Variation of Normal Thymic Veins With Age And Gender in a Large Series of Subjects on Enhanced Multidetector Computed Tomography.

INTRODUCTION/OBJECTIVE: The thymic veins are small vessels located in the anterior superior mediastinum, and thymic tissue undergoes physiological degeneration with age. Multidetector computed tomography (MDCT) with isotropic resolution allows comprehensive visualization of small mediastinal vessels. However, large-sample studies describing normal thymic veins using contrast-enhanced MDCT remain lacking. This study aimed to evaluate the morphologic features of normal thymic veins on venous-phase enhanced MDCT and to analyze their distribution patterns according to age group, gender, and latera

Retrosternal Multinodular Goiter With Extensive Mediastinal Extension Requiring Combined Cervical Thyroidectomy and Median Sternotomy: A Case Report.

Retrosternal goiter is a distinct clinical entity, characterized by extension of thyroid tissue into the mediastinum, often increasing surgical complexity and necessitating individualized operative planning. We report a 55-year-old female patient presenting with a gradually enlarging anterior neck swelling without compressive or thyrotoxic symptoms. Clinical examination suggested mediastinal extension, and imaging with contrast-enhanced computed tomography confirmed a large multinodular goiter extending retrosternally up to the level of the D7 vertebra, abutting major mediastinal vessels witho

Practical approaches to the diagnosis and differential diagnosis of T-lymphoblastic leukemia/lymphoma.

T-lymphoblastic leukemia/lymphoma is a malignant neoplasm derived from T-cell precursors. These neoplasms can present initially as acute lymphoblastic leukemia (ALL) involving bone marrow or lymphoblastic lymphoma (LBL) involving lymph nodes, mediastinum or other extramedullary sites. Bone marrow and extramedullary tissue involvement frequently coexist. In this review, we begin with a short review of the clinical and morphologic features of ALL/LBL. We then focus on the immunophenotypic features of T-ALL/LBL. We delineate lineage-defining T-cell markers and immaturity-associated markers for th

Primary mediastinal large B cell lymphoma in pregnancy: a case report and review of the literature.

BACKGROUND: Primary mediastinal B-cell lymphoma (PMBCL) is a subtype of diffuse large B-cell lymphoma (DLBCL) that typically presents as a large mass originating in the mediastinum and infiltrating the surrounding organs, resulting in pleural and pericardial effusions. The occurrence of PMBCL during pregnancy presents a significant challenge to clinical management, necessitating a comprehensive consideration of the unique risks posed to both the pregnant woman and the foetus when selecting the optimal treatment. This report presents cases of PMBCL diagnosed during late pregnancy and reviews th

Patient-specific assessment of dosimetric deformable registration uncertainties and respiratory motion effects for moving tumours in proton therapy.

PURPOSE: To develop a patient-specific assessment of the dosimetric impact of deformable image registration (DIR) by quantifying DIR uncertainties using the distance discordance metric (DDM), and to demonstrate whether it is sufficient for assessing respiration-induced dose degradation in proton therapy (PT) treatments. METHODS: 4D-CT images were acquired to evaluate tumour motion. The study included eight lung and two liver cancer patients. The DDM was computed for each voxel and patient using the ANACONDA algorithm. DDM was validated on two different thoracic 4D-CT patient datasets with exp

Therapeutic value of lymphadenectomy after neoadjuvant chemotherapy in esophageal squamous cell carcinoma.

OBJECTIVE: Esophageal squamous cell carcinoma (ESCC) is characterized by a high incidence of lymph node metastasis, and prophylactic lymphadenectomy plays a critical role in esophagectomy. However, the optimal extent of lymphadenectomy after neoadjuvant chemotherapy (NAC) remains unclear. We aimed to investigate the extent and significance of lymphadenectomy after NAC by comparing its therapeutic impact with that of upfront esophagectomy. METHODS: In total, 200 consecutive patients who underwent McKeown esophagectomy for ESCC between 2009 and 2021 were eligible. We compared the therapeutic va

Mediastinal Air Entrapment During Extravascular ICD Implantation: A Preventable Cause of Elevated Shock Impedance.

