Thymic hyperplasia in patients with acromegaly: imaging-based assessment and clinical implications.

OBJECTIVE: We aimed to compare thymus visibility and volume in patients with acromegaly to those of controls, and to evaluate the correlation between growth hormone (GH), insulin-like growth factor-1 (IGF-1) levels, and thymus volume. SUBJECTS AND METHODS: This cross-sectional case-control study included 66 patients with acromegaly who underwent thoracic computed tomography scans. An age- and sex-matched control group of 65 trauma patients without comorbidities was selected. All thoracic computed tomography scans were reassessed to measure thymus volume using imaging software. Comparisons of

HSV-Net: hybrid state-space and self-supervised vision fusion with progressive contour learning for CT pulmonary nodule segmentation.

Accurate voxel-level segmentation of pulmonary nodules on chest CT is needed for diameter and volume measurement, growth tracking, and quantitative assessment in lung cancer screening. The task remains difficult: nodules vary in attenuation, size, and contact with vessels or pleura, and clinically relevant morphology often lies in a thin, ambiguous margin. Existing systems commonly trade contour fidelity for speed-large prompt-driven foundation models can be accurate but slow, whereas compact CNN or SSM decoders are efficient yet tend to smooth spiculated and ground-glass borders. We propose H

Utility and Safety of a Novel Preoperative Marking via Ultrathin Bronchoscopy in Thoracic Surgery.

PURPOSE: Virtual-assisted lung mapping (VAL-MAP) is a safe and useful procedure for identifying suitable lines for sublobar lung resections. Nonetheless, the current VAL-MAP technique using a thin bronchoscope limits access to peripheral lung regions, including the upper lobes or the S6 region. METHODS: To overcome this limitation, we developed a new technique utilizing an ultrathin bronchoscope (UTB), termed 'UTB VAL-MAP', enabling precise dye marking of the visceral pleura which is difficult to access with a thin bronchoscope. We retrospectively evaluated the efficacy and safety of UTB VAL-

Development and validation of a CT-based nomogram for predicting invasiveness in solitary pulmonary ground-glass nodules.

This study aimed to develop and validate a nomogram based on clinical and computed tomography (CT) features for the preoperative prediction of invasiveness in patients with solitary pulmonary ground-glass nodules (GGNs). We retrospectively analyzed 846 patients with solitary GGNs who underwent surgical resection at the Lung Cancer Center of West China Hospital between November 2022 and January 2024. All nodules were pathologically confirmed as atypical adenomatous hyperplasia (AAH), adenocarcinoma in situ (AIS), minimally invasive adenocarcinoma (MIA), or invasive adenocarcinoma (IAC). Patient

Safety and efficacy of a novel four-hook needle versus hook wire for CT-guided preoperative localization of pulmonary nodules: a propensity score-matched analysis.

BACKGROUND: To compare the safety and effectiveness of a novel four-hook needle with conventional Hook wire for CT-guided preoperative localization of pulmonary nodules and to identify factors associated with localization-related complications. METHODS: A retrospective study was conducted in patients who underwent CT-guided localization followed by video-assisted thoracoscopic surgery between January 2024 and June 2025. Propensity score matching yielded 248 matched pairs. Post-matching comparisons were performed using paired statistical methods. Generalized estimating equation models with the

Regulation of thymus and thymopoiesis by thyroid hormones and corticosteroids: Insights from vertebrate metamorphosis.

In vertebrates, the thymus is maximally active during the early life, when it coordinates the T cell developmental proceedings with the environmental cues to generate a pool of T cells responsible for life-long immunity. Apart from cytokines and growth factors, hormones play a pivotal role in bridging the thymic microenvironment with the external and internal macroenvironment of the host during the early life. The post-embryonic life in lower vertebrates (like fish and amphibians) involves a period of radical physiological remodeling, known as metamorphosis. Two endocrine axes: the thyroid hor

Case Report and literature review: complications of the association between rheumatoid arthritis and tuberculosis.

