HLA antigens and neural reversal reactions in Ethiopian borderline tuberculoid leprosy patients.

Reversal reactions (RR) or acute neuritis episodes are frequently observed in borderline tuberculoid (BT) leprosy patients during the first year of treatment, and are associated with a rapid increase in cell-mediated immunity. Because HLA-linked genes have been shown to be an important factor in determining the type of leprosy that develops in susceptible individuals and because HLA molecules regulate cellular interactions in the immune system, we have investigated whether RR are associated with HLA antigens in Ethiopian patients. The data reported here indicate that this is not the case: no s

Ultracytochemical studies of lysosomal function in the macrophages of human leprosy--I.

The present study was undertaken to understand the lysosomal status and function in macrophages across the leprosy spectrum, including the reactional states. Acid phosphatase was used as the marker enzyme to demonstrate lysosomes at the electron microscopic level. It was observed that lysosomal morphology was well maintained in the macrophages of tuberculoid, borderline tuberculoid, and borderline tuberculoid leprosy in reaction, while they lost their cellular morphology and cell membrane integrity in lepromatous leprosy and lepromatous leprosy in reaction. The importance of these findings is

Natural killer (NK) cell activity and reversal reaction in leprosy.

Two studies were conducted to assess natural killer (NK) cell activity in leprosy patients and healthy Ethiopian controls. The first study tested 26 untreated leprosy patients across the spectrum of the disease. It was found that lepromatous leprosy and all untreated, nonreactional patients had lower NK activity than healthy controls. However, patients presenting with reversal reaction (RR) had NK activity within the normal range. Heterogeneity was particularly marked in the NK activity of borderline patients. In the second study, NK cell activity was assessed in treated borderline tuberculoid

Defective cell-mediated immunity in leprosy: failure of T cells from lepromatous leprosy patients to respond to Mycobacterium leprae is associated with defective expression of interleukin 2 receptors and is not reconstituted by interleukin 2.

Patients with lepromatous leprosy (LL) but not borderline tuberculoid leprosy (BT) have defective cell-mediated immune responses to Mycobacterium leprae, despite normal responses to other stimuli, as judged by in vivo skin testing and in vitro lymphocyte transformation. To investigate the basis of the immune defect in LL patients, we studied the ability of patient mononuclear leukocytes to produce interleukin 1 (IL 1) and interleukin 2 (IL 2) upon stimulation with M. leprae, and determined the ability of exogenous IL 1 and IL 2 to reconstitute the LL patient response to this antigen in vitro.

An in situ immunohistological study of Mitsuda reactions.

In an attempt to further define their immunopathogenesis, the cellular infiltrates of Mitsuda reactions were studied in situ using immunoperoxidase techniques and monoclonal antibodies. Lepromin A-elicited Mitsuda reactions from six patients with borderline tuberculoid leprosy (TT/BT or BT) and three healthy kindred and contacts of lepromatous patients were examined. In the dermis, cells bearing the Leu4 phenotype comprised a mean of 61% of the infiltrate; the Leu3a, 47%; the Leu2a, 17%; anti-IL-2, 0.2%; anti-Tac, 1.5%; and cells bearing the Ia phenotype were virtually universal; OKT6 positive

Alpha-1-antitrypsin in leprosy.

Estimation of Alpha-1-antitrypsin (AAT) levels was carried out in 52 patients of various types of leprosy. Fifty age and sex matched healthy individuals served as controls. The mean level of AAT in controls was 290.12 +/- 59.56 mg/dl. In patients of tuberculoid leprosy (TT), borderline tuberculoid leprosy (BT) and borderline leprosy (BB), the AAT levels were found to be 284 +/- 47.03, 314.37 +/- 31.56 and 324.44 +/- 32.05 mg/dl respectively. These were statistically insignificantly raised when compared with controls. In borderline lepromatous leprosy (BL), lepromatous leprosy without erythema

Successful plasma exchange in type 1 leprosy reversal reaction.

A 24 year old man admitted to hospital with borderline lepromatous leprosy was treated with rifampicin, dapsone, and clofazimine. After four months he developed a reversal reaction and the diagnosis was modified to borderline tuberculoid leprosy. The dose of clofazimine was raised and prednisolone added to the regimen without any symptomatic response. His condition improved dramatically after five plasma exchanges on five successive days.

