Cat scratch disease (CSD), caused by Bartonella henselae (B. henselae), typically presents as localized swelling of lymph nodes following a scratch or bite from a cat. It is crucial to differentiate CSD from tuberculosis (TB), particularly in regions where TB is prevalent. This report describes a 56-year-old man who exhibited bilateral swelling of the cervical lymph nodes. Initially, he was suspected to have tuberculous lymphadenitis due to the granulomatous changes observed in a biopsy of the lymph nodes, typical signs of TB on a chest CT scan, and a positive result from an interferon-gamma r

Cat-scratch disease (CSD), a rare infectious disorder typically presenting with systemic manifestations such as fever and lymphadenopathy, uncommonly involves ocular structures, with neuroretinitis being one of its more frequent ocular manifestations. We report an unusual case of CSD presenting as isolated neuroretinitis in the absence of constitutional symptoms. The patient is an 18-year-old female who presented with decreased vision in her left eye and the appearance of shadows. She reported no systemic symptoms such as fever or swollen lymph nodes. Fundus examination revealed optic disc ede

Acute lymphadenopathy has a wide range of possible etiologies, ranging from self-limiting viral infections to life-threatening malignancy. Point of care ultrasound (POCUS) can play a crucial role in identifying lymphadenopathy and ruling out other potential causes of soft tissue swelling, such as hernias, abscesses, and malignancies. Furthermore, POCUS enables the characterization of lymphadenopathy by evaluating the size, shape, echogenicity, and vascularity of the involved lymph nodes, thereby helping to identify the presence and extent of pathology. When performed at the time of initial pre

Bartonella henselae is the causative agent of cat scratch disease (CSD), a zoonotic illness primarily transmitted to humans through scratches or bites from cats and, less commonly, by cat fleas or lice. While the most common presentation is self-limited regional lymphadenopathy in immunocompetent individuals, in immunocompromised patients, such as organ transplant recipients, it can cause systemic infections. This report describes a case of disseminated B. henselae infection in a 66-year-old man with relapsed/refractory multiple myeloma following conditioning chemotherapy and autologous stem c

BACKGROUND: Breast cancer has become the second most common cancer after lung cancer. Patients may present with skin manifestations at the time of initial diagnosis, while erysipel-like carcinoma typically appears later, following initial treatment. This delay increases the risk of misdiagnosis.
CASE PRESENTATION: The patient was a 51-year-old female. A modified radical mastectomy for left breast carcinoma (pT2N3M0, stage IIIC; tumor size 4.6 cm × 4.5 cm × 1.6 cm, 14/21 axillary lymph nodes involved), HER2-positive type, was performed on April 21, 2021. In April 2024 (three years post-surgery

We present a mathematical model of the dynamics of Bacillus anthracis bacteria within the lymph nodes and blood of a host, following inhalation of an initial dose of spores. We also incorporate the dynamics of protective antigen, which is the binding component of the anthrax toxin produced by the bacteria. The model offers a mechanistic description of the early infection dynamics of inhalational anthrax, while its stochastic nature allows us to study the probabilities of different outcomes (for example, how likely it is that the infection will be cleared for a given inhaled dose of spores) in

Yersinia pestis, the causative agent of plague, remains a threat to public health worldwide. From the perspective of developing safe and effective vaccines, we present a derived version of our Y. pseudotuberculosis VTnF1 live attenuated vaccine candidate that lacks the pYV virulence plasmid coding for the Type 3 Secretion system (T3SS) and carries no antibiotic resistance cassettes (VTnF1-S). This strain, named VpYV-, fails to cause disease in immunocompromised mice when given orally, and can be considered as avirulent in such conditions. It retains a tropism for Peyer's patches and mesenteric

Enteroinvasive bacterial pathogens are responsible for an enormous worldwide disease burden that critically affects the young and immunocompromised. Yersinia pseudotuberculosis is a Gram-negative enteric pathogen, closely related to the plague agent Y. pestis, that colonizes intestinal tissues, induces the formation of pyogranulomas along the intestinal tract, and disseminates to systemic organs following oral infection of experimental rodents. Prior studies proposed that systemic tissues were colonized by a pool of intestinal replicating bacteria distinct from populations within Peyer's patch

Tularemia, a life-threatening zoonotic infection caused by Francisella tularensis, is a significant public health concern. Diagnosis can be challenging due to nonspecific symptoms, including fever, tonsillitis, and lymphadenopathy. We present the case of a 48-year-old male from a rural area of the Tavush region admitted to our hospital, initially presenting with left-sided tonsillitis, followed by the development of enlarged lymph nodes in the neck and a mild fever. Despite initial treatment with amoxicillin, tularemia was confirmed through positive serological tests, including antibody titers

Tularemia, caused by Francisella tularensis, is a zoonotic infection that typically presents with lymphadenopathy, acute tonsillitis, or ulcerative skin lesions. Various infectious agents and malignancies may underlie lymphadenopathy. Although necrotizing granulomatous reactions are common in suppurative lymphadenitis, caseating granulomas are rare and often raise suspicion for tuberculosis. We report two tularemia cases presenting with lymph node enlargement and caseating granulomas on histopathology. Case 1 involved a 27-year-old woman with axillary lymphadenopathy and no systemic symptoms.

