An exceptional case of intracranial plasma cell granuloma, located in the right frontoparietal convexity in a 16-year-old boy is reported. Reports of these rare inflammatory lesions locate them primarily in the lungs and in other regions of the body, and only exceptionally in the nervous system; indeed, only three intracranial cases and one other arising from the spinal meninges have been reported. The computed tomographic and surgical aspects suggested a falx meningioma in our case. The pathological diagnosis differentiating these from other intracranial lesions with a plasma cell component,

We investigated whether the incidence of post-lumbar puncture headache could be reduced by using a new technique. By rotating the puncture needle the spinal meninges become penetrated with the bevel adjusted parallel to the main fibre direction. Therefore the fibres are only dissociated and not cut. We used this method in 20 patients and only 2 of them complained of headache. By contrast 14 out of another group of 20 patients suffered from headache after conventional lumbar puncture. These results demonstrate that post-lumbar puncture headache can be prevented in a high percentage of patients

A primary cellular blue nevus (melanocytoma) of the spinal canal in a 21-year-old woman is reported. Light microscopic examination revealed a melanotic neoplasm with histological patterns resembling schwannoma, dermal nevi, and neuroblastic-like tumor. The ultrastructural features of the neoplastic cells were similar to those in dermal blue nevi and melanomas. There was no evidence of arachnoidal cell differentiation. Immunohistochemistry revealed positive reactions for S-100 protein and neuron-specific enolase in many cells and no reactions for glial fibrillary acidic protein, cytokeratins, e

The fine anatomy of the human spinal meninges was examined in five postmortem spinal cords taken within 12 hours after death from patients aged 15 months to 46 years. Specimens of spinal cord were viewed in transverse section and from the dorsal and ventral aspects by scanning electron microscopy. Transverse sections of spinal cord and meninges were also examined by light microscopy. The arachnoid mater was seen to be closely applied to the inner aspect of the dura. An intermediate fenestrated leptomeningeal layer was observed attached to the inner aspect of the arachnoid mater and was reflect

We investigated the clinical relevance of a new lumbar-puncture technique for prevention of headache. The spinal meninges were penetrated with a conventional puncture-needle with its bevel adjusted parallel to the main fibre direction, contrary to that in the usual technique. We used this method in 12 patients and only 1 complained of headache. On the other hand 9 patients out of 12 suffered from headache after conventional lumbar-puncture. These results demonstrate that post-lumbar puncture headache can be prevented by this technique in a high proportion of patients.

A male aged 60 is reported who, 5 years after chordotomy at the Th 2/3 level, developed acute paraplegia of the legs and a sensory transverse lesion due to an extradural tumour of the Th 1-4 level. Neuropathology revealed transverse necrosis of the thoracic spinal cord (Th 1-4) due to an intradural tumour at the upper thoracic and lumbar spinal levels, the thoracic malignancy arising at the site of the previous chordotomy, with a suture being observed within the tumour mass. Histology, positive immunostaining of tumour cells with Factor VIII antigen, a specific marker of endothelial cells and

The normal mobility of the vertebral column, particularly in the cervical and lumbar regions, gives rise to major changes in the length and lumen of the spinal canal and of its volume. Its contents must therefore adapt to these changes without disturbance to their several functions. The contents are partly fluid, partly neural tissue, but also connective tissue: all with differing physical characteristics. Thus, all respond differently to changes in the space in which they are contained. When pathological changes supervene, the spinal meninges, the spinal cord and nerve roots may be adversely

A case of sarcoidosis limited to the central nervous system is described in which the diagnosis was suggested by high Ga-67 uptake in the cranial and spinal meninges. The diagnosis was confirmed by meningeal biopsy. Treatment with oral corticosteroids resulted in clinical improvement and marked reduction in Ga-67 uptake in the meninges. This is the first reported case of the central nervous system sarcoid diagnosed by Ga-67 imaging.

The brain and cranial meninges were studied in 61 serially sectioned embryos of stages 8-23. Much earlier stages than those examined by previous authors provided a more comprehensive view of meningeal development. As a result, the possible and probable sources of the cranial and spinal meninges are believed to be: (a) prechordal plate, (b) unsegmented paraxial (parachordal) mesoderm, (c) segmented paraxial (somitic) mesoderm, (d) mesectoderm (neural crest), (e) neurilemmal cells (neural crest), and (f) neural tube. Some of these sources (a, b, d) pertain to the cranial meninges, others (c, d,

