The Hidden Hitchhiker: Retrocaval Ureter Masquerading as Isolated Pelviureteric Junction Obstruction.

The coexistence of true pelviureteric junction obstruction (PUJO) and a retrocaval ureter in a single, non-duplicated collecting system is exceptionally rare, and this case represents a "hidden retrocaval ureter," in which an upstream PUJO masked the classical radiological features, transforming the diagnosis into an unexpected intraoperative discovery. Our case highlights such a peculiar case, where a 70-year-old man with longstanding intermittent right loin pain was found to have right hydronephrosis and a serum creatinine of 1.7 mg/dL. Computed tomography showed a markedly dilated extra-ren

Imaging in Upper Tract Urothelial Carcinoma: Diagnostic Accuracy of Current Modalities for Local Tumor Staging.

PURPOSE OF REVIEW: To summarize current imaging modalities for upper tract urothelial carcinoma (UTUC) and evaluate their diagnostic performance for tumor staging and risk stratification. RECENT FINDINGS: CT urography remains the diagnostic standard with excellent accuracy for tumor detection, though its reliability for local staging is limited. MR urography shows promise for assessing muscle invasion but is operator dependent. FDG-PET demonstrates value in nodal and metastatic staging rather than primary tumor evaluation. Retrograde pyelography maintains a role in tumor diagnosis in circumst

Robot-assisted advancing spiral flap pyeloplasty for complex ureteropelvic junction obstruction and ureteral atresia: a preliminary experience in 27 patients.

BACKGROUND: Long-segment ureteropelvic junction obstruction (UPJO) (typically 12≥3 cm) and ureteral atresia present major reconstructive challenges due to fixed-length limitations of conventional flap techniques. This study evaluates the feasibility and safety of a robot-assisted advancing spiral flap pyeloplasty (RASFP) with dynamically adjustable flap design as a proof-of-concept for complex cases. METHODS: A single-center retrospective study (August 2022-July 2025) included 27 patients with long-segment UPJO (median stricture length 3 cm) or ureteral atresia. All patients underwent RASFP u

Medical Debt and Advanced Cancer Stage at Diagnosis.

IMPORTANCE: Medical debt affects millions of US residents and has been associated with higher cancer mortality. Whether medical debt burden is associated with incidence of advanced stage cancer, a key determinant of survival, remains unclear. OBJECTIVE: To examine the association between medical debt and cancer incidence in the US. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used ecological county-level data linking 2016 medical debt with age-adjusted incidence rates of 9 cancers from January 1, 2017, to December 31, 2021. The study included 2958 US counties with available

Reduced-order, computational fluid dynamics-informed pressure-flow simulation of endoscopic injection therapy in pediatric vesicoureteral reflux: An in-silico evaluation of technique-dependent outcomes.

INTRODUCTION: Endoscopic bulking therapy is widely used to treat vesicoureteral reflux (VUR), yet technique selection remains largely empiric and outcomes vary by reflux grade and bladder dynamics. We developed a hypothesis-generating, reduced-order, computational fluid dynamics (CFD)-informed pressure-flow simulation intended to explore hydrodynamic mechanisms and injection-strategy tradeoffs. METHODS: The model represents the bladder, ureterovesical junction, ureter, and renal pelvis as coupled pressure-flow elements. VUR grade templates modify competence, reverse resistance, dilation, tort

Failure to track a stable AMOC state under rapid climate change.

Persistent olfactory dysfunction (OD) is the second most common symptom of post coronavirus disease-19 (COVID-19) now being termed long-COVID. Its prevalence after recovery from COVID-19 is estimated to be 12% after nearly 6 months of follow-up. It thus becomes imperative for the treating clinicians to update themselves with the pathophysiology of this post COVID disability as well as the tools for diagnosis and the available treatment options. A systematic literature search was performed as per PRISMA guidelines in MEDLINE, Cochrane Library, LILACS, Google Scholar, ClinicalTrials.gov, and med

New Advancements in Prognostic Biomarkers for Upper Tract Urothelial Carcinoma.

As research on upper tract urothelial carcinoma (UTUC) has deepened, the value of biomarkers in the prognostic evaluation of UTUC has been gradually highlighted. As a high-grade epithelial tumor derived from the renal pelvis or ureter, UTUC has a significant prognostic challenge to patients, given its high invasiveness and recurrence rate for the formation of post-operative bladder cancer. To better predict the recurrence and metastasis risk of UTUC, this article provides a comprehensive review of hematologic, urologic, genetic, and histologic biomarker studies on the prognostic assessment of

Study of ureteric jet in kidney transplant recipient.

INTRODUCTION: An intact vescio-ureteric junction (VUJ) complex is necessary to maintain its normal physiological function. A ureteric jet is produced when urine is vigorously propelled from the VUJ into the urinary bladder. The transplanted ureter lacks a normal VUJ but maintains its intrinsic peristaltic activity. this study was conducted to compare prospectively between the ureteric jet parameters in voluntary kidney donors by Doppler ultrasound of the same with patients who underwent ureteric re-implantation during a kidney transplantation procedure. METHODS: There were a total of 44 parti

Identification of molecular changes in organs of obese rats following transcranial direct current stimulation (tDCS) - studies by FTIR microspectroscopy and chemometrics.

