Skin biopsies from 20 patients with indeterminate leprosy were studied histopathologically. The most common clinical manifestation of the disease was a hypopigmented macule. In most cases, 5% to 10% of the dermis was occupied by the infiltrate. Sweat glands were involved in two thirds of the cases, and approximately half the biopsies showed involvement of arrector pili muscle and pilosebaceous glands. Peripheral blood T and B lymphocyte percentage and serum immunoglobulin levels were evaluated in 36 Maculo-anaesthetic (MA) leprosy patients using E and EAC rosette techniques and single radial immuno-diffusion respectively. 21 Tuberculoid, 13 Indeterminate, 16 Borderline, 13 Lepromatous and 20 healthy controls were also studied for comparison. The results showed that the peripheral blood T lymphocyte percentages and humoral immune responses of Maculoanaesthetic leprosy are not significantly different from either Tuberculoid or Indeterminate leprosy types or from control group.

A simultaneous clinical and histological study in 82 new, untreated leprosy patients was undertaken using the criteria of Ridley and Jopling. The disparity between the clinical and the histological diagnosis was abundantly clear, because in only 35 patients the two conformed with each other while in others there was a shift of one step towards either the tuberculoid or the lepromatous end of the spectrum. In 18 patients from other groups of leprosy, histologic delineation had features of indeterminate leprosy.

Two aspects of the phagocytic process in normal and lepromatous polymorphonuclears (PMN) and monocytes (MN) were studied. These aspects were endocytosis and adherence to surfaces. These 2 phenomena are closely related to each other and to the whole phagocytic process as they depend on membrane activity. A retrospective study of liver lesions was made in 13 armadillos infected intracutaneously with 10(7) M. leprae from the same inoculum, to evaluate the pathogenesis of the experimental disease. The objective of this paper is to encourage histopathologists to recognize and diagnose leprosy so that it can be treated effectively at the earliest stage possible, before irreversible deformities result. Recognition of the earliest form of leprosy-indeterminate-is emphasized. Histopathologic descriptions are made and illustrated for the principal types of leprosy. Emphasis is placed on (1) the need for dependable acid-fast staining of leprosy bacillus, (2) the non-specific infiltrate in indeterminate leprosy, and (3) the involvement of nerves in all types of leprosy.

Fourteen cases are described in which the local application of corticosteroid preparations to ringworm infections of the skin have resulted in unusual clinical pictures. A kerion-like lesion due to Trichophyton rubrum, intertriginous infections simulating candidiasis and due to Epidermophyton floccosum, and pictures resembling poikiloderma, papular rosacea, and indeterminate leprosy are among the changes that were seen in these patients.
