Anencephaly Defects Do Not Alter Macroscopic Development of the Ureterovesical Junction in Human Fetuses.

OBJECTIVE: To compare morphometric parameters of the bladder, bladder trigone, and intramural ureter between human fetuses with and without anencephaly, adjusting for fetal age and weight. MATERIALS AND METHODS: Thirty-seven human fetuses were analyzed (23 controls and 14 with anencephaly), ranging from 11 to 35 weeks post-conception. Bladder dimensions, trigonal measurements, and right and left intramural ureter lengths were obtained by standardized microdissection and digital morphometry. Group comparisons were performed using Student's t-test. Multivariable linear regression models adjuste

Two Forgotten Ipsilateral Double J Stents Managed in a Single Session: A Rare Endourologic Challenge.

Forgotten ureteral stents may lead to severe complications such as encrustation, infection, obstruction, and stone formation, especially when retained beyond 12 weeks. Dual forgotten stents in the same renal system managed in a single operative session are rare. A 51-year-old male with no urinary symptoms was incidentally found to have two forgotten left DJ (Double J) stents placed in 2021 and 2024. CT showed encrustation with multiple renal and ureteric stones. Renal function was preserved. A single-session tri-modality approach-cystolithotripsy, ureteroscopic lithotripsy, and lower-pole perc

Validation of a Deep Learning Model for Automated Ureteral Length Measurement on CT Against Gross Specimens.

This study validates a deep learning model for automated ureteral length measurement on CT using gross specimen measurements as the reference standard. A deep learning model was trained on 3629 CT series from 1502 patients (cohort 1) to segment the ureter and extract centerline length. An independent prospective cohort (cohort 2) of 70 patients undergoing radical nephroureterectomy served as the hold-out test set. Each gross specimen was measured twice by one urologist (D1_length and D2_length), with their mean as the reference length (Ref_length). Model-predicted lengths (AI_length) were comp

Gramine Suppresses Myofibroblast Activation and ECM Accumulation in Renal Fibrosis by Targeting the NF-κB/NLRP3 and TGF-β1/Smad Pathways.

Chronic kidney disease (CKD) is driven by persistent inflammation and progressive fibrosis, leading to irreversible renal damage. Gramine (GMN), an indole alkaloid, has broad pharmacological benefits. However, its therapeutic potential against renal fibrosis remains unexplored. This study investigates the antifibrotic potential of GMN in an experimental model of CKD. Potential therapeutic targets of GMN associated with renal fibrosis were identified through network analysis, and functional enrichment analysis revealed the mechanisms regulated by these targets. In vitro validation was conducted

High-resolution pressure myography reveals pressure-dependent local contractile and geometric dynamics in the renal pelvis and ureter.

BACKGROUND AND PURPOSE: Coordinated peristalsis between the renal pelvis and ureter is critical for effective urine transport. However, intraluminal pressure impact on upper urinary tract (UUT) contractility and pharmacologic responsiveness remains poorly understood due to absence of suitable investigative methods. EXPERIMENTAL APPROACH: This study introduces the first application of high-resolution pressure myography to UUT, establishing a novel ex vivo platform enabling simultaneous, intraluminal pressure-controlled local measurement of renal pelvic contraction frequency and amplitude and u

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

Cystoscopic submucosal indocyanine green marking in robot-assisted radical nephroureterectomy: a multicenter cohort study.

BACKGROUND: Distal ureter and bladder cuff dissection can be difficult during robot-assisted radical nephroureterectomy (RA-RNU) for upper tract urothelial carcinoma (UTUC), particularly in the deep pelvis where tactile feedback is limited. We evaluated cystoscopic submucosal indocyanine green (ICG) marking around the ureteral orifice as a navigation aid for this step. METHODS: Adults who underwent RA-RNU with formal bladder cuff excision for clinically localized UTUC (≤T3, M0) at three centers between January 2019 and November 2024 were retrospectively analyzed. Outcomes were compared betwee

Robot-assisted ileal ureter replacement for complex ureteral strictures: a systematic review and single-arm meta-analysis of surgical and functional outcomes.

Complex, long-segment ureteral strictures present significant reconstructive challenges. While open ileal ureter replacement is the historical gold standard, it carries substantial perioperative morbidity. This systematic review and single-arm meta-analysis evaluate the surgical and functional outcomes of robot-assisted ileal ureter replacement (RIUR). Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search of four databases was conducted through September 2026 to identify studies reporting adult RIUR outcomes for complex strictu

The Surgical Management of Prostate Cancer in Renal Transplant Recipients: The Minimally Invasive Approach.

Patients with a renal transplant present unique anatomical challenges during pelvic surgery. The graft is typically located in the iliac fossa with vascular anastomoses to the iliac vessels and a short ureter implanted into the bladder. These features increase the risk of graft injury during radical prostatectomy, especially when lymph node dissection is planned. This case describes the experience in one of the few cases performed in our department. A 66-year-old man with localized prostate cancer (Gleason 3 + 4 = 7a) and a functioning renal transplant in the right iliac fossa. His history inc

Retroperitoneoscopic lymphatic disconnection for non-filarial chyluria in Vietnamese adults: a case series.

