A Crisis of Clarity: When Everyone Is Ordering From a Different Menu.

This commentary explores the fragmented landscape of pharmacy education assessment frameworks and their impact on curricular design. Pharmacy education operates much like a restaurant kitchen, where multiple stakeholders influence what appears on the menu. Tools such as the Accreditation Council for Pharmacy Education Standards 2025 (including the Curricular Outcomes and Entrustable Professional Activities and Appendix 1), American College of Clinical Pharmacy Pharmacotherapy Toolkit, and National Association of Boards of Pharmacy North American Pharmacist Licensure Examination competencies al

A role for vagal activity in preventing the suppression of glucagon secretion by GLP-1 during hypoglycemia.

Glucagon-like peptide 1 (GLP-1) is generally safe against hypoglycemia, although it stimulates insulin and inhibits glucagon secretion. One explanation is that glucagon secretion is not inhibited by GLP-1 during hypoglycemia. We aimed at understanding the lack of suppression of glucagon secretion by GLP-1 by exploring the paracrine and neural regulation of pancreatic hormone secretion during hypoglycemia. Isolated rat pancreas (A) and an organ block comprising pancreas and stomach (B) were perfused. We performed 1) dose-response studies with GLP-1 (7-36) at hypoglycemia; 2) studies with GLP-1

Efficacy of autologous intestinal reconstruction surgery on bowel dilatation in pediatric small bowel syndrome.

OBJECTIVES: Pediatric short bowel syndrome (SBS) may cause excessive bowel dilatation leading to worsened outcomes and autologous intestinal reconstruction (AIR) surgery. We addressed efficacy of AIR surgery by measuring duodenal, small bowel and colon dilatation in SBS children with and without AIR surgery in relation to parenteral nutrition (PN) dependency and healthy control patients. METHODS: SBS children having undergone AIR surgery (AIR+, n = 22) and those without AIR surgery (AIR-, n = 56) were included. Intestinal contrast series performed between 2002 and 2020 were analyzed to measur

Impact of intra- and interfractional motion on the dose distribution and urinary adverse events for magnetic resonance-guided prostate cancer radiotherapy.

PURPOSE: To evaluate the impact of intra- and interfractional motion on dose distribution and clinical outcome in prostate magnetic resonance (MR)-guided adaptive radiotherapy. METHODS: Seventy-six patients were treated with five-fraction stereotactic radiotherapy on a 1.5 T MR linear accelerator. Cine MR images were acquired during treatment and exported to a motion monitoring research package, to generate motion traces in the anterior-posterior (AP), superior-inferior (SI) and left-right (LR) directions. Planned dose distributions were shifted according to motion trace to reconstruct intraf

2026 American Association for Thoracic Surgery (AATS) expert consensus statement: Antithrombotic management of neonates/infants undergoing cardiac interventions.

OBJECTIVES: The hemostatic system in neonates and infants with congenital heart disease is distinct as a result of developmental and disease-specific factors, necessitating population-specific periprocedural evaluation and antithrombotic management. Ongoing practice variation reflects limited evidence and highlights the need for structured guidance and further study. METHODS: To develop a more unified management strategy for periprocedural antithrombotic testing and therapy in this vulnerable population, a group of experts (cardiac surgeons, cardiologists, interventionalist, hematologists, in

Short-Term Outcomes and Quality of Life After Robotic Versus Laparoscopic Double-Flap Technique for Proximal Gastrectomy: A Retrospective Cohort Study.

BACKGROUND: Robotic systems may facilitate the double-flap technique (DFT) after proximal gastrectomy (PG), yet the superiority of robotic DFT following PG (RPG-DFT) over laparoscopic PG with DFT (LPG-DFT) remains unclear. METHODS: Seventy-three gastric cancer patients (28 RPG-DFT, 45 LPG-DFT) were compared for baseline characteristics, perioperative outcomes, and 3-month postoperative data. Gastroesophageal Reflux Disease-Health Related Quality of Life (GERD-HRQL) and weight loss are postoperative primary endpoints. RESULTS: RPG-DFT had significantly longer operative time (308.3 vs. 204.5 m

The impact of war on Primary Health Care in Ukraine: a cross-sectional survey and qualitative interviews with service providers.

BACKGROUND: Primary Health Care (PHC) is vital to supporting emergency preparedness and health care resilience. There is limited evidence of the impact of crises on PHC services and financing. We aimed to explore the impact of the full-scale invasion of Ukraine in February 2022 on PHC services in the country. METHODS: We used a mixed-methods approach. Survey data were collected using an online questionnaire sent to a sample (n = 86) of PHC providers in Ukraine in January-February 2023. Fifteen providers were then randomly selected for semi-structured interviews from among those that reported

Rare Presentation of Necrotic Appendicitis and Small Bowel Obstruction in an Elderly Man: De Garengeot Hernia.

