IMPAACT 2001 (NCT02651259) evaluated the safety and pharmacokinetics (PK) of once-weekly isoniazid and rifapentine in pregnant and postpartum women for tuberculosis prevention. Rifapentine (RPT) and desacetyl rifapentine (desRPT) PK data were collected in maternal plasma, breast milk, infant cord blood, and infant plasma. Longitudinal maternal plasma and breast milk RPT and desRPT concentrations were characterized using a multicompartment PK model with biophase effect compartments (NONMEM v7.4). Rate parameters describing drug entry into breast milk from plasma and breast milk-to-plasma ratio

Placenta accreta spectrum (PAS), and particularly placenta percreta, is one of the most hemorrhage-prone conditions in obstetric surgery. In Rh(D)-negative women, the conventional toolkit of patient blood management is constrained: compatible allogeneic supply is scarce, and intraoperative cell salvage (ICS) raises a specific concern about residual fetal red cell exposure and Rh alloimmunization. We report the perioperative course of a 29-year-old gravida 3, para 2 woman with O Rh(D)-negative blood, preoperative anemia (hemoglobin 86 g/L), placenta percreta with suspected bladder invasion, and

Placental extracts are rich in bioactive molecules and exhibit broad reparative properties, yet the molecular constituents responsible for their effects on skin tissue repair remain insufficiently characterized. In this study, we employed a cell-surface affinity-based screening approach using dermal fibroblasts (BJ) and microvascular endothelial (HMEC-1) cells as dual biological baits to capture bioactive peptides directly from placental extracts. Through liquid chromatography-tandem mass spectrometry (LC-MS/MS) analysis and background subtraction, a peptide with dual-cell targeting potential,

The field of neuroplacentology is a growing area that focuses on the influence of placental function on brain development. This field has expanded our understanding of the placenta's crucial role in proper neurodevelopment as well as placental anomalies that increase the risk of adverse neurodevelopmental outcomes. Neuroplacentology studies have revealed distinct roles of placentally provided hormones and nutrients in neurodevelopment whose disruption by perinatal problems, including inflammation, can increase the risk of neurodevelopmental disorders such as autism spectrum disorder and ADHD.

Placental growth factor (PlGF) and soluble fms-like tyrosine kinase-1 (sFLT-1) are established biomarkers of placental dysfunction, but their functions extend beyond the maternal circulating ratio. This structured narrative review examines the FLT1-PlGF-vascular endothelial growth factor system across implantation, trophoblast differentiation, uterine vascular transformation, foetoplacental angiogenesis, exchange capacity development and late-gestational maturation. Human evidence is integrated with selected non-human mammalian studies according to gestational stage, cellular source, anatomica

The syncytiotrophoblast (STB), the outermost layer of the human placenta, is vital to pregnancy health as it functions as the direct maternal-fetal interface. Being terminally differentiated and multinucleated, STB cannot divide but is formed/sustained by fusion of cytotrophoblasts throughout pregnancy. HtrA4, produced only in placentas of humans and high-order primates, is obligatory for syncytialization, including differentiation of trophoblast stem cells (TSCs) into STB as HtrA4 knockdown (KD) blocks the process; however, how HtrA4 mediates the process remains unknown. The current study aim

Estimating the contribution of preeclampsia (PE) to perinatal mortality in low-resource settings remains difficult due to limited diagnostic capacity. Lack of specific histopathologic changes in stillbirth, foetuses and/or placenta often hampers assignment of PE as cause of death. While PE angiogenic biomarkers (sFlt-1, PlGF) predict adverse pregnancy outcomes, their post-mortem diagnostic utility is unclear. We evaluated the potential role of these biomarkers in identifying PE-related perinatal deaths. This cross-sectional study was conducted among pregnant women who delivered a stillborn inf

Placenta previa is a serious obstetric condition associated with abnormal placental implantation and hemorrhagic complications, for which no targeted pharmacological therapy is established. This study applied an integrated structure-based computational workflow to explore the intracellular VEGFR1 kinase domain as a candidate molecular target. Twenty-one phytochemicals were screened using AutoDock Vina and MOE, with Emodin-8-glucoside consistently prioritized among the highest-ranked compounds. Redocking of the co-crystallized ligand 8ST reproduced its experimental pose with a heavy-atom RMSD o

BACKGROUND: Benzene is a widespread volatile organic compound, but its effects on fetal neuroimmune development remain poorly defined. We tested whether continuous gestational benzene exposure alters fetal placenta-brain signaling, fetal brain myeloid phenotype, and selected postnatal responses to viral challenge.
METHODS: Pregnant C57BL/6 mice were exposed to filtered air or 5 ppm benzene continuously from E0.5 to E12.5 or E17.5. Fetal tissues were collected at the respective endpoints. A separate cohort exposed through E17.5 delivered naturally; offspring received MHV68 at PND10 and were an

Human gestation is characterized by dynamic changes in maternal physiology and maturation of tissue functions before and after birth. The fetus and neonate are highly dependent upon transfer of maternal nutrients via the placenta and lactating mammary gland, which frequently involve selective membrane transporter proteins. At the same time, developmental changes in fetal and infant transport mechanisms critical for regulating absorption at the intestinal and blood-brain barriers are occurring. Coordinate regulation of cellular entry and efflux of nutrients, supplements, and xenobiotics, includ