INTRODUCTION: Retroperitoneal laparoscopic surgery is a widely applied technique in the management of renal, adrenal, ureteral, and retroperitoneal lymphatic disorders. A representative condition is chyluria, a rare disease caused by a lymphatic-urinary fistula, most commonly involving the ureter and renal pelvis. The retroperitoneal approach provides direct access to the affected structures while minimizing intra-abdominal manipulation, thereby reducing the risks of bowel adhesions and visceral injury. This technique enables ligation, excision, or suturing of abnormal lymphatic vessels, poten

Renal Candidal Bezoar in an SGLT2 Inhibitor User with an Indwelling Ureteral Stent: A Case Report and Review of Literatures.

Renal candidal bezoar is a rare cause of obstructive pyonephrosis and usually occurs in patients with diabetes mellitus, immunosuppression, urinary obstruction, or indwelling urinary devices. We report an 88-year-old man with type 2 diabetes treated with dapagliflozin, stage 3 chronic kidney disease, and a history of ureteral stricture managed with ureteral stenting who developed recurrent candidal pyonephrosis and extensive fungal bezoar involving the ureter and renal pelvis. Initial treatment at a referring hospital included percutaneous nephrostomy drainage, fluconazole therapy, ureteroscop

Extraction-string ureteral stents and postoperative infection after flexible ureteroscopy: an IPTW-adjusted analysis.

Extraction-string ureteral stents facilitate non-cystoscopic removal after ureteroscopy, but concerns remain regarding postoperative infection and accidental dislodgement. We evaluated whether extraction-string stents were associated with postoperative infection after flexible ureteroscopic lithotripsy (fURSL) in a cohort restricted to procedures with the distal Double-J (D-J) stent portion positioned within the bladder. This retrospective observational study included 102 procedures in 92 unique patients: 58 extraction-string and 44 conventional non-string procedures. Postoperative infection w

Pressure-flow behavior during RIRS irrigation: a controlled ex vivo porcine kidney study of flow-dependent apparent outflow resistance.

PURPOSE: To measure actual irrigation flow and intrarenal pressure (IRP) under different irrigation modes during retrograde intrarenal surgery (RIRS), and to develop a controlled ex vivo empirical pressure-flow model for the tested endoscope-sheath configurations. METHODS: A controlled ex vivo porcine kidney-ureter model was used, with all measurements taken using a 200 μm holmium laser fiber in the working channel of a 7.5Fr or 6.3Fr flexible ureteroscope and 10/12Fr or 12/14Fr suction access sheaths. Manual syringe, gravity, and automatic pump irrigation were tested across three porcine kid

Slippery when wet: The effect of a novel zwitterionic coating on silicone ureteral stents.

Silicone ureteral stents provide anti-encrustation properties and patient comfort but are more difficult to insert and remove due to increased surface friction. Applying zwitterionic coatings to silicone stents may reduce friction and facilitate stent placement and removal. This study compared the frictional forces of zwitterionic-coated silicone stents with commercial stents using benchtop models. The static and kinetic frictional forces of five ureteral stents were measured: bare silicone stent without coating (BS), single outer-coated zwitterionic silicone stent (Z1), double-coated (inner a

Risk factors and machine learning-based prediction of difficult ureter during retrograde ureteroscopic lithotripsy.

OBJECTIVE: Difficult ureter (DU) is a challenging condition that prevents successful access to the upper urinary tract during retrograde ureteroscopic lithotripsy (URS). The present study aimed to identify clinical risk factors for DU and to evaluate the predictive performance of machine learning-based models in patients undergoing URS for ureteral stones. METHODS: Medical records of patients who underwent retrograde ureteroscopic lithotripsy between January 2023 and January 2024 were retrospectively reviewed. A total of 221 patients were included and classified into difficult ureter (DU) and

The Hidden Hitchhiker: Retrocaval Ureter Masquerading as Isolated Pelviureteric Junction Obstruction.

The coexistence of true pelviureteric junction obstruction (PUJO) and a retrocaval ureter in a single, non-duplicated collecting system is exceptionally rare, and this case represents a "hidden retrocaval ureter," in which an upstream PUJO masked the classical radiological features, transforming the diagnosis into an unexpected intraoperative discovery. Our case highlights such a peculiar case, where a 70-year-old man with longstanding intermittent right loin pain was found to have right hydronephrosis and a serum creatinine of 1.7 mg/dL. Computed tomography showed a markedly dilated extra-ren

Reverse seven ileal ureteroplasty with ileocystoplasty for bilateral radiation ureteral obstruction and a contracted bladder.

Bilateral ureteral obstruction after pelvic radiotherapy is uncommon, and rarer still when the bladder is also contracted, because this removes the low pressure reservoir that ureteral reconstruction depends on. A 39 year old woman developed total obstruction of both ureters and a 100 mL bladder after chemoradiotherapy for cervical cancer. One 35 cm distal ileal segment in a reverse seven configuration replaced both ureters, and its caudal limb was detubularised and anastomosed to the bivalved bladder. At six months she voided spontaneously, was continent and remained free of disease. The reco
