Modified Management of Giant Hiatal Hernia: Laparoscopic Hiatoplasty With Sac Excision and Posterior Mediastinal Drainage.

BACKGROUND: The standard laparoscopic hiatoplasty with Toupet fundoplication is widely regarded as a safe treatment for giant hiatal hernia; however, it is often associated with a high incidence of posterior mediastinal seroma. To address this issue, a modified approach combining sac excision and negative pressure posterior mediastinal drainage has recently been introduced. This study compares clinical outcomes between the conventional and modified techniques to identify the optimal surgical strategy for managing giant hiatal hernia. METHODS: Patients who underwent laparoscopic hiatoplasty wi

Preliminary Experience of Circular Stapler-based Lower Mediastinal Reconstruction in Robotic Total and Proximal Gastrectomy.

BACKGROUND: In minimally invasive surgery for upper gastric and esophagogastric junction cancers, linear staplers are commonly used, and reports of circular‑stapled reconstruction have declined in parallel with the decrease in open surgery. Lower mediastinal reconstruction during robot‑assisted gastrectomy also remains technically challenging. This study describes our surgical technique and preliminary experience with transhiatal lower mediastinal anastomosis using a circular stapler. METHODS: Twelve patients who underwent robot‑assisted total or proximal gastrectomy with lower mediastinal re

Case Report: Rare malignant teratoma in the right posterior thigh.

Malignant teratomas are rare germ cell tumors characterized by undifferentiated embryonic components, exhibiting invasive and metastatic potential. While teratomas commonly arise in the gonads, such as the ovaries and testes, as well as midline locations like the sacrococcygeal region and mediastinum, the occurrence of a malignant teratoma in the femur (right femur) is exceedingly rare, with limited research available. We report the case of a 19-year-old female presented with a subcutaneous mass in the right posterior thigh for 2 months, which was later confirmed as immature teratoma (CK5/6+,

Spontaneous rupture of a giant thymoma causing hemothorax and cardiopulmonary collapse.

Spontaneous rupture of a thymoma is a rare event. An 81-year-old female presented with an asymptomatic 14-cm thymoma in the mid-upper mediastinum, extending to the right diaphragm for thymectomy. Two days before surgery, she developed acute dyspnea and chest pain. Admission chest roentgenogram showed a new large right pleural effusion. Thoracoscopic drainage of 2200 mL of blood mixed with necrotic tumor debris revealed a posterior rupture of her thymoma. The mass was resected via median sternotomy, and the chest irrigated. Her postoperative course was uneventful. Final pathology showed Type A

Bone Tracers for Transthyretin Amyloid Cardiomyopathy: Are [99mTc]Tc-DPD and [99mTc]Tc-HMDP Equivalent?

The management of transthyretin amyloid cardiomyopathy (ATTR-CM) has revolved around the scintigraphic diagnosis since the introduction of a specific treatment; however, the equivalency of the bone radiotracers remains unclear. This retrospective monocentric observational study compared [99mTc]Tc-3,3-diphosphono-1,2-propanodicarboxylic acid ([99mTc]Tc-DPD) and [99mTc]Tc-hydroxy-methylene diphosphonate ([99mTc]Tc-HMDP) intensity of myocardial uptake compared with background noise for ATTR-CM suspicion. Two hundred and seventy-four patients who underwent planar scintigraphy after intravenous inj

Outcomes of radiologically diagnosed solitary fibrous tumours of the pleura.

INTRODUCTION: Solitary fibrous tumours of the pleura (SFTP) are rare neoplasms for which surgical resection is considered the gold standard treatment. Anecdotally, SFTPs are sometimes conservatively managed, but there are little data on this in published literature. In this study, we aimed to collect data on radiological features in SFTP which might be associated with a benign clinical course to assist in risk stratification, and therefore selection for a more conservative management pathway. METHODS: A service evaluation of outcomes of radiologically diagnosed SFTP was completed in 12 UK cen

Diachronous primary Neuroendocrine tumour of the mediastinum: Dual FDG and DOTA avidity on PET/CT.

Primary neuroendocrine tumours (NETs) of the mediastinum are exceedingly rare, accounting for a small fraction of thoracic neoplasms. Their biological behaviour is heterogeneous, ranging from indolent to highly aggressive depending upon their histological grading. Dual-tracer PET/CT imaging with fluorodeoxyglucose (FDG) and gallium-68 DOTA-conjugated peptides provides complementary information, reflecting heterogeneity of tumour including both metabolic activity and somatostatin receptor expression. barovariability.

Perioperative management of an asymptomatic giant retrosternal goiter with high-risk imaging findings and an unanticipated cancer diagnosis: a case report.

