BACKGROUND: Erythema nodosum leprosum (ENL) is a severe, immune mediated type 2 lepra reaction that occurs predominantly in the lepromatous spectrum of leprosy. It is characterized by the eruption of tender inflammatory nodules. The possible contribution of patient belief and the nocebo effect, on precipitating ENL, has not been examined. We present a patient with lepromatous leprosy and probable ulcerated ENL whose flares the patient reported to coincide with the full moon, raising questions about how cultural explanatory models and cognitive pattern recognition intersect with chronic inflamm

OBJECTIVE: to present a case of lepromatous leprosy with two reactional episodes during its course.
CLINICAL CASE: 32-year-old woman from Paraguay, who presented with retiform purpura followed by ulceration on the lower limbs, associated with altered thermal and pain sensitivity. She also had skin infiltration, malar region nodules, madarosis, and a saddle nose. The diagnosis of Lucio’s phenomenon in lepromatous leprosy was made through biopsy for histopathology and bacilloscopy. She began treatment with glucocorticoids and antimicrobials according to the WHO regimen, and subsequently develop

Histoid Hansen disease is a distinct variant of lepromatous leprosy characterized by nodular lesions, spindle-shaped histiocytes and a very high bacillary load, often associated with relapse and drug resistance. While standard multibacillary multidrug therapy (MB-MDT) remains the mainstay of treatment, very short-course bactericidal regimens appear inadequate in bacillary-rich disease. We report a case of histoid leprosy treated with adjunctive rifampicin and ofloxacin for 3 months alongside MB-MDT, resulting in marked clinical regression. Although causality cannot be inferred from a single ca

PURPOSE: The purpose of this study was to determine whether neutrophil extracellular trap (NET)-related markers are elevated in the aqueous humor of patients with glaucoma compared with healthy controls.
METHODS: We performed a case-control study of patients scheduled to undergo cataract extraction and/or glaucoma surgery from 2020 to 2022.
RESULTS: One hundred seventy-seven eyes were included: 127 were healthy controls, 43 were patients with glaucoma, and 7 were patients with uveitis. Pro-inflammatory markers were elevated in glaucomatous aqueous humor as compared with controls including: i

INTRODUCTION: Thalidomide has emerged as a novel antitumor drug with antiangiogenic and immunomodulatory properties. It was taken off the market in the early 1960s due to its infamous connection with congenital defects. Recently, the FDA approved thalidomide for treating erythema nodosum leprosum, adhering to strict guidelines and safety measures. Sensory peripheral neuropathy and teratogenicity, fatigue, vertigo, headache, gastrointestinal issues, skin eruptions, dizziness, galactorrhoea, decreased libido, and constitutional symptoms like fever, weakness, headaches, and weight loss are the ma

Leprosy represents a significant challenge in understanding the multiple immunological mechanisms that influence the distinct clinical forms. This integrative review used the electronic databases MEDLINE/PubMed and the Virtual Health Library (VHL), including 12 studies conducted predominantly in Brazil, China, India, and Ethiopia. We focused on the role of regulatory T (Treg) cells in Hansen's disease, specifically examining their frequency of expression and the functional characteristics of their subpopulations. Findings reveal the ubiquitous nature of Treg across clinical forms, particularly

BACKGROUND: Leprosy is an infection caused by Mycobacterium leprae complex that primarily affects the skin and peripheral nerves but may also involve multiple organ systems. Although globally endemic, the disease is rare in high-income countries, and leprosy during pregnancy is infrequently reported. Immunologic adaptations of pregnancy may precipitate disease onset or exacerbate reactional states, which can complicate anesthetic management. Literature on obstetric neuraxial labor analgesia in women with leprosy is limited, with no previously documented reports describing its use during labor.

Leprosy and tuberculosis (TB) are both chronic granulomatous infections caused by bacilli of the genus Mycobacterium. The simultaneous occurrence of cutaneous tuberculosis and leprosy is rare. We report a 56-year-old male presenting with painful erythematous nodules and plaques over the face and extremities for 1 week, along with asymptomatic right axillary lesions for 1 year. Examination revealed tender nodules and plaques with ulceration on the face and extremities, and firm, matted subcutaneous nodules with sinus tracts in the axilla. Sensory loss in a glove-and-stocking distribution and pe

