Evaluation of parietal pleural invasion in subpleural lung cancer by VEGFR2 targeted microbubble-based ultrasound molecular imaging.

BACKGROUND: The parietal pleural invasion in subpleural lung cancer is associated with poor prognosis and surgical difficulty. The target protein of vascular endothelial growth factor receptor 2 (VEGFR2) is highly expressed in invasive lesions. This study preliminarily investigated the value of ultrasound (US) molecular imaging based on microbubbles targeted to VEGFR2 in early diagnosis of parietal pleural invasion of subpleural lung cancer. METHODS: Animal protocols were approved by the Institutional Administrative Panel on Laboratory Animal Care. The microbubbles targeted specifically to VE

Familial systemic lupus erythematosus with pleural and articular involvement mimicking tuberculosis: a case report.

INTRODUCTION: Systemic lupus erythematosus (SLE), a multisystemic disease with a global prevalence of 20-150/100 000, predominantly affecting women may have a familial clustering, suggesting a strong genetic predisposition and emphasizing the role of heritable factors in disease pathogenesis. In tuberculosis-endemic regions, pleural effusions are frequently misattributed to tuberculosis, leading to delays in the correct diagnosis and appropriate treatment. CASE PRESENTATION: A 42-year-old female with a history of continuous chest pain, aggravated on inspiration, intermittent fever with night

Decoding the Invisible: A Diagnostic Odyssey of Pulmonary Alveolar Microlithiasis From the Desert State of India, Rajasthan.

Pulmonary alveolar microlithiasis (PAM) is a rare depositional disorder that carries an autosomal recessive pattern of inheritance. It may present at any age and is usually detected incidentally with an abnormal chest X-ray. Most patients are asymptomatic or have minimal symptoms at the time of detection of the disease. Here, we report a case of a 30-year-old female patient who presented with an illness duration of six months and complaints of dry cough, chest pain, and shortness of breath. She was started on anti-tubercular treatment on clinical grounds by her physician, but was not relieved

Malignant Pleural Mesothelioma in an Asbestos Industry Worker: A Posthumous Case Report from Haryana, India.

Malignant mesothelioma is an aggressive cancer primarily associated with asbestos exposure, affecting the mesothelial lining of the lungs, abdomen, or heart. Despite global awareness, it remains relatively rare, with an age-standardized incidence of mesothelioma around 3 per million individuals, with Northern Europe exhibiting the highest rates. The occurrence is significantly higher in males compared to females. Malignant pleural mesothelioma (MPM) affects the pleura and often develops after years of asbestos exposure, silently progressing until symptoms appear. MPM predominantly affects men,

[Sonography of the pleura - step by step].

Pleural ultrasonography enables the rapid and reliable detection of pneumothoraces, pleural effusions, and other lung pathologies. Its advantages - such as high image resolution and ease of bedside use - make it particularly popular in acute and intensive care medicine. Despite its limitations in examining deeper structures, it offers many benefits compared to other imaging modalities.

[The 9th TNM classification of thoracic tumors].

The TNM classification applies to three types of thoracic tumors: lung cancer, thymic epithelial tumors (thymomas, neuroendocrine neoplasms, carcinomas), and pleural mesotheliomas. This 9th edition introduces two changes for the lung: category N2 (mediastinal lymph node metastases) is divided into N2a and N2b, when one N2 station or several N2 stations are affected, respectively, regardless of the number of lymph nodes; category M1c (multiple extrathoracic metastases) is divided into M1c1 and M1c2 when multiple extrathoracic metastases are present in one or more organs, respectively. There are

A gastric and pleural metastasis of occult breast lobular cancer in an elderly female and radiomics assisted analysis of a rare metastasis.

BACKGROUND: Metastatic breast cancer involving the gastrointestinal tract is rare, particularly in lobular carcinoma, often emerging later in disease progression. Occult primary breast tumors are exceptionally uncommon. This study reports the first case of occult breast lobular carcinoma (OBLC) with concurrent gastric and pleural metastases in a 65-year-old female. METHODS: A multidisciplinary diagnostic approach integrated histopathological and radiomic analyses. Immunohistochemical (IHC) profiles of axillary lymph node and gastric lesions were compared. Video-Assisted Thoracoscopic Surgery for the Stage II Pleural Empyema: A Prospective Observational Study.

Background and objectives Empyema thoracis is a condition in which purulent fluid collects in the pleural cavity. Video-assisted thoracoscopic surgery (VATS) is a minimally invasive technique that involves insertion of a thoracoscope through small incisions, or ports, in the chest wall. Despite modern diagnostic methods, pleural empyema remains a serious problem. The common causes are complications of pneumonia (fungal and mycobacterial), abdominal infections, or those following trauma and surgical procedures. The aim and objectives of this study are to evaluate the role and efficacy of VATS i

Massive hemothorax following CT-guided lung biopsy: A rare iatrogenic complication managed conservatively.