Thymus-dependent lymphocytes in leprosy. II. Effect of chemotherapy on T-lymphocyte subpopulations.

The basis of the immunological unresponsiveness seen in leprosy patients is unknown. Untreated lepromatous leprosy patients display an unspecific cellular anergy which disappears with treatment, leaving an anergy specific for Mycobacterium leprae. These patients suffer from a complication, erythema nodosum leprosum, characterized by a recurrent eruption of tender skin nodules disappearing in 2 to 3 days. These nodules show a histological picture reminiscent of an Arthus reaction. Erythema nodosum leprosum can occur in untreated patients but it is more frequent in those receiving effective chem

Nerve damage in tuberculoid leprosy.

The pattern of ulnar nerve damage was studied in forty five patients suffering from Borderline Tuberculoid leprosy. All the patients were subjected to sensory and Voluntary Muscle Testings of the hands. It was observed that while all patients should evidence of sensory and autonomic deficit in the ulnar nerve-supplied areas of the hands, only forty patients showed the evidence of motor deficit in the muscles supplied by the same nerve. The probable mechanism of selective sparing of motor function in the remaining five patients is discussed.

Tuberculoid leprosy or sarcoidosis? A diagnostic dilemma.

During an intensive survey in a leprosy endemic area in South India, a young male adult was detected with infiltrated skin lesions and diagnosed as a case of major tuberculoid leprosy. The presence of leprosy in the individual could not be clinically established, despite a histological diagnosis of tuberculoid leprosy made on his skin biopsy. In a eighteen year follow up during which he was examined on six occasions, the skin patches continued very much the same in appearance with a progressive increase in size; he was re-biopsied from the same lesion thrice and was declared tuberculoid or bor

In vitro lymphocyte stimulation in patients with lepromatous and borderline tuberculoid leprosy. The effect of dapsone treatment on the response to Mycobacterium leprae antigens, tuberculin purified protein derivative and non-mycobacterial stimulants.

Lymphocytes from peripheral blood were isolated from leprosy patients and healthy contacts (HC) of leprosy patients and stimulated in vitro with: Mycobacterium leprae and a M. leprae cell wall antigen, MLW 1; tuberculin purified protein derivative (PPD); antigens prepared from Candida albicans, Entamoeba histolytica, Leishmania aethiopica, and parotitis virus; the non-specific mitogens phytohemagglutinin (PHA) and concanavalin A (Con-A). Lymphocytes from patients with untreated lepromatous leprosy failed to respond to the M. leprae antigens, and the median response to PPD was also significantl

The monocyte-macrophage system in granulomatous inflammation.

Granulomas may be immunologically induced or non-immunologically induced. In immunologically induced granulomas cells of the monocyte-macrophage series take on the appearance of epitheloid cells. Ultrastructurally epithelioid cells may have a secretory appearance with much rough endoplasmic reticulum or take on highly degenerate vesicular appearance. Other epithelioid cells look like activated macrophages. Secretory epithelioid cells may be found associated with acute local inflammation as in borderline tuberculoid leprosy in reaction, the lepromin reaction, following injection of BCG vaccine

Borderline tuberculoid leprosy following BCG vaccination. A case report.

Borderline tuberculoid leprosy was diagnosed clinically and histologically in a four year-old boy about 6 months after intradermal vaccination with BCG. His mother reported that a lesion began to appear above the vaccination site on the arm 2 weeks after the vaccination, and a second lesion appeared on the chin 2 months later. Responses in the lymphocyte transformation test to sonicated Mycobacterium leprae, BCG, and to PPD were consistent with a tuberculoid leprosy infection. Precipitation of BT leprosy by intradermal BCG infection may possibly represent the overcoming of a phase of primary s

Leprosy in peripheral nerves: histopathological findings in 119 untreated patients in Nepal.

From a series of 343 nerve biopsies taken by one clinician over a period of 12 years in Nepal, this paper describes the histopathological findings in 153 biopsies from 119 patients suffering from tuberculoid, borderline (dimorphous) or lepromatous leprosy, who were untreated at the time of first presentation and diagnosis. They were taken during the course of other studies, mainly concerned with the mode of transmission of leprosy, and which included biopsies of skin, dartos muscle, nasal mucous membrane and nipple, results of which have already been published. Examination of serial sections b
