Rapidly growing mediastinal mature cystic teratoma with increased FDG uptake: challenging the paradigm of indolent growth.

BACKGROUND: Mature cystic teratomas of the mediastinum are typically germ cell tumors with indolent growth, with documented annual growth rates often less than 5 centimeters. We report a case with an abnormal growth rate (Average diameter growth rate 7.14 mm/month) and significant increased 18 F fluorodeoxyglucose (18 F FDG) uptake, which challenges conventional diagnostic concepts. CASE PRESENTATION: A 34-year-old female presented with progressive chest tightness. Serial CT imaging revealed a right anterior superior mediastinal mass that grew from approximately 1.5 cm in long diameter to 6.5

Transesophageal EUS of mediastinal and pulmonary lesions: An Italian survey from i-EUS Working Group.

BACKGROUND AND OBJECTIVES: Pulmonary and mediastinal masses are diagnostic challenges for digestive endosonographers, and patients with these conditions are usually managed in thoracic or pulmonology units. EUS with tissue acquisition can result in adequate samples for histological examination and molecular analysis. Its spread among digestive endosonographers is limited; thus, we aimed to investigate the current management practices among the EUS Italian centers for those patients with mediastinal or pulmonary lesions to identify the principal areas of disagreement and help guide future resea

Sevoflurane Sedation for High-Risk Airway Management in the Pediatric Intensive Care Unit Following Sclerotherapy for a Pediatric Cervical Lymphatic Malformation: A Case Report.

Cervical lymphatic malformations (CLMs) can cause critical airway compromise due to post-sclerotherapy swelling, requiring precise sedation management in the pediatric intensive care unit (PICU). Precise airway management is especially challenging during the postoperative edema peak, where accidental extubation must be avoided. Inhalational sevoflurane provides rapid titration and predictable emergence, potentially allowing smoother management compared with traditional intravenous sedatives. We report the case of a one-year-old female patient (11.5 kg) with a massive CLM extending into the med

Thoracoscopic Resection of a Bronchial Artery Aneurysm Mimicking Mediastinal Lymph Node Metastasis after Surgery for Papillary Thyroid Cancer: A Case Report.

INTRODUCTION: Mediastinal lymph node recurrence is frequently encountered during the follow-up after surgery for thyroid cancer. We report a rare case of a bronchial artery aneurysm (BAA) that was preoperatively suspected to be a mediastinal lymph node metastasis based on imaging findings and subsequently treated with thoracoscopic resection. CASE PRESENTATION: A 68-year-old male underwent a total thyroidectomy and neck lymph node dissection for papillary thyroid carcinoma (pT1bN1bM0, UICC Stage II), followed by radioactive iodine ablation as adjuvant therapy. Surveillance CT performed 2 year

Extracranial Metastases From FET-CREB Fusion-positive Intracranial Mesenchymal Tumor on FDG PET/CT.

FET-CREB fusion-positive intracranial mesenchymal tumor is a newly recognized provisional tumor type. Usefulness of FDG PET/CT in the management of this tumor is rarely reported. We describe FDG PET/CT findings in a case of FET-CREB fusion-positive intracranial mesenchymal tumor with metachronous extracranial metastases to the mediastinum, mesentery, and left Leg. The metastatic tumors showed mild to moderate FDG uptake. This case indicates that FET-CREB fusion-positive intracranial mesenchymal tumors can develop extracranial metastasis, and FDG PET/CT may be useful for detecting extracranial

Central Venous Catheter Tracking Into a Persistent Left Superior Vena Cava in a Patient With Complete Heart Block.

Persistent left superior vena cava (PLSVC) is the most common thoracic venous anomaly, present in approximately 0.3-0.5% of the general population. Although typically asymptomatic, PLSVC carries significant procedural implications during central venous catheter (CVC) placement, pacemaker implantation, and hemodynamic monitoring. A 75-year-old man presented with symptomatic complete heart block and was transferred for pacemaker evaluation. A right internal jugular (IJ) central venous catheter was inserted for vasopressor administration. Post-procedure chest X-ray revealed the catheter coursing

The Morphologic Spectrum of Mediastinal Yolk Sac Tumors: Diagnostic Challenges and Pitfalls.