BACKGROUND: A retrosternal goiter (RSG) is defined as a thyroid mass with >50% of its volume extending into the mediastinum. Surgical resection is the definitive treatment. For patients with asymptomatic giant RSGs-a condition where watchful waiting is common yet carrying risks of compression and occult malignancy-the perioperative nursing literature lacks specific guidance. This creates a challenge in systematically addressing their dual needs: managing complex physiological risks (e.g., respiratory, neural) and providing psychological support for potential unexpected diagnoses. This case rep

An exploratory nomogram model with 18F-FDG PET/CT metabolic parameters for predicting high-risk status of microvascular invasion in hepatocellular carcinoma patients prior to liver transplantation.

BACKGROUND: There are still difficulties in dividing patients into an microvascular invasion (MVI) high-risk and non-high-risk group among hepatocellular carcinoma (HCC) patients based on the pathology findings. This study investigated preoperative fluorine-18-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)/computed tomography (CT) (18F-FDG PET/CT) quantitative metabolic parameters and developed an exploratory nomogram model with these parameters for predicting high-risk MVI in patients with HCC scheduled for liver transplantation. METHODS: A retrospective analysis was conduct

Age estimation from thoracic CT using 3D convolutional neural networks.

BACKGROUND: This study investigated the feasibility of using three-dimensional deep learning (DL) models to estimate chronological age from routine thoracic computed tomography (CT) scans and examined the contribution of different CT reconstruction windows to model performance. METHODS: This retrospective study included thoracic CT scans from 1278 adults aged 20-80 years obtained from a single tertiary center. Lung, bone, and mediastinal reconstruction windows were generated for each scan. A 3D DenseNet-based DL model was trained using either individual window inputs or a combined three-chann

A standardized, surgically relevant map for emergence of organ-specific branches from the human vagus nerve.

INTRODUCTION: Vagus nerve stimulation modulates airway, cardiac, pulmonary, and gastrointestinal functions through respective organ-specific branches. Knowledge of where along the nerve vagal branches emerge may support alternative surgical placement of stimulation devices to activate targeted functions while avoiding off-target effects. However, no quantified map of the emergence of vagal branches and their relationship to surgically relevant anatomical landmarks exists in humans. METHODS: Fifty-eight vagus nerves (29 left, 29 right) from 29 embalmed donor bodies (15 females) were dissected

What affects adequate lymph node harvest in pulmonary resections?-assessment of compliance with a national quality standard.

BACKGROUND: The Commission on Cancer (CoC) quality standard 5.8 for lung cancer lymph node sampling requires sampling of at least one hilar lymph node and three mediastinal lymph nodes. We hypothesized there may be multifactorial reasons for non-compliance, and in this study, we sought to determine these factors in order to identify targets for local interventions. METHODS: We built a conceptual model for compliance with standard 5.8, considering tumor, patient, provider, and structural factors. We performed a retrospective cohort study of all patients undergoing pulmonary resection for lung

Resection for an Extremely Rare Pleomorphic Carcinoma of the Mediastinal Lymph Nodes of Unknown Primary: A Case Report.

Pleomorphic carcinoma of the mediastinal lymph nodes with an unknown primary site is extremely rare. A 68-year-old man was referred to our department after an enlarged lymph node in the right upper mediastinum was noted on computed tomography. An irregular nodule was also detected in the right upper lobe, showing no change compared with findings from 2 years earlier. FDG-PET (18F-fluorodeoxyglucose positron emission tomography) revealed strong uptake only in the mediastinal lymph nodes. EBUS-TBNA (Endobronchial ultrasound-guided transbronchial needle aspiration) of the lymph nodes revealed fin

First-in-Human Intramediastinal Taurolidine Irrigation for Candida albicans Mediastinitis After Biological Bentall Procedure.

BACKGROUND: Post-sternotomy mediastinitis remains a devastating complication of cardiac surgery. Although most cases are bacterial, fungal mediastinitis due to Candida albicans is rare, aggressive, and particularly difficult to treat because of biofilm formation, prosthetic involvement, and limited penetration of systemic antifungal agents into infected tissues. Taurolidine is a taurine-derived antimicrobial compound with broad antibacterial, antifungal, and anti-biofilm properties that has shown promising results in catheter-related infection prevention and cardiac implantable electronic devi

Beyond the Nodes: Atypical Presentations of Non-Hodgkin's Lymphoma Across Organs.

Introduction Extranodal presentation of non-Hodgkin's lymphoma (NHL) poses a diagnostic challenge due to its diverse clinical, morphological, and pathological features. These lymphomas arise in organs other than lymph nodes. Immunohistochemistry (IHC) plays a crucial role in the confirmation and subtyping of these lesions. Objective  The aim of this study was to identify different patterns of extranodal involvement in NHL and to list the common sites of involvement. Materials and methods This retrospective case series included eight cases of atypical extranodal presentations of NHL across orga
