcinoma underwent 68Ga-PSMA-11 PET/CT for restaging due to significantly increased PSA levels during chemotherapy. Imaging revealed high 68Ga-PSMA-11 uptake in bilateral adrenal uptake, with additional uptake in the prostate and few sclerotic bone lesions. MRI and afterwards adrenal gland biopsy confirmed adrenal metastases, an uncommon metastatic site. As the patient was oligometastatic, refractory to previous systemic therapies, and ineligible for surgery or radiotherapy, 177Lu-PSMA-617 radioligand therapy was initiated. After four

Single-cell transcriptomics reveals transcriptional reprogramming and microenvironmental remodeling in ectopic ACTH/CRH-secreting pheochromocytomas.

BACKGROUND: Ectopic ACTH- and/or CRH-secreting pheochromocytomas (ACTH+CRH+PCCs) are rare tumors that can mimic Cushing's syndrome and complicate the diagnosis and treatment clinically. However, the tumor-intrinsic and microenvironmental features of ACTH+CRH+ PCCs remains poorly understood. METHODS: Fresh tumor specimens from three ACTH+CRH+ and three ACTH- CRH- PCCs were profiled by single-cell RNA sequencing. Developmental trajectories and transcriptional regulatory networks were analyzed to investigate tumor-intrinsic programs, while CellChat and NicheNet were used to characterize cell-cel

Testosterone and DHEA-S Secreting Adrenal Cortical Adenoma in a Postmenopausal Woman: A Rare Functional Incidentaloma.

Postmenopausal hyperandrogenism is uncommon and should prompt evaluation for an underlying androgen-secreting source. Although ovarian causes are more frequent, adrenal tumors remain an important consideration and may occasionally be malignant. We report a 53-year-old postmenopausal woman who presented with progressive hirsutism, scalp hair loss, and mild clitoromegaly. Laboratory evaluation showed markedly elevated total testosterone and dehydroepiandrosterone sulfate levels with suppressed gonadotropins. Pelvic ultrasonography showed no ovarian mass. CT demonstrated a 3.5 × 2.4 cm left adren

Post-dexamethasone cortisol levels in non-functioning adrenal incidentalomas: a pooled individual participant data analysis.

INTRODUCTION: Non-functioning adrenal incidentalomas (NFAI) are defined by 1 mg-dexamethasone suppression test (F-1mgDST) cortisol levels < 1.8 µg/dL. However, many individuals with NFAI may exhibit subtle hypercortisolism associated with increased cardiometabolic and skeletal risk. The aims of the present study are to compare F-1mgDST distributions between individuals with NFAI and population-based controls and to explore whether alternative F-1mgDST thresholds would meaningfully modify patient classification. METHODS: This is a pooled individual participant data analysis of 7 studies evalua