Tularemia, also known as rabbit fever, is a rare zoonotic infectious disease transmitted by air, arthropods, food, and water caused by bacteria of the genus Francisella. Because of its multiple transmission routes and low infectious doses, Francisella tularensis is listed as a biological weapon. In rare cases, tularemia can lead to pneumonia and necrotizing mediastinal and hilar lymph nodes. Herein we present the case of a 43-year-old female patient with an isolated mediastinal mass, initially thought to be a solid tumor. Further investigations led to the diagnosis of tularemia. The patient's

Although often considered a historical disease, plague remains a modern-day public health concern with global relevance. Caused by the bacterium Yersinia pestis and transmitted to humans primarily through infected flea bites or contact with infected animals, plague persists worldwide, including parts of the southwestern United States. The disease presents in three main forms: bubonic, septicemic, and pneumonic. Bubonic plague, the most common form, is characterized by painful swollen lymph nodes known as buboes. Septicemic plague occurs when the infection enters the blood stream, while pneumon

Bacteria in the Mycobacterium tuberculosis complex and nontuberculous mycobacteria may affect a variety of animal species under human care and pose public health risks as zoonotic pathogens. A case of sudden onset of lethargy and increased respiratory effort in a 5-y-old, intact female reindeer (Rangifer tarandus) under managed care had progressed to severe dyspnea despite aggressive treatment. The animal was euthanized due to poor prognosis. Postmortem findings included: disseminated miliary nodules in the lungs, pleura, small intestine, liver, and spleen; enlarged mesenteric lymph nodes; dil

Atypical forms of tuberculosis (TB) are gaining recognition, challenging traditional diagnostic criteria classically linked to chronic cough, lethargy, anorexia, evening pyrexia, and weight loss in immunocompromised patients. We present the case of a 23-year-old immunocompetent man from rural central India who developed unilateral pulmonary miliary TB with concurrent abdominal TB lymphadenitis. The patient presented with a two-month history of chronic cough and diffuse abdominal pain. Initial chest radiograph revealed unilateral, right-sided miliary opacities, while abdominal ultrasonography i

Sarcoidosis is a chronic, idiopathic, multisystemic inflammatory disease characterized by non-caseating granulomas, most commonly affecting the lungs and mediastinal lymph nodes. Radiological imaging plays a fundamental role in the diagnosis, assessment of disease extent, and differentiation from other pulmonary conditions. This narrative review offers a comprehensive overview of the imaging features of pulmonary sarcoidosis, focusing on both typical patterns-such as bilateral hilar lymphadenopathy, perilymphatic nodules, and upper lobe-predominant infiltrates-and atypical manifestations-inclu

Tuberculosis (TB) may co-occur with Takayasu arteritis (TA), a large vessel vasculitis, thus complicating the management, particularly in endemic areas. We report the case of a 17-year-old female who presented with severe headache, blurred vision, giddiness, seizures, syncope, and hypertensive urgency (200/110 mmHg). Renal Doppler demonstrated left renal artery stenosis (RAS), and magnetic resonance imaging showed posterior reversible encephalopathy syndrome (PRES). TA was suggested by raised inflammatory markers (erythrocyte sedimentation rate: 70 mm/h, C-reactive protein: 212 mg/L) and compu

Introduction: Tuberculosis (TB), caused by Mycobacterium tuberculosis, primarily affects the lungs but can involve virtually any organ system, manifesting as extrapulmonary TB. While TB-related hypercoagulability and venous thromboembolism are recognized, such presentations remain uncommon and diagnostically challenging, especially in the absence of classical symptoms. Case Presentation: We report the case of a 24-year-old immunocompetent female who initially presented with painless right-sided neck swelling. Imaging revealed an acute thrombus in the right internal jugular vein (IJV), with no

Tuberculosis of the cerebral nervous system is a diagnostic and therapeutic emergency that can lead to death and sometimes disabling sequelae. It can be complicated by stroke in the context of vasculitis. We report a pediatric case. We report the case of a 13-year-old child who presented with a sudden onset of altered consciousness in the context of fever. He had been experiencing unexamined cervical lymphadenopathy and a cough for a year. On day 4 of hospitalisation, he developed a right pyramidal syndrome. Cerebral magnetic resonance imaging revealed a deep middle cerebral artery infarction.