Pure mediastinal yolk sac tumor is an uncommon and aggressive malignant germ cell neoplasm that presents considerable diagnostic difficulties owing to its pronounced clinical and morphological variability. Mediastinal yolk sac tumors, in contrast to their gonadal equivalents, typically occur at later stages, are typically associated with mixed germ cell components, and have a diverse array of histologic patterns that may resemble both germ cell and somatic malignancies. Accurate identification of these types of cancer is essential since diagnostic misclassification may significantly impact tre

Does needle size affect the diagnostic yield of endobronchial ultrasound-guided transbronchial needle aspiration for malignant lymphoma?-a narrative review.

BACKGROUND AND OBJECTIVE: Bronchoscopic diagnosis of malignant lymphoma is challenging, because bronchoscopic tissues are often relatively small. Recently, a 19-gauge needle (19G) was developed to obtain larger tissue cores; however, its effectiveness in improving the diagnostic yields remains controversial. Therefore, in this narrative review, we aimed to summarize recent findings and evaluate the usefulness of different needle sizes for diagnosing malignant lymphoma. METHODS: A comprehensive and systematic online literature search of PubMed was conducted using the keywords ("malignant lymph

Imaging-based criteria for minimally invasive versus open surgery in thymic tumors: a narrative review.

BACKGROUND AND OBJECTIVE: Thymic epithelial tumors (TETs) require precise preoperative assessment to achieve complete resection and optimal long-term outcomes. With the expanding use of minimally invasive surgery, imaging has become central not only for diagnosis but also for surgical decision-making. This narrative review aims to summarize current evidence on how metabolic, morphologic, and computed tomography (CT)-based imaging parameters can be integrated to guide surgical strategies for TETs. METHODS: A comprehensive literature search was conducted using PubMed and Web of Science for stud

Preclinical and human tracheal replacement with aortic grafts: a narrative review.

BACKGROUND AND OBJECTIVE: Numerous reviews have been published in the field of tracheal replacement. However, most did not differentiate between patch tracheal reconstruction (PTR), non-circumferential tracheal replacement (NCTR), and full-circumferential tracheal replacement (FCTR). This distinction is essential in terms of postoperative mortality. So far, the most widely used tracheal substitutes are the cartilage-reinforced forearm free flap (FFF) and the cryopreserved aortic allograft (CAA), respectively. The aim of our study is to identify and report the results of both preclinical and hu

An Incidental Necropsy Finding: Intrathoracic Ectopic Liver in a Cat.

Ectopic liver is a rare anomaly characterized by the presence of hepatic tissue outside its normal anatomical location without continuity with the main liver. Reports of this condition are sporadic in human medicine, and cases in veterinary species remain limited. This condition may arise as a congenital developmental defect or as a consequence of prior traumatic injury leading to fragmentation and displacement of hepatic tissue. This case study describes an incidental intrathoracic ectopic liver identified in a 6-year-old male cat that died of acute suppurative bacterial pneumonia. Necropsy r

Occult Breast Carcinoma: Pathologic Features of an Uncommon Clinical Presentation in a Large Cohort.

Occult breast carcinoma (OBC) refers to the clinical presentation of breast carcinoma occurring in axillary lymph node(s) without a detectable primary breast cancer. Prior studies of OBC have focused on treatment regimens. We sought to study the clinical, morphologic, and immunohistochemical features of OBCs. We retrospectively identified cases of OBC treated at our center between 1996 and 2021. All patients included in the study had biopsy-proven axillary metastatic breast carcinoma and underwent MRI after a noncontributory mammogram/ultrasound. Patients with a prior history of breast carcino

Optimizing Mediastinal Lymph Node Dissection by Integrating Tumour Location, Consolidation Tumour Ratio, and Size for Lung Cancer.

OBJECTIVES: Several mediastinal lymph node dissection strategies have been proposed for non-small-cell lung cancer based on prognostic analysis. However, as the lymph metastatic pattern of non-small-cell lung cancer has not been fully revealed, there are still needs for optimization of lymph node management. METHODS: Data on 7067 invasive non-small-cell lung cancer patients who underwent pulmonary resection with systematic lymph node dissection were retrospectively analysed. The tumour size, location, and consolidation tumour ratio (CTR) were identified with computed tomography scans. Other c

Endobronchial ultrasound-guided transbronchial fine-needle aspiration of mediastinal lymph nodes: a clinical appraisal of diagnostic accuracy and molecular utility.