BACKGROUND: Pulmonary tuberculosis is one of the major public health issues globally. According to the World Health Organization, it is among the top 10 causes of death caused by a single pathogen. In Romania, its reported incidence rate in 2024 was approximately 60 per 100,000 people, making it the country with the highest incidence in Europe. Rheumatoid arthritis (RA) is a systemic chronic autoimmune inflammatory disease characterized by the destruction of the small joints along with symmetrical distribution and multiorgan involvement. Pulmonary manifestations are more common in males with p

Time trends of human papillomavirus-attributable head and neck cancers: the BROADEN study.

BACKGROUND: The incidence of human papillomavirus (HPV)-driven oropharyngeal cancer (OPC) is increasing in several Western countries, but this trend is less well understood in other geographical regions and for other head and neck cancers (HNC). The BROADEN (absolute BuRden Of HPV relAteD hEad and Neck) study assessed HPV-attributable fractions (HPV-AFs) of HNC including OPC in France, Germany, Italy, Portugal, Spain, China, and Japan over a 10-year period. METHODS: This retrospective study included patients diagnosed with HNC (oropharynx, oral cavity, nasopharynx, hypopharynx, and larynx can

Treatment of Radiation-Associated Dysphagia with Statins (TRADstat): protocol for a prospective, single-site, single-arm pilot trial.

INTRODUCTION: Radiation-associated dysphagia (RAD) is one of the most common and profound side effects of radiotherapy for head and neck cancer (HNC), but there is currently no effective treatment for persistent or late-onset RAD. Pravastatin has shown potential in reversing established fibrosis through modulation of the connective tissue growth factor signalling pathway and decreasing pro-inflammatory and profibrotic cytokines. Treatment of RAD with statins (TRADstat) is a pilot trial designed to evaluate the feasibility (primary outcome), efficacy potential, acceptability and safety of a 12-

Hypopharyngeal granular cell tumor treated with endoscopic submucosal dissection.

A 55-year-old woman was referred to our hospital after screening esophagogastroduodenoscopy revealed an erythematous area in the larynx. Detailed esophagogastroduodenoscopy at our institution initially suggested inflammatory changes. However, pharyngeal observation using the Valsalva maneuver revealed a 4-mm flat, elevated, whitish lesion in the postcricoid region of the hypopharynx. Although endoscopic differential diagnosis was challenging, lymphoid follicles were initially suspected; however, biopsy revealed inflammatory changes. Follow-up esophagogastroduodenoscopy four months later demons

Ganglioneuroma of the sacral canal: an unusual location with MR imaging findings.

Ganglioneuroma is a rare, benign, slow-growing tumor arising from embryonic neural crest cells that differentiate into the sympathetic ganglia. Accordingly, it can occur anywhere along the sympathetic chain, from the clivus to the sacrum, although it is seen most commonly in the mediastinum, retroperitoneum, adrenal glands, and neck. Although involvement at the sacral level is uncommon, ganglioneuroma typically arises from the anterior surface of the sacrum, with presacral lesions occasionally extending through the sacral foramina into the sacral canal. Only two cases of ganglioneuroma confine

Intubation videoendoscopy in a preterm infant with congenital tracheal stenosis and laryngotracheoesophageal cleft: a case report.

BACKGROUND: The co-occurrence of congenital laryngotracheoesophageal cleft and tracheal stenosis is extremely rare. Diagnosis is difficult and many affected neonates die within hours to days. CASE PRESENTATION: A male neonate was delivered at 32 weeks and 5 days of gestation. Birth weight was 1364 g. Severe asphyxia and respiratory failure were apparent soon after birth. Endotracheal intubation was not successful and esophageal intubation did not provide effective ventilation. Flexible intubation videoendoscopy was performed immediately while continuing mask bagging, which identified congenit

Anterior mediastinal epithelioid hemangioendothelioma associated with myelolipoma and extramedullary hematopoiesis mimicking a mature teratoma: A radiologic-pathologic correlation.