INTRODUCTION: Elevated shock impedance during extravascular implantable cardioverter-defibrillator (EV-ICD) implantation may preclude defibrillation threshold (DFT) testing despite appropriate retrosternal lead position. We describe suspected mediastinal air entrapment as a reversible mechanism and a simple preventive strategy. METHODS AND RESULTS: During early EV-ICD implantation experience, two patients developed shock impedance > 200 Ω immediately after retrosternal lead placement, with associated sensing abnormalities. Lead repositioning did not normalize impedance, no pneumothorax was id

Right Atrial and SVC Cardiac Sarcoidosis: Not All Atrial Masses Are Tumors.

BACKGROUND: Right atrial (RA) masses create diagnostic challenges; extension into surrounding venous structures is rare. CASE SUMMARY: A 56-year-old woman presented with variable heart block and was found to have an RA mass extending into the superior vena cava (SVC). 18F-fluorodeoxyglucose positron emission tomography demonstrated metabolic activity within the myocardium, mediastinum, and pancreas concerning for sarcoidosis versus neoplasm. Endomyocardial biopsy was inconclusive. Transvenous pacemaker placement was not feasible given partial SVC obstruction by the mass. Ultimately, the patie

Floating Intercostal Port Technique for Single-Port Robotic Resection of Posterior Mediastinal Schwannomas: A Case Report.

Single-port (SP) robotic thoracic surgery is a minimally invasive option for mediastinal tumor resection; however, intercostal access remains challenging due to narrow intercostal spaces and the risk of postoperative neuralgia. This report describes 2 cases of posterior paravertebral mediastinal schwannoma resected using a floating intercostal port technique with the da Vinci SP system. In each case, a small uniportal incision was made at the lowest feasible intercostal space without rib spreading, and the SP metal port was positioned in a floating configuration above the intercostal space to

Descending Necrotizing Mediastinitis Across Age and Host-Status Groups: Diagnosis, Treatment Phases, and Outcomes in the Literature, with an Illustrative Case.

INTRODUCTION: Descending necrotizing mediastinitis (DNM) is a rare but life-threatening infection originating from odontogenic, tonsillopharyngeal, or deep cervical sources, and propagating through the cervical fascial planes into the mediastinum. Despite advances in cross-sectional imaging, thoracic surgery, intensive care medicine, and antimicrobial therapy, DNM remains associated with mortality ranging from 11 to 40% across contemporary series. The evidence base is fragmented and lacks a unified framework integrating anatomical, physiological, microbiological, and critical care dimensions.

Complete Resection of a Middle Mediastinal Malignant Peripheral Nerve Sheath Tumour via a Transmanubrial Osteomuscular Sparing Approach: A Case Report.

Malignant peripheral nerve sheath tumour (MPNST) is a rare soft-tissue sarcoma, and mediastinal involvement-particularly in the middle mediastinum-is exceedingly uncommon. Complete resection is crucial for favourable outcomes, as MPNST is generally resistant to chemotherapy and radiotherapy. We report a case of a middle mediastinal MPNST completely resected using a transmanubrial osteomuscular sparing approach (TMA). A 35-year-old man with neurofibromatosis type 1 presented with an enlarging 6.8-cm middle mediastinal mass suspected to be a malignant neurogenic tumour. Although initially consid

Endoscopic transcervical mediastinal drainage for treating descending necrotizing mediastinitis.

INTRODUCTION: Descending necrotizing mediastinitis (DNM) is a severe complication of deep neck infection when the mediastinum is involved. Currently, the optimal surgical treatment for DNM, especially for Endo IIA DNM, has not been defined. We aimed to describe a standardized ETMD procedure emphasizing safe catheter placement into the main mediastinal abscess cavities and to evaluate its association outcomes compared with non-ETMD approaches in patients with DNM. METHODS: This multicenter retrospective cohort study included adult patients (≥ 18 years) diagnosed with descending necrotizing med