BACKGROUND: Endobronchial ultrasound-guided transbronchial fine-needle aspiration (EBUS-TBNA) is currently recommended as the technique of choice for mediastinal and hilar lymph node staging in lung cancer, when technically feasible. It is also widely employed in the diagnostic evaluation of mediastinal and hilar lymphadenopathies of uncertain origin. OBJECTIVE: To evaluate the diagnostic performance of EBUS-TBNA for mediastinal and hilar lymphadenopathies and to assess the adequacy of obtained tissue for molecular and immunohistochemical testing. METHODS: Procedures performed by a single ex

Ectopic Hepatocellular Carcinoma in the Mediastinum with Brain Metastasis: A Rare Case Report.

Ectopic hepatocellular carcinoma (EHCC) is an extremely rare neoplasm, especially in the mediastinum, which shares morphologic characteristics with intrahepatic hepatocellular carcinoma (HCC). Its clinical features remain unclear, posing significant diagnostic and therapeutic challenges. The prognosis is also unclear due to its rarity and potential variability. This study reports the first case of EHCC in the mediastinum with subsequent brain metastasis. A 50-year-old man presented with shoulder and chest discomfort persisting for 5 months, accompanied by progressive weight loss and fatigue ov

The Diagnostic Challenge of Sharp Foreign Body Aspiration in Children: A Two-Case Series Highlighting Ingestion vs. Aspiration.

Foreign body aspiration (FBA) is a common pediatric emergency. Sharp objects, such as pins, pose a unique risk of perforation and migration. Differentiating aspiration from ingestion can be challenging when symptoms and radiological findings overlap, particularly for objects lodged in the mediastinum-a "mediastinal dilemma." We present two cases of sharp FBA in school-aged children. Case 1: A 10-year-old girl presented with acute chest pain after inhaling a headscarf pin. Chest X-ray showed a mediastinal radio-opaque shadow, creating a diagnostic dilemma. Sequential endoscopy (negative esophag

Thymoma with suspected secondary myocardial infarction in a red panda.

A 17-y-old, female red panda (Ailurus fulgens) with unusual restlessness and a poor prognosis was euthanized and submitted for autopsy. Postmortem examination revealed a large cystic mass within the cranial mediastinum that severely compressed the lung. Histologic analysis classified the mass as a thymoma with abundant small lymphocytes and recognizable type B2 epithelial cells. Additionally, the heart had several chronic myocardial infarcts within the left ventricular free wall and septum. Granulomatous lymphadenitis and prominent anthracosilicosis were noted in the tracheobronchial lymph nod

Prognostic and predictive biomarkers in thymic epithelial tumors: beyond traditional staging: a narrative review.

BACKGROUND AND OBJECTIVE: Thymic epithelial tumors (TETs), encompassing thymomas (TMs) and thymic carcinomas (TCs), are rare and heterogeneous mediastinal malignancies with variable clinical behaviors and prognoses. Current prognostic assessment primarily relies on histological classification (WHO) and anatomical staging systems (Masaoka-Koga, TNM). However, the rarity and complex biology of TETs necessitate the identification of novel prognostic and predictive biomarkers to improve risk stratification and guide personalized treatment strategies. This narrative review aims to summarize and dis

The evolving role of interventional pulmonology with endobronchial ultrasonography in the diagnosis of lymphoma: a narrative review.

BACKGROUND AND OBJECTIVE: For patients undergoing evaluation for mediastinal/hilar adenopathy or masses suspicious for lymphoma, endobronchial ultrasonography (EBUS)-transbronchial needle aspiration (TBNA) offers a lower-risk alternative to surgical or percutaneous biopsy. While the utility of EBUS-TBNA is well established in lung cancer diagnosis, including biomarker analysis, its role in lymphoma is more limited. This is because for accurate subtyping of lymphoma, larger intact tissue specimens are needed and EBUS-TBNA can only provide cytology specimens. To address this limit, various needl