Epithelioid hemangioendothelioma (EHE) is an ultra-rare vascular neoplasm with variable biologic behavior. Mediastinal EHE is particularly uncommon and may mimic more prevalent prevascular mediastinal tumors. We report a 52-year-old man with a 10.5 × 8.9 × 9.6 cm mixed-density left upper anterior mediastinal mass containing macroscopic fat, soft-tissue components, and coarse calcification. FDG PET/CT demonstrated mild-to-moderate uptake (SUVmax 4.8) without nodal or distant metastatic disease, and the imaging appearance favored mature teratoma. Resection demonstrated a 9.5 cm conventional EHE

Postpartum Spontaneous Hemothorax Strongly Suggestive of Thoracic Endometriosis Syndrome: A Case Report.

A 34-year-old woman developed fever, dyspnea, and hypoxemia 8 days after vaginal delivery and was treated for pyelonephritis with bacteremia. The right dominant pleural effusion was initially clear and transudative, suggesting a systemic postpartum cause. However, sudden chest pain and worsening hypoxemia were followed by the rapid enlargement of the effusion, and repeat thoracentesis revealed hemothorax. Video-assisted thoracoscopic surgery revealed multiple small hematoma-like lesions on the visceral pleura. Markedly elevated pleural fluid CA-125 levels and pigment-laden macrophages suggeste

When Postextubation Stridor Is Not Edema: Bilateral Lateral Subglottic Mucosal Tears.

Iatrogenic tracheal injury following endotracheal intubation is a rare but potentially life-threatening complication. Most reported injuries involve the posterior membranous tracheal wall. We describe a case in which a patient required rapid sequence emergency intubation in the ED and, three days later, following extubation, developed acute stridor and increasing oxygen requirements, found to be secondary to bilateral, inferiorly based lateral mucosal tears of the subglottic larynx and superior trachea with an associated grade 1 subglottic stenosis. This case highlights the importance of maint

Spica Prunellae attenuates ER stress via GRP78/IRE1α/JNK pathway in CCL4-induced acute liver injury in mice.

OBJECTIVE: To evaluate the effects of Spica Prunellae (SP) on CCl4-induced acute liver injury (ALI) in mice and its regulation of the endoplasmic reticulum stress pathway. METHODS: In the toxicity test, ICR mice were administered SP decoction (2 g/kg, crude herb/mouse weight) by gavage for 10 days. Liver and kidney tissues were collected for H&E staining, and serum levels of liver function (AST, ALT) and renal (BUN, SCR) function indicators were measured. After confirming safety, 48 ICR mice were randomly assigned to normal, model, high-, medium-, and low-dose SP, and positive control groups

Common Cold-An Update.

The common cold, a mild yet highly prevalent upper respiratory tract infection (URTI), imposes a substantial societal and economic burden through healthcare costs, lost productivity, and widespread morbidity. Primarily caused by diverse respiratory viruses (e.g., rhinoviruses, coronaviruses), it encompasses a spectrum of acute infections affecting the nasal passages, throat, larynx, and bronchi, manifesting as rhinitis, pharyngitis, or bronchitis. Framed by the "iceberg concept" of disease, most cases are mild or asymptomatic, yet severe outcomes (e.g., exacerbations of chronic conditions) con

Delayed manifestation of infantile nystagmus following a vertigo attack.

Infantile nystagmus syndrome (INS), formerly referred to as congenital nystagmus, typically presents in early infancy. We report a rare case in which nystagmus consistent with INS became clinically apparent after a vertigo attack during adolescence. A 17-year-old female developed acute rotational vertigo. Although the vertigo gradually subsided, she continued to perceive rightward visual motion. Five months later, she was referred to our institution because of persistent spontaneous and gaze-evoked nystagmus. There was no prior history or documentation of nystagmus. Examination revealed jerk-t

Baseline Serum Thymus and Activation-Regulated Chemokine Levels and 1-Year Relapse in Bullous Pemphigoid: A Retrospective Cohort Study.

Baseline serological markers associated with relapse risk in bullous pemphigoid (BP) remain insufficiently defined. Serum thymus and activation-regulated chemokine (TARC) reflects type 2 inflammation and disease activity in BP, but whether its baseline level is associated with subsequent relapse has not been fully clarified. We retrospectively reviewed 109 Japanese patients with BP treated at Toyama University Hospital between January 2006 and September 2025. The primary endpoint was relapse within 1 year after treatment initiation. Associations between baseline serum TARC levels and time to r
