Lyon endoscopic submucosal dissection score: a preprocedure prediction model for operating time in colorectal endoscopic submucosal dissection.

BACKGROUND AND AIMS: Colorectal endoscopic submucosal dissection (ESD) is an effective curative technique for superficial colorectal lesions but remains technically demanding, with variable procedure times. This study aimed to develop and validate a preprocedure predictive model for colorectal ESD duration in the era of traction-assisted strategies. METHODS: We retrospectively analyzed prospectively collected data from patients who underwent colorectal ESD at Edouard Herriot Hospital (Lyon, France). The derivation cohort included 423 cases (January-December 2024), and an independent temporal

Automated dispensing cabinets and nurse-related medication errors in inpatient settings: A systematic review.

BACKGROUND: Automated Dispensing Cabinets (ADCs) are computerized medication storage and dispensing systems increasingly used in inpatient hospital settings to support safe medication administration. As nurses represent the final point of medication delivery to patients, understanding the impact of ADCs on nurse-related medication errors is critical. This systematic review aimed to assess the association between ADC implementation and medication errors involving nurses in inpatient hospital settings. METHODS: A systematic search of PubMed, MEDLINE, Scopus, and Google Scholar identified 16 eli

Subacute Appendicitis Within a De Garengeot Hernia: A Rare Case and Management Considerations.

INTRODUCTION: De Garengeot hernias are a rare type of femoral hernia which contains the appendix inside the hernia sac. Prompt surgical intervention is required when appendicitis develops within the hernia. Diagnosis and management are often challenging due to the uncommon nature of this condition. CASE PRESENTATION: We report the case of a 61-year-old female who presented to the clinic with right lower quadrant pain and an erythematous bulge in the right groin. A CT scan confirmed the presence of a femoral hernia with an incarcerated appendicitis. She was managed by the general surgery team

Anticoagulation, recurrent thrombosis, and major bleeding in antiphospholipid syndrome: UK multicenter observational study.

Antiphospholipid syndrome (APS) is an acquired autoimmune disease characterized by the presence of thrombosis and/or pregnancy morbidity with persistently positive antiphospholipid antibodies (aPL). Due to the heterogeneity and relative rarity of this disorder, clinical practice in the management of APS remains varied. In this retrospective UK-wide multicenter study, we aimed to delineate clinical practice and outcomes in thrombotic APS to improve the areas of limited knowledge, particularly anticoagulant practice, thrombotic recurrence, and bleeding. Anticoagulation in Antiphospholipid Syndro

Incidence and Predictors of Neoplastic Diagnosis in Patients with a Clinically Abnormal Appendix.

BACKGROUND: Appendiceal abnormalities, beyond typical acute appendicitis, are occasionally detected on imaging or endoscopy, leading to surgical evaluation. How often these reflect an underlying neoplasm, however, remains unclear. This study aimed to determine the incidence and associated findings of neoplastic disease in this setting. PATIENTS AND METHODS: Patients undergoing diagnostic appendectomy following abnormal findings in endoscopy or imaging at an academic cancer center were reviewed (2011-2021). Patients with preoperative diagnoses of acute appendicitis without a "suspicious" lesio

Three in one: a case report of mixed neuroendocrine tumor, goblet cell adenocarcinoma and low-grade mucinous neoplasm in a single appendix.

Appendiceal neoplasms (AN) are an uncommon condition that are typically diagnosed incidentally following an appendicectomy for appendicitis. However, there has been an increased incidence of AN, thought to be secondary to the increased use of computer tomography scans. Therefore, understanding of a broad range of differentials and management of potential AN is necessary for general surgeons. This case report describes an exceptionally rare occurrence of three distinct neoplastic processes-mixed neuroendocrine tumor, goblet cell adenocarcinoma, and low-grade appendiceal mucinous neoplasm-coexis

Scimitar Syndrome Incidentally Identified During the Workup for Acute Appendicitis in a Young Adult Female: A Case Report.

While congenital pulmonary venolobar syndrome (CPVS), including Scimitar syndrome and horseshoe lung, is highly uncommon in adults, acute appendicitis is a common surgical emergency. This case report emphasizes the difficulties in diagnosing patients and the possibility of misdiagnosis, highlighting the significance of clinical awareness for the best possible care. A laparoscopic appendectomy was performed on a 21-year-old female patient who presented with symptoms of acute appendicitis. During the procedure, a suspicious cecal tumor and a subserosal, dilated retrocecal appendix were observed.

Digital cholangioscope-assisted endoscopic direct-vision appendicitis therapy versus laparoscopic appendectomy for chronic appendicitis: a multicenter retrospective cohort study.

BACKGROUND: Laparoscopic appendectomy (LA) is the conventional treatment for chronic appendicitis. Endoscopic direct-vision appendicitis therapy (EDAT) has emerged as a minimally invasive, organ-preserving alternative. OBJECTIVES: To compare the efficacy and safety of EDAT versus LA in chronic appendicitis. DESIGN: Multicenter retrospective cohort study with propensity score matching (PSM). METHODS: This multicenter retrospective cohort study was conducted in China between January 2019 and February 2024. PSM was performed to minimize selection bias. A total of 186 patients with chronic appe

Three New Cryptic Species of the Freshwater Shrimp Genus Caridina (Decapoda: Caridea: Atyidae) from Hong Kong, with Notes on the C. serrata Species Group.