A 34-year-old man presented with tumor of his cervical spinal meninges and bone and the dura over the right frontal lobe, which was shown on biopsy to be Hodgkin's lymphoma. Extensive noninvasive evaluation failed to reveal any other sites of disease. Central nervous system involvement with Hodgkin's disease is extremely rare and is almost always a late complication in patients who have widely disseminated disease in the usual nodal sites. The current patient is unique in that his initial symptoms were due to involvement of the central nervous system and he had no evidence of Hodgkin's disease

Five infants with meningitis and ventriculitis, and a sixth patient with meningitis only are reported. In one hydrocephalic infant, infection of the central nervous system (CNS) was not suspected until cerebral ultrasonography revealed features of ventriculitis. It appears that in non-communicating hydrocephalus managed with a ventriculo-peritoneal (VP) shunt, infection may involve predominantly the "sequestered ventricles" rather than the spinal meninges. In four infants, bacterial meningitis had been proven but ventriculitis was not diagnosed until cerebral ultrasonography was performed. In

An intrathoracic meningocele is a protrusion of spinal meninges through an intervertebral foramen into the thoracic cavity. The majority occur in association with neurofibromatosis, and bilateral or multiple lesions are rarely seen. In conventional radiography, small meningoceles may be hidden behind other mediastinal structures. To investigate paraspinal masses, computed tomography (CT) with intrathecal contrast material is likely to result in more frequent detection of unsuspected lateral meningoceles. This report details our experience with CT metrizamide myelography in a case of multiple b

Telecobalt irradiation in combination with intrathecal (IT) methotrexate has been replaced by IT 198Au-colloid and methotrexate for meningosis prophylaxis in leukemia. Seventy-seven children received 56-200 MBq 198Au-colloid. The distribution was measured with a scintillation camera having a data processing facility. The radiopharmaceutical is adsorbed at the surface of the spaces with cerebrospinal fluid (CSF) 10-20 h after application. The normal retention of the administered radioactivity in the intracranial subarachnoid space (ISS) and in the spinal subarachnoid space (SSS) were 52 +/- 10%

Fourteen juvenile patients with small cell gliomas were studied at two institutes. These tumors are believed to form a distinct entity. They arise mostly in the diencephalon or the brain stem and are composed of a poorly differentiated small cell component having a pronounced tendency to differentiate into a glioma. Signs of neuroblastic differentiation were also found with the electron microscope. Small cell gliomas disseminate early and profusely throughout the ventricular walls and the subarachnoid spaces including the spinal meninges. Prognosis is grave, most patients dying within 1 year o

The authors studied the clinical picture of lumbar osteochondritis in 1,020 patients, 151 of whom were operated on. The monoradicular syndrome was diagnosed in 68%, the biradicular (involvement of two radicles) in 27%, and the polyradicular (involvement of three and more radicles) in 7% of the patients. In the authors' opinion, the development of the mono- or polyradicular syndromes in lumbar osteochondritis depends on the character of the disc hernia (lateral, paramedian, median) and on the degree of the reactive inflammatory changes in the radicles, the epidural tissue and the spinal meninge

Results of the clinico-roentgenological observation of butterfly-like deformity of the middle dorsal vertebra body (a rare congenital anomaly) are presented. In the presence of an infection the patient developed a spinal pathology (lower central paraparesis, parahypesthesia, disturbances of the functions of pelvic organs). The neurological symptoms were found to be due to an adhesive process in the spinal meninges at the level of the deformed vertebra and the secondary compression-ischemic myelopathy. The treatment consisted in injecting prednisolone under the spinal meninges. The patient's ro

The spinal cord and meninges were examined, at all levels, in seven anencephalic fetuses of between 19 and 24 weeks of gestation. Four of the seven anencephalics showed dense collections of cells resembling immature neural tissue throughout the spinal subarachnoid space. The leptomeninges were excessively vascular and contained many active macrophages and small round cells in all the anencephalics and the cord was more slender than normal. Control fetuses did not show these features, although inactive macrophages were seen occasionally. It is concluded that the abnormal collections are made up

Two dogs with rapidly progressing paralysis had polyarteritis involving medium-sized to large arterioles and small arteries of the spinal meninges. Rupture of structurally weakened vessels caused massive hemorrhage with compression and infarction of the spinal cord. The cause of the vasculitis was not determined.

A description of 21 patients suffering from lympho- and reticulosarcomas with metastatic affections of the cerebral and spinal meninges (20 patients) and an intracerebral solitary tumour (one patient) is presented. In 17 cases the diagnosis was confirmed on autopsy. The neurological picture was similar to the clinical picture of basal (7 patients) or cerebrospinal (9 patients) meningitis. Isolated spinal lesions were infrequent (4 patients). The cerebrospinal liquor showed, as a rule, a protein-cellular dissociation; in individual cases a high blastic pleocytosis was observed. A favourable eff