De Garengeot hernia, characterized by the presence of the vermiform appendix within the femoral hernia sac, is a rare condition often diagnosed only intraoperatively. The incidence of acute appendicitis within a De Garengeot hernia is even more uncommon, and its clinical presentation may be atypical, making preoperative diagnosis difficult. This report describes the case of a 67-year-old male patient with multiple comorbidities, including an abdominal aortic aneurysm, who presented with right inguinal pain and bulging, culminating in the intraoperative diagnosis of a De Garengeot hernia with a

[Strategies for the prevention and treatment of peritoneal metastases in large Borrmann type III and type IV gastric cancer].

Large Borrmann type III (with maximum tumor diameter ≥8 cm) and Borrmann type IV gastric cancers typically show diffuse infiltration with ill-defined borders and a high frequency of serosal invasion and positive peritoneal lavage cytology, resulting in a high risk of postoperative peritoneal recurrence. Current clinical strategies focus on preventing peritoneal metastasis or recurrence, emphasizing the optimization of perioperative systemic therapy based on precise stratification through staging laparoscopy combined with peritoneal lavage cytology, and exploring the rational combination of tri

Multidisciplinary management of a patient with vesicosigmoid fistula and multisystem diseases undergoing stoma reversal: a case report.

BACKGROUND: Vesicosigmoid fistula is a rare and complex pathological connection between the urinary and digestive systems, most often caused by calculi, diverticulitis, or malignancy. In elderly patients, surgical repair of the fistula and stoma reversal carry high risks, particularly when complicated by dysfunction of multiple organ systems such as the heart, lungs, and kidneys. This case aims to summarize the perioperative risk assessment and comprehensive management strategies for an elderly patient undergoing stoma reversal after vesicosigmoid fistula repair. CASE PRESENTATION: A 73-year-o

Preliminary Investigation of a Transcutaneous Ultrasound-Guided Technique for Pudendal Nerve Block in Six Horse Cadavers.

Regional anaesthesia of the equine anogenital tract is limited to local infiltration, extradural, blind palpation and nerve stimulator-guided techniques which risk iatrogenic damage, recumbency and ataxia. This study aimed to describe and assess the feasibility of transcutaneous ultrasound-guided (USG) pudendal nerve staining in equine cadavers. An initial pilot phase used two fresh equine cadavers and one standing unsedated horse to image the intrapelvic anatomy using ultrasound. One fixed equine cadaver specimen was also dissected to identify the pudendal nerve and refine the dissection appr

Weekly Oral Prophylaxis With MK-8527 Protects Rhesus Macaques From Intrarectal Challenge With Simian-Human Immunodeficiency Virus.

BACKGROUND: MK-8527 is a novel nucleoside reverse transcriptase translocation inhibitor being evaluated for prevention of HIV-1 acquisition. MK-8527 is phosphorylated intracellularly to its active form, MK-8527-triphosphate, which inhibits HIV-1 replication. This study evaluated the efficacy of oral MK-8527 as pre-exposure prophylaxis in rhesus macaques challenged intrarectally with simian-human immunodeficiency virus (SHIV). METHODS: Two groups of male macaques (n = 8 per group) received weekly oral doses of MK-8527 for 12 weeks. Starting 1 week after treatment initiation, the macaques recei

Emerging treatment paradigms for gastric cancer peritoneal metastasis in the era of multimodal precision therapy.

Gastric cancer peritoneal metastasis (GCPM) is usually diagnosed when the disease has already reached a progressive or advanced stage and is associated with a distinctly poor prognosis. Because early detection is difficult and the benefit of conventional systemic therapy remains limited, GCPM has long been regarded as a difficult-to-treat subtype in routine clinical practice. In recent years, advances in molecular classification have driven the exploration of individualised multimodal treatment strategies, with systemic approaches such as immune checkpoint inhibitors, molecularly targeted agen

Integrated optimization of needle paths and dwell time for individualized template-guided interstitial brachytherapy.

BACKGROUND: 3D-printed individualized template (3D-PIT) guided interstitial brachytherapy (ISBT) is an effective treatment modality for cervical cancer. However, the current practice of pre-operative planning requires manual needle paths design, which is highly dependent on operator experience and may lead to unnecessary insertion trauma. PURPOSE: To address this issue, this study introduces and validates an automated integrated optimization method for needle paths and dwell times for an institutional protocol of 3D-PIT based ISBT for locally recurrent cervical cancer. METHODS AND MATERIALS:

Unilateral biportal endoscopic transpedicular intrabody cage augmentation for a metastatic spinal tumor with pathologic fracture: a technical note and case report.

INTRODUCTION: Metastatic spinal tumors (MSTs) may cause pathologic burst fractures with severe pain and neurologic compromise, often requiring circumferential decompression and anterior column reconstruction. Traditional open corpectomy is associated with substantial surgical morbidity. Transpedicular intrabody augmentation has emerged as a minimally invasive alternative; however, an endoscopic approach using UBE for oncologic anterior reconstruction has not been well described. CLINICAL PRESENTATION: A 73-year-old man with polymetastatic colon cancer presented with debilitating axial back pa