Anti-M is usually a naturally occurring, cold-reactive immunoglobulin M (IgM) antibody and is commonly regarded as clinically insignificant. An immunoglobulin G (IgG) component, however, can cross the placenta and has rarely been associated with hemolytic disease of the fetus and newborn (HDFN), including hyporegenerative fetal or neonatal anemia. A 42-year-old woman (G4P2A2) undergoing preconception evaluation had a positive indirect antiglobulin test (IAT) with initially inconclusive antibody identification. Reference immunohematology testing identified anti-M reactivity at 4°C. After dithio

Respiratory distress syndrome (RDS) remains a major cause of neonatal mortality following premature birth. Antenatal betamethasone is the standard of care to accelerate fetal lung maturation and improve neonatal outcomes; however, current dosing recommendations assume uniform maternal and fetal exposure across pregnancies. Gestational age (GA)-dependent physiological changes and maternal obesity may alter betamethasone disposition and fetal exposure. This study developed a maternal-fetal physiologically based pharmacokinetic (m-f-PBPK) model to quantify the effects of GA and maternal obesity o

OBJECTIVE: To characterize placental elasticity during labor using shear wave elastography (SWE) in low-risk term pregnancies and to describe physiological changes across successive intrapartum stages.
METHODS: In this prospective single-centre observational study, low-risk singleton pregnancies at term in spontaneous labor were examined using ultrasound SWE. Placental elasticity was assessed in vivo at multiple labor stages (early labor, active labor, and complete cervical dilatation) and ex vivo after delivery. Measurements were systematically performed in central and peripheral placental r

Coxiella burnetii, the causative agent of Q fever, is an important zoonotic pathogen affecting livestock and humans worldwide. In Pakistan, epidemiological evidence remains limited, particularly at the human-livestock interface. This study aimed to determine the seroprevalence of C. burnetii and identify associated risk factors among livestock and livestock-exposed humans in Southern Punjab, Pakistan. A cross-sectional study was conducted across six districts, including 4758 livestock comprising goats, sheep, cattle, buffaloes, and camels, and 1019 humans with livestock-associated exposure. Se

Mitochondrial dynamics and mitophagy are indispensable for cellular homeostasis in hormonally regulated tissues such as the endometrium and placenta. Decidualization of endometrial stromal cells (ESCs) into decidual cells (DCs) requires extensive mitochondrial remodeling, yet how ovarian steroids regulate this process in vivo and in vitro remains incompletely understood. This study elucidates the modulation of mitochondrial fusion, fission, and mitophagy by estrogen (E2) and progesterone (P4) across the menstrual cycle and during pregnancy. Endometrial tissues were collected from women at earl

Seed shattering, which facilitates seed dispersal, is essential for plant reproduction but causes substantial yield loss in sesame (Sesamum indicum L.). It occurs through capsule dehiscence and/or seed abscission from the placenta, either simultaneously or independently depending on the species. In this study, we investigated cell wall-associated factors underlying delayed seed abscission in the sesame cultivar Suwon195 (SW195) relative to the easy-shattering cultivar IT030100. Histological and transcriptome analyses indicated that cell wall remodeling, particularly involving pectin and lignin

Exposure of pregnant women to toxic metals is an environmental health issue associated with pregnancy complications. Advancing our biological understanding of this problem and mitigating its adverse effects have been challenged by the ethical constraints of human participant research during pregnancy. Here we present a microphysiological system to experimentally simulate and investigate human placental exposure to environmental metals in maternal circulation. An in vitro analogue of the maternal-fetal interface and its dynamic tissue-specific environment are constructed using primary human pla

Single-cell RNA sequencing (scRNA-seq) enables high-resolution profiling of individual cells. However, inferring temporal relationships among cells remains a challenge. Here, we present real-time course analysis (RTCA), a simple direct phylogenetic signal framework that reconstructs cell lineages from somatic variants detected across cells using variants derived from nuclear-encoded transcripts in scRNA-seq data. Our simulations demonstrate that only a modest increase in informative variant sites is sufficient to maintain accurate tree reconstruction, even with a tenfold increase in the number

Parabens (PBs) are widely used preservatives in consumer products and suspected endocrine disruptors associated with adverse pregnancy outcomes. However, a structured framework for quantifying these risks is currently lacking. Employing the Targeted Risk Assessment of Environmental Chemicals (TRAEC) scheme, this study synthesized evidence from 16 epidemiological, 11 in vivo, and 5 in vitro studies, alongside our experimental data. PBs posed a moderate overall risk (score: 6.07), displaying compound-specific profiles. Regarding placental toxicity, methylparaben (MePB) scored highest (6.83), out

The placental labyrinth is the primary site of maternal-fetal exchange, and its disruption contributes to placental insufficiency and fetal growth restriction (FGR). Here, we identify insulin receptor substrate 2 (IRS2) as a key regulator of mouse placental labyrinth development. Irs2 is expressed in the syncytiotrophoblast (SynT) layers of the labyrinth, where it integrates growth factor signaling during placental morphogenesis. Loss of IRS2 disrupts labyrinth organization, causing defective SynT-II differentiation, impaired trophoblast fusion, and reduced fetal vascularization. Using CRISPR-