Background Leprosy and tuberculosis (TB) share common characteristics, such as acid-fastness of causative bacteria, geographic endemicity, route of spread, large number of asymptomatic infections, and requirement of multiple drugs for long periods of time to prevent resistance and provide treatment. Being relatively common, co-infection with the two diseases should occur based on chance alone. However, coinfection is surprisingly rare, with less than 20 cases being reported in the last decade. Aim The purpose of this case series was to study the clinico-epidemiological profile of patients with

Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae, with a predilection for cooler areas of the body such as the external ear. We report a man in his late thirties with lepromatous leprosy, presenting with an eight-month history of facial and auricular nodules that progressed to erythema, ulceration, and sensory impairment. The clinical examination revealed infiltrated, nodular pinnae, a cheek ulcer, and bilateral ulnar neuritis. Slit-skin smear revealed abundant acid-fast bacilli (BI 6 +), and histopathology confirmed the multibacillary spectrum. The acute erythema, p

Leprosy is a chronic infectious disease caused by Mycobacterium leprae that primarily affects the skin and peripheral nerves. Although eliminated as a public health problem in many countries, it remains an important neglected tropical disease, with more than 174,000 new cases reported worldwide in 2023, predominantly in endemic regions such as India, Brazil, and Indonesia. We report a rare case of a presumed very late relapse of lepromatous leprosy occurring 35 years after treatment completion in a patient from a non-endemic European setting. The patient presented with progressive sensorimotor

Leprosy, or Hansen's disease, is a chronic granulomatous infection caused primarily by Mycobacterium leprae. Although its global incidence has declined since the introduction of multidrug therapy (MDT), the disease remains endemic in regions such as India, Brazil, and Indonesia. Additionally, sporadic cases continue to occur in non-endemic areas, including rural areas of the United States, due to migration and travel. We report a 62-year-old male who presented with erythematous, edematous plaques on the arms and trunk, along with progressive hand numbness. He also had a history of frequent tra

Lepromatous leprosy, more commonly seen in endemic areas, is a severe multi-bacillary granulomatous infection of the skin and peripheral nerves, which leads to facial disfigurement and digital auto-amputation. Although treatable with multidrug therapy, diagnosis and early intervention remains of paramount importance. The bacilli are strongly positive with Fite special stain, and to this day this stain remains most widely used especially in areas that do not have access to PCR testing. Herein, we describe a case where the M. leprae bacilli were Fite positive but also very strongly GMS positive.

BACKGROUND: Leprosy is a chronic infectious disease with diverse clinical manifestations. Skin lesions may present as erythema, nodules, ulcers, bullae, or verrucous hyperplasia. When skin lesions and neurological symptoms are atypical, misdiagnosis is common. When patients develop rare skin lesions resembling pretibial myxedema, clinicians often misdiagnose the condition as hyperthyroidism or mucin deposition disorders. Such misdiagnoses can delay appropriate treatment and may lead to serious complications. CASE PRESENTATION: We report a 74-year-old male with progressive bilateral pretibial e

Dermal leishmaniasis, particularly postkala-azar dermal leishmaniasis (PKDL), is an uncommon entity that may closely mimic lepromatous leprosy in endemic regions. Both conditions present with chronic, symmetric nodular or plaque-like lesions, leading to frequent diagnostic confusion, especially when peripheral nerve thickening is absent. We report a 71-year-old male with a 15-year history of diffuse erythematous nodules and scaly plaques over the palms and dorsum of the feet, repeatedly managed as lepromatous leprosy without clinical improvement. The absence of peripheral nerve thickening and

BACKGROUND: Leprosy (Hansen’s disease) is a chronic granulomatous infection primarily caused by Mycobacterium leprae. Since 2008, Mycobacterium lepromatosis has also been identified as an additional causative pathogen. Without timely treatment, the disease can progress to serious neuropathy and permanent disability. Lepromatous leprosy, the most infectious form, is associated with high bacillary load, systemic involvement, and immune-mediated complications such as type 2 reaction. Treatment with multidrug therapy is effective, but patient non-adherence remains a major challenge to disease cont

Lucio's phenomenon is a rare necrotizing skin reaction, almost exclusive to diffuse lepromatous leprosy, that can mimic systemic vasculitis and lead to diagnostic and therapeutic errors. We report the case of a 32-year-old woman from Paraguay with prolonged fever, hepatomegaly, and violaceous, painless necrotic skin lesions on her limbs. She was initially treated with corticosteroids for presumed vasculitis without clinical improvement. Physical examination revealed madarosis, lepromas, and altered thermal-pain sensitivity. Skin biopsy showed a perivascular histiocytic infiltrate with acid-fas
