Revisiting the anatomy of the retroperitoneum and renal fascia. Aspects applied to radiology, laparoscopic and robotic surgery - a narrative review.

This review aims to provide a comprehensive analysis of the anatomy of the retroperitoneal and abdominal fascias. We analyzed papers published in the past 70 years in the databases of Pubmed, Embase and Scielo. We excluded case reports, editorials and opinions of specialists. Studies were excluded if they lacked clear anatomical descriptions of the fascias, if full-text access was unavailable, or if they were published in languages other than Portuguese, English, or Spanish. This narrative review addresses the anatomical integration, surgical techniques, and functional importance of the fascia

Preoperative Preparation With Selective versus Nonselective Alpha-Adrenoceptor Blockade for Pheochromocytomas and Paragangliomas: A Systematic Review and Updated Meta-Analysis.

OBJECTIVE: Preoperative preparation with nonselective alpha-blockers (NABs) or selective alpha-blockers (SABs) is crucial for the maintenance of intraoperative hemodynamic stability during surgical resection of pheochromocytomas and paragangliomas (PPGLs). This study aims to compare the efficacy and safety of NAB versus SAB for preoperative preparation in patients with PPGL. METHODS: We systematically searched PubMed, Embase, Web of Science, and Cochrane CENTRAL databases (up to August 2, 2025) for randomized and observational studies directly comparing NAB with SAB. Meta-analyses were perfor

Catecholamine-Secreting Retroperitoneal Paraganglioma: A Case Report and Literature Review.

INTRODUCTION: Paragangliomas are rare neuroendocrine tumors arising from autonomic paraganglia. These tumors have metastatic potential, and histopathological findings alone cannot reliably predict their biological behavior. Clinical manifestations range from the classic triad of headache, diaphoresis, and palpitations to incidental findings in asymptomatic patients. CASE PRESENTATION: A 75-year-old man was incidentally found to have a 5 cm retroperitoneal mass adjacent to the right renal hilum. Biochemical testing confirmed marked catecholamine hypersecretion, and 18F-FDOPA PET/CT demonstrate

Real-world optimization of cutoff values for enzyme immunoassay-based plasma-free fractionated metanephrines for the diagnosis of pheochromocytoma and paraganglioma: a multicenter retrospective study.

Pheochromocytoma and paraganglioma (PPGL) require sensitive biochemical screening to prevent delayed or missed diagnoses. In Japan, plasma-free metanephrines (pMN/pNMN) measured using enzyme immunoassay have been covered by national insurance since 2019; however, the real-world diagnostic performance of the currently applied cutoff values has not been evaluated. We retrospectively identified consecutive adults who underwent pMN/pNMN testing in routine practice at five endocrinology departments between 2019 and 2025, of whom 817 were included in the final analysis. Patients were classified post

Near-complete Response of Bilateral Adrenal Metastases to 177Lu-PSMA Therapy in Castration-resistant Prostate Cancer on Posttherapeutic SPECT-CT Follow Up.

A 72-year-old man with castration-resistant prostate adenocarcinoma underwent 68Ga-PSMA-11 PET/CT for restaging due to significantly increased PSA levels during chemotherapy. Imaging revealed high 68Ga-PSMA-11 uptake in bilateral adrenal uptake, with additional uptake in the prostate and few sclerotic bone lesions. MRI and afterwards adrenal gland biopsy confirmed adrenal metastases, an uncommon metastatic site. As the patient was oligometastatic, refractory to previous systemic therapies, and ineligible for surgery or radiotherapy, 177Lu-PSMA-617 radioligand therapy was initiated. After four

Single-cell transcriptomics reveals transcriptional reprogramming and microenvironmental remodeling in ectopic ACTH/CRH-secreting pheochromocytomas.

BACKGROUND: Ectopic ACTH- and/or CRH-secreting pheochromocytomas (ACTH+CRH+PCCs) are rare tumors that can mimic Cushing's syndrome and complicate the diagnosis and treatment clinically. However, the tumor-intrinsic and microenvironmental features of ACTH+CRH+ PCCs remains poorly understood. METHODS: Fresh tumor specimens from three ACTH+CRH+ and three ACTH- CRH- PCCs were profiled by single-cell RNA sequencing. Developmental trajectories and transcriptional regulatory networks were analyzed to investigate tumor-intrinsic programs, while CellChat and NicheNet were used to characterize cell-cel

Testosterone and DHEA-S Secreting Adrenal Cortical Adenoma in a Postmenopausal Woman: A Rare Functional Incidentaloma.

Postmenopausal hyperandrogenism is uncommon and should prompt evaluation for an underlying androgen-secreting source. Although ovarian causes are more frequent, adrenal tumors remain an important consideration and may occasionally be malignant. We report a 53-year-old postmenopausal woman who presented with progressive hirsutism, scalp hair loss, and mild clitoromegaly. Laboratory evaluation showed markedly elevated total testosterone and dehydroepiandrosterone sulfate levels with suppressed gonadotropins. Pelvic ultrasonography showed no ovarian mass. CT demonstrated a 3.5 × 2.4 cm left adren

Synergistic antitumor effects of 5-fluorouracil and vorinostat in preclinical models of pheochromocytomas and paragangliomas.