Due to increasing prevalence of obesity new safe and non-invasive therapies are sought to cope with the epidemic. Transcranial direct current stimulation (tDCS) is a neurostimulation technique that targets both the cortical and deeper brain structures. Both experimental studies and clinical research suggest the effectiveness of tDCS in suppressing appetite and reducing body mass. Therefore, to boost current knowledge on possible biochemical mechanisms underlying biological action of tDCS we determined the effects of tDCS on structural changes of lipids and proteins in key organs in obese roden

A feasibility study of a novel irrigation technique for reducing intrapelvic pressure during ureterorenoscopy using conventional devices.

PURPOSE: Ureterorenoscopy (URS) is a widely accepted approach for managing upper urinary tract stones, including renal calculi measuring > 20 mm. However, an elevated intrapelvic pressure (IPP) during URS can lead to infectious and procedural complications. We developed a simple irrigation technique and evaluated its efficiency in reducing IPP during URS. METHODS: In this technique, a 14/16-Fr ureteral access sheath (UAS) was advanced to the ureteropelvic junction or distal to the stone. A 4.7-Fr single-J ureteral stent was inserted through the UAS, with the tip placed in the renal pelvis or u

Clinical, Cytologic, Histopathologic, and Diagnostic Imaging of a Malignant Peripheral Nerve Sheath Tumor in the Renal Pelvis of a Border Collie Dog.

A 12-year-old female spayed Border Collie dog was presented for evaluation of 6 months of intermittent hematuria and weight loss. A highly vascularized right renal mass deforming the renal architecture and paraneoplastic hypertrophic osteopathy were found. Cytologic evaluation of the mass obtained by fine-needle aspiration guided by ultrasound revealed mesenchymal cells with a moderate amount of bluish cytoplasm, moderately defined cell borders, and spindle to stellate or roundish morphology. The nuclei were centrally located, with a coarse chromatin pattern, round to oval, and occasionally be

Renal Granuloma Distinct From the Renal Pelvis Following Bacillus Calmette-Guérin Treatment for Bladder Cancer: A Case Report.

INTRODUCTION: The incidence of renal complication following BCG treatment is rare. Few previous studies have reported a solitary renal granuloma distinct from the renal pelvis following BCG treatment. CASE PRESENTATION: An 85-year-old male underwent transurethral resection of the bladder tumor. A 6-week induction of intravesical BCG instillation therapy was planned; however, the sixth instillation was canceled owing to fever. Regular follow-up revealed no recurrence. However, 4 years later, CT revealed a right renal mass. With a clinical diagnosis of renal cell carcinoma, robot-assisted parti

Assessing inter-rater reliability of objective renal sonographic measurements for identification of clinically significant ureteropelvic junction obstruction.

INTRODUCTION: Anterior posterior renal pelvis diameter (APRPD) and minimal parenchymal thickness ratio (MPTR) on renal ultrasonography (RUS) have been identified as measures to help risk stratify patients with hydronephrosis for adverse findings on diuretic renography (DR). Widespread adoption of these measures requires reliable assessment by providers to ensure accurate risk stratification. OBJECTIVE: To assess the inter-rater reliability between pediatric radiologists measuring APRPD and minimum parenchymal thickness (MPT) on RUS in patients who also underwent DR for evaluation of hydroneph

Successful Treatment of Obstructive Ureterolithiasis with Medical Expulsive Therapy Including Tamsulosin in a Dog.

Ureterolithiasis is a common cause of ureteral obstruction in dogs, often leading to kidney injury. Medical expulsive therapy (MET) using α-adrenergic antagonists has been proposed as a nonsurgical treatment option in selected cases and is thought to facilitate ureteral stone passage by reducing ureteral smooth muscle tone. A 9-year-old castrated male Chihuahua weighing 1.78 kg was presented with anorexia. Physical examination revealed 7% dehydration and pale mucous membranes. Serum biochemistry demonstrated severe azotemia, with markedly elevated symmetric dimethylarginine (>100 μg/dL; refere

The utility of the Whitaker test in the modern era of pediatric urology: A retrospective cohort study.

INTRODUCTION: Pediatric urologists are often tasked with recognizing upper urinary tract obstruction and managing these cases appropriately. The Whitaker test (WhT) is a diagnostic modality created to identify obstruction by measuring pressure differentials along the renal pelvis and bladder via antegrade contrast administration. Over time, the diuretic renogram has become preferred in the workup of pediatric obstruction due to concerns regarding need for percutaneous nephrostomy tube placement and variability in findings based on technique during WhT. OBJECTIVE: This study evaluates the clin

Detection of Cerebrospinal Fluid-venous Fistulas Using Digital Subtraction Myelography in Refractory Spontaneous Intracranial Hypotension: A Single-center Study in an Asian Population.

Cerebrospinal fluid-venous fistulas are increasingly recognized as a cause of spontaneous intracranial hypotension, particularly in patients who do not respond to conventional therapies. Although widely reported in Western populations, their prevalence and diagnosis in Asian cohorts remain underexplored. This study aimed to evaluate the utility of digital subtraction myelography in diagnosing cerebrospinal fluid-venous fistulas in Japanese patients with refractory spontaneous intracranial hypotension. We retrospectively analyzed adult patients with spontaneous intracranial hypotension who unde