Three new species of freshwater atyid shrimp, namely Caridina chui sp. nov., C. bauhinia sp. nov. and C. argilla sp. nov., are described from Hong Kong, southern China using an integrative taxonomic approach. All three species belong to the C. serrata species group and differ from the other members in characteristics of the rostrum, pereiopods, and male first and second pleopods. Phylogenetic analyses based on mitochondrial COI and 16S rRNA sequences show that the C. serrata species group is polyphyletic and comprises at least four lineages. The validity of the C. serrata species group is disc

Microbiota evaluation in acute appendicitis: a preliminary study of appendix and childhood oral microbiota.

Despite the uncertainty of the sequence of events leading to appendicitis, the microbiome is presumed to play a central role in the pathogenesis. This preliminary cross-sectional study aimed to evaluate the intestinal microbes in distinct phases of acute appendicitis compared to the oral microbiota, considering the status of dental caries. Twenty children under 12 years were assigned into two groups: non-complicated appendicitis (NCA, n = 11) and complicated appendicitis (CA, n = 9). Biological material from the oral cavity (saliva) and appendix (mucosal scrapings) was submitted to microbial a

Clinical characteristics and outcomes of patients with goblet cell adenocarcinoma of the appendix treated with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC): A multicenter cohort study.

INTRODUCTION: Goblet cell adenocarcinoma (GCA) is a rare, aggressive type of appendix cancer (AC) often presenting with peritoneal disease (PD). While cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is standard for other AC subtypes, its role in GCA, particularly with high peritoneal cancer index (PCI), remains controversial. METHODS: We conducted a retrospective analysis of two prospective databases, including patients with GCA undergoing first CRS/HIPEC. We described clinical characteristics and compared outcomes between PD patients with high (≥20) and low PC

Operative and non-operative management of acute appendicitis in children: a narrative review.

Appendicitis represents a prevalent medical condition among pediatric and adolescent populations, often necessitating emergency surgical procedures. For nearly two hundred years, the management of appendicitis has been conceptualized as a surgical challenge. In contemporary practice, minimally invasive techniques enable appendix removal with reduced anesthetic and operative risks, as well as shorter hospitalization. Nevertheless, both anesthesia and surgical interventions carry potential adverse effects, such as surgical site infections, hemorrhage, herniation, and visceral injury. Furthermore

[A Case of Appendiceal Neuroendocrine Tumor Diagnosed after Appendectomy].

A 49‒year‒old woman presented to our hospital with right lower abdominal pain that had been presenting for about 6 days without any improvement. An abdominal CT examination demonstrated an enlarged appendix and stercolith of appendix with inflamed surrounding adipose tissue. Antibiotic treatment was administered, but an abdominal CT examination taken 3 days later showed worsening inflammation around the appendix. An appendectomy was laparoscopically performed. Upon resection, the enlarged appendix was found. In immunohistochemical staining, the resected tissue was positive for neuroendocrine m

A protocol for a pilot randomised controlled trial of an online cancer bereavement group.

BACKGROUND: The loss of a loved one to cancer brings unique difficulties that affect the bereavement experience. Mixed evidence exists for the effectiveness of in-person group interventions for bereaved caregivers, with barriers to accessing support including perceived stigmatisation and geographical constraints. Online bereavement interventions offer an accessible means of providing grief support at the individual and group level. Research supports the effectiveness of online groups for those bereaved by cancer although most existing studies have examined peer-led online groups only, while em

Reintroduction of Captive Tigers: Challenges & Concerns.

The tiger (Panthera tigris) is an apex predator and plays a fundamental role in sustaining biodiversity through its native range in Asia. By controlling populations of large herbivores and mesopredators, tigers help maintain the structural integrity of habitats, thereby supporting a diverse array of flora and fauna. Despite its ecological importance, the tiger is one of the most threatened large carnivores globally. It is classified as Endangered on the IUCN Red List, although threat categories vary among subspecies, and it is listed in Appendix I of the Convention on International Trade in En

A Rare Case of Isolated Hindgut Malrotation with Internal Hernia and Coexisting Enteric Duplication Cyst.

An eight-year-old boy presented with a history of intermittent episodes of spastic abdominal pain accompanied by bilious vomiting. Imaging revealed an abnormal position of the large bowel loop, which was associated with an abnormal vascular course. Additionally, a mesenteric cystic lesion was found in relation to the small bowel. A possibility of isolated hindgut malrotation with an enteric duplication cyst was suggested based on these findings. The surgical team confirmed this diagnosis during the procedure. Furthermore, they discovered an internal hernia intraoperatively, with a defect in th

Transcolonic Endoscopic Appendectomy: A Single-Step "Two-In-One" Approach for Recurrent Appendicitis With Appendiceal Orifice Lesions.

OBJECTIVES: Appendiceal orifice lesions are occasionally detected during colonoscopy in patients with recurrent appendicitis. Conventional surgery with partial cecal resection may cause unnecessary trauma. Transcolonic endoscopic appendectomy (TEA) has been proposed as a minimally invasive alternative. We aimed to evaluate the feasibility, safety, and efficacy of transcolonic endoscopic appendectomy and to compare through-the-scope twin clips (TTS-TC) combined with metal clips versus metal clips alone for defect closure. METHODS: Twenty patients with recurrent appendicitis and no prior abdomi