PURPOSE: Therapeutic options for metastatic pheochromocytomas and paragangliomas (PPGLs) are largely limited, highlighting the need for novel strategies. This study aimed to investigate the efficacy and safety of 5-fluorouracil (5-FU) and vorinostat (SAHA), alone or in combination in preclinical PPGL models. METHODS: The growth inhibitory effects of 5-FU and SAHA were investigated in PC-12 cells and human PPGL primary cultures (n = 4) using CCK-8 assays. Drug interactions were evaluated utilizing the Chou-Talalay method. Specific mechanisms were explored via flow cytometry, wound healing, and

Post-dexamethasone cortisol levels in non-functioning adrenal incidentalomas: a pooled individual participant data analysis.

INTRODUCTION: Non-functioning adrenal incidentalomas (NFAI) are defined by 1 mg-dexamethasone suppression test (F-1mgDST) cortisol levels < 1.8 µg/dL. However, many individuals with NFAI may exhibit subtle hypercortisolism associated with increased cardiometabolic and skeletal risk. The aims of the present study are to compare F-1mgDST distributions between individuals with NFAI and population-based controls and to explore whether alternative F-1mgDST thresholds would meaningfully modify patient classification. METHODS: This is a pooled individual participant data analysis of 7 studies evalua

[Stratified and graded diagnosis and treatment strategies for patients with primary aldosteronism].

Primary aldosteronism (PA) is the most common endocrine cause of secondary hypertension and carries a greater risk of cardiorenal complications than essential hypertension. Recent domestic and international guidelines and consensus statements have proposed screening all patients with hypertension for PA. To cope with the massive patient base resulting from universal screening, this article explores practical schemes for the stratified and graded diagnosis and treatment of PA. By employing "non-drug withdrawal" and "minimal-drug withdrawal" testing protocols, PA screening can be feasibly implem

Case Report: Somatic GNAS and CACNA1D mutations responsible for adrenal cortisol-producing adenoma with multiple aldosterone-producing micronodules-concurrence of primary aldosteronism and mild autonomous cortisol secretion.

BACKGROUND: The concurrent presence of primary aldosteronism (PA) and mild autonomous cortisol secretion (MACS) caused by adrenal lesions of a solitary cortisol-producing adenoma, accompanied by multiple aldosterone-producing micronodules (MAPM), is exceptionally rare. Moreover, there is a notable paucity of data regarding the molecular pathogenesis of this disease. CASE PRESENTATION: We report a unique case of a somatic GNAS/CACNA1D-mutant cortisol-producing adenoma accompanied by MAPM and an aldosterone-producing nodule (APN) in the adjacent adrenal cortex. The patient was a 63-year-old wom

Conversion therapy followed by curative resection in a patient with ACTH-secreting pancreatic neuroendocrine tumor G3: a case report.

BACKGROUND: Pancreatic neuroendocrine tumors (pNETs) with ectopic adrenocorticotropic hormone (ACTH) production are uncommon, highly aggressive, and carry a dismal prognosis. Early nonspecific symptoms often delay diagnosis, and treatment strategies remain controversial. For patients with locally advanced pNETs, conversion therapy has only rarely been reported and remains a sparingly applied strategy. CASE PRESENTATION: A 37-year-old woman presented with a six-month history of progressive swelling in both lower extremities and her face. Further workup confirmed ACTH-dependent Cushing syndrome

Adrenomedullary Organoids Derived from Neural Crest and Schwann Cell Precursors.

The adrenal gland, essential for survival, contains the medulla and cortex. Chromaffin cells (ChrCs) in the adrenal medulla control the fight-or-flight response by releasing catecholamines like epinephrine and norepinephrine. Recent findings reveal that ChrCs originate from two progenitors: 20% from developmentally early, neural crest-derived sympathoadrenal progenitors (SAPs) and 80% from the later Schwann cell precursors (SCPs). We generated functional adrenomedullary organoids from human pluripotent stem cells (hPSCs) via both pathways and in mixed (20% SAP, 80% SCP-derived) cultures. When

Birthweight and Breastfeeding Shape Patterns of Biological Aging Across Adult Organs.

Early-life nutrition may influence long-term health, but its impact on organ-specific biological aging is unclear. We investigated associations of birthweight and breastfeeding with multi-organ aging and potential mediating pathways in adulthood. Using UK Biobank data, we examined associations of retrospectively reported birthweight (n = 21,446) and breastfeeding history (n = 29,713) with 19 proteomics-derived aging indices, comprising 18 organ-specific indices and one composite index. Multivariable linear regression assessed individual and joint associations, effect modification by sex and so