CT-guided transthoracic lung biopsy is an essential diagnostic technique for evaluating pulmonary lesions. Although rare, major complications such as hemothorax can be life-threatening. We report the case of a 35-year-old woman who developed a rapidly enlarging right hemothorax several hours after CT-guided core needle biopsy of a pleura-abutting right lower lobe mass. There was no arterial extravasation, and chest tube drainage yielded approximately 1500 mL of dark, clotted blood, suggesting a venous or tumoral source. The patient remained hemodynamically stable and achieved complete recovery

Mesothelioma location influences the tumour microenvironment and immune checkpoint therapy response in preclinical models.

Mesothelioma is a cancer derived from mesothelial cells, most commonly arising from the pleura or the peritoneum. Immune checkpoint therapy (ICT) has shown survival benefit for pleural mesothelioma, but little is known about the response in peritoneal mesothelioma. Most preclinical mesothelioma models involve subcutaneous cancer cell implantation, which lacks the relevant tumour microenvironment of peritoneal mesothelioma and does not resemble the clinical presentation. We therefore set out to explore the influence of location on the mesothelioma tumour microenvironment, comparing pleural, per

Multifocal micronodular pneumocyte hyperplasia in the absence of tuberous sclerosis complex: A case report.

Multifocal micronodular pneumocyte hyperplasia (MMPH) is rare in the absence of tuberous sclerosis complex (TSC). We report a genetically confirmed TSC-negative case in a 50-year-old male patient with numerous bilateral ill-defined ground-glass nodules found on chest computed tomography. The nodules were distributed predominantly in the upper and middle lung zones and showed a centrilobular distribution. The nodules measured 1-15 mm in diameter, and some abutted pleural surfaces. Because several nodules showed slight enlargement (1-2 mm) over 1 year, a diagnostic surgical biopsy was performed.

Computer-assisted analysis of pleural and subpleural lung ultrasound correlates with oxygenation in preterm infants.

Lung ultrasound (LUS) is increasingly used in neonatal intensive care, however visual scoring of LUS images remains subjective. Quantitative LUS (Q-LUS) is objective and whilst it detects small lung aeration changes in preterm animals, human data are limited. We hypothesised that Q-LUS would correlate strongly with oxygenation in very preterm infants born before 32 weeks’ gestation. We examined two quantitative image properties to assess whether Q-LUS correlates with oxygen status in very preterm infants: (1) mean-grey-value (MGV) and (2) second-order textural features (angular second moment,

Subacute pacemaker lead migration with cardiac perforation and extracardiac extension into lung parenchyma with associated pneumothorax.

Cardiac perforation with extracardiac extension by a pacemaker lead is a rare but serious complication of pacemaker placement. We present a unique case with imaging of a patient who underwent pacemaker placement complicated by subacute migration of the pacemaker lead, resulting in perforation involving interventricular septum, right ventricular apex, pericardium, epicardial fat, pleura and left upper lobe parenchyma with left-sided pneumothorax.

Iatrogenic pneumothorax after CT-guided lung biopsy.

INTRODUCTION: CT-guided lung biopsy is an essential diagnostic tool for evaluating pulmonary infiltrates, but iatrogenic pneumothorax (PTX) is a frequent complication. Evidence regarding risk factors and management strategies for post-biopsy PTX is limited, and no specific guidelines exist. METHODS: We conducted a retrospective observational study including 267 patients who underwent CT-guided lung biopsy at Bispebjerg Hospital, Denmark, from 1 January to 30 June 2024. Data were collected on age, sex, smoking status, lung function, lesion location, distance to pleura and final diagnosis. The

Diffuse co-expression of TTF-1 and p40 (ΔNp63) defines a distinct biphenotypic non-small cell lung carcinoma: clinicopathologic and molecular characterization of 19 cases.

BACKGROUND: The diffuse co-expression of thyroid transcription factor-1 (TTF-1) and p40 defines a rare and diagnostically challenging subtype of non-small cell lung carcinoma (NSCLC). Due to its extreme rarity, the clinicopathologic and molecular characteristics of this entity remain poorly characterized. METHODS: We conducted a comprehensive analysis of the largest single-institution cohort to date (n = 19) of NSCLCs with diffuse TTF-1/p40 co-expression. All cases underwent detailed histopathologic review. Immunohistochemistry (IHC) utilized specific antibody clones (TTF-1: 8G7G3/1; p40). Mol
