The effect of toll like receptors in prognosis in patients undergoing radical cystectomy for bladder cancer.

BACKGROUND: We aimed to evaluate the prognostic significance of TLR4 expression in bladder cancer, its association with genes related to EMT, and the potential regulatory role of selected microRNAs. METHODS AND RESULTS: Tumor and adjacent healthy bladder tissue samples obtained from 25 patients who underwent radical cystectomy for bladder cancer were analysed. The expression levels of TLR4, SPRR2A, SPRR1A, SPRR2B, ZEB2 and CDH1, in addition to miR-200c-3p and miR-146a-5p were measured using the RT-PCR method. Additionally, the expression of the TLR4 protein was assessed using the immunohistoc

Molecular insights into uterine fibroids: crosstalk between SHH and Wnt pathways, DNA methylation, and miRNAs regulation.

Uterine leiomyomas (ULMs) are the most common benign gynecological tumors affecting women in reproductive age. Symptoms such as abnormal uterine bleeding, pelvic pain, and infertility may occur, leading to a significant decline in patients' quality of life. In addition, ULMs represent the second leading indication for gynecological surgery in Brazil, imposing a substantial economic burden on the public healthcare system. However, despite their high prevalence and clinical impact, there is still no curative clinical treatment for these tumors, and the molecular mechanisms underlying their devel

Expected costs and benefits of genetic counselling and germline genetic testing in metastatic prostate cancer.

Identifying pathogenic germline variants in men with metastatic prostate cancer is important for therapeutic options and for identifying relatives who may have a high cancer risk. To keep genetic testing costs manageable, it is important to identify which patients should be selected for testing. In this study, we compared expected costs and number of identified pathogenic variants in two scenarios: offering genetic testing to all men with metastatic prostate cancer versus testing only those with additional risk factors linked to a higher likelihood of carrying a pathogenic variant. Costs were

The evolution of hereditary cancer genetic counselling: mainstreaming, service redesign and patient experience in Lynch syndrome.

Genetic counselling for hereditary cancer predisposition has evolved substantially over more than three decades, driven by advances in genomic technologies and the growing use of tumour and germline testing. These developments have expanded access to hereditary cancer testing, treatment and prevention, while also introducing new clinical, psychosocial and system-level challenges. This review examines the transition from traditional specialist-led genetic counselling to mainstreamed testing pathways. Using Lynch syndrome as a paradigm, we explore how service redesign, personalised risk communic

The identification of a PRKACA duplication at 19p13.12 in a female with PPNAD and thyroid carcinoma after a 20-year diagnostic journey.

Rare pathogenic variants affecting components of the evolutionary conserved cAMP/protein kinase A (PKA) signalling pathway are implicated in a spectrum of adrenocortical disorders. Germline inactivating PRKAR1A variants leading to constitutive PKA activation, underlie primary pigmented nodular adrenocortical disease (PPNAD) in Carney complex. More recently, several patients with PPNAD and other types of bilateral nodular adrenal disease (BNAD) have been diagnosed with rearrangements in chromosome 19 at band p13.12, resulting in copy number gains of the entire PRKACA gene. Here, we report the i

Disruption of cellular timekeeping in cancer: links between circadian clocks, metabolism, and tumor progression.

Disruption of cellular timekeeping systems is a key driver of cancer cell development and progression. The circadian clock is a 24-hour regulatory system that relies on core clock genes - CLOCK, BMAL1, PER, and CRY. Maintaining cellular homeostasis is essential, as it regulates processes such as the cell cycle, metabolism, DNA repair, and apoptosis. Disruption of Circadian rhythms is common in cancer, altering gene expression and causing the mis-timing of cellular events. This disturbance stimulates excessive growth, genetic instability, and resistance to cell death. Circadian rhythms and tumo

Eco-Evolutionary Dynamics of Proliferation Heterogeneity: A Phenotype-Structured Model for Tumor Growth and Treatment Response.

Intra-tumor heterogeneity in proliferation rates fundamentally influences cancer progression and treatment resistance. To investigate how continuous phenotypic variation shapes eco-evolutionary dynamics, we develop a phenotype-structured partial differential equation framework that explicitly models proliferation heterogeneity as a dynamic trait. Our model integrates three key biological principles: (1) phenotypic diffusion capturing heritable variation in proliferation rates, (2) global resource competition enforcing density-dependent growth constraints, and (3) an experimentally grounded lif

Real-World-Feasible Immunohistochemistry of ATRX, DAXX, and Menin Identifies a Subgroup of Non-Functioning Pancreatic Neuroendocrine Tumors with low Recurrence Risk to Guide De-Escalating Surveillance.

Non-functioning pancreatic neuroendocrine tumors (NF-pNETs) show a variable prognosis. Despite 40-60% of patients remaining recurrence-free after surgery, guidelines recommend ≥ 10 years of follow-up. Recent studies identify prognostic subgroups based on ATRX, DAXX, and MEN1 mutations and chromosomal aneuploidy, highlighting a subgroup with favorable prognosis. We aimed to classify resected NF-pNETs into molecular subgroups using ATRX, DAXX, and Menin immunohistochemistry as surrogate markers for genomic status. To do so, we retrospectively collected resected primary sporadic grade 1 and 2 NF-

Chrysin treatment is associated with altered microrna expression and reduced levels of HPV16 E6 protein in caski cervical cancer cells.

Persistent infection with high-risk human papillomavirus type 16 (HPV16) is a major cause of cervical cancer and is associated with viral oncoproteins that disrupt tumor suppressor pathways and influence cellular microRNA (miRNA) expression. Given increasing interest in phytochemicals as potential anticancer agents, this study evaluated the effects of chrysin, a natural flavonoid, on cell viability, selected cancer-related miRNAs, and HPV16 E6 protein levels in HPV16-positive CaSki cervical cancer cells. CaSki cells and normal human dermal fibroblasts (HDF) were treated with chrysin (31.5-500

Robotic-assisted compartmental dissection for retroperitoneal sarcoma: a feasibility study on cadavers with the da Vinci Xi system.

Retroperitoneal sarcomas (RPS) pose major technical challenges because of their size, anatomical complexity, and need for multivisceral resection. We evaluated the feasibility of a standardized robotic approach for right- and left-sided retroperitoneal sarcoma (RPS) compartmental resection using the da Vinci Xi system in tumour-free human cadavers. In a two-phase IDEAL-D Stage 0 cadaveric study, two sarcoma surgeons first compared four trocar configurations in two cadavers to identify optimal port templates (Phase 1). Complete six-stage compartmental resection following a standardised protocol

miR-142-3p in cancer: intracellular tumor suppression and extracellular vesicle- mediated microenvironmental signaling.

miR-142-3p has emerged as an important regulator of cancer progression and treatment response, exhibiting predominantly tumor-suppressive functions. Intracellularly, miR-142-3p inhibits proliferation, promotes apoptosis, suppresses epithelial-mesenchymal transition (EMT), and enhances chemosensitivity through the downregulation of key pathways, including HMGB1, Wnt/β-catenin, and SIRT1-mediated autophagy. Notably, miR-142-3p is frequently enriched in extracellular vesicles (EVs), indicating selective export from tumor cells. This EV-mediated trafficking introduces functional complexity, as the
